PURPOSE: The purpose of this study was to evaluate the effects of water-loading weight loss on physiologic response.METHODS: Thirteen consenting university wrestlers (weight 71.5±8.0 kg, BMI 25.0±2.0 kg/m²) randomly divided into the weight loss (WL) group (n=6) and water-loading weight loss (WWL) group (n=7). Wrestlers performed 2-week weight loss program targeting average of 5-10% of body weight reduction, under the supervision of a coach. Participants were instructed to drink 1.5-2 L and 6-7 L daily during the first week in the WL and WWL groups, respectively. Both groups were gradually reduced water intake and didn’t drink water the day before weighing. Anthropometric characteristics, hematocrit (HCT), serum electrolytes, aldosterone, and cortisol were measured before and after weight loss. Data were analyzed with two-way ANOVA using SPSS. The significance level was set at p<.05.RESULTS: After 2 weeks, the weight (p=.004), BMI (p<.001), lean body mass (p<.001), body water composition (p<.001), and SBP (p=.046) were reduced significantly in both groups. However, there was no statistically significant between-group difference in all parameters of body composition and SBP. The HCT and chloride (Cl-) decreased in both WL and WWL groups (p=.018 and p=.006, respectively). Serum aldosterone and cortisol significantly increased in both groups (p<.001). In particular, cortisol was significantly lower in the WWL than in WL group.CONCLUSIONS: This study suggested that a large-volume hydration before water restriction for weight loss can be a safe method.

Roux-en-Y gastric bypass surgery (RYGB) is the most applied technique in the treatment of severe obesity worldwide. However, its impact on anthropometric parameters and the risk for cardiometabolic diseases in obese patients is uncertain. To evaluate anthropometric clinical parameters and the evolution of risk factors for obesity-related diseases in individuals of both sexes undergoing RYGB. Sixty-nine adults subjects from both sexes submitted to RYGB surgery treatment were divided into 3 groups: G1(13 months, n=24); G2 (13 and 25 months, n=21), and G3 (25 and 37 months, n=24). Sociodemographic and anthropometric information before and after surgery were collected. The abdominal perimeter was used in the classification of cardiometabolic risk and the BMI was used for the risk of obesity-related diseases. Hypotheses were tested by Student's t-test and ANOVA, and the significance level adopted was 5%. The average age was 36.0±10.0 years, with 69.6% being male and 30.4% female. Anthropometric parameters (weight, BMI, and abdominal circumference) were higher among women, except for weight loss and percentage of weight loss. There was a difference in weight loss between the sexes in the moments before and after RYGB. There was a decrease in the risk of disease due to obesity and cardiovascular diseases after RYGB. Weight loss and %WL were greater years by year in the short term of 3 years after surgery. RYGB proved to be an effective strategy for both sexes in combating obesity, providing in the short term a significant improvement in clinical-anthropometric parameters and reduction of risk factors for obesity-related cardiometabolic diseases.

Background: In older people with psychoneurological diseases, COVID-19 infection may be associated with a risk of developing or exacerbating dysphagia. The aim of the present study was to examine the relationship between eating/swallowing function and COVID-19 infection. Methods: Subjects were 44 inpatients with confirmed COVID-19 infection being treated for schizophrenia in a psychiatric ward. Eating function was assessed using the Food Intake Level Scale (FILS) before and after infection. We also evaluated age, comorbidities, COVID-19 hospital stay, obesity index, weight loss rate, and chlorpromazine equivalent. Results: Subjects had a mean age of 68.86 years. Pre-infection, 20 subjects had a FILS score of 7–9 (presence of eating/swallowing disorder) and 24 subjects had a score of 10 (normal). Eating function after infection resolution showed decreasing FILS score compared to that before infection in 14 subjects (74.14 years). Six subjects (79.3 years) transitioned from oral feeding to parenteral feeding. A ≥ 10% weight loss during infection treatment was significantly associated with decreased eating function and a transition to parenteral feeding. Chlorpromazine equivalents, comorbidities, and number of days of hospitalization showed no associations with decreased eating function. Conclusions: Preventing malnutrition during treatment for COVID-19 infection is important for improving post-infection life prognosis and maintaining quality of life (QOL).

Objective: Fatigue associated with malignant conditions and their treatments is a disabling condition. This trial assessed the anti-fatigue effects of melatonin coadministration during adjuvant treatment of patients with the breast cancer. Material and Methods: Patients with breast cancer were randomly assigned to receive melatonin or placebo during adjuvant chemotherapy and radiotherapy. Thirty-seven patients were randomly enrolled in each group. The mean ages of patients in the intervention and control groups were 50.47 ± 10.79 and 46.05 ± 10.55 years, respectively ( P = .223). The intervention group received oral melatonin (18 mg/day) from 1 week before until 1 month after the adjuvant radiotherapy. The level of fatigue was assessed before and after intervention using Brief Fatigue Inventory (BFI) in both groups. To analyze data, the Student’s t-test and the Chi-square test were used at a significance level of P ≤ .05. Results: The BFI score was similar before the intervention in both groups, however, after the intervention, it was significantly lower in the melatonin group ( P < .001). Moreover, the frequency of severe fatigue in the melatonin group was significantly lower than in the placebo group after intervention (42.1% vs 83.3%, P < .001). Conclusion: Coadministration of melatonin during adjuvant chemotherapy and radiotherapy of women with breast cancer decreased the levels of fatigue associated with the malignant condition and its treatments.

Gut microbiota-mediated inflammation promotes obesity-associated low-grade inflammation, which represents a hallmark of metabolic syndrome. To investigate if lifestyle-induced weight loss (WL) may modulate the gut microbiome composition and its interaction with the host on a functional level, we analyzed the fecal metaproteome of 33 individuals with metabolic syndrome in a longitudinal study before and after lifestyle-induced WL in a well-defined cohort. The 6-month WL intervention resulted in reduced BMI (−13.7%), improved insulin sensitivity (HOMA-IR, −46.1%), and reduced levels of circulating hsCRP (−39.9%), indicating metabolic syndrome reversal. The metaprotein spectra revealed a decrease of human proteins associated with gut inflammation. Taxonomic analysis revealed only minor changes in the bacterial composition with an increase of the families Desulfovibrionaceae, Leptospiraceae, Syntrophomonadaceae, Thermotogaceae and Verrucomicrobiaceae. Yet we detected an increased abundance of microbial metaprotein spectra that suggest an enhanced hydrolysis of complex carbohydrates. Hence, lifestyle-induced WL was associated with reduced gut inflammation and functional changes of human and microbial enzymes for carbohydrate hydrolysis while the taxonomic composition of the gut microbiome remained almost stable. The metaproteomics workflow has proven to be a suitable method for monitoring inflammatory changes in the fecal metaproteome.

The purpose of this study was to compare the estimates of lean body mass (LBM) and percent body fat (%BF), as predicted by bioelectrical impedance (BIA) and sum of skinfolds (SF), with those derived by hydrostatic weighing (HW) obtained before and after a 10-wk diet and exercise regimen. The experimental (E) group consisted of 17 healthy male subjects; 20 healthy males served as the control (C) group. Post hoc Scheffe contrasts computed on E group data indicated that, for both LBM and %BF, the Lukaski and Segal BIA equations, as well as the Durnin SF equation, derived mean values that were not significantly different (0.05 significance level) from HW in both pre- and postregimen conditions. For LBM, the same equations derived the following significant (P less than 0.01) correlation coefficients for both pre- and postregimen data: Lukaski, 0.87 and 0.85; Segal, 0.89 and 0.87; and Durnin, 0.90 and 0.88. For %BF, the correlation coefficients were slightly lower but remained statistically significant (P less than 0.01). The findings of this study suggest that the BIA method, by use of either the Lukaski or Segal prediction equations, is a valid means of predicting changes in human body composition as measured by the Siri transformation of body density.

The better understanding of the clinically important behavioral features of new instrument systems has an important significance for the clinical endodontics. This study aimed to investigate the shaping and centering ability as well as cyclic fatigue resistance of HyFlex CM (CM), HyFlex EDM (EDM) and EdgeFile (EF) thermally treated nickel–titanium (NiTi) endodontic instrument systems. Sixty curved root canals of the mesial roots of mandibular molars were randomly assigned into three groups (n = 20) and shaped using CM, EDM and EF files up to the size 40 and taper 04 of the instruments. µCT scanning of the specimens before and after preparation was performed and the morphometric 2D and 3D parameters were evaluated in the apical, middle and coronal thirds of root canals. In each group, 40.04 instruments (n = 20) were subjected to the cyclic fatigue resistance test in artificial root canals at 37 °C temperature until fractures occurred, and the number of cycles to failure (NCF) was calculated. The fractographic analysis was performed using a scanning electron microscope, evaluating topographic features and surface profiles of the separated instruments. The one-way analysis of variance with post hoc Tuckey’s test was used for statistical analysis of the data; the significance level was set at 5%. All systems prepared the comparable percentage of root canal surface with the similar magnitude of canal transportation in all root thirds (p > 0.05), but demonstrated significantly different resistance to cyclic fatigue (p < 0.05). The most resistant to fracture was EF, followed by EDM and CM. The length of the fractured fragments was not significantly different between the groups, and fractographic analysis by SEM detected the typical topographic features of separated thermally treated NiTi instrument surfaces.
