<b><i>Objective:</i></b> The objective of this study was to compare the frequency and percentage of fetal hemoglobin (HbF%) by flow cytometry of (1) first-trimester asymptomatic patients with intrauterine hematoma (IUH), (2) first-trimester pregnant patients with vaginal bleeding (VB), and (3) first-trimester asymptomatic pregnant women without hematoma. <b><i>Methods:</i></b> Prospective study involving pregnant women in the first trimester of pregnancy. Patients with ultrasound findings of asymptomatic hematoma and with VB were paired with asymptomatic pregnant women of same gestational age without hematoma (control group [CG]). Maternal blood HbF% was evaluated by flow cytometry. The groups were compared in terms of circulating fetal hemoglobin and HbF%. <b><i>Results:</i></b> Sixty-six patients were selected, 22 with hematoma, 17 with bleeding, and 27 in the CG. Fetal hemoglobin was detected in 15 patients with hematoma (68.2%) and 13 with bleeding (76.5%) and in 20 of the control (74.1%) (<i>p</i> = 0.830). The mean HbF% of each group was 0.054, 0.012, and 0.042 for hematoma, bleeding, and control, respectively, and differences were not significant (<i>p</i> = 0.141). There was a moderate negative correlation between the volume of hematoma and HbF% (<i>r</i>Spearman = −0.527; <i>p</i> = 0.012). <b><i>Conclusions:</i></b> The fetal-maternal hemorrhage expressed by Hbf% in first-trimester pregnancies did not seem to differ between patients with and without ultrasound findings of IUH.

Background: Preeclampsia is a multi system disorder with placenta as the organ of origin and maternal endothelium being the organ of target.  According to recent studies, the cell free haemoglobin induces oxidative stress mediated damage to the blood placenta barrier with consequently elevated levels of HbF in maternal blood. Alpha 1 microglobulin is an endogenous protein with antioxidant property, present in elevated levels in maternal blood in response to oxidative stress. This fact forms the basis for our study. The objective of the present study was to establish association between high levels of fetal hemoglobin and alpha 1 microglobulin in plasma of pregnant women between 10 to 16 weeks of gestational age and subsequent development of preeclampsia. Methods: This was a prospective cohort study undertaken in the Department of Obstetrics and Gynaecology, ISO -KGH, between December 2016 to November 2017. A total of 100 pregnant women were included in the study after getting informed written consent. Both primigravida and multigravida, belonging to age group of 20 to 35 years (singleton/ multiple) between 10 to 16 weeks GA and with BMI between 16 to 35 kg/m2 were included in the study. A woman with Diabetes mellitus, Hypertension, Renal disease, Epilepsy and Vascular disorders were excluded from the study. Results: The cut off value for alpha 1 microglobulin was 1.86ng/ml and the cut off value of fetal haemoglobin was 1.92ng/ml above which the pregnant women develop preeclampsia. Conclusions: Higher values of fetal hemoglobin and alpha 1 microglobulin in pregnant women between 10 to 16 weeks gestational age positively correlates with development of preeclampsia in those women.

Hypothesis/aims of study. Asthma is the most common chronic disease of the respiratory system in women during pregnancy. Numerous studies show an increased frequency of all complications of pregnancy and delivery by cesarean section in women with asthma. Such delivery is reported to be observed more often in patients with severe asthma and asthma exacerbations during gestation. There are conflicting data showing that the use of asthma medication increases the risk of complications of pregnancy and childbirth. This study aimed at analyzing the frequency of cesarean section in patients with asthma, depending on the severity, course, and control of the disease. Study design, materials and methods. This retrospective study involved data of 170 patients with varying severity and control of asthma, who were under the supervision of a pulmonologist from the first trimester of pregnancy. According to the severity of asthma, patients were divided into subgroups with mild intermittent, mild persistent, and moderate and severe course of the disease. The control group was represented by 30 patients who did not suffer from any pulmonary and allergic diseases. Results. The cesarean delivery rate in patients with asthma in the whole group was significantly higher than in patients of the control group. Exacerbation of asthma during pregnancy significantly increases the frequency of abdominal operative delivery, and the use of modern drugs to achieve control of asthma during pregnancy has a positive effect and is associated with a lower birth rate by cesarean section. Conclusion. The active simultaneous management of pulmonary problems in pregnant women allows timely and adequate therapeutic measures aimed to achieve control of asthma during pregnancy, thereby reducing obstetric and perinatal risks, including those associated with the method of delivery.

Women with preeclampsia, independent of obesity and glucose intolerance, exhibit insulin resistance during pregnancy. The purpose of the present study is to determine whether early diagnosis of insulin resistance during pregnancy can predict preeclampsia. Through a case-control study, 675 pregnant women were selected and their first trimester blood was taken. Their fasting blood glucose and insulin were also measured after diagnosis of preeclampsia by 20 weeks of pregnancy. Based on the experiments conducted on 675 women who were 20 weeks past their pregnancy, 375 cases with preeclampsia were selected and assigned to the case group. 35 other pregnant women were put in the control group. Diagnosis criteria for the participants included blood pressure above 140/90 and proteinuria above 300 mg or above +1. Both groups were matched according to age, parity, gestational age, and BMI. Homa-Irand rate of insulin resistance was calculated by HOMA-IR and patients were followed up. Homeostatic model assessments (HOMA-IR) revealed that the average insulin resistance increased during pregnancy among both the case and control groups. There was a significant difference between insulin resistance of these two groups in both first trimester and third trimester and after developing preeclampsia (P< 0.001,P= 0.021). Insulin-resistance of the group with preeclampsia was higher in first trimester prior to diagnosis as well as the third trimester after diagnosis compared to natural pregnancy under similar conditions. Measurement of insulin resistance in first trimester may be useful in predicting the risk of preeclampsia.

Preeclampsia (PE) is characterized by a series of clinical features such as hypertension and proteinuria associated with endothelial dysfunction and the impairment of placenta vascular endothelial integrity. This study aimed to investigate the effect of serum copper (Cu) level on some angiogenesis-related factors including vascular endothelial growth factor-A (VEGF-A), soluble Fms-like tyrosine kinase-1 (sVEGF-R1), soluble endoglin (sEng) and cerruloplasmin (Cp) in Iraqi women with preeclampsia (PE) and control pregnant women. Therefore, 60 women with PE in addition to 30 healthy pregnant women were enrolled in the study. Serum concentration of sEng, VEGF-A, sVEGF-R1, and Cu in PE group significantly increased (p&lt;0.05) in the PE group compared with that in the control group. Increased production of antiangiogenic factors, soluble VEGF-A and sEng contribute to the pathophysiology of PE, indicating the involvement of these parameters in the angiogenic balance in patients with PE. Tests for between-subject effects showed that the circulating angiogenesis factors and Cu were significantly associated with the presence of PE. Serum Cu level was significantly correlated with VEGF- A and VEGF-R1 levels but not with sEng. Multiple regression analysis revealed that only Cp and BP can significantly predict the complications in women with PE. In conclusion, serum Cu has a role in the angiogenesis in women with PE and may be a new drug target in the prevention or treatment of PE.

Objective: Spontaneous abortion is the most common complication of early pregnancy, affecting up to 20% of recognized pregnancies. Kisspeptin is predominantly released by placental syncytiotrophoblasts, and regulates their placental invasion into the uterine matrices. We aimed to establish an association of serum kisspeptin levels with pregnancy outcomes during the early gestational stage of the first trimester. Method: In this prospective study, 90 pregnant women in their 7 to 8 6/7 gestational weeks were classified into three groups: (i) The control group, consisting of healthy pregnant women (n=30), (ii) the threatened abortion group (n=30), and (iii) the spontaneous abortion group (n=30). The maternal serum samples were analyzed for complete blood count parameters and kisspeptin levels. Results: There was no statistical difference regarding body mass index (BMI) and gestational age (p=0.370). Regarding detailed obstetric notations, including gravida, parity, abortion, and living children, socioeconomic levels, and employment rates, all study groups were comparable (p>0.05, for all). No significant association was found regarding the biochemical parameters of complete blood count, including neutrophil, lymphocyte, and platelet concentrations, as well as neutrophil-to-lymphocyte ratios (NLR) and platelet-to-lymphocyte ratios (PLR) (p>0.05, for all). The median serum kisspeptin levels of the study groups did not differ between the groups (p=0.153). Correlation analysis revealed no significant relationship between serum kisspeptin levels and other study parameters in any study groups (p>0.05, for all) Conclusions: We found no statistically significant relationship between serum kisspeptin concentrations and pregnancy outcomes in the early gestational stage of the first trimester, and serum kisspeptin concentrations did not seem to be a reliable marker to distinguish abortion status from viable pregnancy
