Quality has an important role in business. A good quality means it is able to meet the required standards. The concept of quality must be comprehensive, whether the quality of the product or its process. PT Textile Yogyakarta is still having a high rate of defect products. The Waste Assessment Model (WAM) was used to identify the wastes that frequently occur by scoring each type of waste. The results show the waste rank are defect 26.12%, inventory 20.36%, motion 15.26%, transportation 14.51%, over production 12.91%, waiting 7.17%, and process 3.67%. While using the Six Sigma from the 4 CTQ process, the result of sigma level is 3.3. It means that the sigma level of PT Textile Yogyakarta is still under the average sigma level in Japan which is 5 Sigma. As for the improvement part, FMEA AHP was used and the results show that the first rank is the uncleaned woof residue, the second rank is the uncut thread which has an RPN value of 8.04 that was caused when the operator did not clean the machine from the woof residue, and the third rank is the malfunctioned machine which has an RPN value of 7.32 that was caused by broken kleting.

<em>CV Jaya Reksa Manggala (JRM) is one of clothing company. With increasing competition in this clothing company, the companies are required to keep improving the quality of clothing products. One of the important things in quality is reduction of product defects. One of the proven methods of  quality improvement is the Six Sigma DMAIC method, which is a continous improvement method that aims to achieve the best quality. One of the products of CV JRM which is still have a lot of defects is 514 products. From the initial data obtained, DPMO value for process I and II for 514 products is 10,768.52 and 27,341.42 with sigma level 3.79 and 3.43. The steps undertaken in this Six Sigma DMAIC method include define, measure, analyze, implementation and control. Several proposed improvements were then made to reduce the problem of these defects and from the data at the control stage, the DPMO values of processes I and II became 2,941.76 and 1,812.69 with sigma levels of 4.25 and 4.41. From this result it can be said that with Six Sigma DMAIC method, there is improvement of quality for product 514 at CV JRM. </em>

Competition in the corporate world these days of tighter encourage companies to further develop ideas for an effective and efficient in achieving the goals and objectives have been set. Label X is one of the products produced by PT. BPP. For blinding Label X companies also strive to continuously improve product quality by suppressing the number of defects in the production process.Six Sigma is a vision of improved quality to the target 3.4 failures per million opportunities for each transaction of goods and services. So Six Sigma is a method or technique of controlling and improving the quality of which is a dramatic breakthrough in the field of quality management. By using Six Sigma methods can be seen that the quality of label X produced by the company that is pretty good with the 2.79 sigma level of damage to 99.179 million production (DPMO). Implementation of six sigma quality improvement in this study can be concluded that there were three highest causes of product defects: color blur 82%, did not register 9% and cutting 9%.

This study focused on quality improvement on ceramic tiles production process at PT Arwana Citramulia. This study used data defects for 12 months (May 2016 – April 2017) and only focus on one type of defect and it is chop corner. Six sigma with DMAIC (define, measure, analyze, improve, and control) approach was used to improve the process. Each step of DMAIC was conducted to carefully analyze and keep the process precisely. The ceramic tiles production process contains a number of 4375 products defects in million opportunities (DPMO), with sigma level of 4.13. In the improve step of DMAIC, FMEA form was used to propose some recommendations in order to improve the process, some of that that are provision of lubricant periodically by the operator, polishing on the surface of the liner to clean and clear, examination and maintenance periodically. Background: Little has been done regarding the research on quality and quantity of patient support groups (PSGs) and how they can be improved. Here, we present three-year experiences of a quality improvement (QI) program of our PSGs. Methods: We launched earlier on a three-year project to improve our PSGs, including the number and quality of curricula. Data were collected on the number of PSGs, curricula, and participants. Results: In the first year, we organized relevant resources of our hospital and established a standard protocol for applying financial support and reporting the results. In the second year, we elected “the best patient” to promote sense of honor and better peer supports. In the third year, we surveyed through questionnaires participants’ health literacy to improve their feedback. Competitions and exhibitions of achievements were held each year to share results of every PSG. Finally, we had increased the volume of participation of patients and family over these three years (3968, 5401 (+35.5%) and 5963 (+50.3%)). Participation of staff also increased significantly (489 and 551 (+12.7%)). Furthermore, more interdisciplinary curricula were generated, with fewer doctors (38.2% to 29%), but greater numbers of the following: nurses (4.9% to 17.4%), nurse practitioners (0.4% to 14.5%), medical laboratory scientists (2.5% to 16.3%), social workers (4.7% to 41.7%), and teachers from outside (0% to 1.8%). Conclusion: In this first study on QI efforts on PSGs, we enlisted a core change team, drew a stakeholder map, and selected an improvement framework with good results.

This article deals about the application of the Six Sigma methodology by using modern methods in various stages of the DMAIC improvement model. The change management of business processes in terms of quality is determined by the correct choice of methods and tools for reducing discrepancies and non-productive costs. The evaluation of selected processes of furniture production through defects per million opportunities, efficiency, capability and sigma level before the change and after the change in the quality process provides relevant information for ensuring continuous quality improvement. Implementation of assessing changes in the process is a source of information for continuous improvement of process performance. Tatung Electric Indonesia is a peripatetic company in manufacturing industry assembling of electro motor. So that product yielded by PT. Tatung Electric Indonesia can continue to fulfill requirement of customer, company try to improve the quality of its product by minimizing defect which during the time still happened. This defect arising caused by the existence of variation at yielded product and also that happened in the process. Six Sigma represent control technique or method to reach storey or level quality of 3.4 DPMO ( Defects Per Million Opportunities), which applied by Motorola company since year 1986. Applying strategy to be used is Define-Measure-Analyze-Improve-Control (DMAIC). This Method represent recuring step and form a make-up of quality cycle. Calculations show the company is 3.641 sigma level with DPMO 15,830. The sigma levels are on the average level of sigma industries in Indonesia (3-4 sigma). Increased levels of sigma would happen if the company immediately continuous improvement over time. Conclusion of the application of Six Sigma is the DMAIC method in the assembly process of electro motors can analyze and reduce disability that occurs that will increase the quality of products.

PT. Z is a company engaged in the plastic bag industry. The company produces plastic bags, one of which is a type of ATP 12 x 24 that is often used as plastic wrapping meatballs, sugar and others. A company is certainly required to provide the best quality on its products. To meet the needs and expectations of consumers for the quality of products, the company must improve the quality of existing products. Six sigma is a quality tool that can be used to analyze and process data to maintain or improve the quality of a product based on the process. One of the shorts used in the six sigma method is Define, Measure, Analyze, Improve and Control ( DMAIC). This research focuses on the results of product defects produced for 5 months in 2019. Because of the process analyzed there is data of proportions that are out of control in july and november. So that the data is omitted so that the data can be controlled. The sigma production level of ATP 12 x 24 is currently at the level of 4 sigma so it is necessary to improve to reach the level of 6 sigma. Pareto diagrams and fishbone diagrams can be used as tools to analyze significant factors causing product defects. And it can be known that the causative factor that can be done repair is deflated.

Traditional cast brass handicrafts Majapahit is one of the nation's cultural heritage which has a high historical value. The uniqueness and quality become the hallmark of this craft products. The results of handmade products such as sculptures, wall hangings, souvenirs, ornaments doors and so on. To increase the sales of traditional handicrafts of brass castings, required Majapahit applicative technical efforts in improving the quality of products which are ecofriendly. Quality improvement method such as Six sigma. Six sigma focuses on decreasing flawed rate, by reaching the standard 3, 4 flawed in a thousand possibility. Six sigma has 5 phases; define, measure, analyze, improvement, and control. In this research, six sigma is applied in producing cast brass handicraft starting from making the master mold, preparation of smelting raw materials, cooling process, releasing of the mold, and finishing. Purposefully to improve the quality by overcoming and decreasing the high of flawed possibility and also applying marketing mixture in improving the market segmentation of traditional cast brass of Majapahit market. The result is quality improvement is shown by decreasing happen in number of flawed product in total of mean for about 154, 8 decreased to 59,5. While the calculation of the six sigma, flawed product of cast brass handicraft is for about 1,7625 sigma value or being in level 2 and increased to 3,725 or being in level 4 with the flawed possibility in amount 235,000 for thousand times of production. The ability of production for the requirement which is wanted by the consumers and owning a good enough capability but still needed some correction to achieve world class home industry

The aim of this study was to demonstrate the application of Statistics Quality Control in fierce industries. By observing Batik production process at PT XYZ as a case in this work, we applied Six Sigma Method to analyze defective product and their cause while also measure overall quality goodness. Six Sigma is a method to improve a process and reduce defects in productions into 3.4 defects per million productions. We use handprint batik productions at PT XYZ as a case in this study. The method is involving Define, Measure and Analyze (DMA) phases. By using Six Sigma, it was obtained that the quality of handprint batik are quite good with sigma of 3.105 and defect rate of 54.269 million production (DPMO). There are four defect causes of handprint batik namely: ripped fabric, shallowness, perforation, and mispattern which contribute 41,7%, 35,8%, 15%, 7,5% respectively from overall defects. The main cause of defects is the carelessness of workers in production process. Therefore we need to reduce the carelessness to  improve production quality.
