As government operations at all levels have become increasingly computerized, records of those activities have moved from paper to databases. Yet there has been little empirical research about the public’s ability to access such records in practice. This study uses field research to assess how 44 public universities respond to records requests of varying complexity for structured data. Sampled universities produced responsive structured data without a fee in slightly more than a quarter of requests, meaning the vast majority of requests failed to yield the information sought in a structured format and for free.

Abstract Objective: To analyze the prevalence of hypothermia in the first hour of life of preterm infants with birth weight 1,500 g or less. Method: A cross-sectional study performed in a Neonatal Intensive Care Unit. Data obtained from 359 computerized records of premature infants admitted between 2012 and 2016. Descriptive Statistics and Poisson Regression were used. Results: Premature infants (66.9%) presented hypothermia in the first hour of life, with axillary temperature of 36.2ºC (35.7-36.6), associated with: diagnosis of preeclampsia (p = 0.001), small for gestational age (p = 0.029), and the need for chest compression in the delivery room (p = 0.001). In cases of peri-intraventricular hemorrhage grade III (75%) and death (78.9%), there was a prevalence of premature infants with hypothermia in the first hour of life. Conclusion: Hypothermia in the first hour of life was prevalent in preter m infants, being associated with clinical complications. The prevention of hypothermia in the first hour of life is fundamental in the reduction of diseases related to prematurity.

<p><em>PT. CJ Feed Medan is a engaged in the animal feed industry. The purpose of this study is to determine the amount of GM-1C feed production from the forecasting of the number of sales of animal feed products of the previous type. And to plan raw material inventory for production of GM-1C animal feed with Material Requirement Planning method. Forecasting is a process to estimate the level of some future needs that include the needs in quantity size, quality, time and location required in order to meet the demand for goods or services. Material Requirement Planning is a logical procedure, decision rule and computerized records engineering designed to translate master production schedules or MPS (Master Production Scheduling) into Net Requirements for all items or components. From good sales data forecasting method is cyclical method because it has the smallest SEE value. So it can be estimated sales of the last 10 days in October 2017 as much as 70782, 70143, 69266, 68210, 67049, 65860, 64725, 63720, 62915 and 62365. The last process to get data on raw material needs is the making of MRP report. This report is a comprehensive report of the raw material requirements for GM-1C products.</em></p>

Abstract Objective: Although uncommon, Fusobacterium infections have a wide clinical spectrum, ranging from local pharyngeal infections to septic shock. Our aim was to characterize and analyze the clinical features and outcomes in patients with Fusobacterium infections, and determine which variables were able to predict a poor outcome. Methods: We conducted a retrospective, hospital-based study using the computerized records of a second-level Spanish general hospital, serving a population of 155,000 inhabitants. The cohort was enrolled among patients cared for at the hospital between 2007 and 2016. Demographic, clinical data, microbiological characterization and outcomes at discharge, were analyzed. Results: We collected data for all 26 patients over a 10-year period (annual incidence of 1.78 per 100,000), with an incidence of bacteremia of 0.53 cases per 100,000 population per year. F. nucleatum and F. necrophorum were the most frequent isolations (53.8% and 38.5%, respectively). F. necrophorum was found to be associated with a younger population. Although we found no deaths attributable to Fusobacterium, 15 patients (57%) were found to have severe infections due to this pathogen, and 7 patients (26.9%) were admitted to the Intensive Care Unit (ICU). The only identifiable risk factor for a severe infection (sepsis, septic shock or ICU admission) was the presence of bacteremia. Conclusions: Fusobacterium infections are uncommon. F. necrophorum tends to cause infection in younger individuals, while F. nucleatum has a preference for older patients. The clinical spectrum is wide, ranging from local, non-severe infections, such as sinusitis or pharyngitis, to abscess formation and life-threatening infections.

Introduction: In Portugal, 9.8% of patients admitted were inflicted with healthcare associated infections, corresponding to a prevalence of 11.7%. The Hospital de São João has developed a business intelligence platform able to supervise (the patients), monitor (the clinical condition) and notify (the healthcare personnel): HViTAL. This study aims to assess the impact of electronic monitoring on healthcare associated infections since the year of HViTAL implementation.Material and Methods: We evaluated data since January 2008 (moment from which computerized records exist) until December 2011, comparing them with subsequent data, those corresponding to January 2012 (implementation date of HViTAL) until 19 October 2015.Results: There was an upward trend of infection parameters in the 2008 - 2011 period. Since January 2012 and October 2015, all parameters of the infection indicator showed a negative linear trend.Discussion: The results are very suggestive that the HVITAL may have had an impact on improving parameters associated tohealthcare associated infections. Basic measures of infection control were highlighted since 2005, with an increasing number of health professional awareness campaigns, a fact which, although not analyzed in this report, may also have contributed to the observed improvement. Our study did not include other variables such as investment in human capital.Conclusion: There was a clear improvement in all areas characterizing the healthcare associated infections, with obvious positive impact with the introduction of HViTAL.

Aim. Antenatal glucocorticoid therapy (AGT) has been commonly used recently. However, this therapy has severe harmful effects such as maternal hyperglycemia. In Japan, ritodrine hydrochloride has been used as a tocolytic agent. In this study, we performed retrospective casecontrol study to clarify whether concomitant use of ritodrine and glucocorticoid was safe to pregnant women without diabetes mellitus. Methods. We reviewed the computerized records of pregnant women with pregestational diabetes and nondiabetes who gave birth at our hospital between 2002 and 2011. Cases and controls received AGT. Blood glucose after the therapy was analyzed, and additional volume of insulin was compared to that before the therapy. Results. From this study, 30 units of insulin were necessary when performing AGT in diabetic pregnant women. And also, an increase in blood glucose of 40 mg/dL was seen after the therapy even in nondiabetic pregnant women. Blood glucose increased significantly in the group that also received ritodrine, and it was shown that the number of pregnant women who might develop ketoacidosis might increase 11-fold. Conclusions. Ritodrine should be carefully administered during antenatal glucocorticoid therapy. It may be necessary to adequately monitor blood glucose, when performing the therapy, even in nondiabetic pregnant women.
