ObjectivesTraditionally, evaluation is considered a measurement process that can be performed independently of the cultural context. However, more recently the importance of considering raters’ sense-making, that is, the process by which raters assign meaning to their collective experiences, is being recognised. Thus far, the majority of the discussion on this topic has originated from Western perspectives. Little is known about the potential influence of an Asian culture on raters’ sense-making. This study explored residents’ sense-making associated with evaluating their clinical teachers within an Asian setting to better understand contextual dependency of validity.DesignA qualitative study using constructivist grounded theory.SettingThe Japanese Ministry of Health, Labour and Welfare has implemented a system to monitor the quality of clinical teaching within its 2-year postgraduate training programme. An evaluation instrument was developed specifically for the Japanese setting through which residents can evaluate their clinical teachers.Participants30 residents from 10 Japanese teaching hospitals with experience in evaluating their clinical teachers were sampled purposively and theoretically.MethodsWe conducted in-depth semistructured individual interviews. Sensitising concepts derived from Confucianism and principles of response process informed open, axial and selective coding.ResultsTwo themes and four subthemes were constructed. Japanese residents emphasised the awareness of their relationship with their clinical teachers (1). This awareness was fuelled by their sense of hierarchy (1a) and being part of the collective society (1b). Residents described how the meaning of evaluation (2) was coloured by their perceived role as senior (2a) and their experienced responsibility for future generations (2b).ConclusionsJapanese residents’ sense-making while evaluating their clinical teachers appears to be situated and affected by Japanese cultural values. These findings contribute to a better understanding of a culture’s influence on residents’ sense-making of evaluation instruments and the validity argument of evaluation.

ObjectivesThis interview-based qualitative study aims to explore how healthcare providers conceptualise trace-based communication and considers its implications for how teams work. In the biological literature, trace-based communication refers to the non-verbal communication that is achieved by leaving ‘traces’ in the environment and other members sensing them and using them to drive their own behaviour. Trace-based communication is a key component of swam intelligence and has been described as a critical process that enables superorganisms to coordinate work and collectively adapt. This paper brings awareness to its existence in the context of healthcare teamwork.DesignInterview-based study using Constructivist Grounded Theory methodology.SettingThis study was conducted in multiple team contexts at one of Canada’s largest acute-care teaching hospitals.Participants25 clinicians from across professions and disciplines. Specialties included surgery, anesthesiology, psychiatry, internal medicine, geriatrics, neonatology, paramedics, nursing, intensive care, neurology and emergency medicine.InterventionNot relevant due to the qualitative nature of the study.Primary and secondary outcomeNot relevant due to the qualitative nature of the study.ResultsThe dataset was analysed using the sensitising concept of ‘traces’ from Swarm Intelligence. This study brought to light novel and unique elements of trace-based communication in the context of healthcare teamwork including focused intentionality, successful versus failed traces and the contextually bounded nature of the responses to traces. While participants initially felt ambivalent about the idea of using traces in their daily teamwork, they provided a variety of examples. Through these examples, participants revealed the multifaceted nature of the purposes of trace-based communication, including promoting efficiency, preventing mistakes and saving face.ConclusionsThis study demonstrated that clinicians pervasively use trace-based communication despite differences in opinion as to its implications for teamwork and safety. Other disciplines have taken up traces to promote collective adaptation. This should serve as inspiration to at least start exploring this phenomenon in healthcare.

THIS is a report of the plan for teaching basic pediatric pathology in connection with ward teaching during clinical clerkships, which has been in use for several years in the Pediatric Department of the State University of New York at Syracuse. Its success as one of the most practical ways of drawing the preclinical basic sciences closer to actual patient care may be of interest to teachers, either in pathology or on the wards, who feel that there is still too wide a gap between the preclinical years and the clinical teaching years. This problem has been one of long recognition and concern, varying in degree and intensity in different settings dependent in a large measure on the background in basic science training and experience of the clinical teachers.

This study reveals the Baliem ethnic concept of "eternal life" and how it relates to contextual gospel preaching (both potential and crisis). The study was conducted using a qualitative approach with a participant observer method supported by a study of a variety of relevant literature with a discussion of the concept of eternal life of Baliem people in Papua. As for the Baliem Society, Papua with a background of traditional societies with a worldview of animism has an eternal view of life which is lived out as an "ideal situation and condition" in the Nabelan-Kabelan myth and "an ideal person or figure" in the Naurekul myth. Through this view of eternal life, there is a "meeting point" and "difference" with the gospel message and Bible values. Because it is possible to be able to advocate and implement a contextual evangelistic approach for the Baliem people in Papua by touching and empowering their cultural values, Thus the Gospel and Christianity are not just a history or monument but are still present and change society while still paying attention to the integrity of the socio-cultural context, especially the people of Baliem, Papua.  ABSTRAKStudi ini mengungkapkan konsep etnis Baliem mengenai “hidup kekal” dan bagaimana kaitannya dengan pemberitaan Injil yang kontekstual (baik potensi maupun krisis). Penelitian dilaksanakan dengan  pendekatan kualitatif melalui metode pengamatan partisipan yang didukung dengan kajian kepada beragam literatur yang relevan dengan pembahasan mengenai konsep hidup kekal orang Baliem di Tanah Papua. Masyarakat Baliem, Papua dengan latar belakang masyarakat tradisional dengan pandangan dunia animisme memiliki pandangan hidup kekal yang dihayati sebagai “situasi dan kondisi yang ideal” pada mitos atau legenda Nabelan-Kabelan dan “pribadi atau sosok yang ideal” dalam legenda Naurekul. Melalui pandangan mengenai hidup kekal seperti ini, maka terdapat “titik temu” maupun “perbedaan” dengan berita Injil dan nilai-nilai Alkitab. Karena itu memungkinkan untuk dapat menganjurkan dan melaksanakan pendekatan kontekstualisasi Injil bagi etnis Baliem di Papua dengan menyentuh, memanfaatkan dan memberdayakan nilai budaya etnis Baliem, Dengan demikian Injil maupun kekristenan bukan hanya akan menjadi sejarah atau monumen namun akan tetap hadir dan mengubahkan masyarakat dengan tetap memperhatikan keutuhan konteks sosial budaya, khususnya etnis Baliem, Papua.

The required competencies of the medical graduates are changing, and by turn those of medical educators are changing as well. The aim of this study was to compare the faculty development (FD) needs of basic sciences and clinical teachers at the College of Medicine, Prince Sattam Bin Abdulaziz University during reforming of the curriculum to an integrated student-centred curriculum. This study is a descriptive cross-sectional study that included an online survey for faculty members to assess their FD needs and priorities. The survey questionnaire included questions about the experience of faculty members in teaching in integrated curricula, motivators to share in FD activities, preferred formats of FD activities, self-perceived competencies and needs in different areas of medical education, and selfperceived current commitment to the 12 roles of medical educators. Comparison between the basic sciences and the clinical teachers’ perceived competencies, priorities and commitment to the roles of the medical teacher took place. Mann-Whitney U test was used to compare means. The results of the study showed that the highest self-perceived competencies of faculty members were in lecturing (mean = 4.25±0.99) and constructing multiple choice questions (mean = 4.25±0.92). Statistically significant differences were present between basic sciences and clinical teachers in designing integrated courses (p = 0.02) and clinical teaching (p = 0.03). Significant differences were also present in the perceived importance of certain topics in FD programmes, such as course design, blueprinting and simulation. The study concluded that there are differences between basic medical sciences and clinical teachers in the learning needs and the competencies, which should be taken into consideration during planning for FD activities.

<p>Morality, or what is considered right or wrong, varies seemingly across cultures. However, the literature shows that moral psychologists have mainly investigated moral reasoning assuming a universal morality. Cross-cultural psychologists in contrast have widely neglected moral issues such as prescriptive beliefs of what people ought to do in a culture, and have predominantly measured culture through personal values. This thesis attempts to bridge this gap by focusing on the values concept. Four empirical studies were conducted to critically examine values as a measure of culture and their relationship to morality. Chapter one is an introduction into the topics morality, culture and values. Chapter two outlines the conceptual and methodological issues associated with deriving cultural values through the statistical aggregation of individuals' personal values. A value taxonomy is presented in which personal moral values and societal moral values are proposed as alternatives for measuring the cultural context. Following this critique, personal values are examined in two empirical studies in a cross-cultural context scrutinizing the validity of Schwartz' (1994) Culture-level Value Theory. Study 1 is a cross-cultural meta-analysis using the Rokeach Value Survey (Rokeach, 1973) showing that Schwartz' culture-level value structure was replicable with different samples, and a different method for assessing value priorities. Nonetheless, a set of values not included in Schwartz' analysis formed a new value type: Self-fulfilled Connectedness (SFC) which is related to the pursuit of non-material goals and endorsed in countries in which basic needs are fulfilled. Study 2 tested in a multilevel regression model whether Schwartz' cultural values predicted individuals' moral attitudes with data from more than 40 different countries. The findings indicated that the value dimension Autonomy-Embeddedness explained individuals' lenient attitude towards personal-sexual, but not towards dishonest-illegal issues. Study 3 dealt with the fundamental critique raised in chapter two that Schwartz (1994) does not operationalize micro- and macro-level constructs independently when measuring cultural values. To address this empirically, personal moral values and societal moral values were explicitly measured in student samples from eight cultures. Societal moral values showed greater cross-cultural differences than personal moral values. Furthermore, societal moral values at the culture-level conceptually replicated the multilevel findings from Study 2. This suggests that societal moral values are a valid macro-level variable for the measurement of culture. Finally, Study 4 was conducted to elicit implicit moral values. Respondents from four cultures free-listed their associations of a 'moral person'. correspondence analyses revealed that collectivistic-oriented samples mentioned more traditional moral attributes, whereas individualistic samples reported more liberal attributes. Furthermore, accessibility of implicit moral values - matched with the SVS - correlated with explicit ratings of personal moral values from Study 3, providing convergent validity for this kind of values. This multi-method finding corroborated that personal moral values and societal moral values are different concepts. In sum, these studies support the validity of Schwartz' theory and suggest that notions of right and wrong do indeed tell us something about culture, but it depends on (1) the issues studied (personal-sexual issues), and (2) the kind of values measured (societal moral values).</p>

Abstract Background Malnutrition is common in cancer patients. The NUTRISCORE is a newly developed cancer-specific nutritional screening tool and was validated by comparison with the Patient-Generated Subjective Global Assessment (PG-SGA) and Malnutrition Screening Tool (MST) in Spain. We aimed to evaluate the performance of the NUTRISCORE, MST, and PG-SGA in estimating the risk of malnutrition in Chinese cancer patients. Methods Data from an open parallel and multicenter cross-sectional study in 29 clinical teaching hospitals in 14 Chinese cities were used. Cancer patients were assessed for malnutrition using the PG-SGA, NUTRISCORE, and MST. The sensitivity, specificity, and areas under the receiver operating characteristic curve were estimated for the NUTRISCORE and MST using the PG-SGA as a reference. Results A total of 1000 cancer patients were included. The mean age was 55.9 (19 to 92 years), and 47.5% were male. Of these patients, 450 (45.0%) had PG-SGA B and C, 29 (2.9%) had a NUTRISCORE ≥5, and 367 (36.7%) had an MST ≥ 2. Using the PG-SGA as a reference, the sensitivity, specificity, and area under the curve values of the NUTRISCORE were found to be 6.2, 99.8%, and 0.53, respectively. The sensitivity, specificity, and area under the curve values of the MST were 50.9, 74.9%, and 0.63, respectively. The kappa index between the NUTRISCORE and PG-SGA was 0.066, and that between the MST and PG-SGA was 0.262 (P < 0.05). Conclusions The NUTRISCORE had an extremely low sensitivity in cancer patients in China compared with the MST when the PG-SGA was used as a reference.

While involvement of family caregivers can play an important role in the recovery process of persons with serious mental illness (SMI), family caregivers often endure poor health and mental health issues due to caregiving-related distress. These challenges may be exacerbated for Vietnamese American families due to cultural values (e.g., familism and stigma). This qualitative exploratory study examined how Vietnamese American family caregivers of persons with SMI describe their caregiving experience. Using convenience and snowball sampling, the study recruited 21 participants who took part in two Vietnamese-language focus groups. Key findings of the study addressed three themes: (1) the influence of cultural and religious values on caregiving and mental health; (2) the negative impact of caregiving on caregivers’ wellbeing; and (3) the stigma attached to mental illness. The study offers useful insights to assist mental health practitioners in tailoring culturally appropriate and effective services for Vietnamese caregivers.

In 2004, excavation in advance of the construction of a bypass around Mitchelstown in County Cork uncovered a number of pits on the banks of the Gradoge River (Kiely and Sutton 2007). On the bottom of one of these pits, three pottery vessels and a ceramic spoon had been laid on two flat stones. The pots had been deposited in a row: at the centre of the row was a small vessel that clearly models a human face with eyes, a protruding nose and ears, and, at the base of the pot, two feet (cover images). Oak charcoal from the pit returned a date of 1916–1696 cal BC. This find calls into question one of the basic conceptual building blocks that underpins our own contemporary understanding of the world—the distinction between people and objects—for it hints that some artefacts may have been imbued with human qualities and agentive capacities. This book is about the relationship between Bronze Age people and their material worlds. It explores the impact of the post-Enlightenment ‘othering’ of the non-human on our understanding of Bronze Age society. As we shall see, there is in fact considerable evidence to suggest that the categorical distinctions drawn in our own cultural context, for example between subject and object, self and other, and culture and nature, were not recognized or articulated in the same way during this period. So too contemporary forms of instrumental reason—encapsulated in a particular understanding of what constitutes logical, practical action and in the distinction we make between the ritual and the secular—have had a profound effect on how we view the Bronze Age world. Our understanding of the Bronze Age has undoubtedly changed dramatically since Christian Jürgensen Thomsen first popularized the term in his famous formulation of the three-age system in 1836 (Morris 1992). The very notion of a ‘Bronze Age’ foregrounds concepts of technical efficiency and advancement that doubtless chimed with the preoccupations and cultural values of Thomsen’s audience in the industrializing world in the nineteenth century.

BackgroundFeedback is an effective pedagogical tool in clinical teaching and learning, but the actual perception by learners of clinical feedback is often described as unsatisfactory. Unlike assessment feedback or teaching sessions, which often happen within protected time and space, clinical feedback is influenced by numerous clinical factors. Little is known about clinical teachers’ motivations to provide feedback in busy clinical settings. We aimed to investigate the motivations behind feedback being given in emergency departments (EDs).MethodsA qualitative analysis of semi-structured interview data was conducted between August 2015 and June 2016. Eighteen attending physicians were purposively sampled from three teaching hospital EDs in Taiwan. Data were thematically analysed, both inductively (from the data) and deductively (using self-determination theory (SDT)). Themes were mapped to the different motivation types identified by the SDT.Results and discussionDespite working in busy clinical settings, Taiwanese ED clinical teachers reported being motivated to provide feedback when they felt responsible for their learners, when they understood the importance of feedback (patient safety and partner building), or simply because they were committed to following a tradition of passing on their clinical knowledge to their juniors. Suggestions to facilitate the internalisation of external motivations are proposed.ConclusionsIn this qualitative study, motivations for clinical feedback were identified. Although the motivations are mostly extrinsic, the elicitation of internal motivation is possible once true satisfaction is fostered during the feedback-giving process. This understanding can be used to develop interventions to enable clinical feedback to be provided in a sustained manner.

Introduction: There has been a growing number of people from Chinese backgrounds entering England and their perceptions of spirituality and spiritual care need to be addressed when their cultural context changes. Methodology: A Straussian grounded theory method was used. Twenty-five participants were recruited, after which point data saturation was reached. Results: Four themes emerged showing participants’ perceptions of the terms: holistic; family involvement; religious care; abstract and sensitive. Discussion: Participants held holistic and culturally sensitive perspectives of spirituality, which demonstrates that patient-centered care is important. Also, health care professionals need to consider methods to involve family member and use religious or cultural values to support their spiritual needs. Particularly, when implementing spiritual care, they need to be aware that people from Chinese backgrounds blend Confucianism, Buddhism, and Daoism together in their understanding of the terms and may provide contradictory information about their religious belief.
