Toxic epidermal necrolysis is a rare, severe cutaneous reaction, mostly caused by drugs. It affects the skin and mucous membranes, with involvement of more than 30% of body surface. We describe the case of a young woman, previously healthy, who developed skin detachment of more than 90% of the body surface 15 days after being administered lansoprazole for peptic disease. The treatment consisted in discontinuation of the drug involved and early administration of intravenous human immunoglobulin, which led to a satisfactory outcome of the case, substantiating the impact of early diagnosis and treatment on the morbidity and mortality of these patients.

The two-dimensional motion of a rigid body with a smooth surface is studied during its oblique impact on a liquid layer. The problem is coupled: the three degrees of freedom of the moving body are determined together with the liquid flow and the hydrodynamic pressure along the wetted part of the body surface. The impact process is divided into two temporal stages. During the first stage, the wetted region expands at a high speed with jetting flows at both ends of the wetted region. In the second stage, the free surface of the liquid is allowed to separate from the body surface. The position of the separation point is determined with the help of the Brillouin–Villat condition. Calculations are performed for elliptic cylinders of different masses and with different orientations and speeds before the impact. The horizontal and vertical displacements of the body, as well as its angle of rotation and corresponding speeds are investigated. The model developed remains valid until the body either touches the bottom of the liquid or rebounds from the liquid.

BACKGROUND: Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN) are acute, life-threatening drug reactions, which lead to massive epidermal necrolysis. Granulysin plays an important role as a key mediator for keratinocyte apoptosis in these conditions. Erythema multiforme (EM) may have skin manifestation similar to SJS/TEN. AIMS: The aim of the study was to compare serum granulysin levels in patients with SJS/TEN and EM as well as to investigate a possible association between serum granulysin levels and the severity of SJS/TEN. METHODS: In total, 48 patients with SJS/TEN, 43 patients with EM, and 20 health controls (HCs) were enrolled. We measured serum granulysin levels using enzyme-linked immunosorbent assay. RESULTS: The average level of serum granulysin in the SJS/TEN patients was 23.0 ng/ml (range 1.2–144.6 ng/ml), significantly higher than that of EM group (20.1 ng/ml; range 8.5–121 ng/ml, p < 0.05) and HCs group (20.8 ng/ml; range 10.1–46.7 ng/ml, p < 0.05). Of 48 SJS/TEN patients, the 25 samples collected <6 days after onset showed higher level of serum granulysin (27.7 ng/ml; range 2.5–144.6 ng/ml) than those collected ≥6 days after onset (17.9 ng/ml; range 1.2–59 ng/ml; p > 0.05). No significant correlation was found between serum granulysin levels and the body surface area affected and the modified-SCORTEN. At the day of re-epithelialization, serum granulysin levels were not different compared with those at the day of hospitalization. CONCLUSIONS: Serum granulysin levels are significantly higher in SJS/TEN group than in EM group. After the onset, serum granulysin levels in patients with SJS/TEN are not a good biomarker to evaluate the severity of the diseases.

Stevens Johnson Syndrome (SJS) and Toxic Epidermal Necrolysis (TEN) are considered a single entity with variability in the extent of the lesions, characterized by erythema multiforme that may involve mucosa. Severe cutaneous reactions secondary to medications are classified according to the area of epidermal detachment. The activation of cytotoxic T cells and macrophages is mediated mainly by IL-2 and interferon gamma secreted by Th1 lymphocytes, and the activation of eosinophils and B lymphocytes in IgE is mediated by secreted IL-4, IL-5, IL-10 and IL13 by B lymphocytes. The topography of SJS is predominantly central, affecting the trunk and sometimes a generalized dissemination is shown that affects a body surface area of less than 10%, characterized by irregular violaceous erythematous macules of target shooting, which can form confluent blisters. TEN is characterized by a skin detachment greater than 30% of the body surface, whose predominant lesion is diffuse erythema with individual macules, which give rise to detachment surfaces greater than 5 cm. The treatment is symptomatic, nonspecific, and aimed at avoiding complications, carried out in specialized intensive care units, due to ignorance of the pathogenesis. Integral management with different therapeutic alternatives can represent a crucial part in the multisystemic management of SJS and TEN.

Post-mortem examination was carried out on a 2-week old male Friesian crossbred calf to determine the cause of death. The body condition of the calf was fair with pale mucous membranes. There was petechial to ecchymotic epicardial haemorrhages with mild hydroperitoneum and hepatomegaly. The enlarged rumen contained a large amount of white offensive rancid-smelling curdled milk mixed with watery content while the abomasum contained some sand sediment. A diagnosis of acute ruminal drinking was made based on the calf's age, reportedly feeding entirely on milk ration, and the presence of a large amount of curdled milk in the rumen. Early diagnosis and the treatment of underlying pathological conditions, correction of predisposing management practices, and rumenostomy are some of the ante-mortem ways to combat and improve the prognosis of the condition in affected calves.

Background: Steven Johnson syndrome (SJS) is a rare disease that is characterized by acute cutaneous manifestation represented by eruptions of the skin and the mucosal membranes. SJS is an immune-mediated disease, a hypersensitive reaction, characterized by hyperpigmentation of the mucous membranes, rash on the skin and multiple bullae and erosions scattered all over the body especially the face, trunk, and the extremities. Many studies reported that the incidence rate of the SJS was about 1.2 – 6 cases/ million each year and it is more common among males while the toxic epidermal necrolysis (TEN) is more common among females. In addition to the cutaneous manifestations.SJS might show multiple systemic manifestations including the liver, lungs and kidneys. In this case we reported the development of Steven Johnson syndrome in relation to the use of lamotrigine antiepileptic drug.

In the present study, we experimentally investigate water surface deformation due to the impact of rounded cylindrical projectiles with different aspect ratios (1.0–8.0). The subsequent jet and splash formation is closely related to the dynamics of an underwater cavity. To control the cavity formation, two kinds of surface conditions (smooth and rough) are applied to the front parts of the projectiles, and two impact speeds are considered. The Froude, Reynolds and Weber numbers are in the ranges of 32–90, $5\times 10^{4}{-}8.4\times 10^{4}$ and 1700–5000, respectively. When the front is smooth, the water film rises up along the body surface immediately after impact, and the temporal variation of its height is analytically estimated. The film converges at the rear pole to create an apex jet at lower aspect ratios and simply rises up and falls with the body at higher aspect ratios. The jets could be further distinguished as thin and thick jets, whose breakdown is found to be a function of the viscous force and surface tension, i.e. the Ohnesorge number. On the other hand, when the front is rough, the water film cannot rise up along the body surface, and instead early separation occurs to make the splash above a free surface. The splash size is quantified to assess the effects of the aspect ratio and impact speed. Upon splash formation, a cavity is created under the free surface, which emanates from the nose of the projectile. As the body sinks, the cavity pinch-off occurs due to the imbalance between the hydrostatic pressure and air pressure inside the cavity. At higher aspect ratios, cavity pinch-off occurs on the side wall of the projectile and leaves a portion of the cavity bubble on it. When the surface is smooth, no underwater cavity forms. Finally, we compare the hydrodynamic force acting on the sinking bodies with and without cavity formation, based on the underwater trajectory of each projectile. It is found that the underwater cavity reduces the drag force on the sinking body when it fully encapsulates the body; however, if the air bubbles are partially attached to the body after pinch-off, they tend to detach irregularly or impose additional drag on the body.

PurposeThe comfort and fit of clothes are affected by fabric properties, dressed ease and environmental conditions, in which dressed ease is influenced by the interaction among complex shapes of human body, style design and fabric mechanical properties.Design/methodology/approachIn this study, the dressed ease distribution at waist section, which is related to body surface convex angle, was investigated using 3D scanning. A series of surface convex angles on bust and back were formed after adjusting the mannequin. The mannequin was scanned by TC2 separately in garments with eight different ease allowances. Then the dressed ease distributions at waist under different convex angles of body surface have been acquired by calculating the distance between waist points and dressed surfaces along normal directions.FindingsThe results showed that the body surface convex angle was weakly related to the dressed ease when the garments’ bust ease allowance was below 4 cm. When the garments’ bust ease allowance was within 6–12 cm, the body convex angle had a great impact on the dressed waist ease distribution in the condition of 26º–33º bust convex angle and 13.96º–17.96º back slope angle. For slack garments with more than 16 cm ease allowance, the dressed waist ease distribution did not relate to the bust convex angle, while it strongly related to the bust convex angle between 13.96º and 17.96º. The regression model was statistically significant between the dressed ease value and the body surface convex angle.Originality/valueAccording to the dressed waist ease distribution of different body surface convex angles, this paper gives an application of pattern modification in order to optimize the waist fit. The results can provide guidance for the optimization of different body shapes. At the same time, the application of gap data to 3D virtual fitting can greatly improve the authenticity of virtual simulation effect.

Devices are described by means of which the terminals of thermopiles may be held in stable apposition with the mucous membranes of the nasal cavity and postnasal space and local temperature variations thus followed. Chilling of the body surface has without exception caused depression of the temperature of the nasal mucosa surface, amounting in some instances to as much as 6°C., and indicating marked reflex vasoconstriction and diminution of blood supply. With rewrapping, partial recovery of blood supply promptly occurs, although recovery has been incomplete within the duration of the experiments in ten of twelve instances. Application of the wires within the nasal cavity has usually caused pain and discharge of clear mucus, sometimes also lacrimation and sneezing. The rhinorrhea has occurred both on the side directly irritated and on the opposite side, although on the former more abundantly, and has apparently been little if at all affected by the diminished blood supply and shrinkage of the mucous membrane incident to chilling the body surface. Discharge from the nose has been at most a rare occurrence in experiments in which the nasal mucosa was not directly irritated. The temperature of the nasopharyngeal mucosa surface has also been depressed, typically between 1° and 2°C., with chilling of the body surface. This depression has also been shown to have local reflex vasoconstriction and ischemia as its basis. With rewrapping, prompt return toward normal occurs, but here also recovery of blood supply has in the majority of instances not been complete within the duration of the experiments. The thresholds of the chilling vasoconstrictor reflex to the mucous membranes of the nasal cavity and postnasal space, tonsil, oropharynx, and palate and the threshold of the reflex to the skin of the trunk have been found to be lower than the threshold of the like reflex to the skin of the forehead. Disrobing 'the warmly wrapped subject in a room a little below ordinary room temperature has been found sufficient to cause marked vasoconstriction in the sites of the former group, but only slight or no vasoconstriction in the forehead. A number of instances of cold or sore throat occurred among the subjects of the experiments, in several instances correlated with somewhat interesting bacteriologic findings which will be described elsewhere.

Recent clinical studies reported that prosthesis-patient mismatch (PPM) becomes clinically relevant when the effective orifice area (EOA) indexed by the body surface area (iEOA) is <1.2–1.25 cm2/m2. To examine the effect of PPM on transmitral pressure gradient and left atrial (LA) and pulmonary arterial (PA) pressures and to validate the PPM cutoff values, we used a lumped model to compute instantaneous pressures, volumes, and flows into the left-sided heart and the pulmonary and systemic circulations. We simulated hemodynamic conditions at low cardiac output, at rest, and at three levels of exercise. The iEOA was varied from 0.44 to 1.67 cm2/m2. We normalized the mean pressure gradient by the square of mean mitral flow indexed by the body surface area to determine at which cutoff values of iEOA the impact of PPM becomes hemodynamically significant. In vivo data were used to validate the numerical study, which shows that small values of iEOA (severe PPM) induce high PA pressure (residual PA hypertension) and contribute to its nonnormalization following a valve replacement, providing a justification for implementation of operative strategies to prevent PPM. Furthermore, we emphasize the major impact of pulmonary resistance and compliance on PA pressure. The model suggests also that the cutoff iEOA that should be used to define PPM at rest in the mitral position is ∼1.16 cm2/m2. At higher levels of exercise, the threshold for iEOA is rather close to 1.5 cm2/m2. Severe PPM should be considered when iEOA is <0.94 cm2/m2 at rest.
