This report aims to present a clinical case of endodontic re-treatment performed on a left inferior premolar with two canals of difficult localization. The importance of anatomical knowledge and the use of an operating microscope, especially in complex reintervention are discussed. An endodontic re-treatment was performed on element 34, which had a prosthetic restoration and vestibular fistula. In clinical examination, it was observed a negative result to the sensitization test and a positive result to vertical percussion test. After all the obturating material was removed, an operating microscope was requested to assess a possible fracture or the presence of another canal. It was noticed the presence of another canal that was treated and obturated as the existing canal. After 12 months, a new radiographic exam was performed, whose result was found asymptomatic and radiographically normal. In complex peculiar anatomy cases, the operating microscope is vital for the localization of extra canal to guarantee treatment success.

Background: Ascites is one of the most important clinical syndromes, caused by multiple organ disorders, characterized by abdominal distension with accumulation of fluid of various colors and consistencies depending on the etiology that are encountered commonly in canine practice worldwide. Although it has been reported from different countries including India but it has not yet been documented from Bangladesh. Objectives: To evaluate the successful therapeutic management of a clinical case of ascites in dog supported with its brief review for its appropriate application Materials and Methods: A female Spitz dog two and half years old brought for treatment with the history of abdominal distension on 1st November 2009. Clinical examination, abdominocentesis and laboratory examination of ascitic fluid were used for the diagnosis of ascites in dog. Results: Clinical examination revealed dyspnea, discomfort, lethargy, weakness, pale mucous membrane, normal rectal temperature 103.2 0F and distended abdomen with fluid thrill on palpation. Examination of ascitic fluid revealed clear white fluid (pure transudate) which is mainly hepatic origin resulting portal hypertension and hypoproteinaemia. Treatment with restricted sodium diet, antibiotic (amoxicillin), diuretic (furosemide; Lasix, Sanofi Aventis) and vitamin B-complex and C- vitamin supplement with regular monitoring assisted in successful recovery. The recovered dog survived for next five years up to 2014 and then died due to other reasons. Conclusions: This clinical case record on canine ascites with successful treatment along with review especially on the methods of diagnosis and cause-wise treatment would certainly help the clinician for proper management of the clinical cases of canine ascites. We present the clinical case of the patient with gastrointestinal stromal tumor of the duodenum (dGIST).The 34-year-old women had complaints of intermittent dull pain located in the upper abdomen. Clinical examination of the abdomen revealed a duodenum tumor. Resection of the anterior wall of the descending part of the duodenum with a tumor was performed. Histopathology reported that the removed neoplasm was represented by GIST of the duodenum. Immunohistochemistry reported that the tumor cells had an expression of DOG 1, Vim, CD117, CD 34.

Internal resorption is a separate category of diseases of hard tissues of teeth, characterized by the loss of dentine as a result of activation and activity of osteoclasts. The article discuss issues concerning etiology, the prevalence of internal resorption, its diagnosis, emphasise the contribution of computed tomography in the diagnosis and prediction of treatment success, clinical decisions and therapeutic treatment of this disease. Describe modern endodontic treatment methods, including the use of magnification, ultrasound in endodontic treatment, the use of certain materials for temporary and permanent root canal filling. Under these conditions, the prognosis of conservative treatment of internal resorption of dental hard tissues is improved, even if there is a perforation of the walls of the tooth cavity.

Heterotopic pregnancy is coexistence of intrauterine and extrauterine pregnancies that is ectopic pregnancies. It is said to be rare. Here we report a case of 27 years old woman with heterotopic pregnancy. Patient had a typical presentation of severe lower abdominal pain following amenorrhoea for 2½ months. On clinical examination, there was suspicion of ectopic pregnancy but ultrasonography revealed early intrauterine pregnancy along with right tubal pregnancy with huge collection in abdomen . Treacher Collins syndrome usually affects bilateral and symmetric structures that include the orbits, mandible, and ears. The purpose of this report is to describe a clinical case of the syndrome, focusing on the anatomy of the temporomandibular joint, which was assessed using the computed tomography method. Clinical examination included evaluation of mandibular dynamics, investigation of temporomandibular dysfunction, and measurement of bite force. Significant morphological and functional alterations were observed but without significant documented clinical consequences.

The concurrence of COVID-19 with dengue present a similarity in the early stages of both diseases could cause delays in the diagnosis of dengue infection,SARS-CoV-2 or both. This adds an additional difficulty to the diagnosis and suggests that a positive result of IgM or IgG serology would not be sufficient for the definitive confirmation of dengue. Clinical case: 13-year-old female patient in poor general condition who debuts with a clinical picture of 5 days of evolution consisting of fever, anosmia, ageusia, unquantified fever peaks, chills, abdominal pain and vomiting that does not stop.

Abstract. Purpose. To demonstrate a rare clinical case of pyomyoma complicated by perforation and the development of secondary appendicitis Materials and methods. The study involved clinical instrumental examination and surgical treatment of a woman with extragenital pathology, who presented in postmenopausal period with uterine leiomyoma modified into pyomyoma and hystero-appendicular fistula resulting in secondary appendicitis. Results. Diagnosis of pyomyoma is difficult due to absence of pathognomonic symptoms and mimicking into a range of pathological conditions inherent to postmenopausal period. This complication of leiomyoma may result in spontaneous perforation of the uterus involving neighboring organs in the inflammatory process and the development of surgical diseases, particularly secondary appendicitis.  Conclusions. In pyomyoma, even taking into account complications in the form of surgical diseases there is a possibility of performing laparoscopic surgical treatment of the required volume which gives a positive result.

In this article, we demonstrate a clinical case with high efficacy of Karipain electrophoresis in the treatment of a patient with discogenic radiculopathy in the cervical spine. The positive result was a complete regression of motor deficiency, restoration of sensory function and a decrease in the size of a hernia of the intervertebral disc at the level of C5C6 by 40%.

Background: Breast carcinoma recurrence appears in 5%-30% of cases, after the completion of breast cancer treatment. Recurrence appears on thoracic wall (chest wall), on the skin, on the breast tissue remaining after the surgery, equal sided axillary lymph nodes, supraclavicular lymph nodes or on the internal mammary lymph nodes. Recurrence often stays undetected by clinical examination, while it can be detected by ultrasound and punctured under ultrasound control. Cytological confirmation of malignant cells represents a signal to commence a specific recurrence treatment. Methods: Aspiration puncture under ultrasound control has been performed within 128 patients. Only 38 patients, out of 128, did not have distant metastasis at the moment of recurrence detection, so the recurrence was surgically removed and was subjected to the histopathological analysis. The results of definite histopathological analysis were compared to the clinical results and cytological results, which had been obtained preoperatively. Results: 31 patients, out of above mentioned 38 patients, have had breast carcinoma recurrence, while 7 patients have had benign tumor lesion. Clinical diagnoses were correct in 47.4% of cases, and incorrect for 52.6%. False positive finding result happened in 7.89% cases and false negative finding result happened in 39.47% cases. Ultrasound diagnoses were correct for 86.84% of patients and incorrect for 13.16% of patients. Cytological analysis of material obtained by aspiration puncture under ultrasound control produced correct diagnoses for 86.84% of patients. 13.16% of patients had inconsistent cytological and histopathological findings result. Cytological analysis has detected malignant cells for 5.26%, but the recurrence was not histopathologically confirmed, which points that the results were falsely positive. Falsely negative results were confirmed for 7.89%. Conclusion: Clinical examination was reliable for 52.6% of patients examined. Cytological analysis of material obtained by ultrasound-controlled puncture was reliable for 86.84% of patients examined. Aspiration puncture followed by cytological analysis gave falsely negative result for 7.89% of patients examined, while clinical examination gave falsely negative result for 39.47% of patients examined. Aspiration puncture gave falsely positive result for 5.26%, and clinical examination gave falsely positive result for 7.89%. Aspiration puncture is simple, easy to perform, without damaging effect for the patient or the medical personnel, it is not expensive and it is significantly more reliable than clinical examination itself for detection of breast carcinoma recurrence. .

The objective. Prevention of diagnostic errors associated with the rare detection of syphilis in patients in neurological hospitals. Results. A clinical case from the practice of dermatovenereologists is given. Conclusions. For timely detection of neurosyphilis it is necessary to conduct a cerebrospinal fluid examination of all patients in the neurological hospital who have a positive result of non-treponemal and treponemal tests in the serum.
