Abstract Introduction. Atopic eczema (or atopic dermatitis, AD) has a great impact on the quality of life of the affected person as well as the whole family. Hospitalization may be required during exacerbations of the disease.Aim. The authors of this paper have gathered the essential information about this disease and would like to comment on standards of care and management of atopic dermatitis which are implemented according to the current medical knowledge.Summary. Applying emollients everyday is a recommended practice for avoiding a recurrence of the disease. Basic skin care is just as important as pharmacological treatment during both the acute and chronic stage. In certain cases, dietary interventions such as food elimination may be beneficial. We present some useful ways for the management of pruritus, which is a disabling feature of AD. In the present paper, the authors discuss clinical presentation and treatment of atopic dermatitis.

Chronic obstructive pulmonary disease (COPD) is no longer a respiratory disease that predominantly affects men, to the point where the prevalence among women has equaled that of men since 2008, partly due to their increasing exposure to tobacco and to biomass fuels. Indeed, COPD has become the leading cause of death in women in the USA. A higher susceptibility of female to smoking and pollutants could explain this phenomenon. Besides, the clinical presentation appears different among women with more frequent breathlessness, anxiety or depression, lung cancer (especially adenocarcinoma), undernutrition and osteoporosis. Quality of life is also more significantly impaired in women. The theories advanced to explain these differences involve the role of estrogens, smaller bronchi, impaired gas exchange in the lungs and smoking habits. Usual medications (bronchodilators, ICS) demonstrated similar trends for exacerbation prevention and lung function improvement in men and women. There is an urgent need to recognize the increasing burden of COPD in women and therefore to facilitate global improvements in disease management (smoking cessation, pulmonary rehabilitation…) in half of the population. Nevertheless, important limitations to the treatment of women with COPD include greater under-diagnosis than in men, fewer spirometry tests and medical consultations. In conclusion there is an urgent need to recognize the increasing burden of COPD in women and therefore to facilitate globally improvements in disease management in this specific population.

Informasi dalam keperawatan semakin dirasakan pentingnya, baik dalam penyusunan rencana strategis, pengambilan keputusan maupun dalam menilai efektifitas dan efesiensi dari pelayanan keperawatan. Salah satu contoh manajemen informasi yang akan dikemukan adalah tentang kebutuhan anak yang dirawat dengan Bronchopneumonia pada setiap tahapan discharge planning (rencana pemulangan pasien). Tahapan yang satu meyediakan informasi penting, dan sangat menetukkan pada tahapan selanjutnya. Semua data yang didapatkan pada fase akut yang dilaksanakan di klinik anak dan atau di ruang gawat darurat, diproses oleh tim kesehatan dengan menggunakan pengetahuan dasar dan membandingkannya (comparing system) dengan data dasar keperawatan dan pengetahauan kedokteran, fisiologi, patofisiolohi, sosiologi, psikologi, dan farmakolohi. Dengan demikian akan didapat informasi tentang masalah anak tersebut dan kebutuhan pada fase berikutnya yaitu fase transisi yang dijalankan di ruang rawat. Demikian pula proses manajemen informasi dari fase transisi yang dilakukan di ruang rawat akan sangat menentukan langka selanjutnya yaitu perawatan lanjutan di rumah/di masyarakat. Mengingat kegiatan penyusunan rencana pemulangan pasien pada anak yang dirawat dengan BP melibatkan tim multidisiplin, maka penting adanya seorang coordinator yang dapat mengatur kegiatan dari setiap anggota tim, sehingga tujuan dari rencana pemulangan pasien dapat dicapai dengan optimal. Perawat adalah orang yang paling tepat sebagai coordinator penyusunan rencana pemulangan pasien, karena berada di dekta pasien selama 24 jam, memahami segala kebutuhan pasien dengan baik, dan memungkinkan untuk dapat memfasilitasi peran dari setiap anggota tim untuk mengelola informasi rencana pemulangan pasien bagian anak yang dirawat dengan BP. It has been discussed that information in nursing becomes important in making a strategic planning. Decision making or evaluating the affectivity an the efficiency of the nursing service. One of the example of Information Management that would be describe is the children’s need who are hospitalized with Bronchopneumonia in every step of Discharge Planning. Each step provides the essential information, which could determine the next step. All the data which have been gained in the acute phase at the out patient department and/ or at the emergency department are processed by using the knowledge base, compare it with the data base of nursing, medical knowledge, physiology, patophysiology, sociology, psychology and pharmacology. As the outcome from this process, the information about the child’s problems and their needs in the transitional step will be gained. It is also occur on the management information at the transitional step in the ward, it will determine the following step as the preparation for discharge planning. Discharge Planning for the child to have a coordinator, because he/she is in the ward 24 hours and understand the patients’ condition and their needs well. He/she also could facilitate the role of each member of Discharge Planning team, to manage the information about Discharge Planning for a child with Broncho Pneumonia.

Atopic dermatitis (AD) is one of the common multifactorial inflammatory diseases manifesting predominantly in childhood. There is significant number of cases of self-regression of the disease with aging. On the other hand, there is also another scenario ending with AD persistent course and/or development of comorbid allergic pathologies that can significantly worsen patient’s quality of life and finally lead to social maladjustment. The pathogenesis of such way includes epidermal barrier disturbance, transcutaneous sensibilisation and aberrant allergic (Th2) immune systemic response development. Main role in preventing of this pathological pathway is lying on the new class of moisturizers containing active components "emollients plus". They are considered as foundation for the therapy and prevention of the development of AD and other allergic diseases. This literature review provides relevant data on AD pathogenesis and development of comorbid allergic pathologies. This paper also covers data on the effect of emollients in restoration of the epidermal barrier and their use as preventive measures.

The prevalence of allergic disorders has been increasing worldwide and significantly impacts the quality of life of the atopic individual. There has been an increased interest in the role of probiotics for the prevention and treatment of allergic disorders, given the recent evidence that atopy risk may be associated with a dysbiosis of the gut microbiome. Research in this area is ongoing with some studies showing possible benefits of probiotics, with seemingly little to no risk. While these studies suggest that there may be a promise in probiotic use for the prevention or treatment of allergy, further evidence is needed to determine its efficacy, optimal dosing, and strains needed for treatment. In this review, we discuss recently published studies examining the benefits, risks, and role of probiotics in preventing atopic dermatitis, asthma, allergic rhinitis, and food allergy.

Atopic dermatitis is a chronic hereditary recurrent skin disease. One of the most pronounced symptoms of this dermatosis is itchy skin. Pruritus accompanies atopic dermatitis in more than 80% of cases. This review presents modern data on the mechanisms of pruritus formation in atopic dermatitis. The issues of etiological factors, neuroimmune interactions, peculiarities of skin dysfunction, as well as the role of stress are considered. The relevance of studying the topic is due to the high prevalence of atopic dermatitis among the population, a decrease in the quality of life and the lack of effective therapy. Analysis of the literature indicates the need for a comprehensive assessment of the pathogenetic mechanisms of the development of pruritus in atopic dermatitis. A more in-depth study of the mechanisms of neurogenic inflammation in atopic dermatitis will contribute to the development of new methods of diagnosis and treatment.

: Rheumatoid arthritis (RA) is a chronic inflammatory autoimmune disease, which involves a pathological inflammatory response against articular cartilage in multiple joints throughout the body. It is a complex disorder associated with comorbidities such as depression, lymphoma, osteoporosis and cardiovascular disease (CVD), which significantly deteriorate patients’ quality of life and prognosis. This has ignited a large initiative to elucidate the physiopathology of RA, aiming to identify new therapeutic targets and approaches in its multidisciplinary management. Recently, various lipid bioactive products have been proposed to have an essential role in this process; including eicosanoids, specialized pro-resolving mediators, phospholipids/sphingolipids, and endocannabinoids. Dietary interventions using omega-3 polyunsaturated fatty acids or treatment with synthetic endocannabinoids agonists have been shown to significantly ameliorate RA symptoms. Indeed, the modulation of lipid metabolism may be crucial in the pathophysiology and treatment of autoimmune diseases.

Fibromyalgia (FM), chronic fatigue syndrome (CFS) and multiple chemical sensitivity (MCS) are some of the central sensitization syndromes (CSSs). The complexity of their diagnosis, the high interindividual heterogeneity and the existence of multi-syndromic patients requires a multifaceted treatment. The scientific literature is contradictory regarding the role of food in CSS, and evidence on the role of nutrition in MCS is particularly scarce. This review consists in gathering information about the current status of dietary recommendations (i.e., special dietary interventions, the role of additives, presence of micronutrient deficiencies, nutritional supplements and elimination of other nutrients and substances) and discussing the scientific evidence in depth to shed light on appropriate nutritional treatment managements for CSS patients. Current indications show that dietary modifications may vastly improve the patients’ quality of life at a low cost. We suggest personalized treatment, taking into consideration the severity of the disease symptoms, quality of life, coexistence with other diseases, pharmacological treatment, changing clinical characteristics, nutritional status, energy requirements and food tolerances, among others, as the best ways to tailor specific dietary interventions. These approaches will partially overcome the lack of scientific and clinical research on MSC. Patients should also be advised on the serious consequences of following dietary guidelines without a dietitian’s and clinician’s supervision.

Abstract Background The purpose of this paper is to provide an overview of key aspects of specialised dermatology nursing practice in the management of patients with moderate to severe atopic dermatitis. The role of dermatology nurse specialists in supporting patients and promoting disease understanding, education and treatment adherence continues to evolve. As features of specialised nursing care can also inform other nursing staff in a wide range of care settings, an overview of key components is examined. Observations presented are from a pan-European perspective and represent the collected view of a group of dermatology nurse specialists, dermatologists and patient advocates following two round-table discussions. Main body Atopic dermatitis is a common, chronic, inflammatory disease characterised by erythematous/scaling skin lesions, with often intense pruritus. Disease course is cyclic with periodic disease flares of varying intensity, presenting management challenges to patients and families. Dermatology nurse specialists play a key role in providing education and substantial patient support to improve treatment outcomes and quality of life to patients and their family, delivered within a multidisciplinary team framework. Nurse-led education and 'eczema schools’ are of benefit in reducing disease severity and improving quality of life by enhancing self-management, adherence and patient engagement. eHealth tools, such as patient portals or online training platforms, can provide online learning, individualised education, and help to improve engagement. These and other initiatives, such as written action plans, are all essential to improve or maintain treatment adherence, self-management and quality of life. Conclusions Dermatology nurse specialists play a central role in the assessment and management of moderate to severe atopic dermatitis patients and families. This places them in an ideal position to build strong and often long-term relationships with patients and parents. Such engagement promotes trust, assists in setting realistic expectations of treatment and outcomes, and enhances self-management and engagement in their own care. Providing emotional support, as well as formal and systematic education (including individualised practical advice) all contribute to improved treatment adherence and can enhance the quality of life of patients and their families throughout the course of this long-term condition.
