The reversal of epidural xylazine, induced with IV atipamezole was evaluated in 15 buffaloes. Atipamezole at different dose rates (5, 10 and 15 µg/kg) injected 30 minutes after epidural xylazine provoked visible signs of arousal shortly after inection in all buffaloes without any side effects. The mean arousal times were 4.8±0.84 min, 2.4±0.24 min and 2.0±0.00 min while the mean total recovery times were 61.0±2.6 min, 33.0±3.7 min and 32.0±4.3 min following doses of 5, 10 and 15 µg/kg atipamezole respectively. The heart, respiratory and ruminal rates were mildly influenced. Overall, the IV atipamezole at a dose rate of 10 µg/kg was found effective and satisfactory for reversing the analgesic, sedative and cardiopulmonary depression effects induced by epidural xylazine administration in buffaloes.

The gross morphology of the heart of mature Philippine water buffalo was described by determining the weight, length, width and circumference; measured the average external circumference of the pulmonary veins and artery, cranial and caudal vena cava and the aorta; determined the presence and location of ligamentum arteriosum; measured the thickness of the atrial and ventricular wall; described the components of the left and right atrioventricular valves, pulmonary valves and the aortic valves; determined the location and measured the length of trabeculae septomarginalis and determined the location of the os cordis. Hearts of ten mature Philippine water buffalo of both sexes were collected from abattoirs of Cabanatuan City. The absolute and relative weights of the heart were measured using digital weighing scale. Different dimensions like circumferences, length, and width of the heart; the external circumferences of major blood vessels; and the thickness of the wall of heart were determined using measuring tape and Vernier caliper. The number of cusps present in the left and right atrioventricular, pulmonary, and aortic valves was counted and the presence of ligamentum arteriosum was documented. Radiograph was used to determine the presence and location of os cordis. All of the hearts studied were pointed and bilaterally flattened. The base of the heart is markedly surrounded with fatty tissues. The mean absolute weight is 2.42 kg. The heart has a mean dimension of 21.71 cm x 17.49 cm. The mean circumference at the level of the coronary groove, middle and apex were 48.41 cm, 44.10 cm and 20.57 cm, respectively. An average of four pulmonary veins was documented to be present. The right ventricle has a constant three papillary muscles and three cusps while the left ventricle has a constant two papillary muscles and two cusps. The number of cusps of both pulmonary valve and aortic valve was three. Ligamentum arteriosum was present in all samples and it is located between the pulmonary trunk and aorta. The os cordis which is located at the aortic fibrous ring is present in all heart of Philippine water buffalo examined.

Abstract This study presents a detailed measurement of indoor and outdoor terrestrial gamma radiation levels in different cities of Pakistan. The measurements covered dwellings in 27 cities, covering all provinces and region of Azad Kashmir. Most of the houses were of attached type, made of brick walls and concrete roofs. The measurements were made by a handheld radiation survey meter containing Geiger–Muller tube. The average absorbed dose rate in air was 100 ± 32 nGy h−1 for indoor and 74 ± 30 nGy h−1 for outdoor. The population-weighted mean terrestrial dose rates were 90 nGy h−1 for indoor and 78 nGyh−1 for outdoor. The ratio of indoor to outdoor absorbed dose rate was 1.5 as compared to 1.3 for the world average. The estimated average annual effective dose rate was 0.58 ± 0.18 mSv a−1 and the mean excess life time cancer risk was 2.0 × 10−3.

Abstract Absorbed dose rates in air were measured for the whole area of the Kanto region in 2015, 2016 and 2017 (n = 31 147). The mean absorbed dose rates in air for each prefecture measured by car-borne surveys were from 44 to 67 nGy h−1 (13–289 nGy h−1). The absorbed dose rate in air from artificial radionuclides (134Cs + 137Cs) measured by fixed-point observation (n = 507) was from 1 to 14 nGy h−1 (0–105 nGy h−1), and meaning that the contribution ratios of 134Cs and 137Cs were 3–22%. The deposited location of artificial radionuclides was less than 1000 m from ground level and depended on the topography, wind direction and precipitation field.

AbstractThe concentrations of 226Ra, 232Th, 40K and 137Cs radionuclides in soil samples collected from 323 different regions of Nevşehir province were determined by using a gamma spectrometer with an HPGe detector. The mean gamma activity concentrations (ranges) of 226Ra, 232Th, 40K and 137Cs for districts were determined as 49.45 (7.40–193.90), 54.08 (<2.8–122.50), 698.43 (37.67–1370.20) and 8.26 (0.10–52.60) Bq kg−1, respectively. The mean activity concentrations of 226Ra, 232Th and 40K were higher than the world mean value. According to Kolmogorov–Smirnov test, distributions of 226Ra, 232Th and 137Cs show log-normal distributions. Whereas, 40K shows normal distribution. The mean radium equivalent activity was 181.68 Bq kg−1 which is lower than the recommended maximum value of 370 Bq kg−1. The mean external terrestrial gamma dose rate was found to be 85.12 nGy h−1. The calculated external hazard value was 0.49 and within the acceptable limit which is less than unity (Hex ≤ 1). Thermo Scientific RadEye NBR detector was used to determine environmental gamma dose rates. The gamma dose rates were measured at 445 points at a height of 1 m from land surface. The mean outdoor gamma dose rate (range) was found as 150.13 (50–480) nGy h−1. This mean value was found higher than world mean value. The fact that most of the Nevşehir province is based on volcanic rocks explains why the gamma dose rates are high.

Background:During the last decade, the treatment of chronic inflammatory rheumatism (CIR) has been greatly improved with the advent of biotherapy.However, the use of biological treatment can lead to a number of side effects including abnormalities in the blood count.Objectives:The aim of this study was to assess the different hematological side effects of biological treatment in patients with rheumatoid arthritis (RA) and spondyloarthitis (SA).Methods:This study included patients with RA (ACR/EULAR 2010) and SA (ASAS 2009) registred with the Tunisian Biologic National Registry (BINAR).Patients were followed and treated with biologics for 2 years of less. Clinical data relative to biological treatment, including haematological side effects, have been collected.Results:Two hundred and ninety-eight patients (178 women and 111 men) were included in the study.The mean age was 49.2 ± 14.1 years. The male/female ratio was 0.6. The mean diseases durations for RA and SA were respectively 6.7 ± 3.5 years and 6.5 ±3.6 years.Anti-TNFα agents were prescribed in 87.9% of patients (n = 263) with respectively: Infliximab (20.4%) Etanercept (23.1%), Adalimumab (24.6%) and Certolizumab (26.5%).Tocilizumab and Rituximab were prescribed in 10.4% and 5% of the patients, respectively.Blood count abnormalities were noted in 15.4 % of patients (n=46).Neutropenia was the most frequently anomaly met on the hemogram (9.1%) followed by anemia (3.4%) and thrombocytopenia (3%). Pancytopenia was found in 11.4% of patients (n=34).The median time between biological therapy initiation and the onset of hematologic manifestations was 4.8 months [1-12]. Biological treatment was interrupted in two patients.In the other cases, the biological treatment was maintained with close monitoring of blood cell count. No case of death related to these hematological disturbances has been reported.Conclusion:In our registry, hematological side effects of biological treatment were found in 15.4% of cases and were noted with a median delay of 4.8 [1-12] months after the treatment initiation. Introduction: While the operator radiation dose rates are correlated to patient radiation dose rates, discrepancies may exist in the effect size of each individual radiation dose predictors. An operator dose rate prediction model was developed, compared with the patient dose rate prediction model, and converted to an instant operator risk chart. Materials and Methods: The radiation dose rates (DRoperator for the operator and DRpatient for the patient) from 12,865 abdomen X-ray acquisitions were selected from 50 unique patients undergoing standard or complex endovascular aortic repair (EVAR) in the hybrid operating room with a fixed C-arm. The radiation dose rates were analyzed using a log-linear multivariable mixed model (with the patient as the random effect) and incorporated varying (patient and C-arm) radiation dose predictors combined with the vascular access site. The operator dose rate models were used to predict the expected radiation exposure duration until an operator may be at risk to reach the 20 mSv year dose limit. The dose rate prediction models were translated into an instant operator radiation risk chart. Results: In the multivariate patient and operator fluoroscopy dose rate models, lower DRoperator than DRpatient effect size was found for radiation protocol (2.06 for patient vs 1.4 for operator changing from low to medium protocol) and C-arm angulation. Comparable effect sizes for both DRoperator and DRpatient were found for body mass index (1.25 for patient and 1.27 for the operator) and irradiated field. A higher effect size for the DRoperator than DRpatient was found for C-arm rotation (1.24 for the patient vs 1.69 for the operator) and exchanging from femoral access site to brachial access (1.05 for patient vs 2.5 for the operator). Operators may reach their yearly 20 mSv year dose limit after 941 minutes from the femoral access vs 358 minutes of digital subtraction angiography radiation from the brachial access. Conclusion: The operator dose rates were correlated to patient dose rate; however, C-arm angulation and changing from femoral to brachial vascular access site may disproportionally increase the operator radiation risk compared with the patient radiation risk. An instant risk chart may improve operator dose awareness during EVAR.

(1) Background: The epidermal growth factor inhibitors (EGFRIs)/tyrosine kinase inhibitors (TKIs) are effective for cancer target therapy, but acneiform rashes or so-called inflammatory papulopustular exanthemas are common (50% to 90%). The conventional therapy for EGFRIs/TKIs-induced skin toxicity is steroids and antibacterial drugs, but it is still ineffective for some patients, and EGFRIs/TKIs dose reduction/interruption may be needed. In this study, a modified Chinese herbal medicine, Huang-Lian-Jie-Du decoction cream with Yin-Cold (YC) medicine characteristic, was investigated for the effect on patients suffering EGFRIs/TKIs-induced skin toxicity. (2) Methods: The modified Huang-Lian-Jie-Du (mHLJD) decoction cream was made from 10 herbal medicines, including 4 major medicines (Huanglian, Huangqin, Huangbo, and Zhizi) in traditional HLJD decoction. Patients with EGFRIs/TKIs-induced skin toxicity were enrolled. Patients were excluded if they also used other cream for skin toxicity. Skin conditions were monitored by follow up every 2 weeks. The patients’ characteristics, the skin toxicities, treatment response, and adverse events were recorded and analyzed until skin problems resolved or the study ended. (3) Results: The mHLJD decoction cream and its sub-packages were stored at 4 °C before use. Thirty-four patients who had grade 1–3 skin toxicities after receiving EGFRIs/TKIs were enrolled. Seven patients withdrew or were excluded. Finally, data from 27 patients were analyzed. The mean grade of rash acneiform was significantly decreased from 2.19 (ranged 1 to 3) to 0.88 (ranged 0 to 2) after mHLJD decoction cream treatment for 4 weeks and to 0.55 (ranged 0 to 2) after mHLJD decoction cream treatment for 8 weeks. Additionally, the mean grade of dry skin was also significantly decreased from 1.57 (ranged 1 to 2) to 0.71 (ranged 0 to 1) after mHLJD decoction cream treatment for 4 weeks. The changes of skin toxicity were significant, with no obvious adverse events. (4) Conclusions: In summary, the mHLJD decoction cream provides benefits for alleviation of EGFRIs/TKIs-induced skin rash acneiform and dry skin. Additionally, no obvious side effects were found in patients using mHLJD decoction cream.

Abstract. Domestic buffaloes were used to characterize the pattern of progesterone, oestradiol-17β, LH and androgen in the systemic circulation following infertile insemination. Concentrations of hormones were measured by RIA in blood samples collected daily or at alternate days following insemination. The concentration of progesterone was lowest on the day of insemination, and increased significantly to a peak level of 4.00 ± 0.60 ng/ml by day 13 post insemination. After day 17, it declined significantly (P < 0.01) to reach low levels by day 21. The concentration of oestradiol-17β was high at the time of insemination and declined significantly (P < 0.01) by day 2 after insemination. It was maintained around the basal level till day 18 with minor peaks in between this period. It again rose significantly (P < 0.01) at subsequent oestrus. The mean level of LH was highest at the time of insemination, and declined significantly (P < 0.01) by day 1 post insemination. It did not vary appreciably till the animal returned to oestrus. The oestrous value of LH and progesterone were negatively correlated (r = −0.77). The androgen level was observed to be high at insemination in 3 out of 5 animals, but the overall pattern of this steroid was inconsistent during the period studied. A high concentration of androgen was recorded in all the animals from day 2–5 before the onset of oestrus.
