Abstract Background No reliable and comprehensive study has been published on the incidence and epidemiological profile of meningitis in Iran from 2008 to 2014, before pneumococcal conjugate vaccine (PCV) and pentavalent vaccine (DTPw-Hep B-Hib (PRP-T) vaccine (pentavac) (adsorbed)) introduction. The present study aimed to portray the epidemiological profile of meningitis in Iran from 2008 to 2014. Methods Data on meningitis cases aged from 1 day to 110 years were extracted from national notifiable diseases surveillance system from March 2008 to December 2014 in Iran. A total number of 48,006 cases of suspected meningitis were identified and 1468 cases of which met the criteria for diagnosis-confirmed meningitis. Of 1468 cases, 1352 patients were included in the study. Results The great number of cases reported from urban areas. Moreover, males were more predominant than females (58.51% vs. 33.81%) in total. The estimated annual incidence rate of meningitis varied from 0.28/100000 in 2008 to 0.09/100000 in 2014. Streptococcus pneumoniae, Haemophilus influenzae and Neisseria meningitidis were the most leading pathogens causing bacterial meningitis, accounted for 266(23.44%), 145(12.78%), 95(8.37%) of cases, respectively. Each of the three bacterial species showed a descending trend. The majority of infected subjects are children under five years. Conclusions Unlike the decreasing trend of meningitis and high percentage of cultures with negative results, according to World Health Organization recommendation PCV introduction into routine immunization is evident. Implementing an enhanced surveillance system to provide high quality data on epidemiological profile of meningitis in Iran is necessary.

Abstract Background Haiti is one of the Caribbean countries where malaria still persists. Malaria occurs throughout the country at altitudes below 600 meters. More than 99% of the malaria cases are caused by Plasmodium falciparum and the main vector is Anopheles albimanus. This paper aims to describe the epidemiological profile of malaria in Haiti between 2009 and 2018. Methods We analyzed information on malaria cases reported by both the Health Ministry of Haiti (Ministère de la Santé Publique et de la Population-MSPP) and the World Health Organization (WHO) during the study period. Results Between 2009 and 2018, a total of 232,479 malaria cases were reported by the MSPP. There was an increase in the incidence of malaria in the country in 2010 followed by a decrease in 2011. Due to efforts made by Haiti over the past decade to reduce malaria by 2020, malaria incidence has declined from 60,130 cases in 2010 to 8,978 cases in 2018. We verified that the MSPP and the WHO data are conflicting in relation to the number of cases reported. However, the results from both data sets presented the same malaria trend in Haiti from 2009 to 2018. The results also show the endemicity of the disease throughout Haiti, both in rural and urban areas, especially along the coast. Conclusion This work emphasizes the need to promote official data collection and analyses and the application of epidemiological surveillance of malaria for a better knowledge about the real impact of malaria on the Haitian population aiming more appropriate interventions.

Antimicrobial resistance (AMR) is a global problem, and Nepal is no exception. Countries are expected to report annually to the World Health Organization on their AMR surveillance progress through a Global Antimicrobial Resistance Surveillance System, in which Nepal enrolled in 2017. We assessed the quality of AMR surveillance data during 2019–2020 at nine surveillance sites in Province 3 of Nepal for completeness, consistency, and timeliness and examined barriers for non-reporting sites. Here, we present the results of this cross-sectional descriptive study of secondary AMR data from five reporting sites and barriers identified through a structured questionnaire completed by representatives at the five reporting and four non-reporting sites. Among the 1584 records from the reporting sites assessed for consistency and completeness, 77–92% were consistent and 88–100% were complete, with inter-site variation. Data from two sites were received by the 15th day of the following month, whereas receipt was delayed by a mean of 175 days at three other sites. All four non-reporting sites lacked dedicated data personnel, and two lacked computers. The AMR surveillance data collection process needs improvement in completeness, consistency, and timeliness. Non-reporting sites need support to meet the specific requirements for data compilation and sharing.

Allergic rhinitis and eczema represent ailments which are increasingly linked with urban areas. Although they do not belong to illnesses which are predominantly caused by geographical factors, their importance for the development of these illnesses will be considered in this paper. As an example of the influence of urban environment on the development of these illnesses, the city core of Belgrade (the area included in the General Urban Plan) is taken into consideration. The emphasis is put especially on the quality of air, industry, traffic and the conditions for living. The research is based on tracking these illnesses among children, considering the frequent appearance of these illnesses especially among school children. The survey, whose results were partly presented in this research paper, was an important method for examining the subject matter of this paper. Namely, the above mentioned survey was realized in the International Study of Asthma and Allergy among children (ISAAC), which was conducted by World Health Organization (WHO), 156 centres from 56 countries took part. Among them, there were four centres from Serbia: Belgrade, Nis, Podgorica and Sombor.

Australia monitors its polio-free status by conducting surveillance for cases of acute flaccid paralysis (AFP) in children less than 15 years of age, as recommended by the World Health Organization (WHO). Cases of AFP in children are notified to the Australian Paediatric Surveillance Unit or the Paediatric Active Enhanced Disease Surveillance System and faecal specimens are referred for virological investigation to the National Enterovirus Reference Laboratory. In 2017, no cases of poliomyelitis were reported from clinical surveillance and Australia reported 1.33 non-polio AFP cases per 100,000 children, meeting the WHO performance criterion for a sensitive surveillance system. Three non-polio enteroviruses, coxsackievirus B1, echovirus 11 and enterovirus A71, were identified from clinical specimens collected from AFP cases. Australia established enterovirus and environmental surveillance systems to complement the clinical system focussed on children and an ambiguous vaccine-derived poliovirus type 2 was isolated from sewage in Melbourne. In 2017, 22 cases of wild polio were reported with three countries remaining endemic: Afghanistan, Nigeria and Pakistan.

Surveillance plays a pivotal role in overcoming antimicrobial resistance (AMR) in bacterial pathogens, and a variety of surveillance systems have been set up and employed in many countries. In 2015, the World Health Organization launched the Global Antimicrobial Resistance Surveillance System (GLASS) as a part of the global action plan to enhance national and global surveillance and research. The aims of GLASS are to foster development of national surveillance systems and to enable collection, analysis and sharing of standardised, comparable and validated data on AMR between different countries. The South Korean AMR surveillance system, Kor-GLASS, is compatible with the GLASS platform and was established in 2016 and based on the principles of representativeness, specialisation, harmonisation and localisation. In this report, we summarise principles and processes in order to share our experiences with other countries planning to establish a national AMR surveillance system. The pilot operation of Kor-GLASS allowed us to understand the national burden of specific infectious diseases and the status of bacterial AMR. Issues pertaining to high costs and labour-intensive operation were raised during the pilot, and improvements are being made.

Abstract Background In the context of old pharmaceutical legislation and regulations not adapted to current realities, the aim of the present study was to evaluate the existing pharmaceutical system in peri-urban areas of Kinshasa. Methods A prospective study was carried out during the period 2016–2018. The most used antimalarial medicines were identified through household and pharmaceutical establishment surveys. The samples of the obtained medicines were assayed with generic separation methods using the high-performance liquid chromatography technique coupled to a diode array detector. The registration status was checked for 126 antimalarial brand names. A characterization was carried out in 196 pharmaceutical establishments on the basis of standards set out by the Ministry of Health. Results Of the 75 samples assayed, 19% (14/75) were non-compliant. Of the 124 brand names, 46.0% (57/124) were unlicensed and 14.5% (18/124) had an expired licence. Of the 196 pharmaceutical establishments, only 2 (1.0%) had an authorization to practice, none met all the Ministry of Health minimum standards and 24.5% (48/196) met the World Health Organization Guidelines for the Storage of Essential Medicines and Other Health Commodities. Conclusions More resources should be mobilized to apply regulator sanctions.

The Dominican Republic is located in the Caribbean Sea and comprises three-quarters of the island Hispaniola, which it shares with Haiti. According to the 2002 census, approximately 8.5 million people live in the Republic, with 64% residing in urban areas (Oficina Nacional Estadística, n.d.). During 1990 and 2000, the Dominican Republic was a leader in economic development for Latin America and the Caribbean; however, this was not reflected in the areas of human and social development (Pan American Health Organization & World Health Organization, 2007). Less than 1 % of the health budget administered by the Ministry of Public Health and Social Assistance (MISPAS) is allocated to mental health and the public system is generally underfunded (Pan American Health Organization & World Health Organization, 2008). However, there is an array of mental health services within the country when privately funded facilities are taken into account.

<p>Globally, access to improved water sources is lower in rural areas compared to urban areas. Furthermore, in rural areas many people use water from individual systems they have developed with their investments, often without external support. This phenomenon has been called Self-supply. Self-supply ranges from simple to complex systems and different water sources. Water quality varies, from achieving World Health Organization (WHO) standards (0 CFU/100 ml) to systems that provide water posing high risks to human health. While most studies in Self-supply have been developed in Africa, little is known in Latin America and the Caribbean (LAC). This research explores Self-supply in a rural microcatchment in Colombia (LAC). Data was collected through household and drinking water surveys and analysed. Results showed that 40% of households used Self-supply systems taking water from springs and brooks. Thermotolerant Coliforms were below 50 CFU/100 ml, both in dry and rainy season, and between 5 to 7% of samples achieved the WHO standard. These results suggest that Self-supply has potential to offer safe drinking water, provided improvements on source protection and institutional support. Therefore, Self-supply could contribute to address “unfinished business”, including ensuring access for the hardest-to-reach people, as stated in the post-2015 development agenda.</p>

Makassar has a strategic position as it is located in between the south and north in the provinces of South Sulawesi. Thus, the rapid growth of urbanization and industrialization within the area is unavoidable, resulting Makassar to be an area of mixed commercial–residential–industrial along with the problem of air pollution. Hence, it is important to monitor the quality of air in Makassar. This paper presents a preliminary survey of urban air quality in Makassar area based on SO2, CO, NO2, O3, Pb, and TSP (Total Suspended Particle) sampled over ten years period (2001 to 2010), while PM10was monitored for five years (2006 to 2010). The air quality data were obtained from measurements made by the Office of Ministry of Environment Sulawesi, Maluku and Papua and Environment Board of the Province of South Sulawesi as well as Environment agency of Makassar City. The average annual concentrations of SO2, CO, NO2, O3, Pb, TSP and PM10 recorded were 76 μg/m3, 1041 μg/m3, 43.2 μg/m3, 54.5 μg/m3, 0.7 μg/m3, 188 μg/m3, 54.6 μg/m3, respectively. Subsequently, these data are compared to the air quality threshold limits recommended by the Indonesia National Ambient Air Quality Standard (INAAQS) as well as guidelines of the World Health Organization (WHO).

In a world where the population continues to increase and the volume of fishing catches stagnates or even falls, the aquaculture sector has great growth potential. This study aimed to contribute to the depth of knowledge of the diversity of bacterial species found in Sparus aurata collected from a fish farm and to understand which profiles of diminished susceptibility to antibiotics would be found in these bacteria that might be disseminated in the environment. One hundred thirty-six bacterial strains were recovered from the S. aurata samples. These strains belonged to Bacillaceae, Bacillales Family XII. Incertae Sedis, Comamonadaceae, Enterobacteriaceae, Enterococcaceae, Erwiniaceae, Micrococcaceae, Pseudomonadaceae and Staphylococcaceae families. Enterobacter sp. was more frequently found in gills, intestine and skin groups than in muscle groups (p ≤ 0.01). Antibiotic susceptibility tests found that non-susceptibility to phenicols was significantly higher in gills, intestine and skin samples (45%) than in muscle samples (24%) (p ≤ 0.01) and was the most frequently found non-susceptibility in both groups of samples. The group of Enterobacteriaceae from muscles presented less decreased susceptibility to florfenicol (44%) than in the group of gills, intestine and skin samples (76%). We found decreased susceptibilities to β-lactams and glycopeptides in the Bacillaceae family, to quinolones and mupirocin in the Staphylococcaceae family, and mostly to β-lactams, phenicols and quinolones in the Enterobacteriaceae and Pseudomonadaceae families. Seven Enterobacter spp. and five Pseudomonas spp. strains showed non-susceptibility to ertapenem and meropenem, respectively, which is of concern because they are antibiotics used as a last resort in serious clinical infections. To our knowledge, this is the first description of species Exiguobacterium acetylicum, Klebsiella michiganensis, Lelliottia sp. and Pantoea vagans associated with S. aurata (excluding cases where these bacteria are used as probiotics) and of plasmid-mediated quinolone resistance qnrB19-producing Leclercia adecarboxylata strain. The non-synonymous G385T and C402A mutations at parC gene (within quinolone resistance-determining regions) were also identified in a Klebsiella pneumoniae, revealing decreased susceptibility to ciprofloxacin. In this study, we found not only bacteria from the natural microbiota of fish but also pathogenic bacteria associated with fish and humans. Several antibiotics for which decreased susceptibility was found here are integrated into the World Health Organization list of “critically important antimicrobials” and “highly important antimicrobials” for human medicine.
