Abstract Objectives Current WHO hemoglobin (Hb) cutoffs (< 110 g/L, 6–59mo; < 120 g/L, 15–49y non-pregnant women) to define anemia were established in 1967 and validated in 1980 s among iron replete USA survey participants. Our objective was to examine the consistency of Hb cutoffs from multiple countries and age-variations in Hb concentrations among apparently healthy people using contemporary data. Methods We analyzed altitude and smoking (women only)-adjusted Hb data from 25 countries for preschool children (PSC, 6–59mo, 24 surveys, n = 35,088) and women of reproductive age (WRA, 15–49y, 20 surveys, n = 50,846) from the Biomarkers Reflecting Inflammation and Nutritional Determinants of Anemia (BRINDA) project. Healthy was defined as: [ferritin >12 µg/L (< 5y), >15 µg/L ( >5y)], RBP or retinol >0.7 µmol/L, CRP ≤ 5 mg/L or AGP ≤ g/L, and no malaria (if measured)]. We examined Hb levels, cut-offs at specified percentiles (%ile) and country variations with age-adjusted generalized mixed models using country as random intercept. Results The final analytic sample was 41,314; the healthy inclusion criteria resulted in 17–88% data loss of the original data depending on country. Age and country-adjusted mean (SE) of Hb was 116.9 (1.44)g/L for PSC, and 129.1 (1.49)g/L for WRA and was significantly (P < 0.001) associated with age in PSC but not in WRA, and varied by country (pheterogeniety < .001, in each target group). Country explained 22% of the total Hb variance for each group. The pooled countries’ Hb at the 5%ile was 94 g/L for PSC [range: 79 g/L (Pakistan) - 113 g/L (USA], and 110 g/L for WRA [92 g/L (Nigeria) - 121 g/L (USA)]. Conclusions The high heterogeneity in Hb cutoffs among healthy people from different countries suggests that a single Hb cutoff for anemia may not work for every country. This multinational analysis suggests that different approaches may be warranted to establish Hb cutoffs. Funding Sources Bill & Melinda Gates Foundation.

Objectives Occasional leg symptoms, like feelings of heaviness and tension, and occupational or evening oedema are considered typical features of a venous disorder but show low specificity in epidemiological and observational studies. We evaluated the prevalence and nature of such symptoms in subjects with no history or signs of venous disease and investigated the optimal strength that medical compression stockings (MCS) should exert in order to alleviate the symptoms and to prevent leg swelling. Methods Specifically designed questionnaires were used to assess the symptoms of 40 healthy employees of a factory producing MCS. Lower leg volumes were quantified in the morning and evening. Calf size hosiery providing documented ankle pressures of 4–9 (mean 7.3), 12–18 (mean 14.9) and 18–22 (mean 19.5) mmHg, respectively, were tested in a prospective, open-label, randomized trial lasting three weeks. Endpoints were the relief of symptoms, prevention of vesperal oedema and comfort in wearing the stockings. Results Sixty-five percent of the participants reported at least occasional leg symptoms and oedema. Somatic-type symptoms (i.e. pain, heaviness, swelling, unattractive legs) were present in two, psychic-type symptoms (i.e. leg- and personality-related unrest and stress) in 17 and both components in seven of the 40 subjects. MCS exerting 15 and 20 mmHg prevented the symptoms and oedema. Stockings providing <10 mmHg were ineffective and those providing >19 mmHg were not well-tolerated. The effect on the somatic-type symptoms was strongly correlated with the amount of lower leg volume which could be reduced by wearing stockings ( P = 0.005). No correlation was found between the efficacy of compression and the emotional component of the symptoms. Conclusion The cause of occasional pain in the legs of apparently healthy people is unknown. Some features of the syndrome reflect an emotional disorder while others mirror venous insufficiency. MCS of 15 mmHg effectively relieve the symptoms resembling venous insufficiency, prevent oedema and are well-tolerated.

In the existing pandemic of COVID-19, we have been hearing a lot about quarantine. Quarantine is to isolate from normal relations and routine activities or communication and is recommended for apparently healthy people who may have been exposed to the virus (suspects) and may potentially transmit the same to others. These healthy individuals may fall sick due to possible exposure and are therefore advised to go on self-quarantine or may be ordered by state or local administration to stay at home or in facilities like hospitals (public or private), hotels, schools, worship places etc. to isolate them from normal relations in order to prevent transmission of virus to others. Quarantine can be unpleasant and lasts for 14 days which is the presumed incubation period for the virus.

Due to the using of removable laminar dentures made of acrylic plastic, the complications in the form of chronic inflammation of the oral mucosa are developed in a certain category of patients, as a result of the complex impact of dentures on the tissues of the prosthetic bed in terms of mechanical, chemical, toxic, microbiological and immunological factors. In conditions of mechanical or chemical damage to the mucous membrane epithelium by the action of the microbial factor the activation of epithelial cells occurs and they acquire the properties of immunocompetent cells. As a result, the epithelial cells begin to produce cytokines. A special place in the implementation of acute inflammatory process is held by proinflammatory cytokines: interleukin (IL-1β) and tumor necrosis factor (TNF-α). They regulate all successive stages in the development of inflammation and an adequate response to the etiological factor. Interleukin 10 (IL-10) is the main anti-inflammatory cytokine, which inhibits the synthesis of proinflammatory cytokines. Determination of the level of pro- and anti-inflammatory cytokines is a diagnostic criterion in the development of prosthetic stomatitis and at the same time it is used to test the effectiveness of the proposed method of prevention or treatment of complications on the part of the tissues of the prosthetic bed. The aim of the research is to study the concentration of proinflammatory cytokines (TNFα, IL-1β) and anti-inflammatory cytokines (IL-10) in the oral fluid of patients with complete and partial defects of the dentition after prosthetics with removable laminar dentures made of acrylic plastic and dentures coated with fullerene C60 molecules. Material and methods of research. The study included 39 patients who were between 40 to 80 years old, including 19 male (48.7%) and 20 female (51.3%) subjects. The partial and complete defects of the dentition were diagnosed. The control group consisted of apparently healthy individuals, namely 10 people without defects of the dentition and periodontal tissue diseases. The patients were divided into 3 groups: the first group comprised apparently healthy people with intact dentition; the second group included patients with complete and partial defects of the dentition after prosthetics with removable laminar dentures made of acrylic plastic (20 patients) 7 days after prosthetics; the third group – patients with complete and partial defects of the dentition with removable laminar dentures made of acrylic plastic (20 patients) 30 days after prosthetics; the fourth group – patients with complete and partial defects of the dentition after prosthetics with removable laminar dentures made of acrylic plastic and coated with fullerene C60 molecules. The concentration of cytokines IL-1β and TNFα and IL-10 (pg / ml) was determined in the mouth before treatment at day 7, 2 weeks and 1 month. Statistical processing was conducted using STATISTICA 6.0 (StatSoft, USA). Results of the studies and their discussion. The results of the studies show that the level of cytokines in patients before prosthetics is higher than in apparently healthy people. On the 7th and 30th day after prosthetics in the study groups, there was a sharp increase in the concentration of proinflammatory and anti-inflammatory cytokines in the oral fluid as contrasted with the comparison group. These data confirm that activation of local synthesis of proinflammatory cytokines in the oral fluid is an important pathogenetic factor that determines the formation and maintenance of an active inflammatory response in the mucosa of the prosthetic bed. After fullerene C60 coating of the surface of denture, which is in contact with the tissues of the prosthetic bed, there is a significant decrease in the level of proinflammatory (IL-1β and TNFα) and anti-inflammatory (IL-10) cytokines.

The further investigations on this subject which form the basis of this paper have been mainly directed towards phagocytosis and the specific hæmagglutinins and hæmolysins present in the blood of apparently healthy people and those suffering from acute and chronic infections. To these we may possibly add a note on the susceptibility of the red blood corpuscles in various diseases to water containing graduated percentages of sodium chloride. Normal Blood It is stated in a paper published in the ‘Proceedings of the Royal Society’ by one of us (L. S. D.), that while auto-agglutination of the red blood corpuscles does not occur, iso-agglutination is met with in a certain proporation of cases. The Samples of blood used in these experiments is met in a certain proportion of cases. The samples of blood used in these experiments have been obtained from workers in these laboratories and other apparently in the best of health. The technique was similar to that referred to in the preliminary report.
