Developmental Transformations over video chat: An exploration of presence in the therapeutic relationship

This study examined how the phenomenon of presence in the therapeutic relationship shifts when participating in therapy online as opposed to in person. A duoethnographic study was conducted in which the researcher and another drama therapist engaged in two Developmental Transformations sessions, one face-to-face and one via video chat, and wrote about their subjective experiences of presence during each session. From these data, themes arose that confirmed the complexity of presence and suggested presence is further complicated during virtual interactions.

Background Speech-language pathologists make clinical decisions informed by evidence-based theory and “beliefs, values and emotional experiences” ( Hinckley, 2005 , p. 265). These subjective processes, while not extensively studied, underlie the workings of the therapeutic relationship and contribute to treatment outcomes. While speech-language pathologists do not routinely pay attention to subjective experiences of the therapeutic encounter, social workers do. Thus, the field of social work makes an invaluable contribution to the knowledge and skills of speech-language pathologists. Purpose This clinical focus article focuses on the clinician's contribution to the therapeutic relationship by surfacing elements of the underlying subjective processes. Method Vignettes were gathered from clinicians in two community aphasia programs informed by the principles of the Life Participation Approach to Aphasia. Results and Discussion By reflecting on and sharing aspects of clinical encounters, clinicians reveal subjective processing occurring beneath the surface. The vignettes shed light on the following clinical behaviors: listening to the client's “whole self,” having considerations around self-disclosure, dealing with biases, recognizing and surfacing clients' identities, and fostering hope. Speech-language pathologists are given little instruction on the importance of the therapeutic relationship, how to conceptualize this relationship, and how to balance this relationship with professionalism. Interprofessional collaboration with social workers provides a rich opportunity to learn ways to form and utilize the benefits of a strong therapeutic relationship while maintaining high standards of ethical behavior. Conclusion This clinical focus article provides speech-language pathologists with the “nuts and bolts” for considering elements of the therapeutic relationship. This is an area that is gaining traction in the field of speech-language pathology and warrants further investigation.

The execution of academic researches - such as Undergraduate work, Master’s or PhD Thesis - is often supervised by a research advisor. The development process of such works could be characterized as face-to-face, remote or blended orientation, which combines both of former ones. The construction of a proposal for blended academic orientation involves mapping and analysis of elements, as well as didactical and communication structures that would differs face-to-face meetings from virtual interactions. The paper presents some considerations about relevant characteristics related to digital generation and learning theories that value interaction in order to built knowledge, thus allowing the determination of a blended methodology that aims to enhance intrinsic motivation and investigative posture of students at any level.

BACKGROUND During the COVID-19 pandemic, people are being encouraged to maintain social distance. Technology is helping people to reschedule meetings from “face-to-face” interactions to remote videoconferencing. Psychologists are in high demand, due to an increase in stress as a result of COVID. Many seek to both keep treating their current patients, and welcome new ones, given the current high demand for their services. Videoconferencing provides an opportunity to do this. However, shifting treatment from face-to-face to the videoconferencing is not simple as both the psychologist and the patient miss the in-person information and cues, such as body language provides. OBJECTIVE A new theoretical framework is proposed to guide the design of future studies on the impact of the computer as a mediator of psychologist-patient relationships, and the influence of videoconferencing on the whole relationship process. METHODS A literature review has been conducted, screening studies focusing on communication, and the key concepts of therapeutic relationship and therapeutic alliance. RESULTS Studies report that patients are generally satisfied with videoconference therapy in terms of the relationship with their therapists and the establishment of the “therapeutic alliance”. However, psychologists report difficulties in establishing same quality of therapeutic relationship and therapeutic alliance. The analysed studies lead us to interpret data under a different perspective. A new model of relationship is proposed, along with further hypotheses. CONCLUSIONS It is important to consider the computer as having an active role in psychologists and patients’ relationships. CLINICALTRIAL

Health influences general well-being, and well-being affects future health. Oral health professionals report a decreased well-being and a higher burnout. This chapter measures and evaluates the perception of the health professional and the patient about factors of stress and well-being. It evaluates the strategies used to overcome the anxiety and stress that involve the meeting. Two surveys applied by questionnaire, with face-to-face and online dissemination, the first to patients and second to dentists, were done. Of the 245 patients, 46% consider themselves to be anxious. The instruments used in a clinical environment cause discomfort, and their noise is the predominant cause for this fear. Of the 306 dentists, 80% show the ability to face difficult situations. Finally, 90% have an awareness that contributes to the well-being of others. Oral health professionals should prepare themselves with techniques to develop a therapeutic relationship that is more positive, calm, and less stressful.

Caring, a core tenet of nursing practice, grew out of a holistic approach. Nurse theorists often note the establishment of a therapeutic relationship as the beginning point of caring, with subsequent nursing interventions reliant upon this relationship for effectiveness. Relational exchange serves as a source of either stress or healing between participants, and rarely is its impact neutral. Relational stress, in fact, has become a primary contributor to many disease processes in terms of promotion and progression and perhaps even initiation. Patient–provider relationships have a long history in medical and nursing literature as critical to providing effective interventions, but our understanding of relational dynamics between patients and providers remains fairly superficial. This theoretical article adapts a previously described biobehavioral model to illustrate the nature and centrality of caring relationships in nursing practice. The dynamic process of face-to-face engagement is deconstructed from a psychobiological standpoint in order to understand the physiological, emotional, cognitive, and behavioral impacts of relational interaction. This understanding is then applied to the patient–provider relationship. Finally, the utility of biomarkers of stress, positive emotion and resonance, and of disease is discussed relative to the patient-provider relationship. Methodological and interpretive challenges inherent in this line of research, along with suggestions to address such challenges, are also presented.

Psychiatry residents in Norway have 70 hours of mandatory psychotherapy supervision to develop insights into the therapeutic relationship. Six supervision pairs (six candidates and two supervisors) conducted five videoconferencing-based supervision sessions (384kbit/s) and five face-to-face sessions alternating weekly for 10 sessions. Following completion of the 10 sessions for candidates and supervisor B, and the 50 sessions for supervisor A, all subjects completed a semi-structured interview within two weeks. The eight subjects reported a wide range of experiences and attitudes. The results suggested that the quality of supervision can be satisfactorily maintained by using videoconferencing for up to half of the 70 hours required. The precondition for this estimate is that the pair in question have met face to face and established a relationship characterized by mutual trust and respect. Further studies, which include supervision pairs not having previously established relationships, are needed in order to indicate the generality of this precondition. The most obvious implication of this study is the potential for implementing decentralized models for recruiting and educating psychiatrists.

This paper reveals the dynamics of the changing therapeutic relationship of the kiai with students (santri) in learning (ngaji) from face-to-face to online models in the time of COVID-19. The research method is ethnographic-hermeneutic. In the face-to-face learning system, therapeutics occur because of the warm relationship by looking directly at the kiai's face which makes the students feel calm. In the “Ngaji Online” the therapeutic system switches to environmental settings that make students feel safe and comfortable. In the “Ngaji Online” system, the warmth of relationships begins to weaken. However, this weakness can be covered up because the spiritual relationship between the kiai and the students still feels strong. This spiritual relationship is the key to therapeutic for the Islamic boarding school.

The physical co-presence of psychotherapist or counsellor and client has been considered essential to the development of the therapeutic relationship. Psychotherapists have often, therefore, been resolutely opposed to anything other than face-to-face therapy. The richness and ubiquity of internet communication is likely to change that. I consider here the circumstances in which internet psychotherapy is indicated, some of the new problems it poses, including boundary-keeping and ethics, along with some solutions to these problems. The potential of the internet has already been exploited by many clients themselves and I conclude with a consideration of the growth of internet-based peer support groups.
