The penis is the primary site of HIV acquisition in heterosexual men. Elevated penile inflammatory cytokines increase sexual acquisition risk, and topically applied cytokines enhance foreskin HIV susceptibility in an explant model. However, the impact of penile-vaginal sex on these immune parameters is undefined. Heterosexual couples were recruited to the Sex, Couples and Science (SECS) Study, with the collection of penile swabs, semen, cervico-vaginal secretions, and blood after a period of abstinence, and repeated sampling up to 72 hours after either condomless (n = 30) or condom-protected (n = 8) penile-vaginal sex. Soluble immune parameters were quantified by multiplex immunoassay. Co-primary immune endpoints were penile levels of IL-8 and MIG, cytokines previously linked to penile HIV acquisition. One hour after sex there were dramatic increases in penile IL-8 and MIG levels, regardless of condom use, with a gradual return to baseline by 72 hours; similar patterns were observed for other chemoattractant chemokines. Penile cytokine changes were similar in circumcised and uncircumcised men, and repeated measures ANOVA and ANCOVA models demonstrated that the degree of change after condomless sex was explained by cytokine levels in their partners’ cervico-vaginal secretions. This may have important implications for the biology of penile HIV acquisition.

Levels of inflammatory cytokines in the coronal sulcus are associated with an increased HIV risk, and we hypothesized that these may be altered after insertive penile sex. Therefore, we designed the Sex, Couples and Science Study (SECS study) to define the impact of penile-vaginal sex on the penile immune correlates of HIV susceptibility. We found that multiple coronal sulcus cytokines increased dramatically and rapidly after sex, regardless of condom use, with a return to baseline levels by 72 hours. The changes observed after condomless sex were strongly predicted by cytokine concentrations in the vaginal secretions of the female partner, and were similar in circumcised and uncircumcised men. We believe that these findings have important implications for understanding the immunopathogenesis of penile HIV acquisition; in addition, they have important implications for the design of clinical studies of penile HIV acquisition and prevention.

Abstract We conducted a prospective study of 13 heterosexual couples to understand the impact of recent condomless vaginal sex on vaginal immune marker measurement and potential exposure misclassification due to the presence of semen. All immune markers were detectable in semen and concentrations of vaginal immune markers varied by sex recency.

AbstractHuman immunodeficiency virus (HIV) remains a leading cause of global morbidity with the highest burden in Sub-Saharan Africa (SSA). For reasons that are incompletely understood, the likelihood of HIV transmission is several fold higher in SSA than in higher income countries, and most of these infections are acquired by young women. Residents of SSA are also exposed to a variety of endemic infections, such as malaria and various helminthiases that could influence mucosal and systemic immunology. Since these immune parameters are important determinants of HIV acquisition and progression, this review explores the possible effects of endemic infections on HIV susceptibility and summarizes current knowledge of the epidemiology and underlying immunological mechanisms by which endemic infections could impact HIV acquisition. A better understanding of the interaction between endemic infections and HIV may enhance HIV prevention programs in SSA.

Purpose Arthralgia occurs in up to 50% of breast cancer survivors treated with aromatase inhibitors (AIs) and is the most common reason for poor AI adherence. We conducted, in 121 breast cancer survivors receiving an AI and reporting arthralgia, a yearlong randomized trial of the impact of exercise versus usual care on arthralgia severity. Patients and Methods Eligibility criteria included receiving an AI for at least 6 months, reporting ≥ 3 of 10 for worst joint pain on the Brief Pain Inventory (BPI), and reporting < 90 minutes per week of aerobic exercise and no strength training. Participants were randomly assigned to exercise (150 minutes per week of aerobic exercise and supervised strength training twice per week) or usual care. The BPI, Western Ontario and McMaster Universities Osteoarthritis (WOMAC) index, and Disabilities of the Arm, Shoulder and Hand (DASH) questionnaire were completed at baseline and at 3, 6, 9, and 12 months. Intervention effects were evaluated using mixed-model repeated measures analysis, with change at 12 months as the primary end point. Results Over 12 months, women randomly assigned to exercise (n = 61) attended 70% (± standard deviation [SD], 28%) of resistance training sessions and increased their exercise by 159 (± SD, 136) minutes per week. Worst joint pain scores decreased by 1.6 points (29%) at 12 months among women randomly assigned to exercise versus a 0.2-point increase (3%) among those receiving usual care (n = 60; P < .001). Pain severity and interference, as well as DASH and WOMAC pain scores, also decreased significantly at 12 months in women randomly assigned to exercise, compared with increases for those receiving usual care (all P < .001). Conclusion Exercise led to improvement in AI-induced arthralgia in previously inactive breast cancer survivors.

Background: Occupational footwear and a prolonged duration of walking have been previously reported to play a role in maintaining postural stability. The purpose of this paper was to analyze the impact of three types of occupational footwear: the steel-toed work boot (ST), the tactical work boot (TB), and the low-top work shoe (LT) on previously unreported lower extremity muscle activity during postural stability tasks. Methods: Electromyography (EMG) muscle activity was measured from four lower extremity muscles (vastus medialis (VM), medial hamstrings (MH), tibialis anterior (TA), and medial gastrocnemius (MG) during maximal voluntary isometric contractions (MVIC) and during a sensory organization test (SOT) every 30 min over a 4 h simulated workload while wearing ST, TB, and LT footwear. The mean MVIC and the mean and percentage MVIC during each SOT condition from each muscle was analyzed individually using a repeated measures ANOVA at an alpha level of 0.05. Results: Significant differences (p < 0.05) were found for maximal exertions, but this was limited to only the time main effect. No significant differences existed for EMG measures during the SOT. Conclusion: The findings suggest that occupational footwear type does not influence lower extremity muscle activity during both MVIC and SOT. Significantly lower muscle activity during maximal exertions over the course of the 4 h workload was evident, which can be attributed to localized muscular fatigue, but this was not sufficient to impact muscle activity during postural stability tasks.

Background The intent of this study was to evaluate the impact of the commonly used colloids-hetastarch and albumin-on in vivo acid-base balance. From this evaluation, a better understanding of the mechanism of dilutional acidosis was expected. Methods In a prospective, randomized fashion, 11 healthy volunteers were administered 15 ml/kg hetastarch solution, 6%, or 15 ml/kg albumin, 5%, intravenously over 30 min. Four weeks later, the study subjects were administered the other colloid. Arterial blood gas and electrolyte parameters were measured at baseline and at 30, 60, 90, 120, 210, and 300 min after colloid administration. Pre- and postlaboratory values were compared within groups using a paired t test and a Wilcoxon signed rank test and between groups using repeated-measures analysis of variance and a Wilcoxon rank sum test. Results Thirty min after infusion, subjects who were administered hetastarch showed statistically significant changes (P &lt; 0.05) in base excess (from 2.5 +/- 0.9 mEq/l to 0.7 +/- 1.1 mEq/l), HCO3- concentration (from 27 +/- 1.0 mEq/l to 25 +/- 1.3 mEq/l), Cl- concentration (from 108 +/- 2 mEq/l to 112 +/- 2 mEq/l), albumin concentration (from 4.4 +/- 0.2 g/dl to 3.5 +/- 0.5 g/dl), and arterial carbon dioxide tension (Paco2; from 40.8 +/- 2.3 mmHg to 39. 2 +/- 3.2 mmHg), whereas only the albumin concentration (from 4.4 +/- 0.2 g/dl to 4.8 +/- 0.6 g/dl) changed significantly in the albumin-treated group. Conclusions Decreases in base excess were observed for 210 min after hetastarch administration but not after albumin. The mechanism for this difference is discussed.

This study aims to determine to what extent the opportunities and restrictions of the partner market influence educational assortative mating. It also analyzes the interplay between the opportunity structure and preferences. Matching district-based partner market indicators to heterosexual couples when they move in together based on the German Socio-Economic Panel, we find strong effects of the opportunity structure on educational homogamy. The results further imply that the density of the supply of potential partners is more important for educational assortative mating than imbalanced supply and competition. While the impact of partner market imbalances on assortative mating is a mere effect of the opportunity structure, the effects of the partner market density of relevant and available partners in space weakly imply that homophile and maximization preferences are simultaneously at work.

This study investigated the impact of mental fatigue and self-controlled versus yoked feedback on learning a force production task. We randomly assigned 44 non-athlete male students (Mage = 21.4, SD = 1.4 years) to four groups; (a) MF&SCF = mental fatigue & self-controlled feedback, (b) MF&Y = mental fatigue & yoked, (c) NMF&SCF = no mental fatigue & self-controlled feedback, and (d) NMF&Y = no mental fatigue & yoked). SCF group participants were provided feedback whenever they requested it, while YK group participants received feedback according to a schedule created by their SCF counterparts. To induce mental fatigue, participants performed a Stroop color-word task for one hour. During the acquisition (practice) phase, participants were asked to produce a given percentage of their maximum force (20%) in 12 blocks of six trials. We recorded the participants’ absolute error at the end of the acquisition phase, the immediate retention test, the first transfer test, and the second transfer test (after 24 hours and without any further mental fatigue). The acquisition phase data were analyzed in a 2 (feedback) × 2 (mental fatigue) × 12 (block) ANOVA with repeated measures on the last factor, while the retention and transfer data were analyzed in 2 (feedback) × 2 (mental fatigue) ANOVAs. We found that all four groups made significant progress during practice ( p < .001), but there were no significant group differences during this phase ( p>.05). There was a significant interaction effect of self-controlled feedback and mental fatigue at retention ( p = .018) and transfer testing ( p < .001). In the mental fatigue condition, participants in the self-controlled group had poorer learning compared to participants in the yoked group; but when not mentally fatigued, participants in the self-controlled group had better learning than those in the yoked group. These findings suggest that mental fatigue reduces typical advantages of self-controlled feedback in motor learning.

Abstract Suspension exercise systems are being used in strength and conditioning facilities, fitness centers, rehabilitation centers and home gyms. Though some evidence exists regarding the impact of training with these systems, more work is needed for a better understanding. The purpose of the present investigation was to examine the acute effects of an exercise session with 2 (hands only) and 4 straps (hands and feet) in the push-up exercise compared to a work-matched bench press exercise session. The participants for this repeated measures, cross-over investigation were 18 healthy college-aged males (age: 24.8 ± 3.5 yrs, body mass: 81.8 ± 7.8 kg, body height: 178.9 ± 4.5 cm). The conditions were 6 sets of 10 repetitions of suspension push-ups using two straps (DUAL) for the hands, fours straps (QUAD) for hands and feet and a traditional bench press exercise matched to the average resistance during the suspension push-up. The participants performed all repetitions at a controlled cadence. Expired gases, and heart rate were monitored continuously during the exercise session. Pre and post exercise saliva samples were collected to quantify changes in testosterone and cortisol. Upper body isometric strength tests ( UBIST) were performed (Post, 1 hr, 24 hr, 48 hr) to evaluate changes in force production during recovery. Data analysis via repeated measures ANOVA revealed a significant trend for increased oxygen consumption in the QUAD condition compared to the bench press (p = 0.019). Additionally, both suspension conditions resulted in a reduced respiratory exchange ratio as compared to the bench press (p < 0.05). A significant main effect was noted for time in all conditions regarding isometric strength (p < 0.001), but no differences between conditions were revealed. Testosterone and cortisol responses did not differ between conditions. Based upon these data, it appears that when matched for work, suspension exercise results in equivalent reductions in muscle force, but greater oxygen consumption compared to isotonic exercise.
