AbstractObjectiveTo assess the prevalence of anaemia in rural Vietnam and to determine its risk factors.DesignA cross-sectional survey.SettingVietnam, Nghe An Province.Study populationThe total number of participants was 439. Of these participants, one was excluded from the study due to a mental disorder. Forty-seven did not participate in the test for parasites and 68 did not complete at least one of the questions.ResultsThe prevalence of anaemia (haemoglobin (Hb) < 11.0 g dl−1) was 43.2% and of severe anaemia (Hb < 8.0 g dl−1) was 0.5%. Taking iron tablets, the consumption of eggs and the preference for Western medicine significantly and positively correlated with Hb concentration in the pregnant women in a multiple regression analysis. Pregnancy duration and hookworm infestation significantly and negatively correlated with Hb concentration in the pregnant women.ConclusionThe prevalence of anaemia in rural Vietnam has remained as high as that found in the national anaemia survey in 2000. The results of the present study could aid in the development of an iron-deficiency anaemia programme among pregnant women in rural Vietnam that emphasises iron supplementation, parasite control and improved diet, including the consumption of eggs. The programme's focus should be on women who prefer traditional medicine to Western medicine.

ObjectiveTo conduct a systematic review to investigate the safety of induction and/or augmentation of labour compared with spontaneous-onset normal labour among pregnant women with iron-deficiency anaemia.DesignSystematic review.SettingStudies from all countries, worldwide.PopulationPregnant women with iron-deficiency anaemia at labour and delivery.InterventionAny intervention related to induction and/or augmentation of labour.Outcome measuresPrimary: Postpartum haemorrhage (PPH), heart failure and maternal death. Secondary: Emergency caesarean section, hysterectomy, admission to intensive care unit.MethodWe searched 10 databases, including Medline and Embase, from database inception to 1 October 2018. We included all study designs except cross-sectional studies without a comparator group, case reports, case series, ecological studies, and expert opinion. The searches were conducted by a healthcare librarian and two authors independently screened and reviewed the studies. We used the Grading of Recommendations, Assessment, Development and Evaluations (GRADE) approachto ascertain risk of bias and conducted a narrative synthesis.ResultsWe identified 3217 journal articles, 223 conference papers, 45 dissertations and 218 registered trials. Ten articles were included for full-text review and only one was found to fulfil the eligibility criteria. This was a retrospective cohort study from India, which showed that pregnant women with moderate and severe anaemia could have an increased risk of PPH if they underwent induction and/or augmentation of labour, but the evidence was weak (graded as ‘high risk of bias’).ConclusionThe best approach is to prevent anaemia, but a large number of women in low-to-middle-income countries present with severe anaemia during labour. In such women, appropriate peripartum management could prevent complications and death. Our review showed that at present we do not know if induction and augmentation of labour is safe in pregnant women with iron-deficiency anaemia and further research is required.PROSPERO registration numberCRD42015032421.

Introduction: Iron Deficiency Anaemia (IDA) is the most widespread micronutrient deficiency. Globally, nearly two billion people are affected by anaemia. This disease most often affects children, women of child-bearing age, and pregnant women. Nearly half of the pregnant women in the world are estimated to be anaemic. Its prevalence varies according to region and socio-economic conditions. The majority of those who are anaemic live in developing countries where the problem is exacerbated by limited access to inadequate resources and appropriate treatment. Aim: To find out the prevalence of IDA in the reproductive age group women and its associated factors. Materials and Methods: This hospital based, cross-sectional study was conducted among 400 females, of age group 15-49 years. After obtaining the written informed consent, eligible subjects were inquired about socio-demographic variables, socio-economic status, anthropometric measurements and risk factors. Required investigations were done. All the information was recorded on the semi-structured schedule form. Chi-square/Fisher’s-exact test was used to find the significance of study parameters on categorical scale between two or more groups. Results: Prevalence of IDA was found to be 42% (168/400). Prevalence of IDA was significant with habitat (p=0.0180), exercise (p=0.0004), amount of blood loss during menstruation (p<0.0001), duration of flow during menstruation, (p=0.0020), consumption of fish (p=0.0002), consumption of legumes (p=0.002), consumption of green leafy vegetable (p<0.0001). Conclusion: Results confirmed an increased prevalence of IDA in women of reproductive age group.

AbstractObjective:To study the prevalence of anaemia and its association with measures of iron deficiency (ID) among a group of pregnant women.Design:Cross-sectional survey.Setting:Pregnant women identified through house-to-house visits and participating in community-based antenatal care activities in a rural location of Mymensingh, Bangladesh.Subjects:The estimates are based on 214 reportedly healthy pregnant women in their second trimester. Information on socio-economic status and reproductive history were obtained through home visits and venous blood samples were collected at antenatal care centres. Haemoglobin concentration (Hb) was measured by HemoCue, serum ferritin (sFt) by radioimmunoassay and serum transferrin receptor (sTfR) by enzyme-linked immunosorbent assay methods. ID was defined as presence of either low sFt (<12 μg l−1) or high sTfR (>8.5 mg l−1).Results:The prevalence of anaemia (Hb <110 g l−1) was 50%, but severe anaemia (Hb >70 g l−1) was absent. Low sFt was observed in 42%, high sTfR in 25%, either low sFt or high TfR in 54% and both low sFt and high TfR in 13% of the pregnant women. Two out of three anaemic women had an indication of ID, which was present in 80% of women with moderate (Hb 70–99 g l−1) and 50% with mild (Hb 100–109 g l−1) anaemia. Four out of 10 non-anaemic women (Hb >110 g l−1) also had ID, but the prevalence was significantly lower than that observed in anaemic women (P=0.001).Conclusions:Despite the high prevalence of anaemia, severe cases were absent. The prevalence of ID increased at lower Hb. However, an increased prevalence was also found among women in the highest category of Hb.

AbstractObjectiveMenstrual disorders are common in young women, and heavy menstrual blood losses (MBL) are an important cause of anaemia. Menstrual morbidity normally goes untreated in developing countries where cultural barriers also serve to make the problems. We investigated the prevelance of menstrual morbidity, and measured MBL and its relationship to iron deficiency in a rural adolescent population. The rationale was to assess whether or not reducing heavy MBL could be part of a strategy to reduce iron deficiency anaemia.SettingRural village in south-east Nigeria.DesignCross-sectional survey.SubjectsThe studdy included all non-pregnant, unmarried nulliparous girls (< 20 years) who had menstruated, and who lived in K'Dere village.MethodsA field worker allocated to each girl completed a questionnaire, and supervised recovery and collection of soiled pads and ensured blood sampling. MBL was measured using the standard alkaline haematin method. Haemoglobin (HB), serum iron, transferin saturation and protoporphyrin levels (ZPP) were also measured.Results307 girls completed MBL measurements; 11.9% refused to participate. 12.1% had menorrhagia (> 80 ml);. median MBL was 33.1 ml. Menorrhagia was more frequent in girls who had menstruated for > 2 years (p = 0.048), and had longer duration of meneses (p < 0.001). Iron status as measured by haematocrit, serum iron, transferrin saturation and ZPP values was inversely related to MBL. Neither height nor body mass index for age was associated with current iron status.ConclusionsThe level of menorrhagia detected (12%) may be an ‘expected’ level for a condition which often has no underlying pathology. Heavy MBL is one of the most important factors contributing to iron deficiency anaemia. Measures are needed to alleviate menstrual disorders and improve iron status. Oral contraceptives can be part of a strategy to reduce anaemia, particularly for adolescents at high risk of unwanted pregnancies.

Background: Anaemia is the most common nutritional problem in the world with significant impact on the neurological and cognitive development of young, growing children. India has a high prevalence of anaemia among its under-five children. The objectives of the study were to estimate the prevalence of anaemia and iron deficiency anaemia in children in the age group 6 months to 5 years.Methods: A hospital based descriptive study was done on 51 children attending the outpatient department of a tertiary hospital. They were evaluated by obtaining a detailed history, physical examination and haemoglobin and serum ferritin estimation.Results: The prevalence of anaemia in the present study was 41.2%. 13 (25.5%) had mild anaemia, 8 (15.7%) had moderate anaemia and none had severe anaemia. Iron deficiency anaemia was present in 17 out of 21 children (81%) with anaemia (p<0.00001). There were significant associations between haemoglobin levels and age (p=0.049) and nutritional status (moderate acute malnutrition) (p=0.034) of the children in the study.Conclusions: The prevalence of anaemia in children under 5 years in Kerala, which has performed well in other health quality indicators, remains high. A multimodal strategy to address this public health problem is required.

AbstractObjectiveTo determine risk factors for anaemia in preschool children.DesignA cross-sectional study.SettingTigray province, northern Ethiopia.Subjects2080 of 2373 children aged 6–60 months provided blood to assess anaemia.ResultsAnaemia was highly prevalent (42%) and constituted an important nutritional problem in the region. In a sub-sample of 230 anaemic children, 56% had a low red blood cell (RBC) count, and 43% had a serum ferritin of less than 12 μg l−1 indicating that the anaemia was largely due to iron deficiency. Unlike other regions in developing countries, hookworm (0.4%) and malaria (0.0%) were rare and contributed little to the anaemia. Even though their diet lacked variety, the amount of iron consumed through cereal-based staple foods was adequate. However, the iron in these foods was not readily available and their diets were probably high in iron absorption inhibitors and low in enhancers. Dietary factors associated with anaemia included frequent consumption of inhibitors, such as fenugreek and coffee, and poor health in the child such as diarrhoea and stunting.ConclusionsUnderlying causes of anaemia were lack of safe water and inadequate human waste management, maternal illiteracy and mother being ill, and having no food reserves. The root cause of these factors was poverty. The optimal control strategy for iron deficiency anaemia should have a holistic approach which includes the alleviation of poverty, the empowerment of women and the provision of a safe environment.

The purpose of this study is to investigate the prevalence of anemia and IDA and their risk factors in Chinese pregnant women.Methods: A nationwide cross-sectional survey of pregnant women was conducted during their antenatal visits. Using a multi-stage sampling method, 24 hospitals from 16 provinces across China were selected. Structured questionnaires were administered to collect information from participants and to extract clinical data from electronic medical records. Mixed-effects logistic regression models were performed to determine the risk factors associated with anemia and IDA.Results: In total, 12,403 pregnant women were enrolled, including 1,018 (8.2%) at the first trimester, 3,487 (28.1%) at the second, and 7,898 (63.7%) at the third. Overall, 19.8% of women were diagnosed with anemia and 13.9% were diagnosed with IDA. The prevalence of anemia and IDA varied among regions and increased by gestational month, peaking at the eighth gestational month (24.0% for anemia and 17.8% for IDA). Pregnant women at advanced stage of gestation, non-local residents, multiple gestations, multiparity, pre-pregnancy underweight, and those experiencing severe nausea or vomiting during pregnancy, were associated with higher risks of anemia and IDA.Conclusions: The prevalence of anemia and IDA during pregnancy are similar to those from developed countries and vary across regions in China.

INTRODUCTION: The most common disorders presenting with microcytic hypochromic anaemia are iron deficiency anaemia (IDA) and β-Thalassemia trait (β-TT), and each of them has different pathogenesis and treatment modality. Here we intend to see the haematological profile in microcytic hypochromic anaemia in children apart from IDA and Thalassemia with respect to age and gender. MATERIAL AND METHODS: A total of 95 pediatric patients between the ages of 2 to 12 years with microcytic hypochromic anaemia were included in the study. Blood samples obtained from each patient were evaluated for various haematological and biochemical profiles along with haemoglobin electrophoresis. It was a hospital based observational cross-sectional study done for a period of 18 months from January 2019 to July 2020. RESULTS: Severe anaemia was seen in 55 (57.89%) cases, followed by 38 (40.00%) cases with moderate anaemia and 2 (2.11%) cases with mild anaemia. Majority of the cases showed IDA, which were 85 (89.5%) cases, followed by 6 (6.3%) cases with β-TT and 4 (4.2%) cases with β-TM. RBC count, hematocrit and RDW showed significant variation between IDA, β-TM and β-TT. Most number of correctly diagnosed cases were shown by Ricerca Index with 90 (94.74 %). CONCLUSION: Our study concludes RBC count and RDW, along with Srivasthava Index, Ricerca Index and RDW Index could be used as reliable indices to differentiate between iron deficiency anaemia and β-Thalassemia. Red cell indices, serum iron profile and haemoglobin electrophoresis complement each other for the precise diagnosis of underlying cause of microcytic hypochromic anaemia.
