While hybrid managers are increasingly important in contemporary organizations (especially in the public sector), we know little about why or how they become hybrid managers, or how this is shaped by the interplay of professional experience and organizational circumstances. In pursuit of a more variegated, contextualized and dynamic understanding of hybrid management, this article focuses on how individuals transition into managerial hybrids, emphasizing the dynamic and emergent nature of hybrid management identity. Studying managers in English healthcare, we employ the concept of identity work as expressed through career narratives to examine the influence of career trajectories and organizational experiences on emerging hybrid manager identity. The study identifies three broad managerial career narratives – aspirational, ambivalent and agnostic – and relates them to experiences of doctor and nurse hybrid managers in three healthcare settings. An interpretive analysis of these narratives reveals a more variegated, situated and dynamic interpretation of hybrid managerial identities than previously considered and underscores the importance of personal and organizational experiences in shaping emergent hybrid professional/managerial identity.

Both the recent MERS-CoV and the past SARS-CoV indicate that new pathogens would probably emerge. Because it is not possible to predict when or where a new epidemic would occur, this continues to be a challenging issue for physicians and healthcare organizations. Furthermore, there are prophylactic vaccines or effective treatment for these infections and little is known about the origin and the zoonotic transmission of MERS-CoV which hinders the progress of its spread to humans. MERS-CoV is highly pathogenic, exhibiting high fatality rate than the former human corona virus SARS and can obviously be transmitted through several routes, with higher incidence in compromised healthcare settings. Currently, efforts to manage MERS-CoV spared should be directed towards developing educational programs, targeting the public and more importantly health care providers. For one major concern, this infection has and still could pose potential to spread rapidly across the globe, especially during religious mass gathering originating from a MERS-CoV hot spot (i.e., Hajj). Continued epidemiologic surveillance and vigilance remains crucial to compact this virus, or any future mutation.

This article focuses upon gender differences in the satisfaction with career opportunities of civil servants in the Netherlands. Women have become better represented at all levels in the Dutch civil service in recent years, but they are still underrepresented in the higher level positions. Nevertheless, women are slightly more satisfied with their career opportunities than men are and they seem to be increasingly so. Their relatively positive evaluation of extrinsic aspects of their work situation is one of the explanations of this finding, as is their higher intrinsic work motivation compared to that of men. It is suggested that the career orientations and aspirations of women better fit the changing context of career formation in the Dutch civil service and the accompanying new psychological contract. Points for practitioners The Dutch civil service has set ambitious targets with respect to the representation of women at all levels in the service, but insight into the determinants of women’s careers in the public sector is still very scarce. The findings in this article show that there are gender differences in the weighting of intrinsic and extrinsic aspects of the work in evaluating one’s career opportunities. Besides, the article may assist human resource management practitioners in anticipating the impact of changing career trajectories on the career satisfaction of male and female civil servants.

Due to weight stigma, obese and overweight individuals are stereotyped, rejected and victimised by the public. The pervasion of this discrimination in healthcare settings is concerning given that the healthcare setting plays a crucial role in influencing health and shaping perception of health conditions. Without adequate support and little public attention, obese individuals are primarily left on their own to deal with ongoing prejudice unless addressed through vigorous research and strategic interventions at individual, interpersonal, organisational and community levels, and through policy implementation.

Abysmal state of policies governing the health plan providers lead to a huge discontent amongst the public in regards to their health plan besides privacy and security of their medical records. Anyone with access to the patient's medical records could potentially share it with parties like health plan providers or the employers. To address the privacy and the security of patient's medical records, Congress enacted HIPAA in 1996. Chapter starts with discussing the need for HIPAA. Subsequently, we discuss HIPAA at considerable depth. Significant additions and changes were made in subsequent acts and amendments due to pressing policy needs and to address various loopholes. The chapter provides a chronological recount of HIPAA since its introduction. Once the reader develops a complete understanding of HIPAA regulation, we shift our focus to the compliance to HIPAA. We delve deeper into implications of HIPAA on healthcare organizations and the information technology world.


Physicians who choose to serve in public sector mental healthcare settings and physicians-in-training assigned to public sector mental health clinics may not be fully prepared for the many roles of the public and community psychiatrist. This primer offers practical information and guidance to the psychiatrist called upon to serve in the roles of public-sector clinician, team member, advocate, administrator, and academician. Each chapter includes a concise description of these various roles and responsibilities and offers engaging examples of the public psychiatrist at work. The chapters also ask readers to thoughtfully consider case-based problems typical of those faced by the public psychiatrist. Each chapter also features works of art and literature, usually from the public domain. Medical humanities help physicians keep sight of the lived experiences of public-sector patients; this includes not only the pain and suffering endured by them due to both the medical disorders with which they live and the disparities they endure in health, educational and occupational outcomes, but also their resilience while facing so many challenges. Medical humanities also serve to reinforce the physician’s individual and collective will to address the disparities endured by our patients. There are several very comprehensive textbooks available that examine community psychiatry broadly. By contrast, this work is a concise guide for the resident and early-career psychiatrist to the many roles he or she might be asked to provide in a public-sector mental health setting. Our hope is that the primer provides a level of support to psychiatrists that fosters their desire, individually and collectively, to serve the poor and the marginalized with grit and determination, and to broadly consider their potential to improve not only patient well-being but also these patients’ incorporation into their communities.

Even though workflow management systems are currently not being applied on a wide scale in healthcare settings, their benefits with respect to operational efficiency and reducing patient risk seem enticing. The authors show how an approach that is rooted in simulation can be useful to predict the benefits of using a workflow management system. The approach is discussed and its application is demonstrated in the setting of the pre-operative process as being executed in the Bronovo hospital. The approach is considered useful for other healthcare organizations in search for a better foundation for the application of workflow technology.

AbstractConflicts in healthcare settings are quite common events because of the continuous changes and transformations today’s healthcare organizations are undergoing and the vigorous interaction between the medical professionals working in them. AIM: To survey the opinions of medical professionals about the possible destructive effects of conflicts on them in the workplace. MATERIALS AND METHODS: We conducted a direct individual survey of 279 medical employees at four general hospitals. We used a set of questions that reflect the negative effects and consequences of conflict on healthcare professionals as direct or indirect participants. All data were analysed using the descriptive statistics and non-parametric analysis at a significance level for the null hypothesis of p < 0.05. RESULTS: Workplace conflicts contribute a lot to the stress, psychological tension and emotional exhaustion medical professionals are exposed to. The confrontation the conflict brings the participants into acts as a catalyst of the conflict and enhances the manifestation of hostile actions. A conflict generates a situation which has an impact on the behaviour of all participants involved in it giving rise to emotional states such as anger, aggression and reproaches. The destructive consequences resulting from a conflict are seen in the reduced work satisfaction and demotivation to perform the work activity. The contradictions that arise as a result affect negatively the team cooperation and obstruct the collaborative efforts in solving the problems in the healthcare setting. CONCLUSION: A conflict in a healthcare setting exerts a considerable destructive effect on an employee, therefore it requires prompt identification and effective intervention to minimise its unfavourable outcomes.

Purpose – The purpose of this paper is to analyze the structural problems emerging in the course of managing and safeguarding a French association for home care to a thousand elderly or disabled people between 2007 and 2012, employing 150 - 190 people and on the verge of bankruptcy. In France, small local businesses not only compete with major capital outlets in this sector but also with associations of varying size and origin. Free market rules apply, under the legislation of 2003, to what is, in part, “competition free”, being “in the public interest” and within the framework of local and national public funding. Design/methodology/approach – This paper analyses those pragmatic solutions put in place to meet the aim of shared governance and in the context of a generalized financial crisis. Findings – Borrowing from cooperatives and associations, the non-profit-based management structure the authors arrived at, including worker participation in the decision-making processes, raises questions for researchers as to the advisability of any short-term models and the validity of present social and supportive economic models. Originality/value – The hybrid management of this paper is offered as a working model in what the authors have termed an “adhocracy of stakeholders”.

The linguistic-communicative paradigm offers some interesting perspectives in a context where the perception of patient needs is considered a critical step in high-quality care. This study describes healthcare organizations as linguistic communities based on the conceptual framework of Habermas' communicative action theory. Four communicative models are present in healthcare settings: objectifying-instrumental (hegemonic model), where elements of interaction are objectified for clinical purposes; dialogic model with strategic perspectives, in which conversations are used unilaterally as tools to access subjective states; non-dialogic-transmissional model, in which linguistic exchanges are replaced with artifacts to transmit information; and full communicative model (present in palliative care based in homecare and informal caregivers, emphasizing health team/family interactions). Based on these premises, we considered palliative care an emblematic communicative model based on multidisciplinary teams devoted to transdisciplinary collaboration. In these settings, linguistic interaction with patients and their families could provide a solid basis for organization of healthcare networks.
