We retrospectively studied 30 cases of multiple myeloma in patients under the age of 65, diagnosed from 1991 to 2005 in the clinical hematology department of the University Hospital of Yopougon that is a hospital incidence of 2.9 cases/year. The age of patients ranged from 34 to 64 years, with a mean age of 49 years and a sex ratio of 1.73. The professional activity was variable with 3% of radiographers and 10% of farmers. Clinically, the dominant sign was bone pain in 83% of cases. Myeloma was secretory in 93% of cases. It was Ig G-type in 86%, kappa-type in 66% of cases. 86% of patients were anemic, 20% had creatinine >20 mg/L, and 10% had serum calcium >120 mg/L. Geodes were found in 80% of cases. 53% were at stage III of DURIE and SALMON. Complications were infectious (33%), renal (20%), and hemorrhagic (7%). Chemotherapy regimens were VAD (10%), VMCP (30%), and VMCP/VBAP (60%) with 47% of partial responses, 33% of stable disease, and 7% of very good quality partial responses. The outcome developed towards death in 37% and causes of death were renal in 46% of cases. The median survival was only 5.1 months.

Background: Medication-related osteonecrosis of the jaw (MRONJ) is a potentially severe complication of mainly antiresorptive drugs. We evaluated the frequency of dentoalveolar pathologies in patients scheduled for antiresorptive therapy in a ‘real-world’ setting, also including patients with poor oral health potentially requiring tooth extractions and/or other dentoalveolar surgery. This approach is in contrast to the setting of recent randomized trials with restrictive exclusion criteria. Patients & methods: We prospectively included patients suffering from solid tumors with osseous metastases or multiple myeloma. Screening for dentoalveolar pathologies was done prior to initiation of antiresorptive therapy at the specialized MRONJ clinic of the University Hospital for Cranio-Maxillofacial and Oral Surgery, Innsbruck, Austria. Results: 119 subjects could be included. In 76 patients (63.9%), a dental focus was revealed including deep caries (24.4% of patients), chronic apical periodontitis (26.9%), periodontal disease (45.8%), root remnants (16%), jaw cysts (2.5%), partially impacted teeth (5.0%) and peri-implantitis (5.0%). Conclusion: Considering the high number of dentoalveolar pathologies (63.9%), systematic dental focus screening prior to initiation of antiresorptive therapy is of utmost importance to lower the risk for MRONJ.

Light chain multiple myeloma (LCMM) initiates approximately 15 percentage of patients with multiple myeloma (MM). It has a lower prognosis when compared with the variant immunoglobulin (Ig) G or IgA. We report a rare case on Light chain multiple myeloma in 49-year-old male patient who presented with acute kidney injury, hypercalcemia. histopathology examination was found to be plasmacytoma, kappa restricted; and free kappa lamda chain in urine and serum was found to be greater. Bone pain and renal dysfunction were the most common prevalent initial signs and symptoms while extramedullary disease (EMD) was later acquired during disease. Bortezomib demonstrated superior efficacy over nonbortezomib in LCMM patients.

The article considers the formation of professional motivation in future speech pathologists. The importance of solving this issue is emphasized, because the work of a defectologist requires the presence of special characteristics in his personality, the formation of which is carried out during their training. Issues of personal development of the student and the formation of his readiness for future professional activity become the main in the outlined situation. The psychological and personal readiness of the specialist for professional self-realization in modern conditions is important. It is emphasized that the transformation of the requirements for professionals of different natural influences the solution of the question of the complex relationship of personal and professional development of the subject. The question of quality criteria for the training of future speech pathologists can not be considered separately from the requirements of a particular environment in which the specialist works. It is at the stage of professional training in the university begin to develop in a specialist-defectologist those values and motives that determine the success of his work in the profession, and for this it is necessary to study the formation of professional values and motives in the formation of future defectologists.

Professional socialization of future graduates of the pedagogical university is aimed at the formation of competencies essential for the practical activity and solution of vital tasks. One of the components of successful professional activity is the willingness for continuing education. The subject of this research is the phenomenon of continuous pedagogical education as the value. Globalization has encompasses all segments of life of the modern society, including education. The rapidly changing world with its uncertainty imposes new requirements on the system of vocational training. A modern specialist is no longer narrow-focused, but is capable of solving complex and versatile tasks, which is facilitated by constant striving for new knowledge using various forms of learning. However, this is only possible with realization by the individual of the need for continuing education and its utility in different axiological coordinates. For the analysis of axiological basis of continuing education, the author surveyed the students of pedagogical university to determine their perception of continuing education as a personal meaningful value that contributes to the achievement of certain career results. The acquired results on the one hand demonstrate the the vagueness of the concept of “continuing education” for the university students; while on the other hand, prove the hypothesis on the need and importance of continuing education for the pedagogues, but at the same time the unwillingness of the future specialists to consider it as the value of life. The presented materials allow adjusting the educational process of future pedagogues and their motivation for continuing education, since the pedagogical classes to Master’s Degree.

The present role, space, task, mission, function, and outcome of University Medicine in Italy are briefly examined, taking as a pure and trivial pretext the actual professional activity of single, representative physician who changed his role at the same specialistic Department of the same University Hospital, by covering an University role in the past five years, after working at the same facility as an Hospital-affiliated specialist in charge of the same Medical Division during the previous 14 years. The lights and shadows of assistential and academic medicine organisation and integration in Italy are the starting point of our preliminary observations, which may be potentially extended to the University Medicine as a whole, from an organisative, functional, and especially ergonomic point of view.

Abstract BACKGROUND Validation of the 2016 LANO MRI scorecard for leptomeningeal metastasis failed for multiple reasons. The objective of this joint EORTC Brain Tumor Group and RANO effort was to validate the feasibility of the revised MRI scorecard for assessing response in leptomeningeal metastasis. METHODS Coded paired cerebrospinal MRI of 20 patients with leptomeningeal metastases from solid cancers at baseline and follow-up after treatment and instructions for assessment were provided via the EORTC imaging platform. The kappa coefficient (K) was used to evaluate inter-observer pairwise agreement. Statistical analyses were performed using SAS V9.4 software (Cary, NC). The sponsor of the study was the University Hospital Zurich (2018-00192). RESULTS Thirty-five raters participated, including 9 neuroradiologists, 17 neurologists, 4 radiation oncologists, 3 neurosurgeons and 2 medical oncologists. Among leptomeningeal metastases-related items at baseline, the best median concordance was noted for hydrocephalus (K=0.63), and the worst median for spinal linear enhancing disease (K=0.46). The median concordance for overall response was moderate (K=0.44). Notably, the interobserver agreement for the presence of parenchymal brain metastases at baseline was minimal (K=0.29). Significant differences were observed when considering the specialty of the raters. Only 394 of 700 ratings (56%) were fully completed. Among 394 fully completed ratings, perfect concordance was noted in 293 ratings (74%) when comparing the overall response according to the guidelines provided in the scorecard and the overall response provided by the raters. The main discordances were noted for partial response according to the rater versus stable disease according to the guidelines (n=44), followed by progression according to the raters versus stable disease according to the guidelines (n=23). CONCLUSION Electronic case report forms with "blocking solutions" are probably required to enforce completeness and quality of scoring. These results confirm the necessity of central review and the need for training of MRI assessment in clinical trials.

The article examines the methodology of the research presented on the basis of theoretical analysis of modern scientific literature on the subject, generalization of psychological and pedagogical research in the field of training future language specialists in the organization of the educational process. And also in this scientific work the level of methodological and theoretical bases of optimization of training in conditional realization of 12 - year obrozovaniye in R. K. is considered. In this connection, there was a need to improve the training of a competent teacher at the university, corresponding to the requirements of the time. The need to improve the level of scientific organization of professional activity of teachers is particularly emphasized. The paper highlights issues related to the organization of professional activities of personnel. It also provides for science-based planning of organizational scientific and methodological training of teachers. Shows naibolei the full use or scientific achievements of pedagogy, psiology and special sciences in the educational process. This allows you to clearly distribute the functions and coordination of efforts, create the necessary conditions, choose the optimal option of activity, stimulation, regulation, control and accounting. The relevance of research is due to the need for a scientifically based approach to solving these problems. The purpose of retraining and advanced training of teachers is indicated.

Iván Kiss was born in the South-Hungarian town Pécs in 1945. He studied at the University Medical School, Pécs between 1963-1969. After a specialisation in pathology, he qualified also in anesthesiology at the Postgraduate Medical Institute, Budapest. After leaving Hungary in 1977 he gained a position at the University Hospital, Freiburg, Germany, soon appointed as head of the anesthesiology unit. In 1987 he attained the “Doctor Medicinae Habilitatus” degree upon his thesis on cancer pain. Later this work was partly published in “Pain, 29: 195-207 1997” the leading journal of the subject with the title “The McGill Pain Questionnaire - German version. A study on cancer pain”. In 1989 he moved to Essen for holding the job of the chief anesthesiologist at the Krupp-Clinic. By that time, he was acknowledged as an internationally recognized expert in anaesthesiology and pain therapy. His expertise was honoured by the „Doctor honoris causa” title at the Semmelweis University, Budapest in 1999. Dr. med. habil. Iván Kiss died on November 29, 2017. In his person, we lost a colleague and friend, a devoted teacher and researcher, and a committed clinician. We keep him in our memories as a real Paragon of the medical vocation.

In the article, the managerial culture is considered as the main component of the evaluation of the effectiveness of the contemporary teacher, who meets the requirements of training a competitive specialist. The emphasis is made on managerial training of a teacher, who is able and willing to organize a productive educational process. The content of organizational and managerial skills is influenced by psychological characteristics of the coordinator of an activity, such as enhanced responsibility for the effectiveness of professional work, subjects of professional interaction; creative approach to organization and management in solving professional problems; and specificities of communication in the course of solving organizational and managerial problems. It is an indisputable fact that organizational and managerial activities are a substructural component of the integral professional activity of the subject of labor. It involves the presence of formed motives, knowledge, skills, personal and behavioral qualities that enable the manager to effectively organize the process of a particular type of professional activity. This understanding of organizational and managerial activities allows concluding that the readiness of the subject of labor for this activity is closely related to the training system of professionals at high school. The above allows concluding that managerial culture for the education manager is a criterion of efficiency of his organizational and managerial activities, which is essentially identical to professional activity, while the development of this parameter is most effective during the study period at the university.
