Abstract Background Poor oral hygiene and high hormone levels during pregnancy can lead to a deterioration in periodontal health. This study assessed the effectiveness of a family-centered behavioral and educational counselling program on improving the periodontal health of women during pregnancy and postpartum. Methods A randomized controlled trial was conducted among pregnant women (10th-22nd gestational week) and their husbands. Participating families were randomized into test and control groups. Intervention in the test group included explanation of oral health education (OHE) pamphlets, oral hygiene instruction, individualized feedback, and proposed solutions to overcome barriers in self-care. Reinforcements were implemented in the 3rd trimester of pregnancy and six months postpartum. In the control group, only OHE pamphlets were distributed. The assessed outcomes were bleeding on probing (BOP), periodontal pocket (Poc), loss of clinical attachment (LoA), and Visible Plaque Index (VPI). The data collection was carried out at baseline (T0), in the 32nd gestational week (T1), and 12 months postpartum (T2). Results Altogether 589 pregnant women were recruited, and 369 attended all three visits (test:188; control:181). In the test group, the mean VPI score at T0 was 0.19, which decreased to 0.14 at T1 and 0.15 at T2. In the control group, the mean VPI decreased from 0.19 at T0 to 0.16 at T1, but increased to 0.22 at T2. A main effect of time and intervention and an interaction between time and intervention were detected (all p < 0.05), indicating that the intervention effect differed between T1 and T2. The test group showed a significantly greater decrease over time than the control group did. Similarly, the mean BOP% decreased more significantly over time in the test group (T0:57%, T1:46%, T2:35%) than in the control group (T0:58%, T1:52%, T2:46%). For Poc and LoA, there were improvements in both study groups at 12 months postpartum, compared with during pregnancy (p < 0.001). Conclusions Providing family-centered, behavioral, and educational counselling to pregnant women at an early stage of pregnancy and with reinforcements can improve their oral hygiene and reduce gingival inflammation. The effect can be sustained over an extended period and is greater than that of distributing oral health leaflets alone. Trial registration Clinicaltrials.gov, #NCT02937194. Abstract Objectives: Poor oral hygiene and higher hormone levels during pregnancy always lead to poor periodontal health. Oral health education of the pregnant women to improve the periodontal outcomes might be ineffective and inconsistent. The aim of this study was to assess the effectiveness of a family-centered behavioral and educational counselling program on the pregnant women’s periodontal health and long-term effect after delivery. Methods: A randomized controlled trial was conducted among first-time pregnant women (10~20th gestational-week) in Hong Kong. Participating families were randomized into test and control group. The intervention in test group included: explanation on oral health education pamphlets, oral hygiene practice training by the “tell-show-do” technique, individualized feedback, and proposed solutions to overcome barriers on self-care. Reinforcement was implemented by interview, phone call or messaging to assess performance, identify barriers and to encourage participants on self-care, in around 32nd gestational-week and 6 months after delivery. Main outcomes assessed were periodontal health measurement including bleeding on probing (BOP), periodontal-pocket (Poc) and loss of clinical attachment levels (LoA); oral hygiene status was assessed by visible plaque index (VPI). The dental examinations were carried out at baseline, 32nd gestational-week and 12-months post-delivery.Results: 589 pregnant women were recruited and 369 completed all three visits (test 188; control 181). Mixed-effect model ANOVA indicated that in test group, % sites with visible plaque decreased from the baseline to the 32nd gestational-week and maintained to 12-months post-delivery while in control group the effect could not be sustained (p<0.001). Both groups showed significant decrease in BOP over time, and significantly lower % sites with BOP was found in test group than in control group at 12-months post-delivery (p<0.001). Generalized linear mixed model (GLMM) showed that there was significant less in sites with Poc and LoA in the both groups over time at 12-months post-delivery than during pregnancy (p<0.001), whereas there was no significant effect between the two groups (p>0.05). Conclusions: Providing family-centered, behavioral and educational counselling to pregnant women at early stage of pregnancy and reinforcements can improve their oral hygiene and periodontal health, the effect can be sustained over a longer period.

Early childhood caries is common in Hong Kong, and parental practices on maintaining good oral health of their young children are far from satisfactory. This article reports on the effectiveness of a randomized controlled trial on family-centered oral health promotion to new parents in establishing proper feeding habits and oral hygiene practices and in reducing caries risk among 3-y-old toddlers. At baseline, pregnant mothers and their husbands were recruited and randomly allocated into 2 groups. The test group received individualized oral health education (OHE) via a behavioral and educational counseling approach while the control group received the OHE pamphlets only. Information related to the feeding habits, oral hygiene practices, and oral health of the toddlers was collected by parent-completed questionnaires and oral examination annually via home visits. A total of 580 families were recruited at baseline, and 436 toddlers were followed up when they reached 3 y old (test, n = 228; control, n = 208; follow-up rate, 75.2%). The proportions of toddlers who held food in the mouth, fell asleep when milk feeding, had prolonged use of the nursing bottle, ate before bed, and consumed a sweet snack daily were significantly lower in the test group than in the control group (all P < 0.05). Significantly higher proportions of toddlers brushed their own teeth twice daily, were brushed by their parents twice daily, and used fluoride toothpaste than in the control group (all P < 0.001). Toddlers in the test group had better oral health status with a lower level of visible plaque, Streptococcus mutans, white spot lesion, and cavitated lesion (all P < 0.05). Family-centered oral health promotion and individualized OHE for parents via a behavioral and educational counseling approach are more effective in establishing good feeding habits and parental toothbrushing practices and in decreasing the caries risk of their toddlers than the distribution of OHE pamphlets alone (ClinicalTrials.gov NCT02937194).

Abstract Background: In order to prevent oral diseases, the use of appropriate educational methods at childhood is one of the most important determinants of the public health. Therefore, the aim of this study was to investigate the effect of training through animations and games on oral health self-efficacy and self-care behaviors in students aged 6-12 years old. Methods: In this interventional study, 82 students were selected using cluster random sampling (38 subjects in the case group and 44 in the control group). The case group received 4 sessions of blended learning per week including animations and games while the control group received routine school education. Data were collected in six domains, including demographics, self-care, knowledge, attitude, behavior and self-efficacy before and 5 months after the intervention using a questionnaire. SPSS version 20 was used for data analysis. Results: Five months after the intervention, the mean score of self-care increased from 3.8 to 4.8 of 5, the mean score of self-efficacy increased from 36.8 to 48.9 of 70, and the mean score of behavior increased from 17.07 to 18.29 of 32, indicating significant changes (p < 0.05). There were no significant changes in these variables in the control group (p >0.05) Conclusion: Use of combined methods for oral health self-care education has positive effects on the students' performance and self-efficacy. Introduction: Theory of planned behavior can provide a framework for intervention to understand how to prevent health problems. The aim of this study was to determine the effect of training intervention based on the theory of planned behavior in promoting oral health behavior of pregnant women who visit health care centers of Rafsanjan. Methods: In this educational intervention trial, 100 pregnant women in Rafsanjan were selected by multi-stage sampling in 2016. Initially, four centers were randomly selected, followed by two centers selected as intervention groups and two centers as control groups. Data were collected using a three-part researcher-made questionnaire, that were completed by pregnant women in two periods, ie, before and three months after the intervention. Quantitative data were presented as mean ± SD and qualitative data as n (%). First, the homogeneity of the two groups was examined using an independent t-test and chi-square test. Data were analyzed with SPSS 18. The significance level of all tests was considered to be 0.05. Results: Findings showed that there was no statistically significant difference between the demographics of the two groups (p> 0.05). After the implementation of the program, the mean score of intervention in all constructs of the theory of planned behavior model was significant between the control and intervention groups (p < 0.001). Following the educational program, the oral health behavior of pregnant women increased in the intervention group (3.72 ± 4.22) which was significantly higher than the control group (1.48 ± 2.58) (p < 0.001). Conclusion: The results of this study showed that educational interventions can focus on the subjective norm which had fruitful results in the field of oral health. This theory can be used as an appropriate model for oral health promotion programs.

Background: Periodontal infection leads to adverse pregnancy outcomes such as preterm delivery, low birth weight, preeclampsia, miscarriage, or early pregnancy loss. Aim: The aim of this study was to investigate the knowledge, attitude, and practice of pregnant women about periodontal health and oral health and their effect on the fetus. Methods: The present study is a descriptive-analytical study on 210 pregnant women in the first and second trimesters referring to health centers in Birjand in 2019. The tool used in this study was a researchermade questionnaire with four parts: women's demographic information, and women's knowledge, attitude, and practice, the validity, and reliability of which were confirmed. After data collection, it was entered into SPSS software version 22 and then analyzed using descriptive statistics, independent t-test, and one-way analysis of variance. The significance level was considered 0.05 in all tests. Results and Discussion: In this study, the mean scores of knowledge, attitude, and practice of pregnant women were 5.6 ±1.92, 55.9 ± 5.1, and 25.9± 3.7, respectively. There was a significant difference between the mean attitude score and mothers' age. There was also a significant difference between the mean scores of knowledge, attitude, and practice of women and significant difference in knowledge, attitude, and practice of employee and housewife pregnant women; In terms practice, there were significant differences between pregnant housewives and self-employed women. There was a significant relationship between the mean of these scores and mothers' jobs so that the mean scores were higher in employed women than others. Conclusions: The results of our study showed that the average score of knowledge, attitude, and practice of pregnant women in Birjand regarding oral hygiene during pregnancy is not satisfactory and appropriate.

Background/Aim. Promotion of oral health in military population is not only a significant component of general health, but also of the military readiness and represents the strategic orientation of each country. The basic task of military dentistry is to provide oral health of military personnel and to enable their operational readiness at the optimal level. The aim of the study was to assess the periodontal condition in Serbian military population using the community periodontal index of treatment needs (CPITN), and the influence of general life habits and local risk factors on periodontal health. Methods. This prospective cross-sectional pilot study was conducted on 101 examinees at the mean age of 38.94 ? 11.63 years who had dental check-ups at the Dental Clinic of the Military Medical Academy in Belgrade. All the categories of military personnel aged 20-64 years were divided into five groups. The frequency distribution of general and local factors on periodontal health, oral hygiene index, and the assessment of the mean number of sextants by CPITN compared to age were examined. Results. The examinees at the age of 51-60 years had the best oral hygiene index (0.95 ? 0.65), whereas the oldest population had the worst (1.63 ? 0.42). Only one person (5.6%) at the age group of 51-60 years had a completely healthy periodontium. Observed in relation to the age groups, the mean values of sextants increased linearly, but in general population, the most frequent CPITN categories were in sextant with the periodontal pockets 4-5 mm (score 3). Conclusions. Compared to the results from other countries shown by the World Health Organization, the periodontal condition in our examinees is below the average. The appropriate preventive program preparation and its implementation are needed, including primarily the appropriate training on oral hygiene, as well as education based on periodontal disease prevention and treatment.

Background: Poor periodontal conditions among pregnant women might be a risk factor for adverse pregnancy outcomes which include preeclampsia, preterm delivery, intrauterine growth restriction, and fetal demise (stillbirth). Aim: To assess the effectiveness of oral health promotion by community health workers in diminishing periodontal diseases among pregnant women in rural areas of Jammu District. Methodology: In this community-based cluster randomized controlled study, Jammu district was divided into 5 administrative provinces with a total of 16 PHCs. In our study baseline assessment of Oral Health-related Knowledge, Attitude and Practices, and Oral health status using Oral Hygiene Index was done among pregnant women. Trained ASHA workers served as a medium to impart oral health education to pregnant females on a regular basis. After 3 months of follow up in the 2nd trimester, again oral health-related knowledge, attitude and practices, and oral health status using Oral Hygiene Index were assessed and oral prophylaxis was done for all pregnant women. After 4-5 months birth outcomes were collected at PHC's. Results: The knowledge about oral health was poor in pregnant women, and it improved after providing health education in pregnant women. Scaling alone as an independent variable did not influence the reduction in the preterm birth week (p= 1.000) and birth weight (p=0.113) at 2ndtrimester of pregnancy. Conclusion: Sociocultural factors, illiteracy, misconception, and the extra burden of work on ASHA workers are the main reasons for the ineffective transfer of knowledge and very little change in oral health behavior in pregnant women.

Introduction: At Lorulun Public health Center, pregnant women who do Antenatal Care (ANC) visits with complaints around the mouth are still quite high (89.8%), and the implementation of health promotion for maintaining oral health during pregnancy is rarely carried out in a structured manner in conjunction with other pregnancy counseling activities. The aim to determine the effect of oral health maintenance in pregnant women on the growth and development of the fetus during pregnancy. Methods: This study is a quasi-experimental design with a non-equivalent control group design of two groups with a total sample of 64 respondents. Sampling was done using a purposive method. Data analysis of growth and development variable was carried out using independent t-test, oral hygiene variable was analyzed using Mann Whitney test and oral hygiene was analyzed using ANOVA test. Results: The results of this study showed that health education methods (video and demonstrations) were effective in increasing the average growth status of the fetus with TFU 28.06 (cm) TBJ 2334.69 grams and reducing the OHIS value of 1.26 (better oral hygiene), while oral hygiene was not affect the status of fetal growth and development. Conclusion: Oral hygiene does not affect the growth and development of the fetus, but it is hoped that pregnant women will continue to pay attention to their oral hygiene during pregnancy because oral hygiene affects the intake of nutritional needs that enter the fetus.

Abstract Background This study aimed to design a gamified smartphone application (app) and assess its efficacy for education of mothers regarding oral healthcare of their children. Methods In this pretest–posttest controlled clinical trial, a simple app and a gamified version of it were designed to enhance the oral health knowledge and practice of mothers. The app contains information about early childhood caries, health diet, sugars, baby-oral hygiene, fluoride effect, fluoride toothpaste, tooth-brushing training video and regular dental visits. The opinion of experts and 3 mothers were obtained and both apps were revised accordingly. The intervention was implemented on mothers of preschoolers referring to the specialty dental clinic of Tehran School of Dentistry in 2019. The mothers were randomly allocated to the simple app or gamified app group. Before the intervention, all mothers filled out a questionnaire regarding oral health knowledge and practice, and their demographics were collected. The plaque index (PI) of children was also measured. The mothers filled out the same questionnaire 1 month after the intervention, and the PI of children was measured again. Paired t test and linear regression model were used for statistical analysis of the data. Results Totally, 58 mother and child pairs entered the study; 40% of children were boys. The mean age of children was 4.7 ± 1.2 years. The mean knowledge score of mothers in the pretest was 10.5 and 11.3 in simple app and gamified app group, respectively, which changed to 13.1 and 14.3, respectively in the posttest. The mean practice score of mothers was 4.4 and 4.8 in simple app and gamified app groups, respectively in the pretest, which changed to 8.5 and 8, respectively in the posttest. The mean dental plaque index of children in the pretest was 0.8 and 1 in simple app and gamified app groups, respectively, which changed to 0.5 and 0.5, respectively in the posttest. Children had better Plaque control in gamified app group (P < 0.05). Conclusion After 1 month, both apps effectively improved the oral-health knowledge and practice of mothers while oral hygiene as a result of plaque control was superior in children of mothers using the gamified app. Trial registration IRCT, IRCT20131102015238N2. Objective: To investigate the effectiveness of Instagram in improving oral hygiene compliance and knowledge in young orthodontic patients compared to traditional chairside verbal instructions. Design: Single-center, parallel, randomized controlled trial. Setting: Section of Dentistry of University of Pavia. Participants: 40 patients having fixed appliances in both arches were recruited and randomly divided into an intervention (n = 20) and a control group (n = 20). Intervention: At a first appointment, both groups were given verbal instructions and motivated to oral hygiene. In addition, multimedia contents on Instagram were sent weekly to trial participants for six months. Main outcome measures: For all participants, the bleeding index (BI), modified gingival index (MGI), and plaque index (PI) were assessed at baseline (T0), after one (T1), three (T2), and six months (T3). A questionnaire was administered at the beginning (T0) and at the end of the study (T3) to assess participants’ knowledge. Results: In both groups, BI, MGI, and PI significantly decreased (p < 0.05) at T1 (means control group: BI 0.26 ± 0.22, MGI 0.77 ± 0.36, PI 0.53 ± 0.20; means test group: BI 0.24 ± 0.22, MGI 0.65 ± 0.46, PI 0.49 ± 0.21) compared to baseline (means control group: BI 0.56 ± 0.27, MGI 1.23 ± 0.41, PI 0.87 ± 0.23; means test group: BI 0.54 ± 0.26, MGI 1.18 ± 0.39, PI 0.93 ± 0.20) but no significant differences in clinical measures were showed between T1, T2, and T3 (p > 0.05) (intragroup differences). Trial patients demonstrated significant improvements in knowledge with respect to controls comparing scores at T0 and T3 (p < 0.05) but despite this result in the test group clinical outcomes did not report significant intergroup differences at any time (p > 0.05). Conclusions: Presenting multimedia information through Instagram resulted in a significant improvement in knowledge. Therefore, this social media represents an aid to the standard verbal motivation performed by orthodontists towards young patients under an orthodontic treatment.
