May-Hegglin anomaly is the presentation of a qualitative platelet disorder characterised by large platelets, thrombocytopenia and granulocyte inclusions as a result of mutations in the MYH9 gene. Though often overlooked given its rarity, it should be considered in patients who present with epistaxis, bruising, menorrhagia and easy bleeding as it can be mistaken for other diagnoses resulting in unnecessary treatments and tests. Our case study reports one presentation of this anomaly and can help broaden awareness of the presentation of this type of patient.

Abstract Hand pain is a common complaint in the hemodialysis patient on arteriovenous fistula. The problem is related to its etiology: nervous or vascular, which requires different therapeutic solutions. Ischaemic etiology involves a deviation of the distal flow in the anastomosed vein, with the decrease of the distal flow and the installation of distal ischaemia. Nerve etiology involves compression of the median nerve in the carpal tunnel. The purpose of the paper is to present a case study of diagnosis and treatment of median nerve compression in hemodialysis patient.

Constipation is a common complaint in older adults. Although constipation is not a physiologic consequence of normal aging, decreased mobility and other medical conditions may contribute to its increased prevalence in older adults. The prevalence of constipation rises dramatically with age, with some estimates approaching 50% among adults over 80 years of age. There are a variety of over-the-counter and prescription laxatives available for the treatment of constipation. There are many laxatives sold in the market. They have not been studied in controlled trials to make a recommendation. Its side effect may be harmful to patient. There is no one best evidence-based treatment for chronic constipation in the elderly. this study, shows single case of geriatric patient suffering from constipation wherein  Gandharvahasta tailwas given 10ml orally. The reference of Gandharvahasta tail is  Bhaishajya Ratanavali. Gandharvahasta tail acts as mild laxative with sweet taste, pleasant smell. Gandharvahasta Tail shows significant result with no side effect. It is easily palatable so there is no compliant for taking medicine.

AbstractDifficulty hearing in the presence of background noise is a common complaint heard by audiologists. This can be accompanied by additional difficulty in classrooms and other difficult listening situations. This was recently exacerbated by the coronavirus disease 2019 (COVID-19) shutdowns and the accompanying mask usage and virtual meetings. This article describes one such patient seen during the recent shutdown. This patient reported difficulty hearing during her college coursework for her music performance major. She was an established otology patient with an extensive middle ear history including multiple surgeries. During the shutdown, she virtually consulted with the auditory processing disorders clinic at the recommendation of her otologist to discuss her college-related difficulty and pursue educational accommodations. Challenges and solutions for seeing this patient and other, similar patients virtually are discussed as well as a review of how this patient proceeded and how the knowledge gained from this patient could apply to others with similar concerns.

Aim and Background: The term Menorrhagia is from Greek word, men meaning ‘menses’ and ‘rrhagia’ meaning ‘burst forth’10. It denotes cyclic bleeding which is excessive in amount or duration. It is a very common complaint among females in recent years. It has negative effects on women’s quality of life with limited options available in modern medicine, following Ayurvedic principles of diagnosis and treatment can be useful. Case Description: A 42 years old female Hindu patient, housewife by occupation visited our OPD with complaints of Excessive P/V bleeding during menstruation, prolonged bleeding for 7-8 days with interval of 18-20 days which was heavy with passage of big clots for first 4-5days. She changes 7-8 pads/day, next 3-4days moderate bleeding, changes 4-5pads/day was seen. Associated with fatigue patient was not able to do her normal activities. Diagnosed as Raktapradar according to Ayurveda. Treatment: She was given Ashokaghrita 1tsf, twice a day before food daily for 6 months3. Outcome: After three months of treatment patient showed significant relief in symptoms. Significant reduction in PV bleeding, proper intervals in between menses and reduction in passage of clots. Conclusion: Significant relief can be achieved in patients of menorrhagia by applying principles of diagnosis and treatment of Asrigdhar. Abstract: Asthma is a chronic inflammatory disease of the respiratory tract. The purpose of this study was to determine the description of the diagnosis and management of asthma inpatient at RSU Royal Prima Medan in 2018. This research was conducted using a case study design method, descriptive - retrospective, sample size 100 data of medical records of patients taken by purposive sampling, namely medical records the most complete data and analyzed with descriptive statistics. The results show that the youngest is 9 years old and the oldest is 85 years old, with 44 men and 56 women. The most common complaint is 74 shortness of breath and the most pharmacological management is beta2 agonist 100%. The conclusion was that the distribution of asthma was higher in women (56%) than in men (44%) and the most common complaint was shortness of breath (74%) and the most used combination treatment was Beta 2 + Corticosteroid + Ammonium Chloride Agonists. and Diphenhydramine + Paracetamol + Antibiotics (35%). Keywords: Asthma, Diagnosis and Management, Royal Prima Hospital Medan

Objective: To find out the causative organisms of vaginal discharge inreproductive age. Study Design: A descriptive case study. Setting: Department of Obstetricsand Gynaecology, Independent University Hospital Faisalabad. Duration of Study with Dates:One year from 01-01-2012 to 31-12-2012. Subjects and Methods: A total of 369 patients wereincluded in this study One swab for wet mount to see clue cells, pus cells, budding yeastcells and locomotory effect of Trichomoas vaginalis and to see growth by gram staining inpathology department and reported by pathologist. Results: Mean age of the participents was29.66±6.40 years. In 369 patients, 356 (96.5%) were married while remaining 13 patients (3.5%)were unmarried. Frequencies of different organisms responsible for vaginal discharge amongwomen of reproductive age were as follows: candidiasis 114 (30.9%), bacterial vaginosis 182(49.4%), trichomonal infection 22 (6.0%), gonococcal vulvovaginitis 9 (2.4%) and genital herpesnoted in 2 (0.5%). Remaining 40 patients (10.8%) were normal. Conclusion: Vaginal dischargeis a common complaint in Gynae outpatients. In our study, the frequency of bacterial vaginosiswas highest. Remaining causes are negligible except candidiasis.

A Muscle Contusion is a deep bruise due to traumatic blow to the soft structures such as muscle fibres, connective tissues and or blood vessels. In sports activity, Quadriceps Muscle Contusion is a very common complaint. It is a type of closed wound in which there is crushing of the muscle fibres which only allows bleeding beneath the skin. Symptoms include discolouration, swelling, pain and limitation of motion. As per Ayurvedic perspective, Quadriceps Muscle Contusion should be considered as Abhighata Janit Shoth. The present case study aims to investigate whether Ayurvedic intervention can be used as a treatment modality for acute sports injuries such as Quadriceps Muscle Contusion. A 19 years old girl came to the OPD of CBPACS with clinical features indicative of Quadriceps Muscle Contusion with resultant Hematoma. This case of was managed by Ayurvedic intervention of Kaishor Guggulu, Brihat Manjishthadi Kwath and Dashanga Lepa local application at OPD level for 14 days. The treatment gave very promising results with an overall improvement of about 76.66% in the subjective parameter used i.e. Oswestry’s disability index from 30 to 07, VAS improved from 8/10 to 2/10, at the end of 14 days of treatment.

AbstractUsing interviews with former militants and previously unpublished documents, this article traces the genesis and internal dynamics of the Ejército Revolucionario del Pueblo (People's Revolutionary Army, ERP) in El Salvador during the early years of its existence (1970–6). This period was marked by the inability of the ERP to maintain internal coherence or any consensus on revolutionary strategy, which led to a series of splits and internal fights over control of the organisation. The evidence marshalled in this case study sheds new light on the origins of the armed Salvadorean Left and thus contributes to a wider understanding of the processes of formation and internal dynamics of armed left-wing groups that emerged from the 1960s onwards in Latin America.

Abstract Culture shapes our basic sensory experience of the world. This is particularly striking in the study of religion and psychosis, where we and others have shown that cultural context determines both the structure and content of hallucination-like events. The cultural shaping of hallucinations may provide a rich case-study for linking cultural learning with emerging prediction-based models of perception.

Abstract Hoerl and McCormack demonstrate that although animals possess a sophisticated temporal updating system, there is no evidence that they also possess a temporal reasoning system. This important case study is directly related to the broader claim that although animals are manifestly capable of first-order (perceptually-based) relational reasoning, they lack the capacity for higher-order, role-based relational reasoning. We argue this distinction applies to all domains of cognition.
