Primary multiple neoplasms (polyneoplasia) are among the interesting and insufficiently studied fields of oncology. The term polyneoplasia is usually used for two or more malignant neoplasms in a single patient, which occur synchronously or metachronously. Over the last decade, an increase of primary multiple tumors prevalence has been observed. In 2015 39 195 new cases of primary multiple tumors were registered in Russia and comprised 6.7% of all incident malignant neoplasms. Primary multiple metachronous and primary multiple synchronous tumors with two neoplasms development are observed most often, with three more rarely, and with four or more very rarely. In average, 0.1% of a total number of oncologic patients have at least three malignant neoplasms. Primary multiple tumors of gastrointestinal tract are observed most often. The current article presents a clinical case of four synchronous tumors in a single patient localized in major duodenal papilla, sigmoid, transverse colon and lung.

Infantile fibrosarcoma (IF) is a rare soft tissue sarcoma that occurs with a frequency of 1% of all cases of malignant neoplasms in children in the first year of life. IF most often affects the soft tissues of the upper and lower extremities. Rare localizations include the trunk, the head and neck, and the gastrointestinal tract. The article describes a rare clinical case of congenital IF of soft tissues of the chest wall in a newborn, as well as a literature review.

✓ A 28-year-old man presented to the authors' hospital with multiple intracranial tumors. At 2 years of age, he had undergone resection of a medulloblastoma and received adjunctive craniospinal irradiation. Subsequently, he was diagnosed with nevoid basal cell carcinoma syndrome, Gorlin's syndrome. Since his first presentation, he has required surgery for multiple basal cell carcinomas, an osteochondroma of the rib, two meningiomas, a trigeminal schwannoma, and a pleomorphic liposarcoma, all of which arose within the radiation field. Despite this impressive list of benign and malignant neoplasms, the patient is relatively well and leads a normal life. The authors examine the relationships between Gorlin's syndrome and radiation therapy and the subsequent development of tumors.

Cancers of the gastrointestinal tract are widespread among the population and cause significant damage to the health care system. In order to improve the strategy of preventive measures and the detection of oncological diseases at the early stages, it is necessary to provide timely impact on possible risk factors contributing to the onset and progression of malignant neoplasms. This review demonstrates the association between the pathology of the biliary tract and oncological diseases of the digestive system, discusses the possible mechanisms of the influence of cholelithiasis and cholecystectomy on the development of malignant neoplasms of various parts of the gastrointestinal tract.

The Solitary Bone Plasmacytoma (SBP) is a rare oncologic disease corresponding to less than 5% of the malignant neoplasms of the plasma cells. It is characterized as a localized aggressive tumor consequent to the accumulation of monoclonal plasma cell neoplasms and, due to the rarity of the disease, there are only few clinical studies reporting it especially regarding prognostic factors and treatment. Here, we report a fatal clinical case of SPB in Roraima, the northernmost state of Brazil. The study shows a case of solitary bone plasmacytoma on the femur, where the patient was submitted to the preconized radiotherapy cycles. However, over approximately four-years following the diagnose, the patient required multi-modal approach to guarantee quality of life during the survival time. Finally, this study explores SBP-related issues and examines the challenges physicians face when managing the care of patients with SBP.

The problem of treating patients with malignant neoplasms of the extrahepatic bile ducts and the large duodenal papilla remains relevant due to the growing incidence, high mortality, and a pronounced decrease in the quality of life of patients, despite the radical surgery. The purpose of this study was to evaluate the effectiveness of photodynamic therapy (PDT) in inoperable patients with malignant tumors of these localizations.The study is based on the treatment of 79 patients with adenocarcinoma of large duodenal papilla and extrahepatic bile ducts. 29 patients received palliative bile drainage operations with PDT. 50 patients in the control group only had palliative bile drainage operations. Patients in the study group received from 1 to 3 PDT courses in a year. In total, 29 patients received 52 PDT courses. The tolerance to the method and the life expectancy of patients were evaluated. The median survival of patients who underwent PDT was 18 months (11–60 months); in the control group – 11.5 months. PDT, in combination with bile drainage operation, is an effective method for the treatment of inoperable patients with malignant neoplasms of the extrahepatic bile ducts and the large duodenal papilla in the absence of severe complications and with easy tolerance to therapy by patients. For the treatment and prolongation of life of patients whose radical surgical treatment is associated with a high risk of death, PDT seems to be the best treatment option. Results of PDT treatment for cancer of this localization are comparable with the results of radical surgeries and exceed those for palliative surgeries.

Objectives. Actinomycosis in children has not been studied enough, the number of publications is limited. Actinomycosis of the knee joint is very rare, the disease mimics tuberculosis or malignant neoplasms, which can lead to late diagnosis and inadequate treatment. Materials and methods. We report on the first case of an 8-year-girl with actinomycosis knee. Diagnosis is based on histological examination. Results. Surgical resection of infected tissue and longterm antibiotic therapy for 4.5 months was effective in the treatment of pediatric patient with knee actinomycosis. In the scientific literature no such clinical case was found. Сonclusion. Case of successful treatment of pediatric patient with knee actinomycosis is presented for the first time.

The results of the development of a curd functional product with vegetable filler Melissa officinalis L. are presented. An analysis of the health status of the population carried out in recent years by the World Health Organization shows that the world community is facing acute questions related to the need to prevent the so-called “diseases of civilization”, which include diseases of the cardiovascular system, oncology, diseases associated with impaired lipid metabolism and the gastrointestinal tract, al ergii, decreased immunity, disorders of the nervous system. According to experts, many of these diseases leading to death, disability or disability have a steady upward trend. Up to 44% of the population of our country suffer from cardiovascular diseases (as of December 2019), while more than 82% have functional disorders leading to them. Malignant neoplasms and precancerous conditions are observed in 39% of the adult population, diseases of the gastrointestinal tract are found in more than 48% of the working population of the country. The solution to these problems may be the development of functional food products.

This analysis is based on the study of materials from scientific electronic libraries (elibrary.ru, PubMed, scientific library of the Petrovsky National Research Centre of Surgery, Moscow, Russia). The problem of treating the pathology of the upper gastrointestinal tract is caused by the high prevalence of benign and malignant diseases of this zone, their severity, the complexity of diagnosis and often the low efficiency of conservative therapy. Intensive implementation of innovative scientific technologies in medical practice, aimed at increasing the effectiveness of treatment and diagnostic measures, minimizing their negative impact on the patient’s body and reducing the risk of complications, which, at the same time, have sufficient economic attractiveness. In full, all of the above can be attributed to endoscopy, which has been actively developing in recent years, both in the diagnostic and in the operational areas. In the treatment of various diseases of the gastrointestinal tract in recent years, combined methods based on the use of two or more minimally invasive technologies, for example, endoluminal endoscopic and thoraco or laparoscopic methods, have entered the world practice. The data of scientific articles on the problem of surgical treatment of patients with benign and malignant diseases of the upper gastrointestinal tract, carried out using intraoperative intraluminal endoscopic assistant, are analyzed. The use of intraluminal endoscopy, in particular, allows to increase the radicality of surgical interventions in patients with malignant neoplasms of the upper gastrointestinal tract, perform intraoperative topical diagnosis of non-palpable tumors, and use it in the surgical treatment of esophageal diverticulums of various localization. Also, another area of application of intraoperative intraluminal endoscopy is the assessment of tightness and anastomosis zone during surgical interventions on the organs of the gastrointestinal tract. However, this technique is in the process of implementation in clinical practice and has not been sufficiently studied both in foreign and domestic literature.
