BACKGROUND:Helicobacter pylorihas been classified by the World Health Organization as a type I carcinogen. Nearly 50% of the world’s population is estimated to be infected withH pylori. Prevalence patterns of the infection are different between developing and developed countries. The present study had two objectives – to estimate the prevalence ofH pyloriinfection in Ontario, and to evaluate the relationship between the infection and various demographic characteristics and selected lifestyle factors.METHODS: Ten microlitres of plasma were aliquoted from stored blood of 1306 men and women, 50 to 80 years of age, from Ontario. The blood samples belonged to control patients of a colorectal cancer population-based study group. Serological testing was used to detectH pyloriinfection; information was obtained on dietary intake and lifestyle habits, as well as past and present medical history, education, income, number of siblings, ethnicity and place of birth.RESULTS: The overall weighted seroprevalence ofH pyloriwas 23.1% (95% CI 17.7% to 29.5%), with men having higher infection rates (29.4%, 95% CI 21.1% to 39.3%) than women (14.9%, 95% CI 10.1% to 21.4%). Seroprevalence of the infection increased significantly with age and number of siblings. Increased risk was also associated with being nonwhite, being born outside of Canada and immigrating at 20 years of age or older. An inverse association with seroprevalence was found for education and alcohol consumption.CONCLUSION: The prevalence ofH pyloriinfection in Ontario is comparable with that of other developed countries. Age, sex, number of siblings, ethnicity, place of birth and age at immigration are among the factors associated withH pyloriinfection.

Helicobacter pylori (H. pylori) is classified by the World Health Organization (WHO) as a group I carcinogen and is one of the most efficient human pathogens with over half of the world's population colonized by this gram-negative spiral bacterium. H. pylori can cause a chronic infection in the stomach during early childhood that persists throughout life due to diverse mechanisms of immune response evasion. H. pylori has several factors strongly associated with increased risk of disease such as toxins, adhesins, and chemoattractants, some of which are highly polymorphic, phase variable, and have different functions. Conventional treatments involve the use of antibiotics. However, treatment frequently fails due to the resistance H. pylori has progressively developed to antibiotics. This creates the need for different treatments made possible by identifying new therapeutic targets in the pathogen’s genome.The purpose of this study was an in silico prediction of T- and B- epitopes in H. pylori proteins. Twenty-two external membrane proteins from H. pylori Strain 26695 (accession number NC_000915) were identified using the web tool Vaxign (http://www.violinet.org/vaxign/). A total of one-hundred epitopes (60 class I epitopes and 40 class II epitopes) that could be used to develop novel non-antibiotics drugs for an H. pylori infection were predicted.

According to the World Health Organization, the anemia prevalence was estimated reach 9% in developed countries, while in developing countries reached 43%. The most-at-risk groups were children and women at fertile age, with an estimated prevalence of anemia in infants at 47%, pregnant women by 42%, and in non-pregnant women t ages 15-49 reached 30%. This was an observational analytic research with cross sectional approach. The purpose was to analyzed the relationship between the number of parity and the pregnancy age with the incidence of maternal anemia. The population in this study were 111 pregnant womens who checked their pregnancy to Public Healt Center (PHC) Kintamani 1 at Bangli regency, the province of Bali. The  simple random sampling was used in the research with 87 peoples acted as research sample. Using Rank Spearman Correllation test, it was founded that there was a relationship between the number of parity with the incidence of maternal anemia a low  relationship, and there was a relationship between gestational age with maternal anemia incidence with a low degree relationship.

Abstract The relationship between naturally fluoridated groundwater and dental fluorosis has received large attention from researchers around the world. Despite recognition that several factors influence this relationship, there is a lack of systematic studies analyzing the heterogeneity of these results. To fill such a gap, this study performs a systematic review and meta-analysis to understand which factors influence this relationship and how. Selected studies were sampled between 2007 and 2017 from Web of Science, PubMed, Google Scholar and Scopus using keywords and Boolean operators. Results of the systematic review show that dental fluorosis affects individuals of all ages, with the highest prevalence below 11, while the impact of other factors (gender, environmental conditions, diet and dental caries) was inconclusive. Meta-regression analysis, based on information collected through systematic review, indicates that both fluoride in drinking water and temperature influence dental fluorosis significantly and that these studies might be affected by publication bias. Findings show that fluoride negatively affects people's health in less developed countries. The conclusions discuss policy tools and technological innovations that could reduce fluoride levels below that of the World Health Organization (WHO) (<1.5 mg/L).

CONTEXT: Gastric neoplasia is the second most common cause of death by cancer in the world and H. pylori is classified as a type I human carcinogen by the World Health Organization. However, despite the high prevalence of infection by H. pylori around the world, less than 3% of individuals carrying the bacteria develop gastric neoplasias. Such a fact indicates that evolution towards malignancy may be associated with bacterial factors in the host and the environment. OBJECTIVES: To investigate the association between polymorphism in the region promoting the IL-8 (-251) gene and the H. pylori genotype, based on the vacA alleles and the presence of the cagA gene, using clinical and histopathological data. METHODS: In a prospective study, a total of 102 patients with stomach cancer and 103 healthy volunteers were analysed. Polymorphism in interleukin 8 (-251) was determined by the PCR-restriction fragment length polymorphism reaction and sequencing. PCR was used for genotyping the vacA alleles and the cagA in the bacterial strains PCR. Gastric biopsies were histologically assessed. RESULTS: The H. pylori serology was positive for 101 (99%) of all patients analysed, and 98 (97%) of them were colonized by only one strain. In patients with monoinfection, 82 (84%) of the bacterial strains observed had the s1b/m1 genotype. The cagA gene was detected in 74 (73%) of patients infected by H. pylori. The presence of the cagA gene was demonstrated as associated with the presence of the s1b/m1 genotype of the vacA gene (P = 0.002). As for polymorphism in the interleukin 8 (-251) gene we observed that the AA (P = 0.026) and AT (P = 0.005) genotypes were most frequent in the group of patients with gastric adenocarcinoma. By comparing the different types of isolated bacterial strains with the interleukin -8 (-251) and the histopathological data we observed that carriers of the A allele (AT and AA) infected by virulent strains (m1s1 cagA+) demonstrated a greater risk of presenting a degree of inflammation (OR = 24.75 CI 95% 2.29-267.20 P = 0.004) and increased neutrophilic activity (OR = 28.71 CI 95% 2.62-314 P = 0.002) in the gastric mucosa. CONCLUSION: Our results demonstrate that the interaction between polymorphism in the interleukin -8 (-251) gene, particularly with carriers of the A allele and the infecting type of H. pylori strain (s1m1 cagA positive) performs an important function in development of gastric adenocarcinoma.

AbstractObjective:The study aimed to delineate the relationship between18F-fluorodeoxyglucose uptake and histological findings in nasopharyngeal carcinoma.Methods:We conducted a retrospective review of 88 patients referred to our positron emission tomography department with newly diagnosed, biopsy-proved nasopharyngeal carcinoma. Hospital records were reviewed for demographic and clinical data, including age, gender, body weight, histological analysis and clinical tumour stage.Results:Nineteen out of the 88 patients (22 per cent) had nasopharyngeal carcinoma of World Health Organization type I, 39 (44 per cent) had type II and 30 (34 per cent) had type III. The mean standardised uptake value for the primary tumour was 9.4 ± 5.0, ranging from 2.2 to 27.1. The mean standardised uptake values were 8.0 ± 3.9 for type I tumours, 9.7 ± 4.4 for type II tumours and 10.1 ± 6.3 for type III tumours (p = 0.451).Conclusions:Our study found that type III nasopharyngeal carcinoma exhibited a higher18F-fluorodeoxyglucose uptake than either type II or type I nasopharyngeal carcinoma. However, this difference was not statistically significant.

Infectious diseases in the modern world continue to claim millions of human lives despite the achievements of medicine. While in developed countries the main cause of death is cancer and diseases of the cardiovascular system, it is the infectious processes that occupy leading positions in the structure of mortality in the third world countries. About 1.7 million children die from infections that could have been avoided by vaccination according to the World Health Organization. In contrast to the countries of Western Europe, where preventive vaccinations for the population are carried out for a fee, preventive vaccination in the Russian Federation is funded by the state. Immunoprophylaxis includes not only prevention of 12 major infections included in the calendar of preventive vaccinations (diphtheria, polio, tetanus, whooping cough, tuberculosis, measles, rubella, mumps, hepatitis B, pneumococcal infections and haemophilus influenzae, influenza), but also vaccination against 17 additional infections in case of epidemiological indications.

Background: Diabetes mellitus type 2 (DMT2) is an endocrine disease of global proportions which is currently affecting 1 in 12 adults in the world, with still increasing prevalence. World Health Organization (WHO) declared this worldwide health problem, as an epidemic disease, to be the only non-infectious disease with such categorization. People with DMT2 are at increased risk of various complications and have shorter life expectancy. The main classes of oral antidiabetic drugs accessible today for DMT2 vary in their chemical composition, modes of action, safety profiles and tolerability. Methods: A systematic search of peer-reviewed scientific literature and public databases has been conducted. We included the most recent relevant research papers and data in respect to the focus of the present review. The quality of retrieved papers was assessed using standard tools. Results: The review highlights the chemical structural diversity of the molecules that have the common target-DMT2. So-called traditional antidiabetics as well as the newest and the least explored drugs include polypeptides and amino acid derivatives (insulin, glucagon-like peptide 1, dipeptidyl peptidase-IV inhibitors, amylin), sulfonylurea derivatives, benzylthiazolidine- 2,4-diones (peroxisome proliferator activated receptor-γ agonists/glitazones), condensed guanido core (metformin) and sugar-like molecules (α-glucosidase and sodium/ glucose co-transporter 2 inhibitors). Conclusion: As diabetes becomes a more common disease, interest in new pharmacological targets is on the rise.

Abstract After more than 1 year of the SARS-CoV-2 pandemic, a great deal of knowledge on how this virus affects pregnant women, the fetus and the newborn has accumulated. The gap between different guidelines how to handle newborn infants during this pandemic has been minimized, and the American Academy of Pediatrics (AAP)’s recommendations are now more in accordance with those of the World Health Organization (WHO). In this article we summarize present knowledge regarding transmission from mother to the fetus/newborn. Although both vertical and horizontal transmission are rare, SARS-CoV-2 positivity is associated with an increased risk of premature delivery and higher neonatal mortality and morbidity. Mode of delivery and cord clamping routines should not be affected by the mother’s SARS-CoV-2 status. Skin to skin contact, rooming in and breastfeeding are recommended with necessary hygiene precautions. Antibodies of infected or vaccinated women seem to cross both the placenta and into breast milk and likely provide protection for the newborn.

How do COVID-19 experts psychologically manage the pandemic and its effects? Using a full year of press briefings (January 2020–January 2021) from the World Health Organization ( N = 126), this paper evaluated the relationship between communication patterns and COVID-19 cases and deaths. The data suggest as COVID-19 cases and deaths increased, health experts tended to think about the virus in a more formal and analytic manner. Experts also communicated with fewer cognitive processing terms, which typically indicate people “working through” a crisis. This report offers a lens into the internal states of COVID-19 experts and their organization as they gradually learned about the virus and its daily impact.

AbstractLiberia, Sierra Leone, and Guinea lack the public health infrastructure, economic stability, and overall governance to stem the spread of Ebola. Even with robust outside assistance, the epidemiological data have not improved. Vital resource management is haphazard and left to the discretion of individual Ebola treatment units. Only recently has the International Health Regulations (IHR) and World Health Organization (WHO) declared Ebola a Public Health Emergency of International Concern, making this crisis their fifth ongoing level 3 emergency. In particular, the WHO has been severely compromised by post-2003 severe acute respiratory syndrome (SARS) staffing, budget cuts, a weakened IHR treaty, and no unambiguous legal mandate. Population-based triage management under a central authority is indicated to control the transmission and ensure fair and decisive resource allocation across all triage categories. The shared responsibilities critical to global health solutions must be realized and the rightful attention, sustained resources, and properly placed legal authority be assured within the WHO, the IHR, and the vulnerable nations.
