Transformational learning for health professionals through a Theatre of the Oppressed workshop

Theatre of the Oppressed (TO) is a powerful participatory tool for communities to examine their struggles against oppression. The healthcare community has problems inherent to complex, unequal power equations, and TO may be a useful means to understand and respond to their struggle. A 3-day workshop on TO was facilitated by the authors in the Himalayan Institute of Medical Sciences (HIMS) in Dehradun, India, in August 2017. The workshop culminated in the ‘Forum Theatre’, which included five short plays, each depicting a struggle due to real-life oppression faced by one or the other participant. The audience (about 200 invited members of the HIMS community) chose one play depending on the struggle with which they identified most. That play was ‘forumed’: spectators were invited to replace the struggling person and demonstrate how they would handle the oppression. Over the next week, participants reflected on the workshop through a structured online questionnaire. The feedback (n=16/27 participants; response rate 59.3%) was subjected to descriptive statistics and to qualitative analysis. The highest average Likert score (out of a maximum of 5) was given to the following items: TO engages senses and emotions (4.6±0.50), can help inculcate ethical behaviour (4.4±0.81), identifies conflict (4.4±0.51), and resolves issues of attitude, behaviour, communication, diversity and empathy (4.4±0.73). The Forum Theatre was reported to be a means to “express emotions and opinions and to simultaneously gather the same from others”; “make people push their own limits”; “bring out social problems in public”; “examine the root causes behind lived experience”; “provide context for understanding and for exploring alternatives”; and “convert thoughts to action.” In conclusion, TO is an engaging activity that identifies conflict; participants’ initial reactions suggest that it may initiate change in the ABCDE attributes (attitude, behaviour, communication, diversity, ethics and empathy) of medical professionals.

ABSTRACT This paper examines the innovative use of Boal’s Theatre of the Oppressed (Forum Theatre) with a group of 30 street children and young people in East Africa. Drawing upon a project in Burundi, this paper reveals how participants utilized the process of performance making through Forum Theatre as a platform to make visible problems in their lives, and a vehicle to challenge inequalities, abuse and violence. The authors demonstrate how the adoption of this methodology raised questions about interactive theatre as creative activism and a tool for opening up possibilities for dialogue with a community-based audience. This paper illuminates ways in which street children, explored, examined and problematized their lived experience, through the creative lens of Forum Theatre. It argues that this methodology generated a sense of collective consciousness, through which the children and young people created personal and social change, which extended beyond the life of the project.

Automated insulin delivery (AID) is the most recent advance in type 1 diabetes (T1D) management. It has the potential to achieve glycemic targets without disabling hypoglycemia, to improve quality of life and reduce diabetes distress and burden associated with self-management. Several AID systems are currently licensed for use by people with T1D in Europe, United States, and the rest of the world. Despite AID becoming a reality in routine clinical practice over the last few years, the commercially hybrid AID and other systems, are still far from a fully optimized automated diabetes management tool. Implementation of AID systems requires education and support of healthcare professionals taking care of people with T1D, as well as users and their families. There is much to do to increase usability, portability, convenience and to reduce the burden associated with the use of the systems. Co-design, involvement of people with lived experience of T1D and robust qualitative assessment is critical to improving the real-world use of AID systems, especially for those who may have greater need. In addition to this, information regarding the psychosocial impact of the use of AID systems in real life is needed. The first commercially available AID systems are not the end of the development journey but are the first step in learning how to optimally automate insulin delivery in a way that is equitably accessible and effective for people living with T1D.

Background:ANCA-associated Vasculitis (AAV) is a rare, severe small vessel vasculitis that affects multiple organs with a high acute mortality risk. As every patient presents differently, diagnosis is often delayed. Although treatments exist, responses vary, and remission is often not achieved or sustained. From the time of initial diagnosis onwards, patients suffer from an impaired quality of life. Coping with pain, fatigue, ongoing symptoms and combating challenges becomes a complex task and patients may be challenged in how best to communicate these emotions with health care professionals. We aimed to develop an initiative with Art and Voice, that would seek to empower people living with AAV and their carers in feeling understood, seen and heard in a meaningful way. This would invite a collective understanding of ‘how people make sense of key life experiences and what it means to them’ by creating a common language to address poorly addressed issues.Objectives:This project aims to provide a voice to patients to express personal experiences and complexity of everyday living and empower people to feel in control of their own health through an online platform. It should also allow practitioners to gain new awareness about issues faced by their patients, to better understand the relationships between caring and curing, hearing and listening.Methods:We collaborated with 10 patient association groups representatives, 17 AAV patients and 9 of their carers across 7 European countries. A series of workshops were set up to discuss issues faced and aid the subsequent production of a range of materials designed to provide clear, comprehensive content that would help individuals cope with the physical and emotional impact of AAV from diagnosis to living with it. This work was supported by a digital artist who is a rheumatologist living with vasculitis.Results:The co-creation of patient information materials featuring real life patients was successful and led to the development of a creative initiative called SEE ME.HEAR ME with an online platformwww.myancavasculitis.com. This includes: (1) an awareness programme featuring artwork created by the digital artist and advised by the patients which captures the essence of AAV from the patients view (see Figure). (2) a series of first-hand patient and carer stories capturing their authentic voice on ‘what it is like to live with the disease’. (3) extensive written content designed to fill information gaps around AAV diagnosis, investigations and treatment and what to expect during clinical follow up. The platform supports patients in asking questions and seeking information while signposting them to their own healthcare professional for advice and their local country patient association for support.Table 1.Sensitivities and specificities of examinations in gout and calcium pyrophosphate deposition diseaseConclusion:People with AAV need support throughout life, the profound psychosocial influence from illness makes the lived experience, challenging. SEE ME. HEAR ME online patient platform aims to generate awareness around AAV, improve physician and patient dialog, and enhance people’s experiences of living and coping with the disease. Purpose Journals are the essential tools of researchers, especially academicians, to present their scientific findings. So, choosing the right journal helps not only science development but also their academic promotion. The purpose of this study is to examine the factors that Iranian medical researchers consider when selecting scholarly journals in which to submit their work. Design/methodology/approach A self-administered online questionnaire was emailed in May 2021, with 101 responses received. The sample included all the faculty members with the role of “lecturer” in Iranian medical universities and who have 1–5 articles in the Scopus database as early-career Iranian medical researchers. The questionnaire consisted of 36 items, divided into five sections: basic information, attitudes and beliefs, ways to choose a journal, problems and familiarity with the components of scientometrics/validity metrics related to journals. Findings The findings indicate that these researchers value the expertise of experienced researchers and professionals, like librarians, when selecting publication venues. They often use journal indexes to guide journal selection. They also consider factors like the length of typical peer review and the complexity of submission guidelines when making decisions. Research limitations/implications The study of one country, though detecting requirements of journal selection behavior, cannot be generalized to the entire region. Practical implications The current study has academic implications as far as decisions on journal selection are concerned. University policymakers in Iran may consider re-examining their emphasis on academicians’ promotion policies at Iranian universities of medical sciences. Originality/value These findings may support the work of early-career researchers and those individuals (e.g., librarians) that serve them, as well as publishers and editors of scholarly journals.

PurposeThe purpose of this paper is to evaluate short-term, unpaid placements offered to students reading for a degree in public policy. They provide added value to their tertiary education experience. Elective placements were offered in 2012 and became a mandatory requirement for students reading for a three-year bachelor of commerce degree in public policy in 2018. To date, no research has been carried out on these placements and this may serve as a model for a post-evaluation assessment.Design/methodology/approachData were collected from students who undertook placements, embedded in the public policy undergraduate programme. A document analysis of selected student and placement provider's reports was carried out to complement the students' responses to an online questionnaire.FindingsPlacements are of value to students as they served as an introduction to the working world. They enable students to establish connections with the course content and carry out research. They were exposed to real-life situations, developing their knowledge, acquiring soft skills and learning new tools, sought after by employers. These placements were valued as a route to graduate employment tailor-made to the degree's requirements. Students were able to embark on a soul-searching, introspective discovery and journey which made them mature and shed light in the direction of future work prospects.Research limitations/implicationsPlacements give students the opportunity to gain insights into real-work environments and are able to link theories learnt in the class-room with real-life situations. Placements have positive implications on students adjusting to their work life easily after graduation. The limitations are that the sample size was small and that the reflective reports which were randomly selected may not have necessarily been representative of the full complement.Practical implicationsThe practical implications are that the placements system and process can easily be implemented and replicated in other academic disciplines and universities as a compulsory component of their studies.Social implicationsPlacements gave students the opportunity to reflect on their learning, develop non-technical skills and enhance their confidence levels. They were also able to network and communicate with different employees.Originality/valuePlacements provided exposure to relevant organisations and personal enrichment in terms of acquiring skills, autonomy and independence. Students with placement experience are also more likely to secure future employment, relevant to their undergraduate degree.

Objective – To determine reasons authors choose to publish in open access (OA) education journals, which provides readers with unrestricted free online access to published articles, and investigate ways in which publishing practices in the discipline of education affects authors’ willingness to publish in these journals. Design – Web-based survey questionnaire. Setting – The survey was conducted over the Internet through email invitations. Subjects – A total of 309 authors who published in OA journals in education participated in this survey for a response rate of 27.9%. Methods – Researchers surveyed authors who published in selected education journals from 2007 to 2008. The journal titles where generated from the Directory of Open Access Journals (DOAJ). All chosen journals were peer-reviewed and published either original research or overviews of research results. In addition, all were in English and published in the United States. A total of 1,107 authors were invited to participate via email. The survey was delivered through commercial online survey tool SurveyMonkey and consisted of multiple choice and open-ended questions. It was open from early March to April 16, 2009. Main Results – The survey had a response rate of 27.9%. The majority of participants were tenured faculty (42.0%), tenure-track faculty (25.9%), and non-tenure track faculty (12.1%). The rest of participants (20%) consisted of adjunct instructors, graduate students, administrators, and individuals working in non-academic institutions such as government agencies. Most authors surveyed have published between 10 and 20 articles (20.6%), or over 20 articles (30.4%) in print and electronic journals (e-journals). The majority of authors also reported that one (23.3%) or between 2 to 5 (54%) of their articles was published in OA format. When choosing a journal for publications, authors surveyed ranked peer-review to be the most important determinant. Other important determinants included “good match” (ranked second most important) for authors’ manuscripts and reputation of the journal (third) and editorial board (fourth). Citation impact, such as the ISI impact factor (eighth), and copyright retention (tenth) were ranked as some of the least important factors. Researcher also noted a “surprisingly low” (p. 124) correlation between authors’ interest in copyright retention and practices of self-archiving. Thirty-seven percent of authors surveyed reported self-archiving at least one of their publications, but just over 35% of the same group considered copyright retention a determinant when choosing journals for publication. Overall, only 22% of the authors surveyed deemed e-journals to be “less desirable” than print journals. The majority of both tenured faculty (77.4%) and tenure-track faculty (72%) surveyed found e-journals “acceptable” or difference between print and electronic journal format “not an issue.” Only 16.8% of authors surveyed had published in journals that required author fees. Moreover, over 56% of authors indicated they would not publish in journals requiring such fees. Most authors reported they were either very aware (45.1%) or somewhat aware (38.9%) of the concept of OA publishing. However, their perceptions of OA publishing varied: • 47.7% believed OA journals have faster publication times, while 33.6% disagreed and 18.5% offered no opinion. • 57.3% of authors believed OA journals have larger readerships. However, when asked whether OA articles would be cited more frequently than others, only one third of authors agreed, while one third disagreed and one third offered no opinion. • Just under half of the authors (49.4%) thought OA journals are not less prestigious than subscription based journals, while 18.8% had no opinion. Lastly, it should be noted that only 7.1% of authors credited their institution’s library for making them aware of the OA publishing concept. Most credited their colleagues (42.1%), Google searches for publishing opportunities (40.4%), and professional societies (29.3%) for raising their awareness of OA. Moreover, based on voluntary general comments left at end of the survey, researchers observed that some authors viewed the terms open access and electronic “synonymously” and thought of OA publishing only as a “format change” (p.125). Conclusion – The study revealed some discipline-based differences in authors’ attitudes toward scholarly publishing and the concept of OA. The majority of authors publishing in education viewed author fees, a common OA publishing practice in life and medical sciences, as undesirable. On the other hand, citation impact, a major determinant for life and medical sciences publishing, was only a minor factor for authors in education. These findings provide useful insights for future research on discipline-based publication differences. The findings also indicated peer review is the primary determinant for authors publishing in education. Moreover, while the majority of authors surveyed considered both print and e-journal format to be equally acceptable, almost one third viewed OA journals as less prestigious than subscription-based publications. Some authors also seemed to confuse the concept between OA and electronic publishing. These findings could generate fresh discussion points between academic librarians and faculty members regarding OA publishing.

COVID-19 pandemic obligated applying population-level behavioral modifications to effectively prevent the spread of the disease. This necessitated investigating those measures that determine population behavior. Herein we have studied risk perception and information exposure that are among those determinants in Iran. 402 cases from medical sciences students were enrolled during the last week of September 2020. Using an online questionnaire, risk perception and sources of information about COVID-19 were investigated. Although most students considered COVID-19 preventable, merely a few considered the disease curable. A higher risk was perceived concerning the families compared to themselves. Moreover, most of them believed the prognosis good even in high-risk patients. Social media was the most informative source used; however, health professionals were considered the most reliable. The risk perception was equal between those diagnosed with COVID-19 or had a family member diagnosed compared to those without such exposure in most questions. Also, no significant difference was observed in risk perception between those students with serious underlying medical conditions and those without one regarding most items. Lastly, major and grade were the most significant demographic contributors to the risk perception. Moderate risk was perceived overall among the cases in which major and grade were the only remarkable demographic contributors. Unexpectedly, underlying medical history was not significantly correlated with the perceived risk. Lastly, previous COVID-19 exposure merely altered the curability and preventability perception.

Background: Awareness towards Pharmacovigilance among health professionals in any hospital is an important part of health care system. So, the present study was conducted to know the extent of awareness of Pharmacovigilance among health care professionals.Methods: A cross-sectional questionnaire based study about knowledge, attitude and practices towards adverse drugs reaction (ADRs) and Pharmacovigilance was carried out at Shri Ram Murti Smarak Institue of Medical Sciences, Bareilly (U.P). 153 questionnaires were distributed among the Post graduate students, faculties, interns, nurses and pharmacists. 2-3 days time was given to fill the questionnaire.Results: Out of 153 questionnaires distributed, 99 filled forms were turned up for evaluation. Hence, response rate was 64.70%. 73.33% PGs, 100% faculty members, 53.33% interns, 36% nurses and 18.75% pharmacists were aware of adverse drugs reactions (ADRs) and Pharmacovigilance. Regarding reporting of ADRs, 86.66% PGs, 91.66% faculties, 73.33% interns, 88% nurses and 81.25% pharmacists were agreed to report ADR as it is mandatory and related to increased patients’ safety. Regarding discouraging factors for practice of reporting ADRs, 20% PGs, 16.66% faculties, 80% interns, 40% nurses and 68.75% pharmacists found ADR forms difficult to be filled. According to 66% faculties, 80% PGs, 66.66% interns managing patients are more important than reporting ADRs while 93.75% nurses and 76% pharmacists did not know how to report and where to report.Conclusions: The results of our study indicate that the acceptable number of health-care professionals had a good knowledge and attitude towards pharmacovigilance. There was a huge gap between the ADR experienced, and ADR reported by the health-care professionals. So, there is need of conductance of CMEs, workshops and seminars related to pharmacovigilance for all heaith care professionals especially nurses and pharmacists so, that whole community can be benefitted.

Objective: Standardized patients (SPs) are widely used in medical and other health professions education. At xxxx SPs have been used for teaching-learning and assessment of basic science medical students since January 2013. The opinion of SPs about their involvement in teaching-learning and assessment of students has been recently studied. The present study had the objective of studying student perception about various aspects of the program.Materials and Methods: The study was conducted towards end of March and beginning of April 2016. Student perception regarding the program was studied by noting their degree of agreement with a set of 25 statements. Free text comments were also obtained. Gender, nationality and semester of study of the respondents was noted. The total score was compared among different subgroups of respondents.Results: Ninety-eight of the 107 students (91.6%) participated. The mean ± SD total score was 92.33 ± 13.68 (maximum possible score being 125). The scores were significantly higher among first semester students compared to other semesters. Students agreed that interacting with SPs will prepare them for interacting with patients, for their licensing exams and improve their clinical skills. Students learn how to interact with persons from a different cultural background. Suggestions for further improvement were also obtained.    Conclusions: The response rate was high and students’ perception of the SP program was positive. Our experiences would be of special interest to educators in other medical schools with small and/or new SP programs.
