With the rapid increase in computation-intensive tasks, the current research task is to minimize energy consumption due to resource constraints and increased cost. For complex computations where multiple computer systems are required to execute a single task such as in a federated cloudlet environment, load balancing is the main challenge. Load balancing means dividing the total workload between all the present nodes to obtain the maximum benefits from the available resources and to minimize energy consumption. A cloudlet is a resourceful computer that is coupled to the Internet and is accessible for mobile devices in their vicinity. A Cloudlet Federation is the concept of a cooperative framework to share resources and load balancing among various cloudlets. Different tasks consume different amounts of energy for their execution, which results in a large amount of heat dissipation. Due to heat, the performance of the systems is decreased. The more heat is present, the more the performance degrades. To address this problem, this paper proposes a novel scheduling strategy that will assign incoming tasks to systems according to their energy consumption level. The proposed methodology is tested in a Cloudlet Federation environment and the results show improved load balancing in terms of energy consumption and heat dissipation.

The formalism used for barriers calculations is based on a conservative estimation of workload, use factor, and occupancy factor. IMRT techniques (Intensity Modulated Radiation Therapy) and VMAT (Volumetric Modulated Arc Therapy) are known for being superior to conventional techniques, but costly from the shielding standpoint, as they increase the number of monitor units used to deliver the same dose to the patient, increasing the leakage radiation produced and, consequently, the thickness of the secondary barriers. At InRad (Radiology Institute of HC-FMUSP) a 2100CD LINAC already installed was upgraded to perform IMRT/VMAT techniques, and the existing barrier was reassessed. The present study proposes a methodology for acquiring real workload data from the institution's management software (MOSAIQ®) to replace the initially estimated data and recalculate the thickness of the barriers, assessing the impact of the introduction of these techniques and understanding the profile of the treatments carried out at the institution over the years of 2010 to 2020. Through this methodology, a decrease in the workload of 15 MV was observed as the technique of modulated intensity with 6 MV was introduced, reducing the thicknesses calculated for primary barriers. However, no significant changes were observed in the thicknesses calculated for the secondary barriers, because despite the increase in the leakage workload of 6 MV, the total workload of 15 MV decreased. There was also a trend towards an increase in the number of patients treated with modulated intensity year after year, which went from 5% in 2016 to 67% in 2020.

An unequal division of housework has been found to be often regarded as fair, which may explain why women still do most household labor. This study extends previous research by also investigating childcare—an increasingly important part of household labor, which is likely to have a different meaning than housework. It examines how perceptions of fairness for both housework and childcare are influenced by the division of housework, childcare, and paid labor, and whether patterns differ by gender. Data from the Netherlands (men: N = 462; women: N = 638) show that unequal divisions of housework, and especially childcare, are often perceived as fair. When it comes to how an increasingly unequal household labor division is related to unfairness, associations are stronger for women than for men. Fairness of the household labor division is evaluated in relation to total workload and not in isolation from other types of labor.

Background Self-administered massage interventions with a roller massager are commonly used as part of warm-ups and post-workout recovery routines. There is yet no clear consensus regarding the practical guidelines for efficient embedded interventions. Objectives The present randomized crossover pilot trial aimed at examining the effects of a rolling intervention with a roller massager embedded within the rests periods of a resistance training protocol. The rolling intervention targeted quadriceps muscles. Setting Participants (n = 14) performed two resistance training protocols expected to elicit momentary muscle failure. The protocol consisted in 10 sets of 10 rest-pause repetitions of back squats, with a poundage set up at 50% of the maximal one-repetition. Two min were allocated to recovery between sets. During the recovery periods, participants completed a rolling routine with a roller massager for 60 s (Roller-massager), or underwent passive recovery (Control). The total workload, concentric power, thigh circumference rate of perceived exertion (RPE) and delayed onset of muscle soreness (DOMS) from 24 h to 120 h after completion of the protocol were the dependent variables. Results Roller-massager was associated with a reduction in total workload (-11.6%), concentric power (-5.1%) and an increase in perceived exertion compared to Control (p < 0.05). Roller-massager was also associated with reduced thigh circumference after the resistance training protocol, indicating reduced muscle swelling, and reduced DOMS 24 h to 120 h post-workout (p < 0.001). Conclusion These findings support that embedded rolling with a roller massager hinders performance and increases effort perception. Embedded interventions may not be suitable during conditioning periods designed to maximize training intensity.

Introduction Tele-exercise could represent an alternative for remote care in individuals with spinal cord injury at this time of the pandemic of coronavirus disease 2019. However, the differences regarding the training loads and implementation between synchronous and asynchronous types are not yet known. The purpose of this study was to compare the implementation and training load between synchronous and asynchronous tele-exercise programs in individuals with spinal cord injury. Methods Forty individuals with spinal cord injury were recruited and stratified into tetraplegia and paraplegia groups. All subjects performed 3 weeks of both the synchronous and asynchronous tele-exercise programs, after two weeks of familiarization with the exercises, remote connection tools and methods to record information. The primary outcomes were training load (average daily workload and average and total weekly training load) and implementation (adherence and successful exercise recording). Demographic characteristics were obtained from participants' electronic medical records. Results Weekly mean workload, total workload, adherence and successful exercise recording presented significantly higher values in the synchronous compared to asynchronous tele-exercises. Average daily workload did not present significant differences between the tele-exercises. Discussion The training load for each training session presented no differences between synchronous and asynchronous tele-exercises. Both adherence and successful data recording showed more favourable implementation values for synchronous training, thus allowing greater weekly training loads (total and average).

ObjectivesHealthcare professionals need to take into account their knowledge, skills and attitudes to develop a focused clinical question, perform an effective search of the literature, critically appraise the evidence, and apply to the clinical context and evaluate the effectiveness of the process. To date, there is a lack of consensus on evidence-based medicine (EBM) curriculum for undergraduate healthcare students in Brazil. The aim of this study was to develop a consensus on EBM curriculum contents for healthcare schools in Brazil considering expert opinion.DesignModified three-round Delphi methodology.SettingOnline survey.ParticipantsThe expert panel was composed of 40 healthcare professionals from different specialties. Most of the participants (n=24; 60%) were female with the age between 30 and 44 years. Participants were also experts in the field of epidemiology, biostatistics or public health. The mean experience of experts in teaching EBM was 9.5 years.Main outcome measuresAn online questionnaire consisting of 89 items related to EBM was sent to the experts. The experts ranked each item of EBM curriculum considering the importance of each item as omitted, mentioned, explained or practised. The last section of the questionnaire was composed of ‘additional content’ where the experts evaluated only if an item should be included or not, the form of offering the EBM contents and the total workload (in hours/semester). Open-ended questions were present in each section to give the opportunity to experts to insert suggestions. Items that reached values greater than or equal to 70% of agreement among experts was considered definitive for the curriculum. Items between 51% and 69% of agreement were included for the next round and those items with less than or equal to 50% of agreement were considered unnecessary and were excluded. In the third round, the EBM contents were classified according to the degree of consensus as follow: strong (≥70% of agreement), moderate (51%–69% of agreement) and weak (50% of agreement) based on the maximum consensus reached.ResultsOf the 89 initial contents, 32 (35.9%) reached a strong degree of consensus, 23 (25.8%) moderate degree of consensus, two (2.2%) weak degree of consensus and 35 items were not recommended (≤50% of agreement). The workload suggested by experts should be between 61 and 90 hour/semester and an EBM curriculum should be offered with epidemiology and biostatistics as prerequisites. Regarding the importance of each item, 29 (72.5%) should be explained and 25 (27.5%) should be practised with exercises.ConclusionsThe consensus on an EBM curriculum for Brazilian healthcare schools consists of 54 items. This EBM curriculum also presents the degree of consensus (strong, moderate and weak), the importance of each item (mentioned, explained and practised with exercises). A total workload of between 60 and 90 hours per semester was suggested and the EBM curriculum should be offered with epidemiology and biostatistics as prerequisites, but also EBM contents should be included within other disciplines throughout the entire undergraduate course.

The introduction of robotic neurorehabilitation among the most recent technologies in pediatrics represents a new opportunity to treat pediatric patients. This study aims at evaluating the response of physiotherapists, patients and their parents to this new technology. The study considered the outcomes of technological innovation in physiotherapists (perception of the workload, satisfaction), as well as that in patients and their parents (quality of life, expectations, satisfaction) by comparing the answers to subjective questionnaires of those who made use of the new technology with those who used the traditional therapy. A total of 12 workers, 46 patients and 47 parents were enrolled in the study. Significant differences were recorded in the total workload score of physiotherapists who use the robotic technology compared with the traditional therapy (p < 0.001). Patients reported a higher quality of life and satisfaction after the use of the robotic neurorehabilitation therapy. The parents of patients undergoing the robotic therapy have moderately higher expectations and satisfaction than those undergoing the traditional therapy. In this pilot study, the robotic neurorehabilitation technique involved a significant increase in the patients’ and parents’ expectations. As it frequently happens in the introduction of new technologies, physiotherapists perceived a greater workload. Further studies are needed to verify the results achieved.

Abstract Decommissioning of nuclear building structures usually leads to large amounts of low-level radioactive waste. Using a reliable method to determine the contamination depth, it is indispensable prior to the start of decontamination works and for minimising the radioactive waste volume and the total workload. This paper deals with the possibilities of determination of depth distribution of activity of 137Cs and depth of contamination. The method described in this paper is based on concrete core sampling and laboratory gamma-ray spectroscopy measurements.
