Enrolling Caregivers in Obesity Interventions to Improve Obesity-Related Outcomes in Children

In this study, we assessed the influences of change in moderate-to-vigorous physical activity (MVPA)/sedentary time (ST) of caregivers participating in a commercial weight-loss program on their children’s change in MVPA/ST. Data from 29 caregivers and their children were collected over 8 weeks. We used multivariable linear regression to assess associations of changes in caregiver’s percent of time spent in MVPA/ST and changes in their child’s percent of time spent in MVPA/ST. For caregivers that decreased body mass index (BMI) over 8 weeks, changes in caregivers’ MVPA was strongly associated with the change in children’s MVPA (β = 2.61 [95% CI: 0.45, 4.77]) compared to caregivers who maintained/increased BMI (β = 0.24 [–2.16, 2.64]). Changes in caregivers’ ST was strongly associated with changes in children’s ST (β = 2.42 [1.02, 3.81]) compared to caregivers who maintained/increased BMI (β = 0.35 [–0.45, 1.14]). Findings reinforce encouraging caregivers to enroll in weight-loss programs for the benefit of their children as well as for themselves.

Introduction: Much interventional research seeks to reduce childhood obesity by capitalizing on parents’ influence as caregivers. However, few studies have investigated whether grandparent-caregivers might have a similar influence on children’s weight-related health. This dearth of evidence is surprising since there has been a rapid increase in grandparent caregiving in the U.S. Purpose: Examine for a ripple effect on weight-related outcomes in children whose grandparent-caregivers have been enrolled in a weight loss program. Methods: We recruited 9 grandparent-grandchild (aged 6 to 12 years) dyads where both members had overweight or obesity. We collected data on weight/height, physical activity/sedentary time, and dietary behaviors at baseline and at 8 weeks. Physical activity and sedentary time data were collected using accelerometers over two, seven-day cycles (after baseline and 8-weeks’ visits). Dietary data were collected using the Block Food Screener. Spearman’s rank-order correlation was used to quantify associations in changes between the dyad members. Results: Mean age was 64.3 ± 5.2 years and 9.0 ± 1.7 years for grandparents and grandchildren, respectively; 88.9% of grandparents and 66.7% of grandchildren were female. At 8 weeks: (1) there was a positive association between changes in grandparents’ BMI and changes in their grandchildren’s BMI (r=0.40; p<0.01); (2) changes in grandparents’ levels of moderate-to-vigorous physical activity (MVPA)/sedentary time was positively associated with changes in grandchildren’s levels of MVPA/sedentary time (r=0.52, p<0.01; r=0.55, p<0.01, respectively). Conclusions: We demonstrated moderate associations between changes in children’s BMI/MVPA/sedentary time and changes in their grandparents’ BMI/MVPA/sedentary time. Our findings show that enrolling adult caregivers in weight loss programs could represent an effective delivery vehicle for improving childhood obesity. Additionally, our findings can help demonstrate to grandparent-caregivers that they can act as agents for improving health outcomes in their grandchildren by improving their own weight-related health, offering encouragement for grandparent-caregivers to enroll in weight-loss programs.

Men are underrepresented in weight loss programs and little is currently known about the weight loss strategies men prefer. This study describes the weight loss strategies used by men during a men-only weight loss program. At baseline, 3 months, and 6 months, participants reported how frequently they used 45 weight loss strategies including strategies frequently recommended by the program (i.e., mentioned during every intervention contact; e.g., daily self-weighing), strategies occasionally recommended by the program (i.e., mentioned at least once during the program; e.g., reduce calories from beverages), and strategies not included in the program (e.g., increase daily steps). At baseline participants ( N = 107, 44.2 years, body mass index = 31.4 kg/m2, 76.6% White) reported regularly using 7.3 ± 6.6 ( M ± SD) strategies. The intervention group increased the number of strategies used to 19.1 ± 8.3 at 3 months and 17.1 ± 8.4 at 6 months with no changes in the waitlist group. The intervention group reported increased use of most of the strategies frequently recommended by the program (4 of 5), nearly half of the strategies occasionally recommended by the program (10 of 24), and one strategy not included in the program (of 16) at 6 months. The intervention effect at 6 months was significantly mediated by the number of strategies used at 3 months. This study adds to what is known about men’s use of weight loss strategies prior to and during a formal weight loss program and will help future program developers create programs that are tailored to men.

ABSTRACT OBJECTIVE The objective of this study is to establish cutoff points for the number of steps/day and minutes/day of moderate to vigorous physical activity in relation to the risk of childhood overweight and obesity and their respective associations. In addition, we aim to identify the amount of steps/day needed to achieve the recommendation of moderate to vigorous physical activity in children from São Caetano do Sul. METHODS In total, 494 children have used an accelerometer to monitor steps/day and the intensity of physical activity (min/day). The moderate to vigorous physical activity has been categorized according to the public health recommendation (≤ 60 versus > 60 min/day). Overweight or obesity is defined as body mass index > +1 SD, based on reference data from the World Health Organization. The data on family income, education of parents, screen time, diet pattern, and sedentary time have been collected by questionnaires. Logistic regression and Receiver Operating Characteristic curves have been constructed. RESULTS On average, boys walked more steps/day (1,850) and performed more min/day of moderate to vigorous physical activity (23.1) than girls. Overall, 51.4% of the children have been classified as eutrophic and 48.6% as overweight or obese. Eutrophic boys walked 1,525 steps/day and performed 18.6 minutes/day more of moderate to vigorous physical activity than those with overweight/obesity (p < 0.05). The same has not been found in girls (p > 0.05). The cutoff points to prevent overweight and obesity in boys and girls were 10,500 and 8,500 steps/day and 66 and 46 min/day of moderate to vigorous physical activity, respectively. The walking of 9,700 steps/day for boys and 9,400 steps/day for girls ensures the scope of the recommendation of moderate to vigorous physical activity. CONCLUSIONS In boys, steps/day and moderate to vigorous physical activity have been negatively associated with body mass index, regardless of race, family income, education of parents, screen time, diet pattern, and sedentary time. We suggest, for steps/day and moderate to vigorous physical activity, studies with different ages and populations, with different designs, so as to inform the cause and effect relationship with various health parameters.

Objectives: Little is known about how children’s health might be affected by parents’ participation in commercial weight loss programs. Given that more than 3.2 million people subscribe to just one commercial weight loss program, Weight Watchers, any secondary effects on children’s weight-related health behaviors (e.g. dietary behaviors, physical activity, and sedentary time) and body mass index from parent participation in commercial weight loss programs may have significant public health implications. This study examined the feasibility of conducting a study to assess such effects. Methods for recruitment and retention, and perceived acceptability and satisfaction among participants in small-scale preliminary study, were evaluated. Changes in body mass index and health behaviors among the parent–child dyads were also measured to test whether a larger-scale study would be warranted. Methods: This was an 8-week, pre–post observational feasibility study involving 20 parent–child dyads where both members had overweight or obesity. Physical and behavioral data were collected at baseline and 8 weeks from both members of the dyads. Parenting data were collected at the same time periods through parents’ self-report. Bivariate correlation was used to quantify the associations in changes for dyad members. Results: Feasibility goals for retention and perceived acceptability/satisfaction among participants were met. We reached approximately 80% of our enrollment goal. Parents showed a significant decrease in body mass index of 0.53 points (p < 0.05), while children showed a significant increase in raw body mass index (0.42, p < 0.05) and body mass index percentile (0.59, p < 0.05). However, correlation between changes in parent body mass index and changes in children body mass index percentile was positively correlated (r = 0.24, p = 0.31). A decrease in parent intake of total fat was associated with a decrease in the intake of fat in their children (r = 0.47, p < 0.05). Conclusion: Our findings support the viability of a larger-scale follow-up to assess the potential of using parent-only commercial weight loss program as a mechanism for improving health behaviors and body mass index in children with overweight or obesity.

The purpose of this study was to determine where men fall on the motivation continuum based on intuitive eating status and if motivation for physical activity and intuitive eating are correlated. Results indicate that being an intuitive eater was associated with a lower body mass index. In terms of demographic variables, as age increased, intuitive eating status decreased and body mass index increased. Men scored high on the antidieting and self-care subscales of the Intuitive Eating Scale. Men who were classified as intuitive eaters scored higher on the external and introjected regulation of the Behavioral Regulation in Exercise Questionnaire but no significant differences were reported by eating status and the identified and intrinsic motivation subscales. Intuitive eating holds promise as a weight management and weight loss tool for men. Intuitive eating may also influence initial motivation for physical activity for men.

We investigated if cardiorespiratory fitness modifies the association between obesity and the level of physical activity. In this cross-sectional study, we analyzed data from 746 adults, free of diagnosed cardiorespiratory or locomotor diseases. We analyzed sociodemographic and clinical information, cardiovascular risk factors, cardiorespiratory fitness, anthropometry, and level of physical activity (time spent in moderate-to-vigorous physical activity). Those that spent more time in moderate-to-vigorous physical activity were younger, male, with lower body mass index, without self-reported arterial blood hypertension, diabetes and dyslipidemia, non-smokers, and presented with better cardiorespiratory fitness. The linear regression coefficients showed that cardiorespiratory fitness changes according to the level of physical activity and body mass index (obesity in low cardiorespiratory fitness: β 6.0, p = 0.213, 95%CI -3.5 to 15.6; in intermediate cardiorespiratory fitness: β 6.3, p = 0.114, 95%CI -1.5 to 14.2; in high cardiorespiratory fitness: β -6.3, p = 0.304, 95%CI -18.4 to 5.8). This effect modification trend was present after adjusting the model by covariates. Cardiorespiratory fitness potentially modifies the association between body mass index and the level of physical activity. It should be routinely assessed to identify persons with overweight/ obesity with low/ intermediate cardiorespiratory fitness to prescribe individualized training.

Background: The obesity epidemic is a global concern. Standard behavioral treatment including increased physical activity, reduced energy intake, and behavioral change counseling is an effective lifestyle intervention for weight loss. Purpose: To identify distinct step count patterns among weight loss intervention participants, examine weight loss differences by trajectory group, and examine baseline factors associated with trajectory group membership. Methods: Both groups received group-based standard behavioral treatment while the experimental group received up to 30 additional, one-on-one self-efficacy enhancement sessions. Data were analyzed using group-based trajectory modeling, analysis of variance, chi-square tests, and multinomial logistic regression. Results: Participants (N = 120) were mostly female (81.8%) and white (73.6%) with a mean (SD) body mass index of 33.2 (3.8) kg/m2. Four step count trajectory groups were identified: active (>10,000 steps/day; 11.7%), somewhat active (7500–10,000 steps/day; 28.3%), low active (5000–7500 steps/day; 27.5%), and sedentary (<5000 steps/day; 32.5%). Percent weight loss at 12 months increased incrementally by trajectory group (5.1% [5.7%], 7.8% [6.9%], 8.0% [7.4%], and 13.63% [7.0%], respectively; P = .001). At baseline, lower body mass index and higher perceived health predicted membership in the better performing trajectory groups. Conclusions: Within a larger group of adults in a weight loss intervention, 4 distinct trajectory groups were identified and group membership was associated with differential weight loss.

Objective: The objective of this study was twofold. First, analyze physical activity (PA) levels during physical education (PE) with different durations (45 and 90 minutes) according to student’s obesity status. Secondly, we examine the relative contribution of 45 and 90 minutes PE (45PE and 90PE) for the compliance of the daily PA recommendations according to the body mass index (BMI). Methods: Four public schools were analyzed. The sample comprised 472 youngsters (266 girls) aged between 10 and 18 years old. PA was assessed using an Actigraph accelerometer. The participants were categorized as non-overweight (NOW) and overweight/obese (OW) according to the sex-adjusted BMI. Results: The proportion of Moderate and Vigorous Physical Activity (MVPA) was lower than the 50% recommended by guidelines regardless the PE duration. Our data showed that only 26% of NOW and 13% of OW in the 45PE achieved the recommended levels while 17% of NOW and 11% of OW achieved the recommendation in 90PE. Overall, the 90PE had a higher absolute contribution for daily MVPA recommendations compliance than 45PE. Conclusion: During PE classes youngsters spent a reduced amount of time in MVPA, independently of their weight status.Resumen. Objetivo: Este estudio tiene un doble objetivo. En primero lugar, analizar los niveles de la actividad física durante la educación física con diferentes duraciones (45 y 90 minutos) de acuerdo con el estado de la obesidad de los alumnos. En segundo lugar, se analiza la contribución relativa de 45 y 90 minutos de la educación física para el cumplimiento de las recomendaciones diarias de actividad física según el índice de masa grasa. Métodos: cuatro escuelas públicas fueran analizadas. La muestra fue de 472 jóvenes (266 chicas) con edades entre los 10 y 18 años. La actividad física fue medida utilizando un acelerómetro Actigraph. Los participantes fueran clasificados como sin sobrepeso y con sobrepeso/obesidad de acuerdo con el índice de masa grasa ajustado al género. Resultados: La proporción de la actividad física moderada y vigorosa fue inferior al 50% recomendado por las recomendaciones independiente de la duración de la clase de educación física. Nuestros datos muestran que solo unos 26% de los niños sin sobrepeso y unos 13% de niñoss con sobrepeso/obesidad llegaron al los niveles recomendados en las clases de 45 min, mientras el 17% de los jóvenes sin sobrepeso y el 11% con sobrepeso/obesidad han logrado las recomendaciones en las clases de 90 min. En general, las clases de 90 min tienen una mayor contribución para cumplimiento de las recomendaciones diarias de actividad física moderada a vigorosa do que las clases de 45 min. Conclusión: Durante las clases de educación física los jóvenes tuvieran una cantidad reducida de tiempo en actividad física moderada a vigorosa, independiente de su estado de peso.

BACKGROUND Although millions of overweight and obese adults use mobile phone apps for weight loss, little is known about the predictors of success. OBJECTIVE The objective of this study was to understand the relationship between weight loss outcomes and weekly patterns of caloric intake among overweight and obese adults using a mobile phone app for weight loss. METHODS We examined the relationship between weekly patterns of caloric intake and weight loss outcomes among adults who began using a weight loss app in January 2016 and continued consistent use for at least 5 months (N=7007). Unadjusted and adjusted linear regression analyses were used to evaluate the predictors of percentage of bodyweight lost for women and men separately, including age, body mass index category, weight loss plan, and difference in daily calories consumed on weekend days (Saturday and Sunday) versus Monday. RESULTS In adjusted linear regression, percentage of bodyweight lost was significantly associated with age (for women), body mass index (for men), weight loss plan, and differences in daily caloric intake on Mondays versus weekend days. Compared with women consuming at least 500 calories more on weekend days than on Mondays, those who consumed 50 to 250 calories more on weekend days or those with balanced consumption (±50 calories) lost 1.64% more and 1.82% more bodyweight, respectively. Women consuming 250 to 500 calories or more than 500 calories more on Mondays than on weekend days lost 1.35% more and 3.58% more bodyweight, respectively. Compared with men consuming at least 500 calories more on weekend days than on Mondays, those consuming 250 to 500 calories or more than 500 calories more on Mondays than on weekend days lost 2.27% and 3.42% less bodyweight, respectively. CONCLUSIONS Consistent caloric intake on weekend days and Mondays or consuming slightly fewer calories per day on Mondays versus weekend days was associated with more successful weight loss.
