Purposes of the studyTo identify the challenges to well-being experienced by general practice postgraduate trainees and to explore how the trainees respond to those challenges.Study designQualitative focus group study with doctors in their final year of general practice training (n=16). The participants in the study were recruited from one training scheme in South West England. Data were thematically analysed.ResultsParticipants reported challenges to well-being relating to dysfunctional relationships with colleagues and patients, their workload, a perceived lack of support at work and also physical environmental challenges. They identified response strategies focused on cognitive processing, physical self-care, focusing on their professional purpose, building supportive relationships and adapting their working environment where possible. Additionally, there were factors that could support trainee well-being, including personal factors such as adaptability and self-awareness, but also external and organisational factors, such as culture, supportive colleagues and organisational adaptability in relation to workload management. The importance to trainees of the idea of being a ‘good doctor’ arose repeatedly in the data, as did the importance of the organisational environment. Participants reported finding their training placements in secondary care environments particularly challenging.ConclusionThis research highlights the strategies that general practice trainees use in response to challenges, but also that the responsibility for maintaining well-being cannot be borne by individuals alone. This study identifies that supportive approaches by healthcare organisations and educators are vitally important to general practitioner trainees’ well-being.

Cultural Life Scripts (CLS) are shared knowledge about personal events expected to be experienced by individuals within a society, and used as a framework for life story narration. Differences in CLS for individuals with depression and trauma, and their relations to anxiety, stress, and well-being, have not been investigated. Malaysian participants (N = 120) described and rated seven significant events most likely to be experienced by a prototypical infant from their culture, and seven significant events they had experienced or expected to experience in their own life. Participants then answered questionnaires about depression and trauma symptoms and about anxiety, stress, and well-being. The subclinical depression group listed less typical CLS events, whereas the subclinical PTSD group listed less positive individual life story events. The findings indicate that, although individuals with depression and trauma possess knowledge of the CLS, there may be small differences in the cognitive processing of CLS and individual life story events.

Continuing the workshop I've given in the WPC Paris (2017), this article elaborates my discussion of the way I interlace Focusing with Differentiation Based Couples Therapy (Megged, 2017) under the systemic view, in order to facilitate processes of change and healing in working with intimate couples. This article presents the theory and rationale of integrating Differentiation (Bowen, 1978; Schnarch, 2009; Megged, 2017) and Focusing (Gendlin, 1981) approaches, and its therapeutic potential in couple's therapy. It is written from the point of view of a practicing professional in order to illustrate the experiential nature and dynamics of the suggested therapeutic path. Differentiation is a key to mutuality. It offers a solution to the central struggle of any long term intimate relationship: balancing two basic life forces - the drive for individuality and the drive for togetherness (Schnarch, 2009). Focusing is a body-oriented process of self-awareness and emotional healing, in which one learns to pay attention to the body and the ‘Felt Sense’, in order to unfold the implicit, keep it in motion at the precise pace it needs for carrying the next step forward (Gendlin, 1996). Combining Focusing and Differentiation perspectives can cultivate the kind of relationship where a conflict can be constructively and successfully held in the inner world of each partner, while taking into consideration the others' well-being. This creates the possibility for two people to build a mutual emotional field, open to changes, permeable and resilient.

Abstract Background Effective inclusion in society for young people with disabilities is increasingly seen as generating opportunities for self-development, and improving well-being. However, significant barriers remain in the vast majority of activities meaningful for young adults. Research argues that various personal (disabilities, health) and environmental (access to the resources needed, accessible environment, discrimination, lack of personal economic independence) factors contribute to limited participation. However, previous studies conducted in young people with cerebral palsy (CP) mainly investigated the transition period to adulthood, and did not fully consider the whole range of impairment severity profiles or environmental barriers. In this study, we will use the follow-up of the SPARCLE cohort and a comparison group from the general population (1) to investigate the impact of the environment on participation and quality of life of young adults with CP, (2) to determine predictors of a successful young adulthood in educational, professional, health and social fields, (3) to compare quality of life and frequency of participation in social, work and recreational activities with the general population, (4) to document on participation and quality of life in those with severe disabilities. Methods The SPARCLE3 study has a combined longitudinal and cross-sectional design. Young adults with CP aged 22 to 27 years in 6 European regions previously enrolled in the SPARCLE cohort or newly recruited will be invited to self-complete a comprehensive set of questionnaires exploring participation (daily life and discretionary activities), health-related quality of life, body function, personal factors (health, personal resources), and contextual factors (availability of needed environmental items, family environment, services provision) during home visits supervised by trained researchers. Proxy-reports or adapted questionnaires will be used for those with the most severe impairments. The recruitment of a large group from the general population (online survey) will enable to identify life areas where the discrepancies between young people with CP and their able-bodied peers are the most significant. Discussion This study will help identify to what extent disabilities and barriers in environment negatively affect participation and quality of life, and how previous valued experiences during childhood or adolescence might modulate these effects.

This study seeks to add nuance to the definition of binge-watching by identifying the subtypes of the general practice that reflect viewer rituals, motives, and outcomes. The two subtypes are (1) the healthy practice of ‘feast-watching’ and (2) the unhealthy practice of ‘cringe-watching’. While binge-watching as a singular behavior has been associated with anxiety, depression, and loneliness, a survey ( N = 800) finds that binge-watching which is solo, accidental, and distracted (cringe-watching) predicts increased regret and decreased well-being. However, binge-watching that is planned, social, and attentive (feast-watching) predicts positive outcomes. These subtypes add much needed organizational clarity to the discussion of binge-watching, which, due to its popularity, has grown into a catchall for extended video consumption.

Virtual teamwork as a new way of working is becoming increasingly prevalent in a growingly globalized and digitalized working environment. Due to the associated raise in health-related stress factors at the workplace and the central role of leaders in workplace health promotion, the aim of this study is to obtain initial findings on the use of health-oriented self- and employee leadership in virtual teams from the perspective of virtual leaders. Semi-structured telephone interviews were conducted with 13 virtual leaders by using the problem-centered interview method. The collected data were deductively and inductively evaluated and interpreted using the qualitative content analysis according to Mayring. The results show that virtual leaders ascribed great value of health and showed great awareness in health-oriented self- and employee leadership. Physical activity and boundary management were particularly mentioned as health-oriented self-leadership behaviors. The majority of leaders described communication, building trust, support in boundary management and implementation of personal meetings as health-oriented employee leadership behaviors. In addition to social, technical, and personal factors, primarily organizational factors were mentioned as factors of influence in this context. For a more comprehensive understanding of health-oriented leadership, the inclusion of virtual team members in further research studies is necessary.

IntroductionNavy ships and submarines are important military measures that protect the Republic of Korea. They also comprise naval officers’ workplace. However, few studies have examined naval officers’ working environment and their job-related well-being. This study aimed to explore exposure to hazardous work environments among navy officers aboard ships and submarines and their association with job-related affective well-being.MethodsThis was a cross-sectional descriptive study. The sample comprised 146 officers from 4 navy ships and 98 officers from 5 submarines. Items of exposure to the eight types of hazardous work environments and the Job-related Affective Well-being Scale (JAWS) were included in the self-report survey questionnaires.ResultsThe most common hazards reported by officers aboard navy ships were vibration (63.7%) and air pollution (56.2%). For submarine officers, these hazards were lack of personal space (72.4%) and air pollution (67.3%). The average JAWS score for ship officers and submarine officers was 69.81 (SD=10.89) and 70.50 (SD=10.83), respectively. For ship officers, exposure to air pollution, noise, vibration, thermal discomfort during summer or winter and lack of personal space were significantly correlated with lower JAWS scores. For submarine officers, exposure to fire, burning or electrical shock, air pollution, noise, thermal discomfort during summer or winter and lack of personal space were significantly correlated with lower JAWS scores.ConclusionsThis study revealed that some naval officers aboard ships or submarines are exposed to hazardous work environments. Moreover, certain types of hazardous work environments were associated with naval officers’ job-related affective well-being.

BackgroundThe COVID-19 pandemic has drastically increased demands on healthcare workers (HCWs) leaving them vulnerable to acute psychological distress, burnout and post-traumatic stress. In response, supportive services in a central London hospital mobilised mental health support specifically for HCWs.AimsThis rapid evaluation assessed HCW psychological welfare during the acute phase of the COVID-19 pandemic and their use of supportive services made available.MethodsDuring the acute phase of COVID-19 (April to May 2020) all staff working for the hospital were invited to complete an online survey assessing well-being (self-rated health, moral distress exposure, symptoms of burnout and psychological distress) and use of available supportive services (awareness of, use and perceived helpfulness). Associations among personal characteristics and psychological well-being were explored using correlations and linear regression.ResultsA total of 1127 staff participated in the rapid evaluation. On average, psychological distress was high (mean (SD): 22 (7.57)) regardless of role, with 84% of this sample scoring above the general population mean (14.5). Nearly half of the sample reported feeling emotionally drained and a profile emerged displaying higher levels of psychological distress and burnout in those who were younger and exposed to morally distressing situations, with this group also exhibiting greater support service use. Greater levels of burnout were associated with increased psychological distress when controlling for personal factors. During this acute phase of the pandemic, majority of staff used at least one service and rated it as helpful.ConclusionHCWs experienced high levels of psychological distress requiring continued support as the COVID-19 pandemic evolved. Although HCWs were aware of supportive services, uptake varied. In order to mitigate the risk of burnout and post-traumatic stress, long-term, effective strategies that facilitate staff accessing support are urgently required.
