Factors Associated with Severe Periodontitis in Rural Cambodia

An analytical cross-sectional study was conducted with 812 subjects aged 35-74 years in Prek Russey commune, Cambodia to detect some factors associated with severe periodontitis. Four calibrated examiners employed Community Periodontal Index (CPI) to ascertain periodontal pocket depth (PD) and loss of epithelial attachment (LA). A questionnaire was used to obtain tobacco and alcohol consumption habits. One hundred (12.3%) subjects had one or more sextants with ≥ 4mm PD and ≥ 6mm LA (severe periodontitis). Smoking, tobacco chewing and age were significantly associated with severe periodontitis in bivariate analysis while only age and quantified smoking remained significant in a logistic regression analysis. The impact of quantified smoking on severe periodontitis was almost negligible [OR: 1.0006 (95% CI: 1.0001, 1.001)] compared to that of age [OR ranged from 8.3 at 45-54 years to 22 at 65-74 years (95% CI: 3.1, 62.3)]. Nevertheless, both age and quantified smoking can be considered as factors associated with severe periodontitis in rural Cambodians. Asia Pac J Public Health 2004; 16(1): 50-53.

Abstract Background This study was aimed to determine the levels of salivary receptor activator of NF-κB ligand (RANKL) and osteoprotegerin (OPG) and its association with periodontal status among periodontitis patients. Patients and Methods A cross-sectional study was designed and performed at the Dental Clinic, Hospital Universiti Sains Malaysia (HUSM). Random sampling was employed to identify 88 participants into three groups: 30 mild periodontitis, 30 moderate to severe periodontitis, and 28 healthy (nonperiodontitis) patients. Periodontal parameters: periodontal pocket depth (PPD), clinical attachment level (CAL), plaque score (PS), and gingival bleeding index (GBI) were recorded. In total, 4 mL of unstimulated whole saliva was collected to determine the levels of salivary RANKL and OPG proteins by using ELISA technique. Data were analyzed by using SPSS software version 24.0. Results Mean values for PPD (5.3 ± 0.5) and CAL (5.6 ± 0.5) were observed higher for moderate to severe periodontitis as compared with values (4.4 ± 0.2) (4.5 ± 0.2) in mild periodontitis patients. The mean salivary RANKL and OPG was 0.23 ± 0.07 ng/mL and 1.78 ± 0.70 ng/mL respectively in moderate to severe periodontitis. Only salivary RANKL levels were significantly and positively correlated with all the clinical periodontal parameters. Conclusion The levels of salivary RANKL were higher as opposed to lower OPG levels in periodontitis patients in contrast to healthy (nonperiodontitis) patients. RANKL levels were significantly associated with the periodontal parameters. Therefore, we can conclude that RANKL can potentially aid as an adjunctive diagnostic protein in evaluating periodontal disease.

Abstract: Elderly is a phase of declinemind and physical abilities caused by various degenerative diseases, environmental conditions and lifestyles. Changes that occur can lead the elderly to become susceptible to various diseases and one of it in oral is periodontal disease. The purpose of this research is to find out description of periodontal status and treatment need on elderly.This research is a descriptive research of Cross Sectional Study. The research did in the village of Ratatotok Muara with the community sample aged ≥ 55 years as many as 41 responden.Periodontal status examination performed by using an index of the WHO Community periodontal index of treatment needs (CPITN).Results showed that there was no healty periodontal, 1 person (2,44%) had bleeding on probing, 17 person (41,46%) have calculus, 19 orang (46,34%) had periodontal pocket 4-5 mm and patient with periodontal pocket ≥ 6 mm were 4 person (9,76%). Key words: Elderly, periodontal disease, CPITN.     Abstrak: Usia lanjut adalah fase menurunnya kemampuan akal dan fisik akibat berbagai penyakit degeneratif, kondisi lingkungan serta gaya hidup. Perubahan yang terjadi mengakibatkan usia lanjut rentan terhadap berbagai penyakit termasuk penyakit periodontal. Tujuan dari penelitian ini yaitu untuk mengetahui gambaran status periodontal dan kebutuhan perawatan periodontal pada usia lanjut. Penelitian ini merupakan suatu penelitian deskriptif dengan pendekatan Study Cross Sectional.Penelitian dilakukan di desa Ratatotok Muara dengan sampel masyarakat yang berusia ≥ 55 tahun sebanyak 41 orang.Pemeriksaan status periodontal dilakukan dengan menggunakan indeks dari WHO yaitu Community index of periodontal treatment needs (CPITN). Hasil penelitian menunjukkan bahwa tidak ditemukan periodontal sehat, 1 orang (2,44%) yang mengalami perdarahan pada saat probing, 17 orang (41,46%) yang memiliki karang gigi, 19 orang (46,34%) yang mengalami poket periodontal 4-5 mm dan sebanyak 4 orang (9,76%) yang mengalami poket periodontal ≥ 6 mm. Kata kunci : Usia lanjut, penyakit periodontal, CPITN.

Objective. To evaluate if nutritional status is associated with caries and gingivitis in Brazilian schoolchildren. Material and methods. Children of both genders, age ranging from 8 to 11 years old, were included in this study. Caries was diagnosed using ICDAS (International System for Detection and Assessment of Carious Lesions) and gingivitis was diagnosed using the Community Periodontal Index. The nutritional status of each child was defined by BMI Z-score calculation. Data on oral health behavior and dietary habit were collected through parent’s questionnaires. Parametric analyzes were performed to compare the groups. The established alpha was 5%. Results. The sample consisted of 353 schoolchildren: 16 underweight children, 247 eutrophic children, 64 overweight children, and 26 were obese children. Overweight, Obese and Overweight + Obese children presented less cavitated caries lesion than Eutrophic children ( P < .05). Gingivitis was not associated with nutritional status ( P > .05). Conclusion. Caries was associated with overweight and obesity in Brazilian schoolchildren.

Abstract Objective: to verify the prevalence of and factors associated with polypharmacy among elderly residents of the city of Cuiabá, in the state of Mato Grosso. Method: a cross-sectional study of 573 people aged 60 and over was performed. Polypharmacy was defined as the use of five or more medications. To investigate the association between polypharmacy and sociodemographic variables, health and access to medication, the Mantel Haenszel chi square test was used in bivariate analysis and Poisson regression was used in multivariate analysis. The significance level adopted was 5%. Result: the prevalence of polypharmacy was 10.30%. Statistically significant associations were found between polypharmacy and living with others, describing suffering from circulatory, endocrine, nutritional and digestive tract diseases, and referring to financial difficulties for the purchase of medicines. Conclusion: some social and health condition factors play an important role in the use of multiple medications among the elderly.

The aim of the study was to determine influence of intellectually disability (ID) on oral hygiene status (OHS), periodontal status (PS), and periodontal treatment needs (PTNs) and investigate the association of these clinical parameters with various sociodemographic and clinical variables among multicentric institutionalized ID subjects aged 5–55 years in Kolhapur district, Maharashtra, India. This descriptive cross-sectional study was conducted by both questionnaire and clinical examination. OHS, PS, and PTNs of these subjects were assessed by recording Simplified Oral Hygiene Index (OHI-S) and Community Periodontal Index of Treatment Needs, respectively. Analysis of variance and multiple logistic and stepwise linear regression analysis were used for statistical analysis. Of the 576 subjects examined, the mean age of the subjects was 17.50 ± 7.4 years. Subjects with profound level of ID had the highest mean OHI-S, PS, and PTNs scores of 4.47 ± 1.04, 2.84 ± 0.78, and 2.25 ± 0.44, respectively, as compared with other ID groups (P<0.0001). The present study signified that ID subjects had poor OHS and PS with the highest PTNs and were influenced by age (P<0.0001), degree of ID (P<0.0001), SES, location, institution type, and parent’s level education. The present study underlines a considerable need for prevention and treatment of periodontal disease among ID subjects in India.

Abstract: The number of boiler chicken farm located in the community felt starting to annoy people, especially the chicken farm located close to residential areas. Many people complained about the bad effects of the operations of the boiler chicken farm because there are many farmers who ignore the handling of waste of effort. The impact that can arise is the smell of chicken farms, the emergence of flies and fears spread of bird flu virus. The purpose of this study was to analyze the relationship between the sewage smell chicken farm on the incidence of psychosomatic symptoms in a residential community surrounding villages cage Sei Lembu Makmur 2014. The design of this study analytic observational, cross-sectional study approach. This research was conducted in the month of January to August 2014. The sample in this study that people around the cage which amounts to 43 respondents. Data was collected through questionnaires and a doctor's diagnosis. Data were analyzed using univariate and bivariate analysis were processed using the computerized system and Lambda test. The results showed that respondents ammonia contaminated and uncontaminated 21 people 22 people (51.2 %), which has a disturbing perception as many as 23 people (53.5 %) and did not interfere with 20 people (46.5 %) and respondents who had psychosomatic disorders is 13 people (30.2 %) and not psychosomatic 30 people (69.8%) .Based Lambda test showed that there was no relationship between the concentration of ammonia with psychosomatic disorders in settlements around the cage Sei Lembu Makmur (p = 1.000> 0.05) and there was no relationship between perception and psychosomatic disorders in the settlements around the cage Sei Lembu Makmur (p = 0.462> 0.05). For the people around the cage explain to the public that air pollution by ammonia due to the smell of the chicken coop with the highest ammonia concentration of 3.32 ppm which means no influence posed to human health.

Nutrition is a major factor that has an important role in the growth anddevelopment in the toddlers. In the nutritional fulfillment of the toddler,the mother plays very important role. This study aimed to determinethe factors associated with the behavior of parents with the nutritionalstatus of toddlers. The research design was used analytical surveywith cross sectional study approach. The population in this researchwas 213 Balita, the sample in this research was 136 children in BulaloVillage Kwandang District, North Gorontalo District, the samplingmethod was used purposive sampling technique. Data collection ofrespondents in general using questionnaires, data analysis usingSPSS program version 21.0. Bivariate analysis to know the existenceof relation between dependent variable and independet by using chisquared test α = 0,05. The results showed that there was a correlationbetween parents' knowledge on the Nutritional Status of Toddlers (p =0,000), there was a correlation between parental attitudes with thenutritional status in the toddler (p = 0,000), there was a correlationbetween parental action with the nutritional status in the toddler (p =0,000 ). This study recommends to parents to always pay attention tothe type of food consumed and the mothers should often follow thecounseling done by midwives and nutrition officials so that thenutritional status of toddlers has increased.

OBJECTIVE: Analyze the oral health conditions of Caaporã-PB students in the dental caries prevalence, periodontal changes and quality of oral hygiene. MATERIAL AND METHOD: A cross-sectional study with 320 students in the ages of 5, 12 and 15 to 19 years with 62, 112 and 146 students, respectively was conducted. The confidence level was 95%, with a 5% margin of error and the sample wasn’t randomly selected. The dmft and DMFT indices for decayed, missing and filled teeth, the Simplified Oral Hygiene Index (SOHI), and Community Periodontal Index (CPI) to assess bleeding gingival and teeth calculus were employed. A descriptive and inferential analysis of the data with Chi-square test (α=0.05) was conducted. RESULTS: The dmft was 3.69+4.71 at 5 years and 42% hadn’t caries experience. The DMFT was obtained, respectively, 2.44+2.60 and 3.73+3.28 at 12 and 15 to 19 years, with 20.6% and 17.1% without caries experience. The median of SOHI was 1.16±0.51 and there wasn’t difference in relation to caries (p>0.05). It was found that 90.1% of students with gum bleeding and 85.7% with teeth calculus had DMFT>1 (p<0.05). CONCLUSION: The caries prevalence was considered high at 5 years, low to 12 and mean at 15 to 19 years. Oral hygiene was classified as regular, being more deficient in children of 5 years. There was association (p<0.05) between decay and gum bleeding conditions (OR=3.117, CI=1.332-7.296) and dental calculus (OR=1.983, CI=1.027-3.829). These results allow the directing of health actions and therefore obtain better oral conditions.

Background: Adolescence is a period of transition from childhood to adulthood. It is very crucial for adolescents to use the adolescent friendly health services in order to meet sexual and reproductive health needs. This study aimed to assess the factors associated with utilization of adolescent friendly health services in Dang district of Nepal.Methods: A community based analytical cross-sectional study was conducted in Dang district of Nepal. Five adolescent friendly health facilities were randomly selected and proportionate sampling was done to fulfill the required sample size. The adolescents were selected conveniently from the areas. Verbal informed consent was taken from the participants and their parents in case of age below 16 years. Approval was taken from Institutional Review Committee of Chitwan Medical College. Bivariate analysis was performed to detect the factors associated with utilization of adolescent friendly health services.Results: Nearly half (48.7%) of the adolescents had utilized adolescent friendly health services. Different socio-demographic factors like age of the participants, current educational status, mother’s education, ethnicity was significantly associated with utilization of adolescent friendly health services. Other factors like awareness about were services, convenient opening days and hour of health facilities and shyness to utilize the services were significantly associated with the utilization of the services.Conclusions: The utilization of the adolescent friendly services was found to be low among the adolescents of Dang district. In order to increase awareness among the adolescent regarding utilization of the services, there is need of coordination between schools, health institutions, communities etc. to conduct various awareness related program.

Abstract Introduction Contrary to the recommendation, children under the age of 6 months be exclusively breastfed, many infants are also fed with other liquids such as water, non-milk liquids, and other milks before 6 months. Because of level of education and proportion of employed women in Ethiopia has been increasing gradually. Consequently the practice of exclusive breastfeeding became very lower. The main objective of this study was to assess the magnitude and factors associated with the practice of exclusive breastfeeding among employed and unemployed mothers in Ethiopia, using dataset from EDHS 2016. METHOD Data was extracted from Ethiopia Demographic and Health Survey (EDHS-2016). A total of 1089 mothers with infants aged 0 to 6 months in the data set were included in the analysis. Descriptive analysis, chi-square and binary logistic regression models were used. RESULT This study has demonstrated a 64.7%, 95%CI(58.9-70.5) and 75.4%, 95%CI(72.378.2) prevalence of EBF practices among employed and unemployed mothers respectively in the 24h preceding the survey. Other factors associated with EBF practice included being unemployed mother, larger to normal birth weight; infant aged 0-3 month old, infant being male, have radio in the house, infant being wanted, delivery by non CS mode and presence of ANC follow up during pregnancy were found to be significantly associated with exclusive breastfeeding practice. Conclusion the prevalence of exclusive breast feeding practice among employed mothers was significantly lower than that of unemployed mothers. Thus, Having in consideration the impact of appropriate infant and young children feeding practice on children’s nutritional status and mortality rate, policy makers still need to give more emphasis on promotion of exclusive breastfeeding through creating an enabling environment targeting the extension of postnatal maternity leave up to the first six month.
