The aim was to study the activity of progesterone and 5α-pregnan-3.20-dione conversion in the placenta at the exacerbation of cytomegalovirus infection (CMVI) during the first trimester of pregnancy. The study material included 50 villous chorions taken during medical abortions in the period of 8-10 weeks from women with exacerbation of chronic CMVI during pregnancy. The control group had 35 villous chorions of pregnant women of the same gestation period with chronic CMVI in the latent stage. Diagnosis of CMVI was performed taking into account the activity of CMVI with the immune-enzyme method by the level of antibodies of IgM and antibody titers of IgG in the paired serums in dynamics in 10 days. The progesterone content was determined at the “Stat Fax-2100ˮ spectrophotometer. The activity of 4-pregnene-3.20-dione dehydrogenase and 5α-pregnane-3.20-dione dehydrogenase was assessed by the histochemical method. Quantitative evaluation of the reaction products was carried out on sections under the microscope “Meiji Technoˮ connected to the software-hardware complex “SCION Corporationˮ. It was found out that progesterone content in villous chorions of women with reactivation of CMVI during pregnancy was reduced to 21.5±2.73 nmol/L in comparison with the control group (53.4±3.70 nmol/L, p<0.001). The disturbance of progesterone inversion intensiveness in placenta was proved by the decrease of enzyme activity: of 4-pregnene-3.20-dione dehydrogenase till 15.9±0.47 standard units (in control it was 28.6±0.58 standard units, p<0.001) and of 5α-pregnan-3.20-dione till 16.2±1.11 standard units (in control it was 25.0±1.91 standard units, p<0.001). Reduction in the intensity of progesterone conversion into 5α-dihydroprogesterone (5α-pregnan-3.20-dione) in the placenta with the exacerbation of CMVI can negatively affect the formation and functioning of the fetal nervous system.

The aim is to analyze androstenediol converting in villous chorions at gestation against cytomegalovirus infection activation during the first trimester of pregnancy. 48 villous chorions samples taken during medical abortion at 8-10th weeks of gestation of women with chronic cytomegalovirus reactivation during pregnancy became the material for study, and 35 villous chorions samples were taken as the control. The results of pregnant women examination were analyzed from the point of cytomegalovirus infection activity with immune-enzyme method by the presence of IgM antibodies or by the value of 4-times and more growth of IgG antibodies titer in paired sera in dynamics in 10 days. IgG avidity index to cytomegalovirus was more than 75% in all women. Androstenediol converting was assessed with histochemical methods. Quantitative evaluation of the reaction products was carried out on sections under the microscope MT (Japan) associated with software-hardware complex «SCION Corporation» (USA). Histochemical reaction product of the androstenediol dehydrogenase was well detected in cyto- and syncytiotrophoblast of chorious villi. Reaction activity in the material from women with reactivation of cytomegalovirus infection during pregnancy was low, which indicated a reduction in the reserve for the synthesis of estrogen. The concentration of estriol and estradiol in villous chorions during reactivation of cytomegalovirus infection decreased till 24.9±2.01 nmol/l and 15765.1±107.77 pmol/l, respectively.

The aim of the work is to assess the effects of cytomegalovirus infection on the 20α-hydroxysteroid dehydrogenase activity in the villous chorion syncytiotrophoblast in the first trimester of pregnancy. The materials for the study included 48 villous chorions samples taken during spontaneous abortions at 8th–10th weeks of gestation from women with reactivation of cytomegalovirus infection (the main group). The control group included 35 villous chorions samples from pregnant women with chronic cytomegalovirus infection in the latent stage taken at medical abortions at the same stage of gestation. The results of women’s examination were studied taken into account the activity of cytomegalovirus infection by immunoassay by detection of IgM antibodies or by 4 times and more increase of IgG antibody titers in the paired serums in dynamics in 10 days. The activity of 20α-hydroxysteroid dehydrogenase was assessed by the histochemical method. Quantitative evaluation of the reaction products was carried out on sections under the microscope MT (Japan) connected to software-hardware complex «SCION Corporation» (USA). At histochemical specimen of villous chorion of pregnant women who had a reactivation of cytomegalovirus infection during pregnancy (main group), there was a decrease of cytophotometric parameter of 20α-hydroxysteroid dehydrogenase activity till 30.1±2.12 pixels/μm2 (p<0.001) compared with the control group. The decrease in the reaction activity in syncytiotrophoblast indicated a decrease in the contents of 20α-dihydroprogesterone in placenta, which contributed, in our opinion, to spontaneous abortion.

ABSTRACT The fractional determination of 17-ketosteroids in the daily urine was performed in nine cases of hyperemesis gravidarum and in four control cases, in the first trimester of pregnancy both before and after corticotrophin administration. The excretion of total 17-KS is similar in the two groups. Only in the hyperemesis group does the excretion of total 17-KS increase significantly after corticotrophin administration. The fractional determination reveals no difference between the two groups of patients with regard to the values of the fractions U (unidentified 17-KS), A (androsterone) and Rest (11-oxygenated 17-KS). The excretion of dehydroepiandrosterone is significantly higher in the hyperemesis group than in the control group. The excretion of androstanolone seems to be lower in the hyperemesis group than in the control group, but the difference is not statistically significant. The differences in the correlation between dehydroepiandrosterone and androstanolone in the two groups is significant. The high excretion of dehydroepiandrosterone and low excretion of androstanolone in cases of hyperemesis gravidarum is a sign of adrenal dysfunction.

AbstractOne of the most frequent causes of maternal and perinatal morbidity is represented by hypertensive disorders during pregnancy. Women at high risk must be subjected to a more intensive antenatal surveillance and prophylactic treatments. Many genetic risk factors, clinical features and biomarkers have been proposed but none of these seems able to prevent pre-eclampsia onset. English literature review of manuscripts focused on calcium intake and hypertensive disorders during pregnancy was performed. We performed a critical analysis of evidences about maternal calcium metabolism pattern in pregnancy analyzing all possible bias affecting studies. Calcium supplementation seems to give beneficial effects on women with low calcium intake. Some evidence reported that calcium supplementation may drastically reduce the percentage of pre-eclampsia onset consequently improving the neonatal outcome. Starting from this evidence, it is intuitive that investigations on maternal calcium metabolism pattern in first trimester of pregnancy could represent a low cost, large scale tool to screen pregnant women and to identify those at increased risk of pre-eclampsia onset. We propose a biochemical screening of maternal calcium metabolism pattern in first trimester of pregnancy to discriminate patients who potentially may benefit from calcium supplementation. In a second step we propose to randomly allocate the sub-cohort of patients with calcium metabolism disorders in a treatment group (calcium supplementation) or in a control group (placebo) to define if calcium supplementation may represent a dietary mean to reduce pre-eclampsia onset and to improve pregnancy outcome.

Abstract Context Medical treatment of Graves disease during the first trimester has been the subject of controversy ever since treatment with an antithyroid drug during the first trimester was reported to possibly be associated with an increased risk of birth defects in newborns. Objective We investigated whether the incidence of birth defects among newborns born to mothers with Graves disease (GD) treated with propylthiouracil (PTU) during the first trimester of pregnancy was higher than in a control group that was not exposed to any medication. Methods We reviewed the cases of 1913 women with GD who gave birth between January 1, 2015, and May 31, 2019. Detailed information concerning the outcome of pregnancy and the presence of birth defects was collected at the first visit after the delivery and again 1 year after delivery. We classified the mothers and infants into 3 groups according to the treatment the mother had received for GD in the first trimester of pregnancy: a group in which the mothers had been treated with PTU alone (PTU group), a group in which the mothers had not been treated with any medication (control group), and a group in which the mothers had received some other medical treatment, such as thiamazole, potassium iodide, or 2 or more drugs (other treatment group). Results The incidence of malformed infant births was 5.5% (30/541 infants) in the PTU group and 5.7% (27/ 475 infants) in the control group. There were no specific birth defects in the PTU group, and there were no significant differences between PTU dosages or maternal thyroid function according to whether mothers had delivered a child with a birth defect. Conclusion The results of our retrospective study showed that treatment with PTU during the first trimester of pregnancy did not increase the incidence of birth defects among newborns.

The aim is to study the features of changes of free-radical status, antioxidant protection and morphologic changes of erythrocytes of the peripheral blood of pregnant women of the first trimester with exacerbation of cytomegalovirus infection (CMVI). 35 pregnant women with exacerbation of CMVI in the first trimester of gestation (the main group) and 35 pregnant women without this pathology of the same age and term as the patients of the main group were examined. Erythrocytes of the venous blood stabilized by heparine as well as blood plasma became the material of the study. There has been studied the condition of lipid peroxidation (LP) by the contents of diene conjugates and active products reacting with thiobarbituric acid; the state of nonenzymatic antioxidant protection by the contents of α-tocopherol as well as of the enzyme of erythrocytes (superoxide dismutase); morphologic changes of erythrocytes. It was found out that the exacerbation of CMVI in the first trimester of pregnancy is associated with activation of LP processes which include modification of not only primary, reverse stages of this process – the increase of the concentration of diene conjugates (p<0.001), but they lead to an increased formation of secondary active products reacting with thiobarbituric acid (p<0.001), intensifying the destructive action on the membrane of erythrocytes. Primary and secondary products of LP against the decrease of concentration of erythrocyte superoxide dismutase (p<0.001) and α-tocopherol (p<0.001) are accumulated in increasing concentrations and become potential factors of erythrocytes damage causing their functional and metabolic inferiority. Metabolic disturbances and damages of cellular membranes lead to the increase of population of echinocytes, target-like and degenerative forms of erythrocytes, which can be accompanied by a high probability of intravascular hemolysis and contribute to the development of hemic hypoxia complicating the infectious process and the course of pregnancy. Within the present hyperoxidation and suppression of antioxidant protection that lead to the worsening of the rheology of blood, women with exacerbation of CMVI in the first trimester of pregnancy should be prescribed the medicine stabilizing the lipid exchange.

The purpose of the study is to evaluate the effect of placental dysfunction caused by gestational endotheliopathy on the course of labor and the condition of the newborn. The first group consisted of 70 patients with placental dysfunction with gestational endotheliopathy confirmed by laboratory-instrumental findings in the first trimester of pregnancy. The control group included 30 pregnant women with physiological gestational course. PD secondary to GE leads to preterm birth, fetal distress, increases the percentage of caesarean section, contributes to the delay of fetal growth and birth weight, poor infant status and perinatal complications.

Introduction. It is now generally acknowledged that cytomegalovirus infection (CMVI) is one of the main causes of miscarriage. However, the mechanism of this effect has not been sufficiently studied. At the same time, the influence of acids of the ɷ-3 family and α-tocopherol (αTP) on the placentation process through a pro-angiogenic action is shown.Aim. To study the concentration of αTP and ɷ-3 family acids in the peripheral blood and establish their role in miscarriage in CMV-seropositive women with CMVI reactivation.Materials and methods. A case-control study included 64 women in the first trimester of pregnancy (7-10 weeks), of which 36 were CMV-seropositive with CMVI reactivation (main group) and 28 were CMV-seronegative (control group). CMVI was diagnosed by the determination of class M and G antibodies by ELISA, as well as CMV DNA detected by PCR. The concentration of ɷ-3 acids of the family (eicosapentaenoic – EPA, docosahexaenoic – DHA) in blood serum was studied by capillary gas-liquid chromatography (J.P.Carreau, J.P.Dubacq). The αTP concentration was determined by the fluorometric method (L.G.Hansen, W.I.Warwich).Results. In women of the main group, a significant (p<0.001) decrease in the concentration of αTP to 1.32±0.025 μg/mL was observed in the peripheral blood compared to the same indicator in the control group (1.49±0.029 μg/mL). At the same time, the levels of EPA and DHA were also statistically significant (p<0.001) lower than the same indicator in the control group and amounted to 1.09±0.012 and 6.09±0.015%, respectively (in the control, 1.29±0.071 and 8.80±0.071%, respectively). Conclusion. The obtained results of the study allow us to establish the important role of disorders in the content of α-TF, EPA and DHA in the pathogenesis of miscarriage during reactivation of CMVI in the early periods of gestation, which can serve as a basis for expanding diagnostic and therapeutic measures in this pathology of pregnant women.

The contents of β-subunit of chorionic gonadotropin (β-CG), estriol, progesterone, dehydroepiandrosterone (DEAS), placental lactogen and cortisol were studied in 164 women of child-bearing age at the 7-9th weeks of pregnancy. The first group (control) consisted of 30 women with physiologic course of pregnancy. The second group (comparison) had 32 patients with the threat of premature delivery of non-infectious etiology. The third group included 36 patients with influenza virus А(Н3N2) (anti-virus antibody titers were 1:4-1:16); the fourth group had 34 women with influenza virus А(Н3N2) (antibody titers were 1:8-1:32); in the fifth group there were 32 patients with influenza virus А(Н3N2) (antibody titers were 1:32-1:128) and the threat of premature delivery in the period of reconvalescence. The patients of the fifth group had the decrease of the level of β-CG till 40.5±2.0 mМЕ/ml, of estriol till 3.4±0.21 nmole/l, of progesterone till 35.1±3.13 nmole/l, of DEAS till 7.3±0.68 nmole/l, of placental lactogen till 12.1±1.0 nmole/l, as well as the increase of cortisol till 738.7±22.08 nmole/l in comparison with 73.0±2.0 mМЕ/ml (р&#60;0.001), 5.9±0.33 nmole/l (р&#60;0.001), 70.6±4.14 nmole/l (р&#60;0.001); 7.3±0.68 nmole/l (р&#60;0.001), 12.1±1.0 nmole/l (р&#60;0.001) and 567.8±24.02 nmole/l (р&#60;0.001) in the first group, respectively. The differences of hormonal status in the fifth and third group consisted of the drop of concentration of β-CG by 22.8% (р&#60;0.001), of estriol by 26.1% (р&#60;0.01), of progesterone by 32.1% (р&#60;0.001) and placental lactogen by 32.4% (р&#60;0.001). At the same time there were no significant changes in the concentration of DEAS and there were registered higher indicators of cortisol, i.e. the increase by 11.5% (р&#60;0.05), which suggested the significance of the growth of antivirus antibody titers in the suppression of synthesis of hormones in ovaries and in syncytiotrophoblast as a result of direct and indirect negative influence of influenza infection.

The aim of the work is to study the content of 11-deoxycorticosterone in the placenta during pregnancy complicated by exacerbation of cytomegalovirus infection. Histochemical method was used to determine the content of 11-deoxycorticosterone in 72 placentas, including 37 cases from women with acute cytomegalovirus infection and simultaneously diagnosed moderate preeclampsia, and 35 placentas from women with exacerbation of chronic cytomegalovirus infection without concomitant diseases. The control group consisted of 30 placentas from mothers with chronic cytomegalovirus infection in the latent stage. It was found out that an increase in the content of 11-deoxycorticosterone in placenta by 25% in comparison with the control group occurred during gestation with an exacerbation of cytomegalovirus infection. A significant increase (by 40%) in the content of this hormone in the placenta was noted during the reactivation of the viral process accompanied by preeclampsia. We believe that an increase in the production of 11-deoxycorticosterone with mineralocorticotropic effect in the placenta could contribute to the development of hypertensive disorders in pregnancy complicated by cytomegalovirus infection.
