Emotions and Experience of Fathers applying Kangaroo Care in the Eastern Anatolia Region of Turkey: A Qualitative Study

The aim of this study was to investigate the emotions and experiences of fathers in Eastern Anatolia Region of Turkey who applied kangaroo care in the neonatal intensive care unit. This study applied the qualitative descriptive design. The study included 12 fathers at the NICU of a university hospital. The fathers practiced kangaroo care with their babies two times a day for 15 days. Content analysis was then conducted to determine the main themes and sub-themes of the interviews. Three main themes and six sub-themes emerged from the analysis: (1) Emotions of being a father (feeling that the baby belongs to own and feeling the warmth and scent of the baby); (2) Confidence in fathering roles (self-confidence and caring for the baby); and (3) Happiness in the new parent role (seeing the baby calm down, hugging the baby and touching the baby’s skin).

ResumoDurante a gestação, os pais idealizam a imagem de um filho perfeito, mas por complicações alguns são internados na Unidade de Tratamento Intensivo Neonatal (UTIN). As causas são variadas, podendo ser decorrentes de complicações pré, peri e pós-natal. O objetivo deste estudo foi compreender a participação do pai no cuidado e no acompanhamento de bebês internados na UTIN. Trata-se de um estudo de caráter qualitativo, descritivo e exploratório, para investigar a participação do pai no cuidado e acompanhamento das crianças internadas. Foi utilizado um questionário aberto com sete questões. As entrevistas foram gravadas e transcritas na íntegra. Os resultados foram analisados segundo análise de conteúdo de Bardin. Os resultados evidenciaram que os pais vivenciam fortemente os efeitos da internação do filho, com sentimentos complexos como frustração, impotência, medo do desconhecido e preocupação com a mãe. Contrariando a hipótese do estudo, os pais se sentiram acolhidos pela equipe da UTIN e foram participantes ativos nas decisões e cuidados com o bebê. Os resultados indicam que os pais têm consciência e buscam realizar seus papéis de cuidador da criança e como cônjuge de forma participativa. Concluiu-se que o processo de humanização dos hospitais que acolhem as famílias nas UTIN tem sido muito importante para que os pais consigam superar as dificuldades relacionadas à internação do bebê, ainda mais quando há apoio e suporte da equipe. Abstract During the pregnancy the parents idealize the image of a perfect child, but due to complications some of them are hospitalized in the Neonatal Intensive Care Unit (NICU). The causes are varied, stemming from pre, peri and postnatal complications. The goal of this study was to comprehend the father’s participation in the care and monitoring of babies interned in the NICU. It is a study of qualitative, descriptive and exploratory nature, to investigate the participation of the father in the care and monitoring of interned babies. An open questionnaire with seven questions was utilized for the interviews, which were recorded and transcribed; the results were analyzed according to Bardin’s content analysis. The results made evident that the fathers experience intensely the effects of the child’s hospitalization with complex feelings ranging from frustration, impotence, fear of the unknown and concern with the mother. In contrast to the hypothesis of the study, the fathers felt welcomed by the NICU team and active participants in the decisions and cares regarding the baby. Results indicate that the fathers are aware and aim to fulfill their roles as the baby’s caretaker and of partner in active fashion. It was concluded that the humanization process of the hospitals that foster the families into the NICU has been important for the fathers to overcome the difficulties related to the baby’s hospitalization, seeing help and support in the team.Keywords: Humanization; Paternity; Neonatal Intensive Care Unit.

Aim: to  analyze  the knowledge  of  nurses  in the  Neonatal  Intensive Care  Unit  on  the  entry,  handling,  maintenance  and  withdrawal  of  the  peripherally inserted  central  catheter.  Method:  This  is  a  descriptive,  exploratory,  and  qualitative study involving nine  nurses from  the  Neonatal intensive care  unit  of  the  Antonio  Pedro University  Hospital,  Fluminense  Federal  University,  interviewed  based  on  a  semi-structured  script.  The  data  submitted  to  content  analysis  have  originated  thematic categories.   Results:   knowledge   of   nurses   regarding   the   statement,   insertion, maintenance  and  removal  of  the  catheter,  standards  and  protocols  and  professional training  for  the  care  process  are  decisive  for  the  safety  of  care  to  the  newborn. Conclusion: The use of the peripherally inserted central catheter (PICC) is important in neonatology  for  its  benefits  to  the  infant.  However,  nurses  need  to  deepen  their knowledge so that the care process in the NICU is guided by ethics and based on nursing protocols aiming to base and legitimize such assistance.

This study aimed to evaluate how parents of former neonatal intensive care unit patients with cerebral palsy perceive prognostic discussions following neuroimaging. Parent members of a cerebral palsy support network described memories of prognostic discussions after neuroimaging in the neonatal intensive care unit. We analyzed responses using Linguistic Inquiry and Word Count, manual content analysis, and thematic analysis. In 2015, a total of 463 parents met eligibility criteria and 266 provided free-text responses. Linguistic Inquiry and Word Count analysis showed that responses following neuroimaging contained negative emotion. The most common components identified through the content analysis included outcome, uncertainty, hope/hopelessness, and weakness in communication. Thematic analysis revealed 3 themes: (1) Information, (2) Communication, and (3) Impact. Parents of children with cerebral palsy report weakness in communication relating to prognosis, which persists in parents’ memories. Prospective work to develop interventions to improve communication between parents and providers in the neonatal intensive care unit is necessary.

Background: Describe the profile of newborns discharged from the neonatal intensive care unit (NICU) sent to a kangaroo ward and their neonatal variables.Methods: Retrospective and documentary study with a quantitative approach, performed at the General Hospital César Cals, whose sample consisted of 30 charts. The following variables were analyzed: weight, gestational age, Apgar score, gender, race, adequacy of the pregnancy and assistance provided in the NICU and kangaroo ward. The variables were analyzed using Microsoft Excel® 2010 program to obtain percentages.Results: There was a prevalence of extremely premature infants with high underweight and small size for the gestational age, male gender and browns, with Apgar score at 1st and 5th minutes more than 7, born by cesarean section, who used mechanical ventilation and surfactant, with prevalence of respiratory distress syndrome, with admission weight in Kangaroo ward less than 1.250g, making use of exclusive breast milk, who were attended by physiotherapy and with weight less than 1.600g at discharge, of the variables studied.Conclusions: Kangaroo care is an excellent cost-effective model for the newborn coming from the neonatal intensive care unit.

Infants who are born premature require hospitalization in the neonatal intensive care unit (NICU). In this study, 10 parents and seven grandmothers were interviewed with the purpose of exploring their experiences of having a premature infant in the NICU being cared for by nurses whose practice was grounded in Watson's theory of human caring caritas processes. Qualitative descriptive analysis revealed expressions of the caring moment lived as an intentional presence, within the context of the caritas processes to care for the infant and family, with loving-kindness, helping trusting relationship, creating caring healing environment, and allowing for hope and miracles.

ABSTRACT Objective To evaluate the quality of raw human milk distributed in the Neonatal Intensive Care Unit of a University Hospital of the city of São Paulo. Methods A cross-sectional study with raw human milk samples from mothers who attended the Human Milk Collection Station of a University Hospital, analyzed between May 2016 and January 2017, excluding mothers of twins. The quality of the raw human milk was assessed by verifying the presence of dirt, the coloration of the milk, the titratable acidity using the Dornic method, and through its energy content. Kruskal-Wallis and Mann-Whitney tests were used for the analysis of the energy profile and the degree of Dornic acidity, according to the stage of the raw human milk and the gestational age of the child. Results The study was composed of 40 samples of 40 different women, with a mean age of 27 years, an average of 11.8 years of education, most of them were multiparous and with a partner. Regarding milk analysis, 55.0% was classified as colostrum, 27.5% as mature milk and 17.5% as transitional milk. All samples presented negative results for dirt and normal coloration. The mean milk acidity was 3.24º Dornic and most of the samples were classified as hypercaloric energy content. There was no association between the lactation stage and gestational age with the acidity value and energy content. Conclusion The quality of raw human milk distributed in the Neonatal Intensive Care Unit of the institution evaluated was considered adequate and the samples analyzed had a high energy content and excellent Dornic acidity.

Early detection of hearing loss in neonates is important for normal language development, especially for infants admitted to the neonatal intensive care unit (NICU) because the infants in NICU have a higher incidence of hearing loss than healthy infants. However, the risk factors of hearing loss in infants admitted to the NICU have not been fully acknowledged, especially in Korea, although they may vary according to the circumstances of each country and hospital. In this study, the risk factors of hearing loss in NICU infants were analyzed by using the newborn hearing screening (NHS) and the diagnostic auditory brainstem response (ABR) test results from a 13-year period. A retrospective chart review was performed using a list of NICU infants who had performed NHS from 2004 to 2017 (n = 2404) in a university hospital in Korea. For the hearing loss group, the hearing threshold was defined as 35 dB nHL or more in the ABR test performed in infants with a ‘refer’ result in the NHS. A four multiple number of infants who had passed the NHS test and matched the age and gender of the hearing loss group were taken as the control group. Various patient factors and treatment factors were taken as hearing loss related variables and were analyzed and compared. From the 2404 infants involved, the prevalence of hearing loss was 1.8% (n = 43). A comparison between the hearing loss group (n = 43) and the control group (n = 172) revealed that history of sepsis, peak total bilirubin, duration of vancomycin use, days of phototherapy, and exposure to loop-inhibiting diuretics were significantly different, and can be verified as significant risk factors for hearing loss in NICU infants.
