Subjective tinnitus is an enigmatic and chronic condition that is predominantly managed as symptomatic. Little high-level evidence exists for the efficacy and specificity of the various tinnitus management strategies currently used, and this is reflected in documents that aim to guide clinicians. As a consequence, there are clear gaps in evidence-based practice linking diagnosis to the most effective management strategies as well as a general lack of consensus about which are appropriate strategies for assessment and management. Several guidelines have been produced from research efforts and from expert opinion. All recommend standardization of assessment and a range of management options but do not yet provide a means to link the two. The authors call for clinicians, scientists, and policy makers to work together to address this barrier to good practice.

Objective: To formulate a protocol for safe and effective management of an ophthalmologic practice in a hospital or a clinic during the Covid - 19 pandemic. Methods and analysis: Literature was reviewed regarding Covid-19 pandemic and ophthalmology. The following key words were used. Covid-19, telemedicine, challenges, management strategy. Result: An efficient protocol for ophthalmology practice during the Covid-19 pandemic was established which covered various protective measures that are essential and vital in the Covid-19 pandemic. Conclusion: An evidence-based protocol was formulated which may be modified in future as per the latest developments and trends in the ongoing pandemic.

Increasingly, 18–24-year-old women from hereditary breast/ovarian cancer (HBOC) families are pursuing genetic testing, despite their low absolute risks of breast and ovarian cancer and the fact that evidence-based management options used with older high-risk women are not generally available. Difficult clinical decisions in older carriers take on substantially more complexity and value-laden import in very young carriers. As a result, many of the latter receive highly personal and emotionally charged cancer risk information in a life context where management strategies are not well defined. We analyzed 32 in-depth interviews with BRCA1/2 mutation-positive women aged 18–24 using techniques of grounded theory and interpretive description. Participants described feeling vulnerable to a cancer diagnosis but in a quandary regarding their care because evidence-based approaches to management have not been developed and clinical trials have not been undertaken. Our participants demonstrated a wide range of genetic and health literacy. Inconsistent recommendations, surveillance fatigue, and the unpredictability of their having health insurance coverage for surgical risk-reducing procedures led several to contemplate risk-reducing mastectomy before age 25. Parents remained a primary source of emotional and financial support, slowing age-appropriate independence and complicating patient privacy. Our findings suggest that, for 18–24-year-olds, readiness to autonomously elect genetic testing, to fully understand and act on genetic information, and to confidently make decisions with life-long implications are all evolving processes. We comment on the tensions between informed consent, privacy, and the unique developmental needs of BRCA1/2 mutation-positive women just emerging into their adult years.

There is a desire among many tinnitus researchers and clinicians for greater standardization in the assessment and management of tinnitus. In their commentary on the complexity of tinnitus, Hoare et al. have highlighted the need for strong evidence-based protocols. The authors make many valid and important conclusions as to the current state of clinical guidelines; they identify clear gaps in evidence and limited consensus as to clinical practice. While I agree with the bulk of their commentary there are a few areas where I will offer a counter view. In particular I will address their view that only high-level evidence has a place in forming practice guidelines for tinnitus.

Around two-thirds of the earth’s surface covered with water, it is obvious that water is among the most significant aspects that are essential for the life of human being. It is not only important to live, but also important to take a part in social and financial development. Water is God’s personalized gift, as well as the necessity of life. Due to high level of gluttony, misused play of people with the environment and the global climate, shift results in progressive diminishing of natural resources. The problems of freshwater and food security at global level linked to the overwhelming population of the world. To overcome the food and water scarcity challenge, there is required to transfer the freshwater from agriculture sector to other straining purposes. Pakistan is facing the meager water scarcity crises in the last few years, as water is becoming scarce to any specified usage. The quickly growing population, expansion of drainage regions, increasing urbanization and industrialization put a lot of stress on available water supplies. At the moment, there is dire need to harvest rain water by constructing more dams and focus on effective management strategies for further use. Specifically, authors are recommended that, water should be provided demand base instead of supply base irrigation system. Moreover, in addition to land tax, tax on irrigation water quantity/usage should be rectified.

On Thursday, 30 April 2020, the EdTech Hub participated in an “Ask me anything” session for policy-makers and funders in Nepal. The session focused on designing high-quality, effective, distance education programmes during the COVID-19 pandemic. Participants included high-level officials from the Nepalese government (e.g., the Ministry of Education, Science and Technology, the Curriculum Development Office and the Education Review Office), representatives from development partners (e.g., the World Bank, UNICEF and USAID) and other education organisations (e.g., OLE Nepal).  The session was convened for two purposes. First, to consider international good practice and current trends in distance education during the COVID-19 pandemic, presented by the World Bank EduTech team and the EdTech Hub. Second, for the EdTech Hub team to gather questions from participants, to be able to target guidance specifically to the situation in Nepal.  This document provides answers to a consolidated list of 10 questions received from stakeholders during the session. To consolidate any overlap, we have occasionally combined multiple questions into one. In other cases, where multiple important issues required a focused response, we split apart questions.

Infertility is defined as a failure to achieve a positive pregnancy test over 12 months of regular unprotected sex. It is a devastating condition affecting 15% of couples and is socially marginalizing. Despite significant focus on the female, nearly half of cases are in fact due to poor sperm function in the male partner. It is therefore surprising that despite the existence of numerous diagnostic tools and management options currently available for female infertility, very few exist for their male counterparts. So, what can we do to diagnose men with impaired fertility promptly and direct couples to effective management strategies?

Dittrichia viscosa (L.) Greuter is a shrub native to the Mediterranean, however, declared as a very invasive species in Australia and North America. Environmental (climatic) and socio-economic (land abandonment) changes can trigger different adaptive mechanisms and cause changes in species behavior, influencing invasion dynamics. Motivated by the recently noticed change of D. viscosa behavior in its native Mediterranean habitat, we discuss the invasion properties, its behavior in the native habitat and new areas, and its management options. We review the species’ adverse effects and its positive ecosystem services in the Millennium Ecosystem Assessment framework. In this review, we provide information on the phytochemical properties of D. viscosa and highlight its potential use in ecological agriculture, phytopharmacy, and medicine. The presented data is useful for developing effective management of this contentious species, with emphasis on mitigating environmental and economic damages, especially in agriculture. The final aim is to achieve a balanced ecosystem, providing a high level of possible services (provisioning, regulating, cultural and supporting).

The management of symptomatic osteochondral lesions of the talus (OLTs) can be challenging. The number of ways of treating these lesions has increased considerably during the last decade, with published studies often providing conflicting, low-level evidence. This paper aims to present an up-to-date concise overview of the best evidence for the surgical treatment of OLTs. Management options are reviewed based on the size of the lesion and include bone marrow stimulation, bone grafting options, drilling techniques, biological preparations, and resurfacing. Although many of these techniques have shown promising results, there remains little high level evidence, and further large scale prospective studies and systematic reviews will be required to identify the optimal form of treatment for these lesions. Although generalized tonic-clonic status epilepticus (SE) is frequently seen, an evidence-based approach to management is limited by a lack of randomized clinical studies. Clinical practice, therefore, relies on a combination of expert recommendations, local hospital guidelines and dogma based on individual preference and past successes. This review explores selected and controversial aspects of SE in adults and provides a critical appraisal of currently recommended management strategies.

Plant diseases caused by vector-transmitted viruses like begomovirus inflict serious damage in terms of yield and colossal loss in many crops, predominantly Chilli. Coupled with bleak farm awareness and lack of management options, they could severely hamper remuneration prospects. In this connection, the profitability of the farm level begomovirus management strategies has been analyzed in this study. A sample of 100 chilli farmers were surveyed with focus on their perceptions of begomovirus caused diseases. Findings revealed a majority of farmers (80 per cent) experienced cross loss due to viral diseases. Positive changes in output and production cost were also reported with the adoption of management strategies, which in turn were categorized in to: low, medium and high. Besides, it was also revealed that when compared with no management strategy, the change in the gross margin was 17 per cent, 26 per cent and 80 per cent after the adoption of low, medium and high level of management strategies, respectively.
