Health services for older people in the NHS have developed pragmatically, and reflect the nature of disease in later life and the need to agree objectives of care with patients. Although services are likely to be able to cope with the immediate future, the growth of the elderly population anticipated from 2030 calls for long–term planning and research. The issue of funding requires immediate political thought and action. Scientifically the focus needs to be on maximizing the efficiency of services by health services research and reducing the incidence of disability in later life through research on its biological and social determinants. Senescence is a progressive loss of adaptability due to an interaction between intrinsic (genetic) processes with extrinsic factors in environment and lifestyle. There are grounds for postulating that a policy of postponement of the onset of disability, by modifications of lifestyle and environment, could reduce the average duration of disability before death. The new political structures of Europe offer underexploited–unexploited opportunities for the necessary research.

In Vietnam, as in much of the world, the elderly population is growing rapidly. Two-thirds of Vietnam’s elderly live in rural areas, where there may be unique health profiles and needs. Furthermore, the sex ratio in the elderly population is highly skewed. The relationship between sex, health-seeking behavior, and health outcomes in elderly individuals residing in rural areas has not been well-characterized, and needs to be elucidated in order to better serve this population. This study sought to explore the associations of sex with health and health-seeking behavior among the elderly in Soc Son, a rural district of Hanoi. A cross-sectional design was employed; elderly women and men were surveyed across the domains of socioeconomic information, health status, and healthcare service utilization. We found differences between women and men in sociodemographic characteristics, quality of life, health outcomes, and patterns of health services utilization. Overall, women had poorer health and quality of life, but no difference was found between sexes regarding level of health services utilization. Future research should expand upon these findings and, moreover, characterize their underlying systemic, sociocultural, and psychological mechanisms, in order to help bring solutions to improve the health of elderly persons in rural parts of Vietnam.

In Europe, the share of people aged 65 years and above is expected to increase exponentially, and in 2050, for the first time in human history, the number of older people will be larger than the number of children under the age of 15. Aging is associated with increased vulnerability and dependence on medical care services. For these reasons, providing access to health services to people aged 65 years and over is crucial. From the perspective of social equity, everyone should have the opportunity to access healthcare services equally, but due to economic and geographical issues, achieving this level of equity is challenging. The aim of this study was to fill the gap between scientific and actual practices using an accessibility measure to evaluate urban accessibility to primary healthcare services and to support decision-makers to better allocate resources in local restructuring of welfare policies. The accessibility measure was designed considering both the land use and the transportation components, the local healthcare supply system, and a multimodal transportation network. The methodology was applied to the city of Naples considering Local Health Agency (ASL) healthcare services to the elderly. The ASL is the largest health agency in Europe, consisting of 17 buildings serving nearly 200,000 elderly. The results show that the entire elderly neighborhood population suffers from poor accessibility to primary health services, especially in the city suburbs, and that the methodology could be effectively applied to urban planning strategies to achieve a high quality of life for elderly people. Due to poor dialogue between the authorities, this could help to improve the decision-making process through the lens of social equity.

Background Population ageing is a common problem faced in many countries world-wide. Due to physiological and biochemical changes in the elderly, increased incidence of diseases is observed. There is often low use of health services by the elderly for a variety of reasons. Objective To examine the status of health care utilization and to determine the factors associated with utilization of health care among the elderly population of Dhulikhel Municipality. Methods A quantitative descriptive-cross sectional study, with a total number of 200 elderly people residing in Dhulikhel Municipality, was selected for the study. Data were collected across the months of June - July 2011 applying two-staged cluster and systematic random sampling method. Both descriptive and inferential statistics were measured. Results Sixty eight percent of the elderly visited medical personnel in the past year. Eight percent of them visited the emergency department where most of them reported with symptoms attributed to heart disease. Among 200 elderly, 12.5% of them were admitted to the hospital and 53.0% utilized diagnostic services. Age, marital status, activities of daily living, and regular medication showed significant association with health services utilization at 95% level of confidence (p >0.05). Conclusion Marital status, daily living habits, existence of chronic disease, and regular medication demonstrated significant association with the utilization of health care. Social support services, informal education, and awareness programs targeting the senior citizens and studies covering a diverse population are recommended. Purpose The purpose of this study is to investigate the status of perceived need, seeking behavior and utilization of health services in the elderly population of Sanandaj (west of Iran). Design/methodology/approach This is a cross-sectional study conducted on 800 elderly people in Sanandaj. Subjects were selected using multistage sampling and data were collected using self-report questionnaires. A multivariate logistic model with odds ratios (ORs) was used to determine the relationship of independent variables with seeking perceived need. Also, the concentration index was used to measure the inequality in using health services. Findings The perceived need for outpatient (during the last 30 days) and inpatient health-care services (during the past 12 months) was 69.7% and 29.7%, respectively. Among them, the unmet need for outpatient and inpatient health-care services was 46.6% and 17%, respectively. Having health insurance (adjusted OR 12.08; 95% confidence interval [CI] 1.04–140.11), middle economic status (adjusted OR 5.18; 95% CI 1.30–20.51) and being in an age group of 65–70 years (adjusted OR 7.60; CI 1.42–40.61) increased the chance of seeking inpatient care. Also, being in an age group of 60–65 years (adjusted OR 0.41; 95% CI 0.18–0.95) reduced the chance of seeking outpatient care. There was also a pro-rich inequality in using outpatient health services. Originality/value The elderly population suffers from unmet health-care needs, especially in outpatient services. The most important reason for not seeking outpatient and inpatient services was financial barriers and self-medication, respectively. So, designing targeted policies and interventions to address barriers in the conversion of need to demand in the elderly population is essential.

By 2050, the growth of the elderly population in Colombia is estimated at 10% and thus a greater demand for special services (such as health services) for the elderly. This justifies the exploration of digital health content as an important source of information for this population. The accessibility and usability guidelines for website design –e.g., TAW and WACG– do not have specific guidelines to mitigate the motor, cognitive or visual disabilities characteristic of aging, which become a barrier for this group to consult necessary information for administrative processes that involve health. This review of accessibility and usability guidelines is presented, facilitating the consumption of specific contents and generating better interactions with such systems, which will lead to the construction of guidelines based on existing recommendations that allow the development of aspects related to interaction, legibility and usability in digital content for the elderly.

ResumenObjetivo: Explorar las percepciones sobre salud bucal y género con relación a la población adulta mayor que consulta la red hospitalaria pública de Medellín desde la perspectiva de los prestadores de servicios de salud. Materiales y métodos: Estudio cualitativo con perspectiva etnográfica mediante 34 entrevistas semi-estructuradas en personal que presta servicios de salud en la Empresa Social del Estado -E.S.E- Metrosalud. Las entrevistas se transcribieron textualmente y se realizó análisis de contenido según categorías propias de los discursos de los participantes. Resultados: Se encontraron tres categorías principales: 1) Necesidades y problemas de salud general y bucal y sus determinantes; 2) Símbolos y significados de la población adulta mayor; 3) Perspectiva y roles de género y su relación con las prácticas sociales en salud bucal. En general, los prestadores perciben alta carga de enfermedad y barreras de acceso a los servicios de salud. Se percibieron diferencias de género relacionadas con los roles que asumen las personas, las prácticas de autocuidado, y los perfiles de utilización de los servicios de salud. Conclusiones: Los prestadores entrevistados perciben como la población adulta mayor que consulta a la red prestadora de servicios pública de Medellín se encuentra en situación de vulnerabilidad social que impacta en las condiciones de salud bucal. De igual manera, se perciben diferencias de género relacionadas. Se requieren políticas sociales que garanticen equidad para este grupo poblacional.AbstractObjective: To explore the perceptions about oral health and gender in relation to the elderly population assisted by the public hospital network of Medellin from the health personnel perspective. Materials and methods: A qualitative and ethnographic study was conducted through 34 semi-structured interviews in health personnel in the State Social Enterprise E.S.E Metrosalud. Interviews were transcribed and a content analysis was carried out according to categories from the participants’ discourses. Results: Three main categories were found: 1) Needs and problems of general and oral health and their determinants; 2) Symbols and significances of the elderly; 3) Perspectives and gender roles and its relationship with social practices on oral health. In general terms, participants perceived high disease burden and barriers of access to health services. In addition, gender differences were perceived related to the roles that people assume, self-care practices and health services utilization profiles. Conclusions: Interviewed providers perceived that the elderly population assisted by the public hospital network of Medellin is exposed to social vulnerability that affects oral health conditions. In addition, gender related differences were found. Social policies that ensure equity in this population group are required.
