Objective: To evaluate the postoperative sensitivity in posterior restorations with different resin composites and adhesive systems as well as the influence of the depth and extent of the dental cavity. Material and Methods: A double-blind clinical trial was carried out with 80 class I restorations of 16 patients. The participants were divided into 4 groups according to the adhesive system + composite: F + P (Filtek P90™ + P90™); R + S (Rok™ + Stae™); P + A (P60™ + Adper SE PLUS™); E + X (Evolux™ + XPBond™ Adhesive). After 7, 15 and 30 days, the presence of postoperative sensitivity was evaluated and classified according to type and intensity. The data were submitted to Pearson's chi-square test, Fisher's exact teste, Student’s t-test and ANOVA. A significance level of 5% was used for all tests. Results: The presence of postoperative sensitivity was approximately 6% of the total sample. The sensitivity decreased with the evaluation time, with the smallest reduction occurring from the 7-day evaluation compared to the other evaluations. Conclusion: There was found no evidence of influence of the resin composite and adhesive type, depth and extension of the cavities for the presence of postoperative sensitivity. <p><strong>Objective</strong>: The aim of this study was to evaluate the nine-year clinical performance of Class III composite restorations using two microhybrid anterior composite resins [Magic™-Vigodent/(F) and Z100™-3M ESPE/(Z)]. <strong>Material</strong> <strong>and</strong> M<strong>ethods</strong>: The study was a randomized controlled trial, following the split mouth design. Seventy restorations were placed, thirty-five for each resin composite into 35 patients. The restorations were placed by one operator according to the manufacturers’ specifications. Two independent evaluators conducted the clinical evaluation using modified USPHS criteria. After nine-years, 56 restorations (28F-28Z) were evaluated. Data were analyzed using Chi-square, Exact Fisher and McNemar tests (p&lt;0.05). <strong>Results</strong>: No postoperative sensitivity, secondary caries and loss of anatomic form was observed after nine-years for both composites. There were no significant differences between the two composites tested at baseline and after nine-years. Significant differences for Z and F restorations between baseline and nine-year with respect to color matching and for F regarding the marginal integrity were detected. <strong>Conclusion</strong>: The clinical performance of both materials was considered acceptable after the 9-year evaluation.</p>

SUMMARY The aim of the present study was to evaluate the three-year clinical performance of a nanofilled resin composite, a packable resin composite, and silorane-based resin restorations in Class I occlusal cavities. Twenty-eight patients with at least three similar-sized occlusal lesions in molar teeth participated in the study. A total of 84 Class I occlusal restorations were placed: 28 with nanofilled resin composite (Filtek Supreme), 28 with packable resin composite (P60), and 28 with silorane-based resin (Filtek Silorane). Filtek Supreme and P60 were used with their respective etch-and-rinse adhesive system, Adper Single Bond 2, and Filtek Silorane was used with its respective self-etch adhesive, Filtek Silorane Adhesive. All restorations were placed by the same operator. The restorations were evaluated at baseline, at six months, and annually for three years according to modified US Public Health Service criteria by two calibrated examiners who did not know which restorative resin had been used. The three restorative materials for each category were compared using the χ2 test at a significance level of 0.05. Cochran's Q test was used to compare the changes across the five time points for each restorative material. McNemar's test followed by Bonferroni adjustment was used when significance differences were found. At the end of the three years, 60 restorations were evaluated in 20 patients, with a recall rate of 71.4%. The retention rate was 100% for all restorative resins. Eight restorations from the P60 group, ten from the Filtek Supreme group, and nine from the Filtek Silorane group were rated Bravo for marginal discoloration. For marginal adaptation, three P60, five Filtek Supreme, and 11 Filtek Silorane restorations were rated Bravo. No statistically significant differences in overall clinical performance were found between the restorative materials except for marginal adaptation. P60 showed the best marginal adaptation at the end of the three years. No differences were observed between the restorative resins for any of the evaluation criteria tested (p&gt;0.05). None of the restorations showed postoperative sensitivity, secondary caries, or loss of anatomic form. All restorative resins performed equally well in clinical conditions during the three-year evaluation, and no significant differences were found among them, except for marginal adaptation, in which P60 showed superior results.

Marginal microleakage is one of the major disadvantages of resin composite restorations. The purpose of this study was to evaluate in vitro the effect of reinforcement with fiberglass in Class I restorations with composite resin on the marginal microleakage.The sample was composed by 30 human third molars, which were divided into 3 groups: G1 (no fiberglass – control group), G2 (1 layer of fiberglass) and G3 (2 layers of fiberglass). The restorations were subjected to a total of 300 thermal cycles and were immersed in solution of methylene blue 2%. The analysis of microleakage was performed in a stereomicroscope and measured Data were analyzed statistically by the Chi-square and ANOVA tests at 5% significance level. G3 presented the lower infiltration index, but it was not statistically significant (p>0.05), indicating that use of fiberglass did not appears effective against microleakage. Although the use of layers of fiberglass slightly reduces polymerization shrinkage, the incremental technique seems to be more viable clinically.

The concept of preemptive analgesia, albeit long-standing, has reemerged. Consequently, recent research has focused on testing a variety of drugs preoperatively to prevent the occurrence of postoperative pain, a major factor of morbidity. Amitriptyline is a tricyclic antidepressant used to treat chronic pain. Because amitriptyline acts on pain transmission pathways, it could theoretically be used as an agent for the prevention of postoperative pain. This study evaluated the effectiveness of amitriptyline in preventing pain in patients submitted to hysterectomy, the most commonly performed gynecological surgery. A randomized, double-blind clinical trial was conducted with 145 patients, 72 of these receiving amitriptyline and 73 placebo. All patients were evaluated at 6, 12, 24 and 48 hours after surgery using a visual analog scale (VAS) for pain and algometry to determine the pressure-pain threshold. Statistical analysis was conducted using the chi-square test of association, Student's t-test, and the Mann-Whitney test, with Fisher's exact test being used whenever appropriate. No statistically signicant difference was found between the two groups with respect to pain at any of the time points evaluated, leading to the conclusion that at a dose of 25 mg, amitriptyline is ineffective in preventing postoperative pain in patients submitted to abdominal hysterectomy

Objective: Fatigue associated with malignant conditions and their treatments is a disabling condition. This trial assessed the anti-fatigue effects of melatonin coadministration during adjuvant treatment of patients with the breast cancer. Material and Methods: Patients with breast cancer were randomly assigned to receive melatonin or placebo during adjuvant chemotherapy and radiotherapy. Thirty-seven patients were randomly enrolled in each group. The mean ages of patients in the intervention and control groups were 50.47 ± 10.79 and 46.05 ± 10.55 years, respectively ( P = .223). The intervention group received oral melatonin (18 mg/day) from 1 week before until 1 month after the adjuvant radiotherapy. The level of fatigue was assessed before and after intervention using Brief Fatigue Inventory (BFI) in both groups. To analyze data, the Student’s t-test and the Chi-square test were used at a significance level of P ≤ .05. Results: The BFI score was similar before the intervention in both groups, however, after the intervention, it was significantly lower in the melatonin group ( P < .001). Moreover, the frequency of severe fatigue in the melatonin group was significantly lower than in the placebo group after intervention (42.1% vs 83.3%, P < .001). Conclusion: Coadministration of melatonin during adjuvant chemotherapy and radiotherapy of women with breast cancer decreased the levels of fatigue associated with the malignant condition and its treatments.

The objective of this study was to evaluate the alteration of the optical properties of bulk Fill resin-based composites after 2 years of simulated toothbrushing. Three high-viscosity bulk fill resin composites and one conventional sculptable resin composite (control) were analyzed. Five specimens of each resin composite were prepared, and roughness, color, translucency and gloss were evaluated initially and after one and two years of simulated toothbrushing. The specimens were submitted to electric simulated toothbrushing (14600 strokes/100g) using a commercially available toothbrush with soft fibers and toothpaste slurry in a proportion of 1:2 by weight. The brushing scheme was of 40 cycles per day (2 cycles per second / 280 cycles in a week). The optical properties and the roughness data were analyzed by a multifactor analysis of variance (MANOVA) and a Tukey post hoc test. The baseline and final values of Ra and Sa were submitted to linear regression analyses, all of which were performed at a significance level of p=0.05. Gloss and roughness presented statistical differences between the resin-time interactions in all composite resins. The gloss (72.5%) and roughness (89.4%) worsened significantly after brushing, but the authors didn’t find statistically significant differences when evaluating translucency and color. Based in the results, it could be concluded that the simulated brushing degraded the brightness and roughness of the used high-viscosity bulk fill composites.

Kepuasan pasien merupakan merupakan tingkat kepuasan dari persepsi pasien terhadap layanan kesehatan dan merupakan salah satu indikator kinerja. Apabila pasien menunjukan hal – hal yang bagus mengenai layanan kesehatan dan pasien mengindikasikan dengan perilaku yang positif maka akan dapat mengambil kesimpulan bahwa pasien merasa puas terhadap layanan tersebut. Penelitian ini bertujuan untuk mengetahui hubungan antara lima dimensi mutu pelayanan rawat jalan dengan kepuasan pasien di RSUD Arjawinangun Kabupaten Cirebon Tahun 2015. Jenis penelitian deskriptik analitik dengan desain Cross Sectional. Populasi dalam penelitian ini adalah jumlah seluruh pasien yang berobat ke rawat jalan RSUD Arjawinangun pada bulan Januari tahun 2015 sebanyak 9.707 pasien. Jumlah sampel sebanyak 385 pasien yang diambil secara Accidental Sampling. Data dianalisa secara statistic menggunakan uji Chi Square pada tingkat kemaknaan 5% (0,05). Variabel yang diteliti yaitu variabel dependen: kepuasan pasien, variabel independen: Bukti Fisik (Tangibles), Kehandalan (Reliability), Ketanggapan (Responsiveness), Jaminan (Assurance), Dan Empati (Empathy). Hasil penelitian ini menunjukan ada hubungan yang bermakna antara Bukti Fisik (Tangibles) dengan kepuasan pasein (p = 0,034), ada hubungan yang bermakna antara Kehandalan (Reliability) dengan kepuasan pasein (p= 0,000), ada hubungan yang bermakna antara Ketanggapan (Responsiveness) dengan kepuasan pasein ( p= 0,000), ada hubungan yang bermakna antara Jaminan (Assurance) dengan kepuasan pasein (p= 0,000), ada hubungan yang bermakna antara Empati (Empathy) dengan kepuasan pasein (p= 0,000).Kata Kunci: Kepuasan Pasien, Lima dimensi mutu ABSTRACTPatient’s satisfaction is the satisfaction level of the patient’s perception of the health services and is one of the performance indicators. If the patients shows the good things about the health care service and the patients indicated by the positive behavior will be able to infer that patients are satisfied with the health care service. This research aimed to determine the relationship between the five dimensions of quality of outpatient care with patient’s satisfaction in RSUD Arjawinangun Kabupaten Cirebon 2015. The type of this research is descriptive analytic with Cross Sectional Design. Population in this research is the total number of outpatients treated at RSUD Arjawinangun Kabupaten Cirebon through Januari 2015 as many as 9707 patients. The total sample of 385 patients were taken by accidental sampling. Data were analyzed statiscally using Chi-squared test at 5% (0.05) significance level. The dependent variable analyzed in this research is patient’s satisfaction and the Independent variable in this research are Tangibles, Realiability, Responsiveness, Assurance, and Empathy. Judge from the analysis above, we conclude that the result of this research showed the relationship between Tangibles with patient’s satisfaction (p = 0.034). There is relationship between Reliability with patient’s satisfaction (p = 0.00). There is relationship between Responsiveness with patient’s satisfaction (p = 0.00). The relationship between Assurance with patient’s satisfaction is also influence with p = 0.00, and the last of variable, there is relationship between empathy with patient’s satisfaction (p=0.00). The result of this research can be followed up by efforts to improve the Tangibles, Reliability, Responsiveness, Assurance, and Empathy by providing technical training to every officer to increase the ability to carry out the services.Key Word : Patient’s Satisfaction, Five dimensions of  quality

Objectives: identify the incidence and characterize pressure injuries in an adult intensive care unit regarding the occurrence, locations and risk factors, and verify whether there is an association between these and the appearance of the injuries. Method: observational, cohort, prospective study, developed in an intensive care unit of a tertiary hospital, from October to December 2019. The population consists of adults on the first day of admission to the unit, without pressure injury at admission. Participants were monitored during hospitalization, sociodemographic and clinical variables, and risk assessment of developing a pressure injury, skin assessment and Braden scale were collected daily. Pearson’s chi-square tests and student’s t-test were used to assessing the relationship between variables and injuries. For the analyzes, a significance level (α) of 5% was considered. Results: 40 participants were included, 20% had pressure injuries with a predominance of stages 1 and 2; the main affected sites were the sacral region followed by the calcaneus. The average hospital stay was 23.38 days for the injured group and 5.77 days for the non-injured group; time showed a significant relationship with the appearance of lesions (p = 0.002). Conclusion: it was concluded that the most affected site was the sacral region and grade 1 was the most frequent staging; length of stay was the variable that influenced the appearance of injuries.

SUMMARY This clinical study assessed the performance of posterior composite resins applied with the Adper™ Single Bond Plus (SB) and Adper™ Scotchbond SE (SE) adhesive systems and Filtek™ Supreme Plus composite resin, using modified US Public Health Service criteria. A total of 97 restorations were placed in posterior teeth by two calibrated operators. Application of the materials followed manufacturers' instructions. The restorations were evaluated by two examiners at baseline and after one year. Statistical analyses were conducted using the proportion test at a significance level of 5% (p&lt;0.05). All the restorations evaluated (ie, 100%) received an alpha rating for the criteria of marginal discoloration and marginal integrity at baseline. At one year, for marginal discoloration, 64.6% of SB and 61.2% of SE received an alpha rating. For marginal integrity, 72.9% of SB and 77.6% of SE received an alpha rating. The other restorations received bravo ratings for both criteria. None of the teeth that received the restorative systems presented caries lesions around the restorations. A total of eight teeth presented postoperative sensitivity one week after baseline, five with SB and three with SE; the symptom had disappeared one year later. One year later, composite resin restorations using either adhesive system showed satisfactory clinical performance.

The aim of this study was to evaluate the bond strengths of composite restorations made with different filler amounts and resin composites that were photoactivated using a light-emitting diode (LED). Thirty bovine incisors were selected, and a conical cavity was prepared in the facial surface of each tooth. All preparations were etched with Scotchbond Etching Gel, the Adper Scotchbond Multipurpose Plus adhesive system was applied followed by photoactivation, and the cavities were filled with a single increment of Filtek Z350 XT, Filtek Z350 XT Flow, or bulk-fill X-tra fil resin composite (n= 10) followed by photoactivation. A push-out test to determine bond strength was conducted using a universal testing machine. Data (MPa) were submitted to Student’st-test at a 5% significance level. After the test, the fractured specimens were examined using an optical microscope under magnification (10x). Although all three composites demonstrated a high prevalence of adhesive failures, the bond strength values of the different resin composites photoactivated by LED showed that the X-tra fil resin composite had a lower bond strength than the Filtek Z350 XT and Filtek Z350 XT Flow resin composites.
