Over the last 10-15 years, treatment of tuberculosis patients with artificial pneumothorax has become widespread and there are many works devoted to collapse therapy in the press; details of the technique, efficiency of treatment, and complications are discussed. Spontaneous pneumothorax (SP) is one of the most dangerous complications of pneumothorax. Under p. p. we understand gas accumulation in pleural cavity in case of lung perforation, as the result of pathological process in the lung, more often of subpleural cavernous cavity breakthrough, caseous focus, or due to lung parenchyma needle trauma while applying pneumothorax.

Background: Pneumothorax is a clinical condition which can cause respiratory distress. It can have as its origin traumatic causes or even classified as spontaneous, mainly related to diseases of the lung parenchyma. Lipoid pneumonia is rarely described in dogs, and it is characterized by globules of lipid in the alveolar spaces. Endogenous lipoid pneumonia (EnLP) occurs when lesions on pulmonary cells release cholesterol and other lipids in the alveoli. There is no clinical approach established for EnLP in veterinary patients. The aim of this report is to describe a case of a young Maltese dog, with recurrent spontaneous pneumothorax in which EnLP was diagnosed Post mortem.Case: A 2-year-old sexually intact male Maltese dog was evaluated for restrictive dyspnea. Clinicopathologic findings included cyanotic, muffled chest auscultation with hypersonic thoracic percussion. Chest x-ray demonstrated an increase in pleuropulmonary radio transparency and a floating-looking heart, indicating pneumothorax. Complete blood counts andbiochemical panel results were normal. Dirofilaria immitis antigen test results were negative. Computed tomography demonstrated slightly hyper-expanded pulmonary fields, with slightly enlarged reticular marking with areas of mild multicentric panlobular emphysema and a fracture on the sixth left rib. The treatment was focused on improving the breathing pattern through sedation, supplementation with oxygen, and thoracentesis. Owing to the reserved prognosis of the case, the unknown etiology of the recurrent pneumothorax, and the clinical worsening of the patient, the owner opted for euthanasia. Necropsy displayed multiple, circular whitish areas in the lungs, distributed over the surface of all lobes. Histopathological examination revealed pulmonary tissue with the subpleural micronodular foci, multifocal to coalescent, with a moderate accumulation of foamy intra-alveolar macrophages, occasionally multinucleate, associated with cholesterol crystals compatible with endogenous lipid pneumonia. Discussion: The patient presented with clinical signs and physical examination characteristics of pneumothorax at the first visit. After the pneumothorax diagnosis, and clinical stabilization of the patient. No predisposing factor for the formation of the pneumothorax was identified as the radiography revealed only bronchitis and blood tests were normal, the patient was thus discharged after 24 h, with the recommendations for observing the breathing pattern. Initially, spontaneous pneumothorax was suspected. The antibiotics were administered since bacterial pneumonia, although not confirmed on chest x-ray, is the main cause of pneumothorax in dogs is lung parenchyma disease. With the worsening of the clinical condition of the patient, CT was performed and did not demonstrate any findings that would justify the presence of pneumothorax. Despite the placement of the chest tube for facilitating the management of thoracentesis, there was no stabilization of the condition, enhancing the frequency of centesis procedures, which led to the decision to euthanize. The microscopic examination of the pulmonary alterations was decisive for the diagnostic conclusion. The visualization of the accumulation of foamy intra-alveolar macrophages, occasionally multinucleate, associated with cholesterol crystals, was responsible for the diagnosis of EnLP. This condition is rarely described in dogs and as in the present report, it is a noninfectious inflammatory condition, characterized by intra- or extracellular globules of lipid in the alveolar spaces. In the present report, although it was not possible to determine the etiology of EnLP, we can conclude that although rare, it can affect dogs and can generate severe clinical repercussions.

Spontaneous pneumothorax in the newborn period was recognized in fifteen infants over a 6-year period. Seven of the 15 infants were symptomatic in the delivery room, which suggests that lung rupture occurred with the first few breaths. Three of the seven were meconium stained. The remaining eight infants were symptomatic within the first 24 hours. The unique features of the first breath of the newborn infant, namely, the serial opening of ventilatory units and high applied pressures, suggest that any foreign material in the airway would make rupture of opened units likely. Studies on the inflation of airless lungs demonstrate the opening phenomenon. Four of the 15 infants died, 2 from their pneumothoraces. This bad experience led us to examine principles of treatment of spontaneous pneumothorax with the recommendation that oxygen breathing in some term infants, and prompt aspiration in others would be useful. The majority, however, can be expected to resolve spontaneously. Measurements of the rate of absorption of air in the pleural cavity in the rabbit show a six-fold increase with 100% O2 breathing.

This is a prospective clinical study aimed at introducing a method to visualise the location of an air leak and to identify the bulla responsible on three-dimensional (3-D) cine CT. In 10 patients with spontaneous pneumothorax, dynamic 320-detector row CT was performed with injection of 0.9% saline into the affected pleural cavity via a preplaced chest tube. In eight cases, 3-D cine CT thoracography revealed the location of the air leak and the bulla responsible (7 cases: air stream sign; 1 case: repeated collapse and expansion of a bulla with the patient's breathing).

Ligoniai ir metodai 2000–2003 metais Torakalinės chirurgijos klinikoje gydyti penki vienos giminės nariai, sirgę spontaninio pneumotorakso komplikuota plaučių emfizema. Įgimta plaučių emfizema šeimoje sirgo ir šiuo metu serga 12 žmonių. Visi ligoniai buvo operuoti. Atliktos torakotomijos, rezekuota pažeista plaučio dalis, atlikta dalinė arba visiškoji pleurektomija, drenuota pleuros ertmė. Rezultatai Pooperaciniu laikotarpiu visiems ligoniams padidėjo kraujavimas į pleuros ertmę. Netekto kraujo tūris buvo papildomas hemotransfuzija, šviežiai šaldytos plazmos infuzijomis. Dėl kraujavimo į pleuros ertmę ar susidariusio hemotorakso buvo atliktos trys retorakotomijos. Pakartotinių operacijų metu buvo rastas įvairaus dydžio hemotoraksas (600–1100 ml), difuzinis kraujavimas iš krūtinės sienos be aiškaus vieno kraujavimo židinio. Visi pacientai pasveiko. Išvada Turimi duomenys yra būdingi įgimtam alfa-1 antitripsino sintezės defektui. The data from the case histories of all the patients are presented and the possible reasons for blood clotting dysfunction are discussed. Patients and methods Twelve members of the family have been or still are suffering from congenital pulmonary emphysema. Five members of the family were operated on. The surgey involved thoracotomy, resection of the damaged section of a lung, partial or total pleurectomy, pleural cavity drainage. Results All the patients underwent surgery, and in all cases the postoperative intrapleural bleeding was uncommonly intensive. In three cases rethoracotomies were performed. All patients survived. Abstract: Radiology examination especially chest x-ray can enforce various kinds of pulmonary diseases inter alia pneumothorax. Pneumothorax is defined as the presence of air in the pleural cavity. The causes of pneumothorax are very diverse ranging from idiopathic, infection, trauma, and iatrogenic. This study was aimed to obtain the profile of chest x-ray in patients with pneumothorax. This was a retrospective descriptive study by using secondary data from the medical records at the Department of Radiology Prof. Dr. R. D. Kandou Hospital Manado from January 2015 to August 2016. Samples were the medical records of patients that were radiologically diagnosed as pneumothorax. There were 41 patients that were diagnosed radiologically as pneumothorax. The majority of cases were male (90.2%), age group >50 years (36.6%), location of lesion in the right hemithorax (53.7%), and secondary spontaneous pneumothorax as the etiology (43,9 %). Conclusion: In this study, pneumothorax was more common among males, age group of ≥50 years, and secondary spontaneous pneumothorax as the etiology of pneumothorax.Keywords: pneumothorax, radiology, chest x-ray Abstrak: Pemeriksaan radiologi khususnya foto toraks dapat menegakkan berbagai macam diagnosis penyakit paru, salah satunya ialah pneumotoraks. Pneumotoraks adalah terdapatnya udara bebas didalam rongga pleura dengan penyebab yang sangat beragam mulai dari idiopatik, infeksi, trauma, maupun iatrogenik. Penelitian ini bertujuan untuk mengetahui profil hasil pemeriksaan foto toraks pada pasien pneumotoraks. Jenis penelitian ialah deskriptif retrospektif dengan pengambilan data di Bagian Radiologi RSUP Prof. Dr. R. D. Kandou Manado pada bulan Januari 2015 sampai dengan Agustus 2016. Sampel yaitu data rekam medik pasien yang didiagnosis pneumotoraks secara radiologis sebanyak 41 pasien. Yang tersering ditemukan ialah pasien laki-laki sebanyak 37 orang (90,2%), kelompok usia >50 tahun sebanyak 15 orang (36,6%), lokasi lesi hemitoraks deksra sebanyak 22 kasus (53,7%), serta etiologi pneumotoraks spontan sekunder sebanyak 18 kasus (43,9%). Simpulan: Pada penelitian ini didapatkan pneumotoraks paling banyak pada laki-laki, kelompok usia ≥50 tahun, dengan pneumotoraks spontan sekunder sebagai etiologi tersering. OBJECTIVE. The article analyzed the experience of treatment of endometriosis-related pneumothorax (ERP). MATERIAL AND METHODS. The diagnosis of ERP was detected in 30 women at the period from 2004 to 2015. A control group consisted of 149 women. RESULTS. Statistically significant differences associated with presence of ERP were the elder age, right-side localization and recurrence course of disease. Diaphragmatic fenestrations and endometriotic ectopy and their combinations were specific findings in ERP-group. This group of patients characterized by frequent recurrences and higher rate of complications. The most effective method of treatment of ERP was diaphragm resection with pleurectomy and hormone therapy from 3 to 6 months after surgery. CONCLUSIONS. Endometriosis-related pneumothorax could cause up to 34 % cases of spontaneous pneumothorax in women of reproductive age. Diaphragmatic fenestrations and endometriotic lesions were specific signs of EAP. Direct visual examination of the pleural cavity was inevitable for reliable diagnostics of the disease. Surgical treatment of ERP was determined by higher rates of complication and recurrence. Postoperative hormone therapy could significantly improve the results of surgical treatment of ERP.

The article presents a literature review on the VATS treatment of spontaneous pneumothorax as a complication of bullous lung emphysema. The review considers questions of etiology, diagnostics and therapeutic tactics in VATS treat-ment of spontaneous pneumothorax with bullous emphysema. The main target of surgeons is not only elimination of spontaneous pneumothorax, but quick exposure of lung parenchyma pathology in order to predict possible reoccurrence of spontaneous pneumothorax. Video-assisted thoracoscopy in this case is a mini-invasive, less traumatic and highly efficient method of treatment of spontaneous pneumothorax. Anti-relapsing surgical interventions with pleurodesis are unanimously considered to be necessary. An integrated and differentiated approach to the treatment of spontane-ous pneumothorax as a result of bullous emphysema helps to determine diagnostic and therapeutic tactics of VATS treatment and choice of pleurodesis induction leading to improved treatment outcomes in this category of patients.

X-rays, creating favorable conditions for optimal determination of air in the pleural cavity, greatly facilitated the diagnosis of spontaneous pneumothorax and enriched our experience in this matter. "Over the past few years, there has been a rich literature on spontaneous pneumothorax. Numerous cases of spontaneous pneumothorax with the most unexpected etiology and the most diverse clinical picture have been published, which gave an impetus to revise our previous attitudes on the issue of spontaneous pneumothorax and, first of all, its pathogenesis.

Introduction. Pneumothorax is a common clinical problem in thoracic surgery. Leading professional associations have adopted a number of guidelines and recommendations for the treatment of pneumothorax, but their clinical use is often insufficient. This study was aimed at analyzing the incidence of pneumothorax, profile of patients and surgical treatment of pneumothorax at the Clinical Center Kragujevac, in a five-year period. Material an methods. This retrospective, non-interventional study used data collected from the medical records and operative protocol of the Department of Thoracic Surgery, Clinical Centre Kragujevac in a fiveyear period. All data were statistically analyzed. Results. In the observed period, 492 patients with diagnosis of pneumothorax were hospitalized. The majority of patients were treated in 2009 (140), and the fewest in 2005 (68). Four-fifths of patients were male. Primary spontaneous pneumothorax was found in 25% and traumatic pneumothorax in 50%. Most patients were aged 21-30 years (83). The number of cases of secondary spontaneous and traumatic pneumothorax increased with the age of patients and the number of cases of primary and recurrent spontaneous pneumothorax decreased with their age. Drainage as a method of treatment was prevalent in all types of pneumothorax (p < 0.01). Conclusion. The results of study show similarities with other studies regarding both the incidence of certain forms of pneumothorax and the gender-age profile of patients and their treatment, taking into account the observed period and the target population.
