Experiences from accidents show that the majority of critical injuries die within the first 1-4 h if they do not receive some appropriate medical treatment. The problems of an accident with many casualties is dealt with by: (a) maximum utilization of resources; and (b) sorting the tasks.By rapid and skillful evaluation, we may sort out the critical injuries and give priority for treatment and transport. Through experience we have learned the few and very simple principles of dealing with critical injuries: (a) rapid and sufficient first-aid at the scene, (b) rapid evacuation to a hospital, (c) efficient final treatment, and (d) the need for an effective system of communication between the scene, the ambulances and the hospital.The principle objective of medical first-aid on the scene of an accident is to stabilize the patient and to make him fit for transportation. Only thereafter is the most necessary temporary treatment carried out. The resuscitation must be continued during transportation. Qualified personnel with equipment must accompany critical injuries. During the last decades there has been a considerable development in the first-aid on the site. Each wounded individual must get the maximum benefit. This requires a high degree of field administration and co-ordination of medical activity on site. This may differ from country to country, but in general the principles will be the same.

In the context of the worldwide acute shortage of qualified personnel in healthcare organizations, one of the most relevant tools for attracting talented doctors to work is an employer brand. The authors analyzed the views of potential job seekers of a medical organization about an attractive, “ideal” employer. Basic research methods are a content analysis of scientific works and an online survey. Based on the content analysis of publications of two abstract databases – eLIBRARY and Web of Science, the work presents the dynamics of the authors’ activity over the past five years, the key focus of the work, and the representation of an employer brand in medical organizations. The analysis reveals that among the abundance and against the background of positive trends in the number of works, only four papers on the employer brand deals with the study of the context in health care organizations, which does not allow judging the disclosure of this topic and offers opportunities for further scientific research. The authors identified the main components of the concept of an employer brand followed by the world scientific community – the presence of a positive image; the set of benefits associated with working in a particular company; and a holistic employer image. These elements formed the basis of the survey. One hundred twenty-nine respondents took part in the empirical part of the study. The study results reflect key factors of the attractiveness of a medical organization as an employer – reputation, personnel care, the existence of a strong leader, competitive salary, possibility of development, and human relations. The paper presents the ideal employer characteristics, according to the respondents, stated in the authors’ concept.

Introduction: Few studies are available to appreciate the broad spectrum of dive-related injuries (DI), which are not limited to decompression illness (DCI) and fatalities. Studies supporting injury-management efficacy from early recognition to first-aid, final treatment and outcome are also lacking. This study aims at making an epidemiologic inventory of DI among French scuba divers. Methods: This online, retrospective, cross-sectional survey analyzed self-reported symptoms, context of occurrence, initial response and outcome. The relationships between symptoms and diver characteristics were assessed and severity scores created from the reports. Results: A total of 799 divers responded, of whose questionnaires 784 were sufficiently complete to be analyzed. Approximately one-third (35%) of respondents had never experienced a DI. DCI-like symptoms represent a small fraction of DIs, the most commonly reported being ear barotrauma. Self-reported symptom rates decreased with increasing age and male sex. The ranking dive leader was the primary care provider in 58% of reports and 32% of injured divers never sought help. Management decisions (first aid and/or hyperbaric oxygen treatment) were related to the severity score. Complete resolution was achieved in 84 (74%) of 114 DCI cases, whilst mild (n = 22, 19%) and severe (n = 8, 7%) residual symptoms were reported. One in 10 divers who did not seek treatment for symptoms believed to be related to DCI declared some residual symptoms. Conclusion: Based on these results, diving injury rates may be higher than previously reported. However, the most frequent symptoms appear to be of only a modest nature.

SummaryThe fatigue life of a pin-jointed connection can be optimised by using a moderately high degree of interference between the loading pin and plate. Where a joint has to be assembled in confined conditions, difficulty may be experienced in inserting the interference-fit pin and one possible solution is to use a pre-assembled interference-fit bush in the plate, leaving only a light interference-fit pin to be pressed in on assembly of the joint. It is shown that a relatively thick bush of diametral ratio 4/3 will give a reduction in shear stress concentration factor for the plate comparable with that obtained with a solid pin, but that maximum benefit is not obtained with a thinner bush of diametral ratio 8/7. Where thin bushes are essential in order to maintain the ultimate tensile and fatigue strengths of the plate and /or the ultimate and fatigue strengths of the pin, the shear stress concentration factor for the plate is reduced as the modular ratio of bush to plate is increased and as the interference fit of the loading pin in the bush is increased.

The article addresses application of digital technologies in inspection activities. The article describes the scope of use of advanced technologies in practical inspection activities demonstrating a high degree of digitalization and wide range of inspection tasks. Opportunities, restrictions and risks of use of digital systems are analyzed. The article also clarifies unacceptability of “total digitalization” of inspection activities and infeasibility of inspectors’ replacement with digital systems without involvement of qualified personnel.

Providing quality pharmaceutical care is a key task of healthcare; any medical organization must respond efficiently to a change in both external and internal factors that affect the medical support of patients. The aim of this study was to analyze the structure of medication consumption in comparable hospitals in the penitentiary and civilian healthcare systems. Materials and methods. The study was conducted on the basis of content analysis, documentary observation, ABC analysis, and comparison of performance indicators of medical organizations. Results and discussion. This study analyzed the assortment of commodity stocks of medications in the regional clinical hospital that is a part of the structure of FSIN and a typical central regional hospital (CRH). Using ABC analysis it was established that the Group A (the most costly) medications in the penitentiary medical organization consisted exclusively of anti-viraldrugs used for treating HIV-infected patients, which is 54.91% of the stock that are represented by 18 international non-proprietary medication names. In turn, Group A of the medication assortment of the State Budgetary Healthcare Organization of the Republic of Adygea (GBUZ RA) “Krasnogvardeiskaia CRH” consists of three pharmacological groups – plasma substitutes, cardiovascular medications, and antibiotics. On the basis of the obtained data, the study identified different groups of medications and studied their assortment based on international non-proprietary namesand medication forms that received preference either in the branch of Clinical Hospital-2 (KB-2) FKUZ MSCh-23 FSIN of Russia or GBUZ RA “Krasnogvardeiskaia CRH” (38 ATC groups in all). The first group consists of medications from the following 12 ATC groups (31.6%), the assortment of which can be found exclusively in the branch of “KB-2” FKUZ MSCh-23 of the Federal Penitentiary Service of Russia: antiretroviral, muscle relaxants, anticonvulsants, adaptogens, antidepressants) or significantly exceeds similar stocks in GBUZ RA “Krasnogvardeiskaia CRH”: anti-asthmatic agents (3 times), anti-fungal drugs (3 times), hormones except for insulin (2.2 times), respiratory regulators (2 times), non-steroidal anti-inflammatory drugs (1.7 times), medications regulating the gastrointestinal function (1.5 times), and antibacterial agents (1.4 times). The second group consists of medications of the following 9 ATC groups (23.7%) that are stocked only by GBUZ RA “Krasnogvardeiskaia CRH”: antifermental drugs, radiocontrast agents, chondroprotective and antiparasitic medications, and -- within a wider diapason of non-proprietary drugs and forms of manufacture – diuretics (5 times), antitubercular (3 times), antiviral with the exception of anti-HIV (2 times), spasmolytics (2 times), and hypertension drugs (1.5 times). It was determined that the main medical activity of medical organizations subordinate to FSIN of Russia, is aimed at treatment and prevention of complications of the most socially dangerous diseases – HIV/AIDS and tuberculosis – as well as therapy of the associated diseases. It was shown that pharmacists do not use methods of pharmacological-economic analysis in their work, and the medication needs of medical organizations are only 20% satisfied. Conclusion. The identified shortcomings attest to the need to develop and implement a more effective system for the formation of stockpiles in the pharmacies of the penitentiarymedical organization.

Essence and features of the system of financial stability of enterprises of aviation industry of Ukraine is certain. The enterprises of the aviation industry of Ukraine occupy a special place in the development of the country's economy. Taking into account the peculiarities of the aviation industry, the financial stability of an enterprise is the ability of an aviation enterprise to function on the basis of effective management of financial resources through an adequate response to changes in the external and internal environment. The main conditions for ensuring the financial stability of aviation industry enterprises are: the provision of assets with a high degree of liquidity; predominance of own sources of property formation over borrowed and borrowed ones; continuity of business activities; legal capacity and legal capacity of a legal entity. The analysis of indexes of estimation of modern development of aviation industry of Ukraine status is carried out. Based on the study, we note that the aviation industry of Ukraine in 2021 is focused on the same problems as during the second half of 2020: managing daily financial losses, improving safety and offering attractive prices aimed at returning passenger demand, as causes the need to build a more effective system for assessing and managing the financial condition of enterprises in a particular industry. Separate conclusions are formed in relation to determination of separate terms of the use of indexes of financial stability taking into account the modern state of aviation industry of Ukraine. Presented recommendation in relation to the process of management financial stability on the enterprises of aviation industry of Ukraine taking into account the modern terms of menage.

The results of a prospective study of 66 patients aged 32,63,8 years infected with the human immunodeficiency virus who were treated in an anti-tuberculosis medical organization for tuberculosis meningoencephalitis are presented. Of these, 49 (74,2%) were newly diagnosed with tuberculosis, and 17 (25,8%) had relapses and chronic forms of tuberculosis. According to social characteristics, patients were distributed as follows: working - 9 (13,6%), non-working - 57 (86,4%), disabled people with tuberculosis - 4 (6,1%), persons who previously served their sentences in prison - 48 (72,7%). All the surveyed patients had bad habits: they continued to actively use psychoactive substances - 52 (78,8%) people, abused alcohol - 43 (65,2%), smoked - 60 (90,1%). In 97% of cases, tuberculosis of the meninges and Central nervous system was a complication of respiratory tuberculosis: tuberculosis of the intra-thoracic lymph nodes - 42,4%, disseminated pulmonary tuberculosis - 27,3%, infiltrative pulmonary tuberculosis - 22,7%, and caseous pneumonia- 7,6%. The most effective method for diagnosing the tuberculosis etiology of meningoencephalitis in patients infected with the human immunodeficiency virus is the study of cerebrospinal fluid using a polymerase chain reaction. Deoxyribonucleic acid of Mycobacterium tuberculosis was detected in 31 (47%) patients. The characteristics of the cerebrospinal fluid in the examined patients are presented as follows: a decrease in glucose in 45 (68,2%) patients, an increase in protein in 59 (89,4%), and cytosis at the upper limit of normal values in 10 (15,6%). High mortality rates (47,7%) in patients infected with the human immunodeficiency virus in combination with tuberculosis meningoencephalitis are associated with a high degree of immunosuppression.

This study aimed to evaluate the occluding/remineralization performance and resistance to acid attacks of the mineralization layer formed by a tooth-desensitizing gel containing amorphous calcium magnesium phosphate (ACMP) particles and compare it to six other desensitizing products available on the market. Similar comprehensive studies are few and there is especially a lack of studies that are up to date. A dentin-disc model was used for in vitro evaluation of the desensitizing toothpastes/gels. Application of the products was performed twice daily for seven days. One set of specimens were evaluated using scanning electron microscopy (SEM) directly after the final treatment and another set was evaluated after an acid challenge, exposing specimens to 2 wt% citric acid. The ACMP desensitizing gel was the only product resulting in complete occlusion by the formation of mineralized material on the dentin surface and inside the tubules. Particle deposition was dominant after treatment with the other desensitizing products, with little or no mineralization, resulting in partial occlusion only. Sensodyne Repair & Protect and Oral-B Pro-Expert showed the highest resistance toward acid attacks. Material inside the tubules remained relatively unaffected by acid attacks in all specimens. The results in this study indicated a great variability among the occluding agents in terms of occlusion and acid resistance of the mineralization layer. The high degree of occlusion and intra-tubular mineralization that could mitigate the effect of acid solubilization indicate that the ACMP desensitizing gel may be a superior option for the treatment of dentin hypersensitivity.
