Objective — to study the modern classification, mechanisms of action and clinical effects of vitamin A derivatives, to analyze retinoid for local treatment of various dermatoses with a universal mechanism of action at the epidermis and dermis levels. Materials and methods. A review of the literature and an analysis of the results of international clinical trials of drugs based on the natural retinoid of the first generation — tretinoin (retinoic acid) is presented. The works of dozens of authors since 1980s to the present day are analyzed. Most sources provide detailed information on the results of topical retinoids in acne therapy, which are the base of clinical guidelines. Long-term (6 months or more) studies of retinoic acid-based preparations carried out in recent decades have discovered the unique clinical effects of tretinoin in the treatment of skin photoaging, actinic keratosis, etc. They are achieved due to the effect of tretinoid on the nuclear receptors of keratinocytes and fibroblasts. Results and discussion. The molecular mechanisms of action of retinoic acid, realizing the cellular and tissue effects of the most studied retinoid, are systematized and grouped in a single review. It has been proven that a unique feature of tretinin is its ability to activate directly all subtypes of RARs- and, indirectly, RARs-nuclear receptors of skin cells. A new modern drug for external use is presented — AltrenoТМ lotion containing micronized 0.05 % tretinoin in combination with sodium hyaluronate, soluble collagen and glycerin. This combination exhibits the expected clinical efficacy in acne therapy and prevents side effects such as dryness, redness and exfoliation. AltrenoТМ is approved for use in children of 9 years of age and older. Conclusions. Tretinoin (retinoic acid) is a modern powerful retinoid with a universal mechanism of action, recommended for the treatment of acne.

Acne vulgaris is a common condition that affects both men and women of all age groups. As a result of the outbreak of the COVID-19 pandemic and the introduction of restrictions, mainly the obligation to cover the nose and mouth with protective masks, a large percentage of the population is now struggling with acne. The aim of the research was to determine the opinion on the impact of wearing protective masks on the deterioration of the skin condition and the awareness of the respondents about the treatments and preparations used in acne therapy. The article also attempted to explain the causes of acne lesions.

Background: Acne vulgaris (AV) is an inflammatory disease of the pilosebaceous unit, which often causes stress and disrupts quality of life. Correctly identifying precipitating factors and preventing disease development will holistically increase the effectiveness of acne therapy, enabling better therapeutic outcomes to be achieved. Purpose: The objective of this study is to identify the clinical profile and precipitating factors of AV patients who visited Dr. Saiful Anwar Hospital between 2015 until 2017. Methods: This was a descriptive study, using a retrospective case study approach. The sample data was collected from medical records of AV patients from the period of 2015–2017 who met the following inclusion criteria: new AV cases with a complete medical record including gender, age, and precipitating factors. The dependent variable of this study was new AV patients, while the independent variables were the patient clinical profile and precipitating factors. Results: This study included 930 samples, with the number of female patients significantly higher (75.48%) than the number of males. The highest AV prevalence was found in the 15–24 years age group. The most common precipitating factors in females were hormonal issues, stress, and cosmetics; in males, these were stress, genetics, and dietary factors. Conclusion: The incidence of AV at Dr. Saiful Anwar Hospital mostly occurs in the 15–24 years age group, with varying trigger factors. The most frequent AV trigger factor in female patients was hormonal issues; in males, this was stress.

The aim of the study was to show the effectiveness of treatment of acne with moderate severity in women with ovarian hyperandrogenism. A total of 25 female reproductive voerast patients with moderate acne and ovarian hyperandrogenism who were not planning pregnancy were examined. All patients were examined, which included a consultation with a gynecologist with a gynecological examination, ultrasound examination of the pelvic organs, a study of the hormonal status, a biochemical blood test, as well as a dermatological examination with the determination of the dermatological index of acne (DIA). After the examination, the patients underwent complex therapy using the external treatment recommended by the dermatovenerologist (skin cleansing, gel with azelaic acid and / or gel with adapalene), as well as the appointment of a combined oral contraceptive (COC) Jes Plus. Based on this study, it can be concluded that complex therapy, including local treatment and taking Jes Plus, is the first choice in the treatment of androgen-dependent dermopathy, manifested by moderate acne, in patients with ovarian hyperandrogenism due to PCOS. Such therapy contributes not only to the formation of a pronounced clinical result, but also causes a low probability of side effects. The administration of COC containing drospirenon and the active form of folate, in the form of the calcium salt levomefolate, allows a favorable effect on the metabolic processes in the body as a whole, and in particular, provides a positive effect on the skin, the cardiovascular system, reduces the risk of systemic and local inflammation, relieving the state of hyperhomocysteinemia, providing prevention of cardiometabolic risks.

Abstract Background Acne is a common disorder of the pilosebaceous follicle. The face, back and chest are usually involved. It leads to significant diminution in quality of life. Numerous treatments are documented in therapeutic guidelines. Naturopathic approaches have been proposed in some, but the role of homeopathy is not examined. Methods In this study, 83 patients treated for acne with individualised homeopathic medicine alone were reviewed. Most had received conventional acne treatment, with limited success prior to presentation for homeopathy. Each patient was prescribed a single homeopathic medicine and followed up at 6- to 8-week intervals. The individualisation process resulted in 17 different medicines being used in this group. Photographic documentation was obtained per patient, with informed consent. Patients were classified as mild (comedonal acne with no papules or pustules), moderate (inflammatory and non-inflammatory lesions) and severe (predominantly inflammatory lesions: pustules, cysts, nodules). Results of treatment were recorded as remission (decrease in new lesion number, duration and intensity), failure to respond, and lost to follow-up (LTF). Results The average age of patients was 21.5 years (range 11–45 years). The F:M ratio was 55 (66.3%):28 (33.7%). Average pre-treatment duration was 5.5 years (0.25–22 years). Seven (8.4%) patients had mild acne, 37 (44.6%) moderate, and 39 (47%) severe acne. There were 13 (15.7%) LTFs, two (2.4%) failed to respond, and 68 (81.9%) went into remission. Average time to remission was 1.9 months (range 1.5–6 months), with no relapses or side-effects. The most commonly prescribed medicines were Lycopodium (38.6%), Palladium (15.7%) and Platinum (12.1%). Conclusion Individualised homeopathy may be useful for acne therapy. The most useful medicines appeared to be Lycopodium, Palladium and Platinum, though 17 different medicines were used in this study, underscoring the value of individualisation of therapy, a key characteristic of homeopathy.

BACKGROUND: Acne vulgaris (acne vulgaris) is an urgent interdisciplinary problem. Analyzing modern scientific data on the development of acne vulgaris, it is necessary to focus on the role of inflammation that accompanies dermatosis at all its stages of development, sometimes preceding the formation of clinical symptoms. Prolonged chronic recurrent course, difficulties in choosing the correct therapy tactics, sometimes unreasonable medical interventions, exacerbation, improper skin care lead to the formation of a number of complications. AIMS: To optimize and improve the effectiveness of therapy in patients with moderate and severe degrees acne using isotretinoin and the combined innovative gel Contractubex Anti-Red. MATERIALS AND METHODS: The study included 60 patients aged 18 to 45 years with moderate to severe acne. Isotretinoin was prescribed in standard dosages. To study the effect of Сontraktubex Anti-Red on the dynamics of post-acne, patients with atrophic post-acne scars were divided into 3 subgroups, depending on the regimen of drug use. RESULTS: Almost complete relief of the inflammatory process, regression of elements was noted 12 months after the use of complex therapy, the value of the dermanologic index of acne (DIA) index was more than 70% noted in 62% of patients, significant improvement in 38%. The severity of post-acne scars on the SCAR-S scale changed significantly during treatment and in 53.3% this indicator was 1. The Cadi index decreased by 70% compared to the baseline. CONCLUSION: Good cosmetic effect, safety, absence of side effects, comfort and ease of use make it possible to recommend Contractubex Anti-Red together with basic acne therapy.

The аim of research – the clinical results of complex therapy in patients with acne vulgaris- residents of the iodine deficiency region should be determined and analyzed consideringthe state of their pituitary-thyroid system, intestinal microbiota and immunologicalparameters.Material and methods. The study involved 109 patients with moderate to severe acnevulgaris, who are the residents of Chernivtsi region - a region with natural iodinedeficiency. Examination of the patients revealed changes in the pituitary-thyroid system,immunological parameters and intestinal biocenosis, which corresponded to the severityof dermatosis. During treatment, patients were divided into 3 groups: I comparative - 36people who received conventional acne therapies and medicines to normalize thyroidhomeostasis; group II comparative - 36 patients who were additionally prescribedprobiotics, and ІІІ (main) - 37 persons who received complex treatment with the additionaladministration of probiotics and immunotropic drug. The effectiveness of treatment ofpatients was evaluated by their earliest and long-term clinical results using statisticalanalysis methods.Results. It has been established that complex therapy of patients with moderate and severeacne vulgaris - residents of the iodine deficiency region with the administration alongsidewith the conventional acne therapy of medicines that normalize detected changes in thepituitary-thyroid system, immunotropic drug glucosaminylmuramyl dipeptide (licopid))and probiotic (lacium) reliably reduces (p <0.01) in comparison with conventional acnetherapy or conventional acne therapy with additional probiotics administration the termsof regress of acute inflammatory elements of acne with the achievement of the state ofclinical remission of dermatosis, (respectively: by 1,54 and 1,41 times). The frequency ofexacerbations acne process of patients with moderate and severe acne vulgaris duringthe year were also reduced (respectively: 1.59 and 1.26 times) relatively to patients in thecomparison groups.Conclusion. The complex therapy of patients with moderate to severe acne - residentsof the iodine deficiency region with administration of medicines to normalize thyroidhomeostasis, immunotropic drug glucosaminylmuramyl dipeptide (licopid) and probiotic(lacium) reliable improves the earliest and long-term clinical results of their treatment incomparison with conventional treatment or treatment using only the probiotic additionalprescription.
