Media literacy and cigarette smoking in Hungarian adolescents

Objective: To assess smoking media literacy in a sample of Hungarian youth and to determine its association with current smoking and susceptibility to future smoking. Design: Quantitative cross-sectional survey. Setting: Four elementary and four high schools in Mako, Hungary. Method: A survey form was administered in regularly-scheduled classes to 546 eighth- and twelfth-grade students that included the smoking media literacy (SML) scale and items assessing cigarette use. Logistic regression was used to examine the relationship of smoking media literacy with current smoking, and also separately for susceptibility to smoking in the future, as dependent dichotomous variables. Results: Smoking media literacy was lower among the Hungarian adolescents than what has been previously reported in American adolescents. Multivariate logistic regression analysis results showed smoking media literacy to be associated with reduced risk of current smoking status at a similar level to that found in American adolescents. However, unlike previous research in American adolescents, smoking media literacy and susceptibility to future smoking was not associated. Reduced smoking may be most associated with the representation-reality domain of media literacy, which relates recognition of what is portrayed in the media with reality. Conclusion: Based on this study’s findings, prevention and health promotion planners in Hungary should consider media literacy training as a possible addition to smoking prevention efforts in community- and school-based efforts.

Abstract Background: Both lipid and glucose abnormalities are associated with hypertension (HTN). However, it is unclear whether the triglyceride-glucose (TyG) index is associated with HTN. Therefore the aim of this study is to investigate the association of the TyG index and HTN and to compare the discriminative power of the TyG index, lipid, glycemic parameters for the risk of HTN in elderly individuals.Methods: The present study was nested in a longitudinal (REACTION) study from May 2011 to December 2011, which was designed to demonstrate the association of abnormal glucose metabolism with the risk of cancer in the Chinese population. In total, 47808 participants were recruited in this cross-sectional study. The TyG index was divided into five groups: the <20% group, the 20-39% group, the 40-59% group, the 60-79% group and the ≥80% group, according to quintile division of the subjects. Three multivariate logistic regression models were used to evaluate the association between the TyG vs. lipid parameters, glycemic parameters and HTN.Results: Multivariate logistic regression analysis shows that compared with lipid and glycemic parameters, the TyG index remains significantly associated with HTN in either total subjects or subjects separated into men and women (odds ratio (OR) 1.33, 95% confidence interval (CI) 1.18-1.51, p <0.0001 in total subjects; OR 1.39, 95% CI 1.11-1.74, p=0.0042 in men; OR 1.28, 95% CI 1.11-1.49, p=0.0010 in women). In a stratified analysis, an elevated TyG index is significantly associated with HTN in the subgroup of the oldest age (≥65) (OR 1.67, 95% CI 1.30-2.14, p <0.0001), as well as with obesity (Body mass index (BMI) ≥28 kg/m2) (OR 1.85, 95% CI 1.29-2.66, p=0.0009) or lower estimated glomerular filtration rate (eGFR) (<90 mL/ (min·1.73 m2)) (OR 1.72, 95% CI 1.33-2.21, p <0.0001).Conclusion: The TyG index is significantly associated with HTN and shows the superior discriminative ability for HTN compared with lipid and glycemic parameters in the Chinese elderly population.

Konsep literasi media (smoking media literacy [SML]) dikembangkan sebagai  strategi dalam pengendalian tembakau berbasis sekolah.Tujuan. Untuk mengetahui hubungan SML dengan status merokok siswa SMA negeri di wilayah Kecamatan Purwakarta Kabupaten Purwakarta.Metode. Penelitian ini merupakan penelitian kuantitatif dengan desain cross-sectional yang dilaksanakan pada April-Mei 2018 di Kecamatan Purwakarta Kabupaten Purwakarta. Data dikumpulkan dengan kuesioner yang diisi sendiri oleh responden yang berjumlah 310 siswa-siswi SMA negeri. Analisis multivariat dilakukan dengan uji regresi logistik ganda. Variabel yang dikontrol dalam analisis adalah jenis kelamin, pendidikan orang tua, parenting, orang terdekat yang merokok (orang tua, saudara kandung, dan teman sebaya), capaian prestasi di sekolah, depresi, self-esteem, sifat memberontak, dan sifat mencari sensasi.Hasil. Hasil penelitian menunjukkan bahwa terdapat 14,2% responden yang berstatus merokok. Nilai rata-rata skor SML responden adalah 68,94 (dalam skala 100). Hasil regresi logistik ganda menunjukkan bahwa ada hubungan bermakna antara SML dengan status merokok setelah jenis kelamin, saudara yang merokok, teman sebaya yang merokok, capaian prestasi di sekolah, dan sifat memberontak dikendalikan (nilai p=0,048; CI=1,008-7,085).Kesimpulan. Pemahaman siswa SMA tentang literasi media masih rendah, dan SML berhubungan dengan status merokok siswa. ABSTRACTBackground. Media exposure is a very influential factor in the initiation of adolescents to smoke. Therefore, media literacy (smoking media literacy [SML]) was developed  as a school-based tobacco control strategy. Objective. The purpose of this research was to examine the relation of SML with the smoking status of high school students in Purwakarta Regency Method. This research used quantitative research with a cross-sectional design conducted in April-May 2018 in Purwakarta Regency. Data were collected by self-administered questionnaires from 310 students of public senior high school. Data were analyzed using logistic regression test. Statistical adjustments were made for gender, parental education, parenting, smoking status of parents, siblings, and peers, school performance, depression, self-esteem, rebellion, and sensation seeking character.Results. There were 14.2% smokers among respondents. The mean of SML score was 68.94 (on a scale of 100). The result of logistic regression showed that there was a significant correlation between SML and smoking status with adjustment by sex, the sibling who smokes, peers who smoke, school achievement, and rebelliousness (p-value=0.048; CI=1.008-7.085).Conclusions. High school students' understanding of media literacy was still low, and SML was related to students' smoking status.

Abstract Introduction Up-to-date monitoring of non-combustible nicotine products (e.g. e-cigarettes, nicotine replacement therapies (NRT), heated tobacco products (HTP); NNP) is important to assess their impact. To date, there is little evidence on the association between ever regular use (defined here as 1 year or more) of NNP and current smoking status. Aims/methods The purpose of this study was to examine the prevalence, and sociodemographic, alcohol and smoking status correlates, of ever regular use of NNP in England in 2020. A cross-sectional survey of adults in England was conducted between February and June 2020. Results A total of 8486 adults were surveyed; 94.9% (8055) were complete cases. The weighted prevalence of ever regular NNP use was 5.4% (n = 436; 95% CI 5.0–6.0), of which 82% (n = 360; 95% CI 78.7–85.8) was single and 18% (n = 79; 95% CI 14.8–22) multiple product use. Amongst ever regular NNP users, the prevalence of ever regular NRT, e-cigarette and HTP use was 64.7% (95% CI 60.1–69), 43.4% (95% CI 38.8–48) and 2.5% (95% CI 1.4–4.5), respectively. In adjusted analysis, ever regular NNP use was associated with smoking status, being significantly higher among current (22.3%; adjusted OR (aOR) 34.9, 95% CI 24.0–50.8) and ex-smokers (12.7%, aOR 19.8, 95% CI 11.1–14.4) than among never-smokers (0.6%). More advantaged occupational grade (aOR, 1.27 95% CI 1.02–1.57) and at least hazardous alcohol use (aOR, 1.38 95% CI 1.06–1.78) were associated with greater prevalence of ever regular NNP use. Conclusions Ever regularly using NNP was highest among smokers and ex-smokers and rare among never-smokers. Among people who have ever regularly used NNP, NRT is the most popular.

ObjectivesTo explore smoking prevalence and attitudes towards smoking among Estonian physicians in 2002 and 2014.DesignTwo self-administered cross-sectional postal surveys were conducted among practising physicians in Estonia.ParticipantsInitial sample consisted of all practising physicians in Estonia. The corrected response rate was 67.8% in 2002 and 53.1% in 2014. Present study sample was restricted to physicians younger than 65 years (n=2549 in 2002, n=2339 in 2014).MethodsAge-standardised prevalence of smoking and prevalence of agreement with seven statements concerning attitudes towards smoking was determined. To analyse association of physicians’ attitudes towards smoking with study year and smoking status, logistic regression analysis was used. Adjusted ORs of agreement with the seven statements were determined. Corresponding 95% CIs were calculated.ResultsThe age-standardised prevalence of current smoking among men was 26.8% in 2002 and 15.3% in 2014, among women 10.4% and 5.8%, respectively. Compared with the year 2002, in 2014, prevalence of agreement with statements declaring harmfulness of smoking was higher and prevalence of agreement with statements approving smoking was lower. Adjusted ORs showed that compared with 2002, physicians’ attitudes towards smoking were less favourable in 2014, and physicians’ attitudes towards smoking were associated with their smoking status.ConclusionsCompared with 2002, the age-standardised smoking prevalence among male and female physicians was lower, and attitudes towards smoking were less approving in 2014. The smoking physicians had more approving attitudes towards smoking than their non-smoking colleagues.

Abstract Background Poor self-rated health (SRH) and smoking have consistently been shown to be related to mortality. The aim of this study was to explore SRH and smoking among physicians and general population with higher education in Estonia in 2002 and 2014 and to analyse the association of SRH with smoking and sociodemographic factors. Methods This study was based on cross-sectional postal surveys among physicians and general population with higher education in Estonia n in 2002 and 2014. Calculation of age-standardized prevalence of SRH and current smoking with 95% confidence intervals (CI) was performed. Multivariate logistic regression analysis was used to measure association between SRH (at-least-good vs less-than good) and smoking status, study year, age group, ethnicity, and marital status. Fully adjusted odds ratios (OR) with 95% CI were computed. Results Age-standardized prevalence of at-least-good SRH was 71.3 and 80.6% among male physicians, 68.4 and 83.1% among female physicians, 45.4 and 67.4% among men with higher education, and 44.7 and 63.1% among women with higher education in 2002 and 2014, respectively. Age-standardized prevalence of current smoking was 26.0 and 15.6% among male physicians, 10.2 and 5.9% among female physicians, 38.7 and 22.2% among men with higher education, and 20.9 and 16.4% among women with higher education in 2002 and 2014, respectively. There was no significant gender difference in at-least-good SRH, but prevalence of current smoking was significantly higher among men in both study groups in 2002 and 2014. Compared to year 2002, odds to have at-least-good SRH was higher in 2014 (OR = 1.64; 95% CI 1.16–2.31 among male and OR = 2.36; 95% CI 2.02–2.75 among female physicians, OR = 1.49; 95% CI 1.07–2.07 among men and OR = 2.40; 95% CI 1.84–3.13). Odds to have at-least-good SRH was significantly higher among non-smokers (except female physicians), in the youngest age group, and among Estonians. Conclusions This study gave an overview of differences in SRH and smoking between two target groups with higher education in two timepoints highlighting the importance of addressing smoking cessation counselling and health promotion campaigns in the population by different subgroups in Estonia.

Abstract Background:Online ride-hailing is a fast-developing new travel mode, and tobacco control policies on it have not yet been improved. This study aims to reveal the smoking status and influencing factors of ride-hailing drivers, so as to provide a basis for the formulation of tobacco control policies.Methods:The cross-sectional data used in this study were derived from an online survey of full-time ride-hailing drivers in China. Questionnaires were employed to collect variables including sociodemographic and work-related characteristics, health status, health behavior, health literacy, and smoking status. Chi-Square test and multivariate logistic regression were used to analyze the influencing factors of current smoking.Results:A total of 8990 ride-hailing drivers were investigated, in which 5024 were current smokers, accounted to 55.9%. Current smokers (53.7% (2696/5024) v 44.2% (1752/3966); P<0.001) and drivers who smoked on the car (85.8% (1389/1618) v 38.4 (1307/3406); P<0.001) were more likely to allow passengers to smoke. Logistic regression analysis showed that men (OR=0.519, 95%CI (0.416, 0.647)), central regions (OR=1.172, 95%CI (1.049, 1.309)), eastern regions (OR=1.330, 95%CI (1.194, 1.480)), working at both daytime and night (OR=1.287, 95%CI (1.164, 1.424)), and working at non-fixed time (OR=0.847, 95%CI (0.718, 0.999)), 35-54 years old (OR=0.585, 95%CI (0.408, 0.829)), current drinker (OR=1.663, 95%CI (1.526, 1.813)), eating very irregularly (OR=1.370, 95%CI (1.233, 1.523)), the number of days a week of engaging in at least 10 minutes of moderate or vigorous exercise ≥ 3 (OR=0.752, 95%CI (0.646, 0.875)), taking the initiative to acquire health knowledge occasionally (OR=0.882, 95%CI (0.783, 0.992)) or frequently (OR=0.675, 95%CI (0.591, 0.770)) , underweight (OR=1.249, 95%CI (1.001, 1.559)) and overweight (OR=0.846, 95%CI (0.775, 0.924)) were associated with the prevalence of current smoking among online ride-hailing drivers (P<0.05). Conclusions:The smoking rate of ride-hailing drivers was high, and the social demographic and work-related characteristics, and health-related factors all affected their smoking behavior. Tobacco control measures targeted at online-hailing drivers should correct their cultural beliefs about smoking, increase their health literacy, guide them to exercise more and keep a regular schedule, as well as combine with drinking intervention and weight intervention.

The Bacillus Calmette–Guérin (BCG) vaccine affords indirect protection against COVID-19, which is presumably due to priming of the innate immune system. It was hypothesized that the live attenuated Varicella Zoster (LAVZ) vaccine, recommended for the elderly population, would also protect against COVID-19 infection. A retrospective population-based cross-sectional study was conducted using the Leumit Health Services (LHS) database. LAVZ-vaccinated patients were matched with controls based on a propensity score model using 1:9 nearest-neighbor matching. Matching was based on age, gender, and the presence of some chronic disorders, which were selected according to their association with COVID-19 infection. Multivariate logistic regression analyses, adjusted for sex, age, smoking status, comorbidities, and chronic medications associated with COVID-19 risk, were used to estimate the association between LAVZ vaccination and COVID-19 RT-PCR results. Subjects (625) vaccinated with LAVZ and RT-PCR-tested for COVID-19 were identified. After 1:9 matching of subjects who received the LAVZ vaccine, 6250 subjects were included in the study. Multivariate logistic regression analysis demonstrated a significant and independent negative association between having received the LAVZ vaccine and the likelihood of COVID-19 infection (adjusted OR = 0.47 (95% CI 0.33–0.69, p < 0.001)). This association was further strengthened after separate analysis based on the time of LAVZ vaccination before COVID-19 RT-PCR testing. Individuals aged ≥50 years vaccinated with LAVZ had a decreased likelihood of being tested positive for COVID-19.

With relatively sufficient antenatal health service supplies in eastern rural China, the utilization still needs to be improved. The objective of this study was to identify factors that correlate with antenatal care (ANC) utilization from the demand-side in Jiangsu, China. In a cross-sectional survey, a sample of 896 rural women who had a childbearing history in the previous 5 years answered ANC questions and formed the final analysis. Questionnaire was designed based on Andersen’s behavioral model. The outcome variables included receiving times and items of prescribed ANC utilization, and the explanatory variables were organized into 3 hierarchical levels: predisposing, enabling, and need factors. Univariate analysis and multivariate logistic regression analysis were conducted. In the results of multivariate logistic regression, factors significantly associated with ANC examination times included income, odds ratio (OR) (95% confidence interval [CI]) = 2.90 (1.92-4.39); the distance from the nearest hospital, OR (95% CI) = 0.67 (0.47-0.95); chronic disease, OR (95% CI) = 1.77 (1.15-2.72); and parity, OR (95% CI) = 0.66 (0.46-0.95), while factors significantly associated with ANC examination items included education, OR (95% CI) = 8.02 (1.08-59.67); income, OR (95% CI) = 3.90 (1.72-8.85); female medical staff in towns and villages, OR (95% CI) = 2.64 (1.39-5.02); and parity, OR (95% CI) = 0.41 (0.23-0.75). In reducing inadequate ANC utilization in rural area, efforts should be made not only to target the rural women with lower income, lower educational level, and multi-parity, but also to further improve the accessibility of the primary medical facilities and female staff at the grassroots level.

Antenatal care is a service provided to pregnant women to monitor, support maternal health and detect mothers whether normal or problematic pregnant women. According to the WHO, globally more than 70% of maternal deaths are caused by complications of pregnancy and childbirth such as hemorrhage, hypertension, sepsis, and abortion. Based on data obtained from the profile of the North Sumatra provincial health office in 2017, in the city of Padangsidimpuan in 2017 the coverage of ANC visits reached (76.58%) and had not reached the target in accordance with the 2017 Provincial Health Office strategy plan (95%). This type of research was an observational analytic study with a cross sectional design. The population in this study were independent practice midwives who were in the Padangsidimpuan, the sample in this study amounted to 102 respondents. The technique of collecting data used questionnaires and data analysis used univariate, bivariate and multivariate analysis with logistic regression analysis. Based on bivariate analysis showed that there was a relationship between facilities, knowledge and attitudes of independent midwives with compliance with the standards of antenatal care services with a value of p <0.05. The results of the study with multivariate logistic regression analysis showed that the factors associated with the compliance of independent midwives in carrying out antenatal care service standards were attitudes with values (p = 0.026).
