Pre-analytical errors in clinical chemistry laboratory of a tertiary care hospital in western Uttar Pradesh, India

Background: Pre-analytical errors account for up to 70% of all mistakes made in laboratory diagnostics, most of which arise from problems in patient preparation, sample collection, transportation, and preparation for analysis and storage. Pre-analytical errors influence the total error thus hindering TQM in laboratory, consequently decreasing the accuracy and reliability of the results generated. This study was conducted with the aim to determine nature and frequency of the occurrence of pre-analytical errors.Methods: This prospective analytical study was designed to evaluate the pre-analytical errors observed in a total of 13,892 out-patient and inpatient samples. Samples received for routine clinical chemistry analysis were screened for pre-analytical errors. Samples received for other investigations were excluded. We recorded all nonconformities and errors occurring over a 3-month period and corrective measures were suggested to minimise them. Laboratory personnel were asked to register rejections, and pre-analytical causes for rejection of ward as well as out-patient samples collected in the laboratory. Types of inappropriateness were evaluated as follows: hemolyzed, blood collection in wrong tubes, clotted blood, inappropriate timing of collection, improperly labelled samples, insufficient volume of specimen and lipemic samples.Results: A total of 13,892 samples from the outpatient department and in-house patients were received by our clinical biochemistry laboratory during the period from April 2019 to June 2019. Out of these 404 samples were found unsuitable for further processing. This accounted for 2.9% of all samples collected in the laboratory and pre-analytical errors were responsible for these samples to be rejected over a period of 3 months. Rejections arose as a result of the following reasons: 0.92% were rejected due to hemolysis; 0.58% were blood collected in wrong tubes; 0.55% were clotted blood; 0.26% had inappropriate timing of collection; 0.24% were mislabeled samples; 0.20% had insufficient sample quantity and 0.14% were lipemic samples.Conclusions: Of all the samples received in the lab, the overall percentage of rejection is 2.9%. Substantial number of samples undergo repeated testing because of rejection owing to pre-analytical errors. The efforts should be aimed to reduce the rates of rejected samples can provide to improve the quality of laboratory based health care processes.

Abstract Objective The aim of this study was to study the incidence of preanalytical errors in the clinical chemistry laboratory attached to a tertiary care hospital. Design and Methods The study was conducted in a clinical chemistry laboratory using the samples and forms received for analysis. Five hundred random samples were analyzed using a predefined set of quality indicators (QIs) over a period of 3 months. The incidence of each preanalytical error was described as a percentage of the total samples analyzed in the study. Statistical Analysis Individual QIs were assigned values as 0 and 1 and were used to assess each sample; 0 if the error was present, and 1 if absent. The incidence of each preanalytical error was described as a percentage of the total samples analyzed in the study. Result Out of the 500 samples observed, 138 samples were error free, while 21 samples had the maximum number of errors, that is, 6. The error committed most often was the omission of provisional diagnosis being mentioned on the requisition form. No preanalytical error was observed for QIs: selecting the appropriate blood collection vial or storage of sample. Conclusion This study confirms that error rate in the preanalytical phase is high and vastly ignored. Errors committed here may be overlooked, given the large number of samples received in the clinical laboratory of a tertiary center. To reduce these errors, the laboratory should provide training to all workers involved in the preanalytical phase. Daily or weekly QI scores should be recorded to assess and rectify shortcomings, thereby improving patient care.

Background: Six sigma is a powerful tool which can be used by laboratories for assessing the method quality, optimizing Quality Control (QC) procedure, change the number of rules applied, and frequency of controls run .The aim of this study was to quantify the defects or errors in the analytical phase of laboratory testing by sigma metrics and then represent the sigma value in Method Decision Chart.Methods: A retrospective study was conducted in a tertiary care hospital in Bhubaneswar, India. The clinical chemistry laboratory has been NABL accredited for the past 5 years and strictly quality checked.  Internal and external quality control data was collected for a period of six months from January - June 2018 for 20 biochemical analytes. Sigma metrics for each parameter was calculated and plotted on method decision chart.Results: The sigma metrics for level 2 indicated that 6 out of the 20 analytes qualified Six Sigma quality performance. Of these seven analytes failed to meet minimum sigma quality performance with metrics less than three and another seven analytes performance with sigma metrics was between three and six. For level 3, the data collected indicated that seven out of 20 analytes qualified Six Sigma quality performance, six analytes had sigma metrics less than 3 and seven analytes had sigma metrics between 3 and 6.Conclusion: In our study Sigma value was highest for amylase and lowest for potassium. Use of alternative methods and/ or change of reagents can be done for potassium to bring the sigma value within an acceptable range.

Background: Every medical student in India have to undergo a compulsory rotatory internship for completion of their course where they encounter various medical emergencies and apply their medical knowledge. An early encounter to a basic life support course and training will increase the efficacy of cardiopulmonary resuscitation and thus the outcome of the patient. This study was designed to test knowledge of MBBS students in a tertiary care hospital.Methods: This observational study was conducted in a tertiary care hospital in Uttar Pradesh and used a preformed validated questionnaire to test awareness and knowledge of basic life support and cardiopulmonary resuscitation in a sample of 500 MBBS students. Descriptive analysis was performed on the questionnaire responses. All data obtained from the questionnaire was evaluated and statistically analysed using software IBM SPSS Statistics software version 24 (IBM Corp., Armonk, NY, USA) for MS windows.Results: With a response rate of 47% among 500 MBBS students, the mean score obtained was 2.34±1.066 out of a maximum score of five. A maximum score of 2.804±1.055 obtained by 5th-year students. Surprisingly, first-year students achieved an average score of 2.66±0.97, which was higher than that of 2nd, 3rd, and 4th year students. 87% of students were like-minded to participate in the cardiopulmonary resuscitation (CPR) awareness program. Only 45% of students correctly answered the order of CPR as C-A-B (chest compression-airway-breathing).Conclusions: The study showed that though the awareness and importance of basic life support (BLS) are high among the medical students, the accurate knowledge required in performing BLS is inadequate. This study also showed that the National medical commission has taken a positive step in the incorporation of BLS in the curriculum.

BACKGROUND: Maternal health is an important aspect for the development of any country in terms of increasing equity and reducing poverty. For reduction of maternal mortality and morbidity Government of India launched National health mission (NHM). Some of the major initiatives under NHM are– ASHA (Accredited Social Health Activist), JSY/JSSK (Janani Suraksha Yojana / Janani Shishu Suraksha Karyakaram), National mobile medical units, National ambulance services. METHODS: A retrospective observational study was performed in department of Obstetrics and Gynaecology, G.S.V.M Medical College, Kanpur, Uttar Pradesh which is a low performing state, during the period of 2018 to 2019. A total of 5134 obstetric patients including 1032 low general condition patients were included over duration of 20 months since January 2018 to august 2019. This observational study collected information regarding number of deliveries in different years in this tertiary care hospital, effect on availability of health facilities and antenatal care to the antenatal women after introduction of JSY, ASHA and ambulance services. CONCLUSIONS: After JSY there is major difference in OPD admission and institutional delivery rate. Ambulance services had major contributory role in reducing maternal mortality and morbidity. There is need to improve performance of ASHA by proper supervision and monitoring.

Background: Nucleic acid amplification test (NAAT) is considered gold standard in the molecular diagnosis of CoV-2 infection but since it is costly, labor intensive and needs technical expertise, rapid chromatographic immunoassay for the qualitative detection of specific antigens to SARS CoV-2 have been devised. Objectives of this study was to compare the results of Antigen test and NAAT for CoV-2 infection carried out during the months of July and August 2020 by single tertiary care hospital in Lucknow, Uttar Pradesh and to determine the utility of rapid antigen test in the SARS CoV-2 diagnosis.Methods: All the patients who came to our hospital seeking admission during July 2020 and August 2020 were included in the study. A total of 1000 patients were included in this study.Results: Out of a total 1000 cases which were included in the study, 769 cases (76.9%) were found to be SARS CoV-2 negative by both antigen and CBNAAT, 100 cases (10.0%) were SARS CoV-2 positive by both antigen and CBNAAT tests. But in 131 cases (13.1%), antigen was not able to pick up the disease. It was also found that the Cycle Threshold (Ct) value for the discordant group was higher (Mean E= 28, Mean N2=33) when compared to the group where antigen was positive.Conclusions: The present study establishes the role of rapid antigen tests in contributing to the quick, point of care diagnosis of SARS CoV-2. These assays are safe, simple, and fast and can be used in local clinics and hospitals. These tests are very important for real-time patient management and infection control decision.


ABSTRACT Blindness is one of the significant social problems in India with 7 million of the total 45 million blind people in the world residing in our country. Major barriers to cataract surgery are poverty, no transportation, need not felt, and sex related. The low literacy rate among females and poor accessibility of the surgical sites were identified as important barriers in rural areas. There was also association found between socioeconomic status and cataract among cataract patients. In this study, a total of 208 participants who attended the ophthalmology outpatient department were studied for the observation on perception of cataract patient regarding cataract surgery.
