In 2016, J. D. Vance, a previously unknown author, surprisingly hit the bestseller’s list with his memoir, Hillbilly Elegy: A Memoir of a Family and Culture in Crisis. However, historically scholars have pushed back against the stereotypes that Vance portrays and have argued for a narrative that embraces the diversity of the Appalachian region. Using content analysis, this research study investigated the frequency and context of key terms related to Appalachia across the K-12 social studies standards of states labeled as being part of the region by the Appalachian Regional Commission (ARC). Results illustrate the lack of inclusion of these terms and that, when included, they do not represent the rich history or culture of the region. These results suggest that researchers, teacher educators, and teachers should consider the ways in which standards define curriculum on topics such as Appalachia and how these definitions interact with the powerful narratives being portrayed within our mainstream democracy. Guidance and suggestions for disrupting Appalachian stereotypes within social studies education are provided.

Red spruce (Picea rubens) was historically an important and dominant timber species in the central Appalachian mountain range. The tree species is now found in a small fraction of its original home range. Threatened and endangered organisms such as the Cheat Mountain Salamander (Plethodon nettingi) rely on red spruce associated forests for survival. This review provides a background on the history of forest management of red spruce in the central Appalachian region. A meta-analysis was conducted on recent literature (published 2000 or later) of red spruce in the central Appalachian region to highlight key management and conservation concerns. In particular, forest health concerns related to air pollution and climatic stress also are addressed. Approaches to examine the impact of environmental factors on red spruce site productivity are covered. This review also provides sustainable management options for restoration of red spruce in the central Appalachian mountain range.

PURPOSE With the introduction of precision medicine, treatment options for non–small-cell lung cancer have improved dramatically; however, underutilization, especially in disadvantaged patients, like those living in rural Appalachian regions, is associated with poorer survival. Molecular tumor boards (MTBs) represent a strategy to increase precision medicine use. UK HealthCare at the University of Kentucky (UK) implemented a statewide MTB in January 2017. We wanted to test the impact of UK MTB review on overall survival in Appalachian and other regions in Kentucky. METHODS We performed a case-control study of Kentucky patients newly diagnosed with non–small-cell lung cancer between 2017 and 2019. Cases were reviewed by the UK MTB and were compared with controls without UK MTB review. Controls were identified from the Kentucky Cancer Registry and propensity-matched to cases. The primary end point was the association between MTB review and overall patient survival. RESULTS Overall, 956 patients were included, with 343 (39%) residing in an Appalachian region. Seventy-seven (8.1%) were reviewed by the MTB and classified as cases. Cox regression analysis showed that poorer survival outcome was associated with lack of MTB review (hazard ratio [HR] = 8.61; 95% CI, 3.83 to 19.31; P < .0001) and living in an Appalachian region (hazard ratio = 1.43; 95% CI, 1.17 to 1.75; P = .004). Among individuals with MTB review, survival outcomes were similar regardless of whether they lived in Appalachia or other parts of Kentucky. CONCLUSION MTB review is an independent positive predictor of overall survival regardless of residence location. MTBs may help overcome some health disparities for disadvantaged populations.

This study analyzes land-cover transitions in the headwaters of the Big Coal River in the Central Appalachian Region of the US, from 1976 to 2016, where surface mining was found as the major driver of landscape change. The land-change analysis combined Multi-Level Intensity Analysis for two-time intervals (1976–1996, 1996–2016) with Difference Components, to differentiate suspected misclassification errors from actual changes. Two land cover classifications were obtained with segmentation analysis and machine learning algorithms from historical high-resolution aerial images and ancillary data. Intensity Analysis allowed for the inspection of transitions across five land cover (LC) classes and measure the degree of non-stationarity of land change patterns. Results found surface mining-related classes and their transitions, including the effects of reclamation processes on areas mined before the enactment of the Surface Mining Control and Reclamation Act (SMCRA, 1977). Results included changes in settlement distribution, low vegetation, water bodies, and forest class transitions. The findings can be applied to infer similar land-change processes in the more extensive Appalachian region where Mountain Top Removal (MTR) operations are widespread. The overall method can be used to address similar problems and inform landscape managers with detailed data to support land use alternatives and conservation in regions that experienced intense changes and are characterized by anthropogenic disturbances and novel ecosystems.

Objectives Electronic cigarette (e-cigarette) use has increased steadily in the United States, but little research has examined its prevalence in states comprising Appalachia, a rural region known for high rates of tobacco use. This study assessed lifetime and current e-cigarette use among adults by sociodemographic characteristics, geographic region, and cigarette smoking in Kentucky, with a focus on the Appalachian region. Methods We used data from the 2016-2017 Behavioral Risk Factor Surveillance System (BRFSS) surveys to calculate the prevalence of lifetime and current e-cigarette use, and we used weighted multivariable logistic regression analyses to examine the relative influence of other factors. Results Among adults in Kentucky, 5.8% (95% CI, 5.2%-6.4%) were current e-cigarette users and 27.0% (95% CI, 25.9%-28.0%) were lifetime users, compared with state medians of 4.6% (95% CI, 4.0%-5.1%) and 21.4% (95% CI, 19.4%-23.5%) for the United States. Multivariable regression models showed similar patterns for all regions: higher prevalence odds of current e-cigarette use among adults aged 18-24, current conventional smokers, and adults unable to work. Generally, Appalachian residents of Kentucky did not have significantly higher rates of lifetime or current e-cigarette use as compared with other non-Appalachian residents of Kentucky. Hispanic residents of Appalachian Kentucky, however, had higher rates of e-cigarette use than Hispanic residents of other regions of Kentucky. Conclusions Rates of e-cigarette use were higher in Kentucky than in the United States but were not further elevated in Kentucky’s Appalachian region. High rates of e-cigarette use among Hispanic residents of Appalachia indicate a need to focus future interventions in the region.

While land reclamation efforts of surface mines have considerably increased soil stability since the implementation of SMCRA (Surface Mining Control and Reclamation Act), research suggests that resulting soil compaction hinders the productivity of forests post-mining. The Forestry Reclamation Approach (FRA) was developed to improve forest health in the Appalachian region through a five-step process that minimizes soil compaction and establishes a productive forest. The FRA has not yet been tested in the western Gulf Coastal Plain (GCP). The higher clay content of some GCP soils and the dearth of coarse fragments (e.g., cobbles, stones and boulders) may affect reclamation practices and the ability of these methods to create productive forests. Compaction caused by conventional reclamation methods in the GCP has not been studied in great detail. Thus, this study attempts to provide a comparison of two reclamation methods, FRA low-compaction method used in the Appalachian region with that of conventional scraper-pan (scraper) methods in the GCP. This study used the FRA with common silvicultural practices of the western Gulf. The two hectare study site was installed with a randomized complete block design with three replicates comparing conventional scraper reclamation used in the region with that of an unmined control and the FRA-style low compaction treatment. Following soil reclamation, containerized loblolly pine (Pinus taeda L.) seedlings of a western Gulf provenance were hand-planted. Soil chemical and physical parameters were assessed on each treatment to determine the effect the FRA and scraper method had on resulting tree seedling growth and survival. After three growing seasons, seedlings in the FRA plots had significantly greater tree volumes than both the scraper (p = 0.0139) and the control (p = 0.0247) treatments. The FRA plots also had a 97% survival rate, while scraper plots had a survival of 86%. The FRA plots had significantly lower soil bulk densities than scraper (p = 0.0353) and control (p < 0.0001) plots which likely influenced growth and survival trends. Soil nutrients were increasingly available on the FRA and scraper plots, likely due to mixing of the soil profile when compared to the unmined control. Leaf-level water potential and gas exchange were not correlated to growth and survival and did not differ among treatments. These results suggest reclamation practices modeled after FRA methods may benefit tree growth and survival in the Western Gulf.

Summary Consensus does not exist for which cost forms (i.e., one accounting solely for explicit cost and the other for both explicit and opportunity costs as in relative opportunity cost) are used in calculating return on investment (ROI) for conservation-related decisions. This research examines how the cost of conservation investment with and without inclusion of the opportunity cost of the protected area results in different solutions in a multi-objective optimization framework at the county level in the Central and Southern Appalachian Region of the USA. We maximize rates of ROI of both forest-dependent biodiversity and economic impact generated by forest-based payments for ecosystem services. We find that the conservation budget is optimally distributed more narrowly among counties that are more likely to be rural when the investment cost measure is relative opportunity cost than when it is explicit cost. We also find that the sacrifice in forest-dependent biodiversity per unit increase in economic impact is higher when investment cost is measured by relative opportunity cost rather than when measured by explicit cost. By understanding the consequences of using one cost measure over the other, a conservation agency can decide on which cost measure is more appropriate for informing the agency’s decision-making process.

Abstract This article documents how the hardwood industry, in the absence of a standardized, industry-wide log grading system, has gone about grading and scaling hardwood logs by surveying mills, primarily in the Appalachian region. In total, 135 surveys were completed by respondents and returned via mail, with only 110 of those surveys considered usable for further analysis, after a thorough review of the individual surveys. Survey responses were grouped around annual production level, with three defined levels; ≤2.5 million board feet (MMBF), &gt;2.5 and ≤8.0 MMBF, and &gt;8.0 MMBF. Responding mills used some variation of a log grading system based on the number of clear faces on the log and the small end diameter of the log. The most common log rule used by mills in this study was the Doyle log rule, with over 75 percent using Doyle for scaling logs. Nearly 90 percent of all mills sampled graded logs without rolling the log to examine all four sides/faces. Half of all the sawmills surveyed pay the same price per thousand board feet (MBF) for butt logs and uppers. When asked if they would support the development of a standard log grading system, about two-thirds of the respondents (66%) indicated they would indeed support a standardized system for Appalachian hardwoods. These findings can help guide the development of a set of log grading standards for buyers and sellers in the Appalachian region and other parts of the world where hardwood lumber is produced.
