Clinical Analysis of Qinglongdingchuan Decoction on Acute Exacerbation of Chronic Obstructive Pulmonary Disease

Objective To explore the clinical effect of QinglongDingchuan Decoction on patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD) and analyze its mechanism. Methods From June 2017 to June 2019, patients with AECOPD were randomly divided into the observation group and the control group according to the time sequence of treatment, 40 cases in each group were observed. Control group: conventional western medicine treatment (controlled oxygen therapy, antibiotics, expectorants, bronchodilators, etc.), observation group: QinglongDingchuan decoction. 6 days is a course of observation. The routine pulmonary function, blood gas analysis, serum inflammatory factors, clinical symptoms and signs were observed. Results The observation group was superior to the control group in clinical control rate and effective rate (P < 0.05), suggesting that the treatment group was superior to the control group in overall effective rate. There were differences in symptoms, physical signs and total symptom scores between the two groups before and after treatment. The improvement of serum inflammatory factors in the treatment group was better than that in the control group. The improvement of PaC02 and Pa02 in the treatment group was better than that in the control group. Conclusion QinglongDingchuan decoction can better improve the clinical symptoms, physical signs, blood gas analysis indicators, improve the overall efficiency, and has a better effect on patients with fever AECOPD.

Objective. This study was to compare the effects of symptomatic treatment and high flow nasal cannula (HFNC) treatment on patients with advanced lung cancer complicated with chronic obstructive pulmonary disease (COPD) and to explore the clinical application effect of HFNC treatment in such patients. Methods. 80 patients with advanced lung cancer and COPD admitted to the hospital were selected as the research objects. They were randomly divided into a control group (n = 40) and an observation group (n = 40). The computed tomography (CT) image data of all patients were classified. The neural network was trained to obtain the network weights. Based on surgery, radiotherapy, and chemotherapy, patients in the control group received anti-inflammatory, phlegm, and other symptomatic treatments, while patients in the observation group received HFNC treatment on this basis. The blood gas analysis results, clinical symptoms (cough, wheezing, rales, etc.), inflammatory factors (high-sensitivity C-reactive protein (hs-CRP), plateletcrit (PCT), tumor necrosis factor-α (TNF-α), and interleukin-6 (IL-6)), and quality of life of the two groups were compared and analyzed. Results. When the model MSE was the smallest, the corresponding hidden layer neuron node value was 49, so 49 was set as the optimal number of hidden layer neuron nodes. CT images were imported into the constructed model system, and the model diagnosis system could still diagnose and classify under the premise that the pathological characteristics were not obvious. There was no significant difference in clinical data between the two groups of patients before treatment ( P > 0.05 ). After treatment, the clinical symptoms, arterial partial pressure of carbon dioxide (PaCO2), arterial partial pressure of oxygen (PaO2), hs-CRP, PCT, TNF-α, and IL-6 levels were greatly reduced; those of patients in the observation group were much better in contrast to those of the control group ( P < 0.05 ). The total effective rate in the observation group was 97.5%, and the effective rate in the control group was 87.5%. After treatment, the functional assessment of cancer therapy-lung (FACT-L) score was obviously higher than that before treatment ( P < 0.05 ). After treatment, the quality of life in the observation group was increased by 45.69% compared with that before treatment, and the quality of life in the control group was increased by 35.77%. Conclusion. HFNC therapy can improve the lung function of patients with advanced lung cancer and COPD, alleviate the development of the disease, and improve the quality of life of patients.

To explore the clinical value of octreotide therapy and nursing intervention in patients with acute pancreatitis. 120 patients with acute pancreatitis admitted to the Department of Digestive Medicine from June 2018 to August 2019 were selected. Two groups of patients were treated with octreotide. They were randomly divided into observation group (intensive nursing intervention) and control group (routine nursing) with 60 patients in each group. Compared with the control group (81.67%), the total effective rate (93.33%) was higher in the observation group (P<0.05%); After treatment, C reactive protein, lactate dehydrogenase, WBC and hemodiastase of patients in the observation group were better than those in the control group (P<0.05); compared with the control group (18.33%), the occurrence rate of hypoglycemia in the observation group (6.67%) was lower (P<0.05%); At the same time, the Nursing Satisfaction Scale of the observation group (95.00%) was higher than that of the control group (78.33%), (P<0.05%). Patients with acute pancreatitis treated with octreotide and strengthened nursing intervention can obtain more obvious therapeutic effect, so that the clinical symptoms and related laboratory indicators of patients have been significantly improved. At the same time, the clinical value was significant with a lower occurrence rate of hypoglycemia and higher nursing satisfaction scale.

Introduction: Chronic obstructive pulmonary disease (COPD) is the leading cause of morbidity and mortality worldwide. Cigarette smoke and noxious agent result in oxidative stress and activate release of inflammatory mediators such as Interleukin-8 (IL-8). Quercetin is a flavonoid compound containing anti-inflammatory effects which can be used as an adjuvant therapy in stable COPD. Objective: To analyze the effect of quercetin on serum IL-8 levels, % VEP1, and CAT score of stable COPD patients. Methods: Experimental clinical trial with pre-test and pasca-test design was performed in 30 patients with stable COPD in Dr. Moewardi Surakarta between December 2017 and January 2018. The samples taken by using purposive sampling were divided into two groups treatment groups received standard therapy and quercetin 500mg/day for 28 days and control groups only received standard therapy. The decrease in inflammation was measured by serum IL-8 examination, improvement of obstruction measured by %FEV1 and clinical improvement measured by CAT score. Results: IL-8 serum level was significantly lower in treatment group than of in control group (p=0,001). The percentage of FEV1 was insignificant different between the two group (p=0,236). However CAT score was significantly lower in treatment group compared to that of in control group (p=0,001) Conclusions: Quercetin can decrease IL-8 serum level and decrease CAT score when given in combination with standard therapy for COPD patients. Background: Amplification of inflammation in acute exacerbation of chronic obstructive pulmonary disease (COPD) increases inflammatory mediators and oxidative stress in the airways, pulmonary and systemic circulation that are characterized by increased plasma level of IL-6 and MDA, resulting in worsening of clinical symptoms. Xanthones in mangosteen pericarp have anti-inflammatory and antioxidant effects, potentially as an adjuntive therapy in acute exacerbations of COPD. Methods: The aim of this study was to determine the effect of mangosteen pericarp extract to clinical improvements, plasma level of IL-6 and MDA of acute exacerbation COPD patients. A clinical trial of experimental with pretest and posttest was conducted on 34 acute exacerbation of COPD patients in Dr. Moewardi Hospital Surakarta and Dr. Ario Wirawan Lung Hospital Salatiga from April until May 2016. The sample was taken by consecutive sampling. Subjects were divided by randomized double blind technique into the treatment group (n=17) received mangosteen pericarp extract 2x1100mg/day and control group (n = 17) received placebo. Clinical improvements were measured in CAT score and length of stay. CAT score, plasma level of IL-6 and MDA were measured on admission and at discharge. Length of stay based on the number of days of care in hospitals. Results: There was significant difference (p=0,011) towards decreased of IL-6 plasma level between treatment group (-2,17 ± 3,46 pg/ mL) and control group (+1,67 ± 6,81 pg/mL). There were no significant difference towards decreased of length of stay (p=0,34) between treatment group (4,12 ± 1,54 days) and control group (5,24 ± 2,49 days), towards decreased of CAT score (p=0,252) between treatment group (-19,18 ± 3,96) and control group (-18,24 ± 2,75), and towards decreased of MDA plasma level (p=0,986) between treatment group (+0,03 ± 0,36μmol/L) and control group (+0,35 ± 1,58). Conclusions: The addition of mangosteen pericarp extract 2x1100mg/day during hospitalization was significantly lowered plasma levels of IL-6, but were not significant in lowering the CAT score, shortening the length of stay, and reducing the increase in plasma level of MDA.

Objective: To discuss the specific application of Zhuang Medicine Crystal Sugar Ye’An Decoction in senile skin pruritus and its effects on serum IgE detection level. Methods: 90 patients with senile skin pruritus admitted to our hospital from February 2019 to April 2020 were randomly grouped. The general treatment effect, itching, secondary skin lesion severity, serum IL-6, IgE level and adverse reactions between 45 patients treated with Chlorcyclizine Hydrochloride Tablets (The control group) and 45 patients treated with Crystal Sugar Ye’An Decoction were compared. Results: The total effective rate of the treatment in the observation group was 93.33%, much higher than that in the control group, 75.56%. The difference was verified and statistically significant (P<0.05). Before the treatment, the itching area, frequency, duration and severity of secondary skin lesions were at the same level (P>0.05). After the treatment, the above data were all reduced, and the reduction of the observation group was more significant than that of the control group, indicating the difference was statistically significant (P<0.05). Before the treatment, there was no difference in serum IL-6 and IgE test data between the two groups (P>0.05). After the treatment, the tendency all declined, and that of the observation group was more obvious, indicating the difference was statistically significant (P<0.05). During the experiment, no obvious adverse reactions were observed in both groups. Conclusion: In the treatment of patients with senile pruritus, the application of Zhuang Medicine Crystal Sugar Ye’An Decoction is not only safe and reliable, but can promote the improvement of patients’ symptoms, adjust the level of inflammatory factors, and enhance clinical efficacy.

<p class="16"><em>Background and Objective: Pneumatic nebulizers (PN) are commonly used to treat COPD. We aimed to evaluate and compare two PNs with respect to effectiveness and delivery, and the effect of using a membrane filter to prevent drug wastage.</em><em></em></p><p><em>Methods: COPD patients (240) were divided into an experimental group (EG); treated with a YZB/GEM1058-2009 medical atomizer, and a control group (CG); treated with a DNA100 medical atomizer. A polypropylene membrane was then applied to each nebulizer.</em><em></em></p><p class="16"><em>Results: Wastage per inhalation was 79.56% in the EG, and 87% in the CG and reduced to 35.3% in the EG and 42.1% in the CG following application of a polypropylene membrane. </em><em>The  ratio of drug not atomised </em><em>per inhalation was 10.32% in the EG, and 24.52% in the CG and altered to 30.2% and 37.3% with the polypropylene membrane. </em><em> </em><em>The total effective rate, cure rate and weekly efficiency were 96.7%, 73.3% and 93.3%, respectively in the EG, and 93.3%, 60% and 83.3% respectively in the CG, and increased to 100%, 93.3% and 100%, respectively in the EG, and 100%, 83.3% and 93.3%, respectively, in the CG with the membrane application. The number of days required to be effective was 4±2.4 days in the EG, and 7±3.8 in the CG and decreased to 3±1.4 in the EG, and 5±3.1 in the CG with the membrane. </em><em></em></p><p class="16"><em>Conclusions</em><em>: There was a high percentage of drug wastage with both PNs. YZB/GEM1058-2009 was significantly more effective than DNA100 but a </em><em>polypropylene </em><em>microporous membrane significantly improved the effectiveness of both.</em><em></em></p><p class="16"><em>Keywords: Chronic Obstructive Pulmonary Disease; Pneumatic Nebulizer; </em><em>Drug waste; Polypropylene microporous membrane</em><em></em></p>

Objective: To investigate the impact of self-management intervention on the effectiveness of emotional regulation in patients suffering from chronic obstructive pulmonary disease (COPD). Materials and Methods: This study was conducted in the Pulmonology Department of Jinnah hospital Lahore in Collaboration with community medicine department for Six months duration from March 2018 to August 2018. 86 consecutive chronic COPD patients were diagnosed in the stable phase. 43 patients in each group were divided into a control group and a randomized observation group. In the control group, the continuous feeding mode outside of a conventional hospital and an enhanced guide to self-care programs in the observation group (information about the disease, breathing exercises, emotions, home oxygen therapy, medication technique, healthy life behaviors and deteriorating action plans) were used to compare differences in results. Results: For six-month follow-up visits, personal hygiene behavior, physical education, cognitive symptoms and management of medical care results, and observation groups in both groups were more visible and the differences were statistically significant (p <0.05); Self-regulation of emotions regulation in two groups was increased, including positive effect, deterrence / anguish, anger / irritation and total results. In addition, the extent of growth in the observation group was clearer and the differences were statistically significant (p <0.05). Conclusion: intervention of the self-monitoring program can improve the self-control behavior of patients with COPD and is important for improving the effectiveness of regulation and prediction of emotions.

Objective: This study aims to investigate the effect of optimized ultrafiltration on perioperative electrolytes, acid–base balance, and pulmonary function during cardiopulmonary bypass (CPB) in infants with low body weight (under 10 kg), using traditional balanced ultrafiltration and modified ultrafiltration.Methods: A total of 30 children who underwent surgical correction for congenital heart disease in Changzhou Children's Hospital between January 2018 and December 2019 were randomly divided into two groups. In the treatment group, ultrafiltration pre-treatment was carried out with blood-containing priming fluid prior to CPB. Balanced ultrafiltration was performed during the operation, and optimized and modified ultrafiltration were conducted before closing and extubation. In the control group, traditional balanced ultrafiltration was used during the operation, and a modified ultrafiltration combination was used before closing and extubation. Indexes such as blood gas analysis and electrolytes were measured perioperatively, and pulmonary function was observed.Results: No deaths were reported in either group. The ventilator-assisted breathing time was shorter in the treatment group than in the control group (P &lt; 0.05). The indexes of the treatment group were closer than those of the control group to the optimal physiological values. The concentrations of potassium ion (K+), lactate (Lac), and blood glucose (Glu) decreased, and there was significant difference between the two groups (P &lt; 0.05) at the end of CPB. Hemoglobin (Hb) and hematocrit (HCT) in the treatment group increased (P &lt; 0.01). Alveolar-arterial differences for oxygen (A-aDO2) and respiratory index (RI) increased significantly in both groups after operation. Children in the treatment group began to recover lung function earlier than children in the control group. Both A-aDO2 and RI were lower in the treatment group than in the control group at each time point after operation (P &lt; 0.05).Conclusion: Optimizing and modifying the traditional ultrafiltration combination method can effectively shorten the ultrafiltration time, reduce the adverse impacts of the ultrafiltration technique, and improve the lung function of infants after operation.

Objective. This study aimed to explore the clinical efficacy and relevant mechanism of Tripterygium glycosides combined with low molecular weight heparin calcium (LMWH) in the treatment of Henoch–Schönlein purpura nephritis (HSPN) in children. Methods. 64 cases of children patients with HSPN treated at Qilu Hospital (Qingdao) from January 2015 to May 2020 were selected and randomly divided into the control group and the observation group and 32 cases in each group. Conventional medical treatment was applied in the two groups, besides which the control group was given LMWH while the observation group was given Tripterygium glycosides based on the control group. The clinical efficacy and the indexes of clinical symptoms of the two groups were compared. Immune globulin level, fibrinogen content (FIB), prothrombin time (PT), platelet level (PLT), and activated partial thromboplastin time (APTT) level of the two groups were compared before and after the treatment. Results. The total effective rate in the observation group was significantly higher than that of the control group, and the recurrence rate in the observation group was lower than that in the control group. After treatment, urine red blood cell count and 24 h urine protein were obviously better than those of the control group. There was no statistically significant difference in PT between the two groups of children before and after treatment. The levels of PLT and FIB in the two groups of patients after treatment were significantly lower than before treatment, and the PLT levels in the observation group were lower than those in the control group. Conclusion. The combination of Tripterygium glycosides and LMWH had good clinical effects in the treatment of children with HSPN, and it could improve the clinical symptoms, the mechanism of which might be related to the increase of PT, a decrease of PLT, and the improvement of coagulation function.
