Abstract Background Hypertension is among the growing non-communicable diseases (NCDs) in developing countries and the leading cause of death worldwide. Pastoral areas have been identified to be at a higher risk of diseases due to challenges in their daily food production, livelihoods or mobility. Unfortunately, the prevalence of hypertension and the risk factors particularly affecting rural and pastoral populations are not fully understood, making intervention efforts challenging. The aim of this study was to determine the prevalence of hypertension and identify the risk factors among adults living in Monduli district in Tanzania. The findings will be useful for the provision of tailored interventions focused on community-specific nutritional and behavioral practices. Methods We conducted a community based cross-sectional study involving a sample of 510 adults aged above 18 years selected using a multistage cluster sampling in the Monduli district of Arusha region, Tanzania. Data were collected by using interviewer-administered questionnaires containing socio-demographic, physical activity, smoking and alcohol consumption. Anthropometry, systolic (SBP) and diastolic blood pressure (DBP) levels were measured. A one-day 24 h diet recall was conducted to evaluate the dietary habits of all participants. Both linear and logistic regression analysis were used to identify the independent predictors for hypertension and blood pressure levels. Results The prevalence of hypertension in this study was 25.7% (n = 131, 95% CI; 22.1–29.7). The odds of hypertension increased with being male (AOR = 1.75, 95%CI, 1.06–2.88), belonging to the older age group of 30–39 year olds (AOR = 3.3, 95%CI, 1.76–6.38), 40–59 year olds (AOR = 3.34, 95%CI, 1.75–6.37) and ≥ 60 year olds (AOR = 4.2, 95%CI, 2.02–8.87), being overweight or obese (AOR = 3.37, 95%CI, 1.18–9.62), have more hours spent sedentary (AOR = 3.19, 95%CI, 1.61–6.32), and consumption of fatty foods (AOR = 2.23, 95%CI, 1.27–3.93). The odds for hypertension was significantly reduced among participants who reported higher income (AOR = 0.47, 95% CI, 0.25–0.91), high level of physical activity (AOR = 0.55, 95%CI, 0.31–0.96) and those reported to consume fruit (AOR = 0.37, 95% CI, 0.18–0.77). Consumption of cereals was negatively associated with levels of SBP (β = − 17.4, 95% CI, − 23.8; − 11.0) and DBP (β = − 6.6, 95% CI, − 11.5,-1.79). Conclusion About one in every four adults living in pastoral communities have been found to have hypertension in this study. Our findings suggest that older age, obesity or overweight, low physical activity, low income, and consumption of fatty foods increase the risk of hypertension among study population. Their diet was dominated by cereals with moderate intake of meat and milk and low fruits. There is a need to promote physical activities and consumption of fruits in the study population in order to fight against hypertension. Further research should be done to confirm the associations.

The present study aims to evaluate the association between the high intensity of OPA and cardiovascular risk factors of employees.MethodsA cross-sectional study was carried out in the “Fomento de Construcciones y Contratas S.A. Company” (FCC S.A. Delegation) in 2017, Spain. The intensity of OPA of each employee was classified by work categories in low (≤ 3 METs) or high (> 3 METs). Health and lifestyle data of employees were collected on medical consultation in the workplace.ResultsAssociations were analyzed between the high intensity of OPA and cardiovascular risk factors of employees. 751 employees, 55.5% (n = 417) with high intensity of OPA. An 82.2% (n = 245) of employees with low intensity of OPA presented overweight/obesity compared with 69.0% (n = 171) with high intensity of OPA employees (p < 0.001). In men, high intensity of OPA is negatively associated with Body Mass Index (BMI) (B: -0.042, p = 0.003), waist circumference (B: -0.027, p < 0.001) waist-hip ratio (B: -5.484, p < 0.001), cholesterol (B: -0.007, p = 0.001) and triglycerides (B= -0.003, p = 0.025). In women, high intensity of OPA was positively associated with blood pressure (BP) (systolic BP: B = 0.036, p = 0.005 and diastolic BP: B = 0.040, p = 0.021).ConclusionsIn conclusion, the high intensity of OPA is associated with less cardiovascular risk factors in men, whereas women are associated with high levels of blood pressure, suggesting an influence of gender and work categories.

The understanding of cardiovascular disease (CVD) risk factors and their association with food and physical activity is not yet completely clear. This study aimed to evaluate the association between CVD risk factors with dietary intake, according to the physical activity level. A cross-sectional study was conducted with Brazilian individuals attended by the Public Health System. Demographic, blood pressure, physical activity practice, anthropometry and food intake data were collected and evaluated. Of the 83 participants, 61.4% were active. No difference were observed in the frequency of CVD risk factors, anthropometric data, blood pressure, estimated energy requirement, energy and nutrient intake between the active and inactive subjects (p ≥ .05). There was also no difference in the frequency of energy and nutrient intake adequacy between groups (p ≥ .05). Among the inactive subjects, it was found that the consumption of total (OR: 1.021, p = .035) and saturated (OR: 1.060, p = .033) fat was predictor of being overweight, with no relationship between food intake and the risk factors for CVD when the total participants or active individuals were considered (p ≥ .05). No difference was observed in the frequency of CVD risk factors between active and inactive individuals, however, total and saturated fat consumption increased the chance of being overweight among the inactive individuals. Resumo O conhecimento da associação entre os fatores de risco para doenças cardiovasculares com a alimentação e atividade física ainda não está totalmente elucidada. Este estudo teve como objetivo avaliar a associação entre os fatores de risco cardiovascular e o consumo alimentar, segundo o nível de atividade física. Foi realizado um estudo transversal com individuos brasileiros atendidos pelo Sistema Único de Saúde. Foram coletados e avaliados dados demográficos, pressão arterial, prática de atividade física, antropometría e consumo alimentar. Dos 83 participantes, 61,4% eram ativos. Não foi observado diferença na frequência dos fatores de risco cardiovascular, dados antropométricos, pressão arterial, estimativa da necessidade energética, consumo de energía e nutrientes entre os individuos ativos e inativos (p ≥ 0,05). Também não houve diferença na frequência de adequação no consumo de energia e nutrientes entre os grupos (p ≥ 0,05). Entre os participantes inativos, observou-se que o consumo de gordura total (OR: 1,021; p = 0,035) e saturada (OR: 1,060; p = 0,033) foram preditores do sobrepeso, sem relação entre o consumo alimentar e os fatores de risco cardiovascular quando se considerou todos os participantes ou apenas os indivíduos ativos (p ≥ 0,05). Não foi observado diferença na frequência dos fatores de risco cardiovascular entre os sujeitos ativos e inativos, entretanto o consumo de gordura total e saturada aumentou a chance de sobrepeso entre os indivíduos inativos. Introduction: Hypertension remains an emerging public health problem globally, particularly in developing countries. Age, income level, obesity, alcohol consumption, smoking, vegetables and fruit consumption, physical activity and chat chewing were some risk factors of hypertension. However, there are limited data on the epidemiology of hypertension in Ethiopia. This study aimed to assess Epidemiology of Hypertension among the community of selected towns of Wollega zones. Methods: A community-based cross-sectional study was conducted from 1 to 30 June 2019 in selected towns of Wollega zones. A multistage sampling technique was used to select 840 study participants. Data were collected using the WHO STEP wise approach. The data were coded and entered into EpiData 3, and exported to SPSS version 20.0 for analysis. Bivariate and multivariable logistic regression analyses were conducted. Statistical significance was declared at p-value < 0.05. Results: The study included a total of 838 respondents with a response rate of 99.7%. The prevalence of hypertension was found to be 189 (22.6%) (95% confidence interval = 19.9%–25.2%). Of this, 108 (12.9%) and 81 (9.7%) of female and male were hypertensive, respectively. Age groups of 30–44 years (adjusted odds ratio = 2.65 (1.43, 4.89)), 45–59 years (adjusted odds ratio = 3.55 (1.79, 7.04)), above 60 years (adjusted odds ratio = 2.97 (1.43, 6.18)), having history of alcohol consumption (adjusted odds ratio = 4.29 (2.4, 7.66)), involving in vigorous physical activity (adjusted odds ratio = 0.096 (0.028, 0.33)), not walking to and from the work (adjusted odds ratio = 13.12 (8.34, 20.67)), being overweight (adjusted odds ratio = 1.98 (1.21, 3.25)), inadequate fruits serving per day (adjusted odds ratio = 2.93 (1.75, 4.88)) were significantly associated with hypertension. Conclusion: The prevalence of hypertension was found to be high in the study area. Older age, alcohol consumption, not engaging in vigorous activity, physical inactivity, being overweight and inadequate intake of fruits were found to be risk factors for hypertension. Therefore, health care providers should provide extensive health education and promotion on recommended lifestyle modification to tackle the burden of hypertension.

Abstract Introduction: A better understanding of socio-demographic characteristics of subgroups, which have a high risk to develop chronic diseases, is essential to develop more efficient interventional programs especially for youth. This study aimed to determine the association between clusters of non communicable diseases (NCDs’) risk factors and the socio-demographic characteristics among a sample of Tunisian school children. Materials and methods: We conducted, in 2013/2014, a cross-sectional study among a proportional and stratified school children sample, selected in 17 elementary public schools in Sousse (Tunisia). A cluster analysis was used to identify different NCDs risk factors clusters, based on tobacco use, physical inactivity, unhealthy diet, and excess weight. Subsequent χ2-tests were used to identify differences between the NCDs risk factors clusters in regards to socio-demographic characteristics. Results: Four clusters of NCDs risk factors were found: 1) Cluster 1: physical inactivity behavior with normal weight, 2) Cluster 2: physical inactivity behavior associated to excess weight, 3) Cluster 3: unhealthy diet associated to excess weight and low practice of physical activity, and 4) Cluster 4: smoking behavior with physical activity behavior. The pattern of cluster membership differed across sex (<10–3), school level, and socioeconomic level (<10–3) but there was no significant difference between clusters for mother’s education levels and household tenure. Conclusion: This study can have important implications for health policy and practice. Indeed, it found that many subjects have simultaneous multiple NCDs risk factors which leads to identify groups at risk and implement integrated intervention program.

Objective: This study aims to assess the relationship that frailty has with nutritional status and functional risk factors in community-dwelling older adults. Methods: Cross-sectional study in community-dwelling older people, independent for walking and without impaired cognition. Frailty was assessed by Fried criteria. Nutritional status was analyzed by the Mini Nutritional Assessment Short Form (MNA-SF), biochemical markers (albumin, total proteins, cholesterol, lymphocytes, and hemoglobin); and anthropometric parameters (body mass index [BMI], body fat percentage, handgrip, and perimeters). A comprehensive geriatric assessment analyzed other risk factors: functionality, cognition, falls, comorbidity, polypharmacy, physical activity, and quality of life (QoL). Results: We included 564 elderly people with a mean age of 76.05 (standard deviation 3.97) years; 63.1% (n = 356) were women, and 83.9% (n = 473) were prefrail, and frail. The sample presented high functionality and a nutritional status with a predominance of overweight and obesity. Factors associated with frailty (R2 = 0.43) were age over 75 years (odds ratio [OR] 3.31, 95% confidence interval [CI] 1.76, 6.21; p < 0.001), female gender (OR 2.37, 95% CI 1.24, 4.52; p = 0.009), anemia (OR 2.45, 95% CI 1.19, 5.02; p = 0.015), falls (OR 1.94, 95% CI 1.12, 3.25; p = 0.016) and the fear of falling (OR 4.01: 95% CI 1.76, 9.16; p = 0.001). Performing more than 3 weekly hours of physical activity was found to be a protective factor (OR 0.23, 95% CI 0.15, 0.35; p < 0.001). Conclusions: The relationship between frailty and malnutrition in functionally independent community-dwelling older people is unclear. More studies are needed to know what nutritional markers are related to frailty, cognition, and functionality in order to discriminate the risk factors for community-dwelling older people at risk of malnutrition and dependency.

ABSTRACT Background: Actually the lifestyle exposes the population to several risk factors related to alimentary habits and less physical activity that contributes to chronic diseases appearance worldwide. Aim: To analyze the association between salivary cortisol and the components of metabolic syndrome. Methods: This is a cross-sectional study. As part of it, 28 individuals aged 30-59 years presenting three or more of the following findings: CA: ≥88 cm for women and ≥102 cm for men; SBP>130 mmHg and DBP>85 mmHg; GL>100 mg/dl; TG>150 mg/dl; HDL<40 mg/dl for men and <50 mg/dl for women. Was performed analysis of salivary cortisol (by radioimmunoassay) from 25 salivary samples collected throughout the day, for evaluating changes in the circadian rhythm of this hormone (8AM, noon and 8PM). Results: 28 evaluated individuals had a mean age of 51.9±7.5 years, mostly women (64.3%) and a mean of BMI 33.6±3.2 kg/m². The cortisol level from the 8AM averaged 18.7±4.8 ng/dlL. Individuals with FPG>110mg/dl, have significantly lower average levels of cortisol than ones with FPG <110 (12.8±5,2 vs. 17.3±4.2). Significant correlations were HOMA vs. WC (r=0,465; p˂0,005) and TG (r=0,473; p˂0,005), WC vs. FG (r=0,446; p˂0,005) and BMI (r=0,730; p˂0.0001); TG vs. HDL (r=0,441 p˂0,005) and FG (r=0,440; p˂0,005). Conclusion: Morning salivary cortisol in subjects with chronically elevated blood glucose can represent a downregulation of the hypothalamic-pituitary adrenal axis. This is an important finding not yet well investigated.

Background The prevalence of overweight and obesity is rising at a rapid pace and is associated with negative health consequences like cardiovascular diseases, type 2 diabetes and cancer. Obesity is a multifactorial problem that develops mainly from lifestyle factors including physical inactivity and poor dietary intake. Dietary diversity is a simplified method for assessing the adequacy and quality of diet and is associated with nutritional need and overall health status. Therefore, we conducted this study to synthesize the associations between consumption of a diversified diet and overweight/ obesity among adults living in pastoral communities in Monduli district in Tanzania. Methods This was a cross-sectional study conducted among 510 adults aged ≥ 18 years old in the Monduli district, Arusha region in Tanzania. We conducted face-to-face interviews to collect information about socio-demographic characteristics, 24-hours dietary recall, and anthropometric measurements. The dietary diversity score (DDS) was constructed and used to determine the diversity of the diet consumed. We performed the multivariate Poisson regressions to determine the prevalence ratio (PR) with 95% confidence intervals (CI). The dependent variables were overweight and general obesity as measured by body mass index (BMI), abdominal obesity measured by waist-hip ratio (WHR) and waist circumference (WC). Results The prevalence of general obesity based on BMI was 20.2% (95%CI; 16.9–23.9), abdominal obesity based on WHR was 37.8% (95%CI; 33.7–42.1), and WC was 29.1% (95%CI; 25.2–33.1). More than half (54.3%) of the participants consumed an adequate dietary diversity (DDS ≥4). After adjustment for potential confounders, the prevalence of abdominal obesity by WHR decreased with higher DDS among male (APR = 0.42; 95% CI, 0.22–0.77) and female participants (APR = 0.63; 95% CI, 0.41–0.94). There were inconsistent positive associations between DDS and prevalence of overweight and general obesity among male and female. There was no association between DDS and abdominal obesity by WC. Conclusion More than half of the pastoralists have consumed an adequate diversified diet. Given the inconsistent findings on associations between dietary diversity and obesity measures, this study suggests that targeting dietary diversity as an overweight/obesity prevention strategy requires careful consideration.

The aims of this study were a) to assess whether obesity acts as a mediator between i) cardiorespiratory fitness (CRF) and mean blood pressure; and ii) between between physical activity (PA) and mean blodd pressure in children and adolescents. A cross-sectional study was conducted with a 632 children and adolescents. It was measured mean blood pressure, body mass index, fat mass and waist circumference. CRF and PA was assessing with Course Navette test and ActiGraph. The analysis of the mediation was performed using Process macro for SPSS. The results indicate that obesity acts as a partial mediation in the association between CRF and mean blood pressure in 10-12 years old children (z=from -5.81 to -5.40; all p˂0.000). These results indicate that obesity acts as a complete mediator in the association between PA and mean blood pressure in 10-12 years old children (z=from -4.49 to -1.94; all p˂0.000). Our result reinforces the relevance of prevent weight increse and improve cardiorespiratory fitness level since erly age in children and adolescents to prevent high mean blood pressure. Increasing the level of physical activity can influence on obesity and cardiorespiratory fitness.
