Objective: to evaluate the efficiency of combination therapy with methotrexate (MTX) and hydroxychloroquine (HC) in rheumatoid arthritis (RA) in real clinical practice.Patients and methods. The investigation enrolled 430 patients with documented RA who had been followed up for 16 weeks by rheumatologists in different regions of the country. Individual schedules were filled out for the patients, by adding demographic, clinical, and laboratory parameters that made it possible to assess the activity of the disease and to estimate the level of glucose, cholesterol, and low-density lipoproteins (LDL). The results of the investigation were evaluated at the inclusion of patients in it and then at 8 and 16 weeks. Taking into account the insufficient previous effect of MTX, the treatment was enhanced by HC.Results and discussion. By the end of the investigation, the combination therapy with MTX + HC led to significant decreases in the number of swollen and tender joints, the duration of morning stiffness, the severity of pain, and the integrated DAS28 index. The therapy showed a positive effect on the levels of fasting glucose, cholesterol, and LDL. The quality of life improved in patients by Visit 3. The results obtained are in good agreement with the data by foreign and Russian investigators on the greater efficiency of combination therapy with MTX + HC than that of MTX monotherapy and on the pleiotropic (hypolipidemic and hypolipidemic) effect of HC.Conclusion. It is concluded that in Russian practice when monotherapy with MTX shows an insufficient effect or its high doses cannot be administered to enhance the efficacy of RA therapy, it is possible to use a combination of MTX + HC, especially in older patients with comorbidity (hyperglycemia and hypercholesterolemia).

Abstract Objectives To estimate the prevalence of impaired morning function (IMF) in rheumatoid arthritis (RA), and to understand physicians’ and patients’ perceptions on the impact of IMF and treatment in selected Asian countries. Methods A survey on the impact of IMF was conducted in Asia on rheumatologists and patients with moderate-to-severe RA who experienced IMF for ≥3 mornings a week. Participants underwent comprehensive face-to-face interviews using structured questionnaires. Results Sixty physicians and 300 patients from Hong Kong, Philippines, Singapore, South Korea, and Taiwan were surveyed. Rheumatologists estimated that two-thirds of patients with RA experienced IMF and believed that the prevalence of IMF increased with RA severity (present in 42%, 73%, and 87% of patients with mild, moderate, and severe disease, respectively). Patients’ survey revealed that, on an average, patients with RA experienced IMF 5 days a week for 2 h each day. Thirty-eight percent of patients with RA considered a reduction in morning stiffness as an important treatment goal, but this was agreed by only 3% of rheumatologists. Only 22% of rheumatologists modified the treatment regimen specifically for IMF. Physicians considered prednisolone and other glucocorticoids (GCs) to be the most effective medication for the treatment of IMF. Fifty-one percent of patients with RA did not find their current medication effective in relieving IMF, and as a result, they reported negative emotions such as frustration, defeat, and anger, and 56% had missed work because of this symptom. Conclusions IMF is prevalent in RA and significantly affects patients’ quality of life (QoL). A higher proportion of patients compared to physicians view the reduction of morning stiffness as an important goal. More should be done to address the dissonance between physicians’ and patients’ views on IMF.

AbstractIt is shown for the first time that the structural and optical functional characteristics of integrated GaAs/Si(100) heterostructures can be controlled by using misoriented Si(100) substrates and their preliminary etching. The growth of an epitaxial GaAs layer on a Si substrate without the formation of antiphase domains can be carried out on a substrate deviated from the (100) singular plane by an angle smaller than 4°–6° or without a transition layer of GaAs nanocolumns. Preliminary treatment of the silicon substrate by etching makes it possible to use it for the vapor-phase epitaxial growth of a single-crystal GaAs film with a considerably smaller relaxation coefficient, which has a positive effect on the structural quality of the film. These data are in good agreement with the results of IR reflectance spectroscopy and photoluminescence and ultraviolet spectroscopy. The features of the optical properties of integrated GaAs/Si(100) heterostructures in the infrared and ultraviolet spectral regions are also defined by the relaxation coefficient.

Background:Upper extremity functions affect the quality of life at different levels in patients with rheumatoid arthritis (RA). In the current literature; it has been shown that grip endurance is associated with upper limb functions (1). However, there is no study investigating the relationship between grip endurance and quality of life in patients with RA.Objectives:To investigate relationship between grip endurance, disability of upper extremity and quality of life in patients with RA.Methods:In our cross sectional study, 23 RA patients [Mean age; 52.7±12.6, BMI;26.9±5.7 kg/m2, Women;20(87.0%)] who were classified according to the ACR 2010 criteria. Demographics and clinical characteristics of patients were recorded (Table 1). Das28 for disease activity score, Static and dynamic grip endurance measurements using Hand Dynamometer (Lafayette Proffessional Hand Dynamometer, USA) for grip endurance, Disability of Arm, Shoulder and Hand Survey (DASH) for disabilities and symptoms of upper extremity and Short Form-36 Health Survey for quality of life were performed. DASH was correlated negatively with SF-36 physical function, rol limitataion due to physical health, pain subparameters and positively correlated with Das28 score (p<0.05).Conclusion:In our study, it was concluded that grip endurance was related to upper extremity functions and quality of life in patients with RA. The article presents the studies carried out in 2017 and 2018 in the central zone of the Krasnodar Territory on the basis of the Agrotechnological Department of the FSBSI «National Center for Grain named after P.P. Lukyanenko». The purpose of the research was to study the dose and frequency of application of mineral fertilizers with a complex of NanoSilicon microelements, stimulating growth and development of corn and, ultimately, its yield and quality of the obtained products. Two year studies revealed the effectiveness of the positive effect of the studied drug on grain corn in all studied options. It was noted that at low or high doses of its application, a number of indicators that determine the structure of the crop and, ultimately, yield, decreased. It was found that foliar treatments increased the growing season of corn for a day. The introduction of the preparation promoted the formation of taller plants, the height of which varied from 201,3 to 212,4 cm, while in the control it was 184 cm (milk ripeness phase). In the same phase the plants had a large biological weight, which in the control was 1010,1 g/plant, and when the preparation was applied, it was 90,2–275,5 g/plant higher. Ultimately, mineral fertilization with the NanoSilicon microelement in all studied dosages had a positive effect on the corn yield, the maximum value, which was obtained when it was applied at doses of 40/75/75 (seed treatment with NanoSilicon with a rate of 40 g/ha + application by seedlings at a dose of 75 g/ha + in the phase of 7–8 leaves 75 g/ha) and 40/100/100 (seed treatment – 40 g/ha + seedlings – 100 g/ha + in the phase of 7–8 leaves – 100 g/ha), which amounted to 59,9 and 59,8 c/ha, respectively.

Sudden cardiac death and the heaviest arrhythmic events are connected to ventricular arrhythmias (VA). The issue of optimization of drug therapy for VA remains relevant. Given the possible multidirectional effects of antiarrhythmic drugs on the functional state, in order to determine the effectiveness of pharmacotherapy in patients with VA it is advisable to use a stepwise integrated approach. Aim. Compare the effects of bisoprolol, nebivolol and sotalol on the functional state of patients with VA. Materials and methods. 120 patients with VA of grade IIV based on the В. Lown grading system, III groups based on J. Bigger grading system against the background of essential hypertension or its combination with coronary heart disease took part in the research, they were randomized into three groups for treatment with bisoprolol, nebivolol and sotalol. As part of combination therapy, patients were administered lisinopril, and if indicated, acetylsalicylic acid, atorvastatin. Initially and after 24 weeks of therapy the following was done: quantitative assessment of regulatory-adaptive status by cardio-respiratory synchronism test, treadmill test, six-minute walk test, subjective assessment of quality of life, all-day monitoring of blood pressure and electrocardiogram. Results. With comparable hypotensive and antiarrhythmic effects, therapy with nebivolol had a positive effect on the regulatory-adaptive status, had a better effect on exercise tolerance in comparison with bisoprolol and sotalol, more improved the quality of life, in comparison with bisoprolol and sotalol. Conclusion. In patients with VA against the background of essential hypertension or its combination with coronary heart disease as part of combination therapy the use of nebivolol may be preferable to bisoprolol or sotalol due to a more pronounced positive effect on the functional state.

Recently, the preparation of the endometrium for embryo transfer, namely the achievement of optimal thickness and receptivity of the endometrium, the prevention of unsuccessful attempts to implant embryos, is gaining more and more attention among doctors in the field of reproductive medicine. Since cryptocycles of embryo transfer preparation are often discontinued in the practice of a reproductive physician due to insufficient endometrial thickness, high doses of estrogen and negative IVF results provided the satisfactory condition of the endometrium on the day of transfer and quality of the embryos. The article briefly presents the views on the effectiveness of PRP therapy of the endometrium in the protocols of preparation for embryo transfer. The main mechanisms of the effect of platelet-rich blood plasma on the endometrium are elucidated. The positive effect of this technique on regeneration, increase in thickness and improvement of endometrial receptivity is proved. Conclusion. PRP-therapy of the endometrium is a modern, affordable, effective and safe method of treatment of thin and damaged endometrium in reproductive medicine.

Material and methods.120 patients with VA against the background of essential hypertension or its combination with coronary heart disease took part in the research, they were randomized into three groups for treatment with bisoprolol, nebivolol and sotalol. As part of combination therapy, patients were administered lisinopril, and if indicated, acetylsalicylic acid, atorvastatin. Initially and after 24 weeks of therapy the following was done: echocardiography, treadmill test, six-minute walk test, subjective assessment of quality of life, all-day monitoring of blood pressure and electrocardiogram. Results.With comparable hypotensive and antiarrhythmic effects, therapy with nebivolol increased exercise tolerance to a greater extent, in comparison with bisoprolol and sotalol. Nebivolol and sotalol improved the quality of life, in comparison with bisoprolol. Conclusion.In patients with VA against the background of essential hypertension or its combination with coronary heart disease as part of combination therapy the use of nebivolol may be preferable to bisoprolol or sotalol due to a more pronounced positive effect on the functional state.

Background:The level of quality of life in patients with rheumatoid arthritis (RA) is often overlooked. These patients suffer from an often-precarious quality of life resulting in pain, joint destruction and fatigue.Objectives:The main objective of this study was to compare the level of quality of life in patients with RA receiving treatment with disease-modifying anti-rheumatic drugs (DMARDs): monotherapy (biological only) versus combination therapy with methotrexate.Methods:We performed a descriptive study of 56 patients with RA meeting the criteria of the ACR 2010. The SPSS.20 software was used for statistical analyses.Results:These were 56 patients: 93% women and 7% men, mean age 46.74 years with an average duration of the disease of 14.74 years. 28 patients were on rituximab, 19 patients on tocilizumab and 9 patients on TNFi. The mean HAQ was 1.11. 52.38% of patients were on biological alone as monotherapy. The Compliance Questionnaire Rheumatology score (CQR19) was 55.15 / 100, the RAID (Rheumatoid Arthritis Impact of Disease) score was 3.08 / 10, the SF36 of 56.01 / 100, the AIMS score of 2.10 / 10 for social activity, 2.44 / 10 for pain, 3.32 / 10 for depression and 4.06 for physical activity.The comparison between the 2 groups (Combination therapy vs monotherapy) did not find any significant difference in terms of quality of life parameters: An SF-36 score> 55 was found in 56% in patients on combination therapy vs. 44% on monotherapy, the RAID score was 3.02 vs. 3.12, AIMS social activity 2.08 vs 2.13, AIMS pain 2.33 vs 2.59, AIMS activity physics of 4 vs 4.11.Conclusion:Our study did not demonstrate any superiority of the combination with methotrexate in improving quality of life. The use of biotherapy in patients with RA has been shown to be an important pharmacological strategy for the overall management of the disease.References:[1]Lavielle.M and Dougados.M.
