Background: Health care has emerged as an industry with potential source of stress in the workplace environment. Burnout is characterized by emotional exhaustion, depersonalization, and reduced personal accomplishment. The objectives of the study were to study proportion and associated factors of burnourt among health professionals in medical college.Methods: Data was collected using a questionnaire by using Copenhagen burnout inventory. From a sample frame from of 856, proportionate sampling was to get 187. Data was expressed in proportions and Chi square test was used as test of significance.Results: There are 187 subjects participating in the study which comprised of 52 (27.8%) males and 135 (72.2%) females. Most of the participants who are stressed, falls in the age group less than thirty five years age, i.e. 59.9%. Client related stress was seen in subjects who were staying alone without family. Those who worked more than six hours per day had more personal burn out 17% and the result were significant. 60.4% had job related stress due to night shifts. Those subjects whose income was less than forty thousand rupees had high job related burn out 51.3%. Work related burn out was increased in subjects who had no exercise 47% at all and the result was very highly significant.Conclusions: Professional development programmes should be incorporated to improve the fit between the organisation and the professionals.

BACKGROUND: Psychosocial risks and work-related stress are one of the most complex, most significant and most challenging problems that organizations around the world are facing. Stress negatively affects all participants in the organization and significantly contributes to negative health consequences and economic outcomes. OBJECTIVE: With timely identification and adequate assessment of psychosocial risks, it is possible to create healthy workplaces and healthy organizations where employees are satisfied and motivated. The paper will present the results of the research of psychosocial risks on a large sample of respondents in the Republic of Serbia. METHODS: A questionnaire was distributed to 1,212 participants of which 1,140 answered all the questions asked. Data from the questionnaire were analyzed using multiple regression analysis, Kolmogorov-Smirnov test, Spearman’s correlation coefficient and Pearson’s chi-square test (χ2). RESULTS: The results of multiple regression analysis showed that all psychosocial risks taken together are statistically significantly (p <  .001) and explain 24%of the variance of the criteria. CONCLUSIONS: Psychosocial risks identified as statistically significant predictors are Labor Demands, Interpersonal Relations, Lack of Support, Work Role and Job Uncertainty. All predictors are statistically significant predictors at the level.01.

Introduction: Nursing is considered as one of the most stressful and demanding profession. In the health care organizations, work stress may contribute to absenteeism and turnover, both of which detract from the quality of care. The aim of this study was to assess the job-related stress and to identify and compare the factors associated with job stress among nurses’ working in selected hospitals of Pokhara, Nepal. Methods: The descriptive research design based on simple random sampling method was used. The sample size was 238. The data was collected using self-administered questionnaire consisted of socio-demographic information and Expanded Nursing Stress Scale. Both descriptive and inferential statistics was used for data analysis. Results: The study findings showed that majority of nurses were in moderate level of stress and ‘patients and their families' were the most stressful factors perceived by both government and private hospital nurses. ‘Problems with peers’ was the least stressful factor perceived by both group of nurses and experiencing ‘discrimination’ was also the least stressful factors for private hospital nurses. Furthermore, a chi-square test revealed significant association between work setting and job stress in government hospital nurses. Conclusions: Nurses working in the both government and private hospitals were working under stressful conditions.

Budaya keselamatan merupakan kunci untuk mendukung tercapainya peningkatan keselamatan dan kesehatan kerja dalam organisasi. Upaya membangun budaya keselamatan merupakan langkah pertama dalam mencapai keselamatan pasien. Terdapat beberapa faktor yang berkontribusi dalam perkembangan budaya keselamatan yaitu; sikap baik individu maupun organisasi, kepemimpinan, kerja tim, komunikasi dan beban kerja. Penelitian ini bertujuan mengetahui faktor determinan yang berhubungan dengan terciptanya budaya keselamatan pasien di RS Pemerintah Kabupaten Kuningan. Teknik pengambilan sampel yang digunakan incidental sampling 88 orang perawat pelaksana. Rancangan penelitian menggunakan survey analitik dengan pendekatan cross sectional, uji hipotesis digunakan Chi Square dan regresi logistik ganda. Hasil penelitian menunjukan terdapat pengaruh yang signifikan antara persepsi terhadap manajemen (p 0.0005, odd rasio 21.3), dukungan tim kerja (p 0.0005, odd rasio 13.34), stress kerja (p 0.006, odd rasio 3.94), kepuasan kerja (nilai p 0. 002) dengan budaya keselamatan pasien. Tidak terdapat pengaruh yang signifikan kondisi kerja dengan budaya keselamatan pasien dengan nilai p 0.507. Berdasarkan analisis multuvariat diperoleh persepsi terhadap manajemen menjadi factor determinan dengan nilai p 0.000 < α 0.05. Simpulan; unsur pimpinan memiliki pengaruh yang signifikan dalam menciptakan budaya keselamatan pasien. Pimpinan memiliki kewenangan dalam menerapkan system yang berlaku dalam organisasi, oleh karena itu gaya kepemimpinan, teknik komunikasi serta kemampuan manajerial merupakan suatu hal yang sangat perlu diperhatikan dalam menciptakan atmosfer kerja yang kondusif sebagai upaya terciptanya budaya keselamatan pasien. Berdasarkan hasil penelitian bahwa model kepemimpinan transformasional merupakan model yang sesuai diterapkan untuk meningkatkan budaya keselamatan pasien, pelatihan keterampilan komunikasi efektif serta pengembangan model pendidikan antar profesi sebagai upaya peningkatan kemampuan kolaborasi.Kata kunci:Budaya keselamatan pasien, stress kerja, kepuasan kerja.Determinant factors that are Influencing Patient Safety Culture in a Government-owned Hospitals in Kuningan Regency AbstractSafety culture is a key to support the achievement of occupational health and safety in an organization. An effort to build safety culture is the first step in ensuring patient safety. There are some factors that contribute in the development of safety culture, namely, individual and organizational attitude, leadership, team work, communication, and work load. This study aimed to identify the determinant factors that are related to achievement of patient safety culture in a government-owned hospital in Kuningan Regency. Eighty eight samples of nurses were recruited using incidental sampling technique. The research design was using cross sectional study, the hypothesis testing were using Chi Square and multiple logistic regression. The results showed that there were significant influenced between perception towards management (p= 0.0005, odd rasio 21.3), team work support (p= 0.0005, odd rasio 13.34), work-related stress (p= 0.006, odd rasio 3.94), work satisfaction (p= 0. 002) with patient safety culture. There was not significant influenced between work condition and patient safety (p= 0.507). The multivariate analysis showed that perception towards management was the determinant factor for patient safety culture (p 0.000 < α 0.05). In conclusion, leaders have significant influence in creating patient safety culture. Leaders have authority to implement systems in the organization. Therefore, leadership style, communication technique, and managerial ability are important in order to create a conducive atmosphere for developing patient safety culture. As recommendation, transformational leadership is a model that is appropriate to be applied in order to increase patient safety culture, trainings of effective communication and inter-professional education model are also needed to increase the collaboration skills among health professionals.Keywords:Patient safety culture, work-related stress, work satisfaction.

Owing to the nature of their jobs, nurses all over the world experience burnout. The aim of this descriptive and correlational study was to describe the job characteristics, job satisfaction and burnout levels of home care nurses, and to predict what factors contributed to their job satisfaction and burnout levels. The study population consisted of 80 nurses working in home care units. Of them, 71 participated in the study. A socio-demographic questionnaire, the Minnesota Satisfaction Scale and the Maslach Burnout Inventory were used. Of the participants, 85.9 per cent were female, 56.4 per cent had a bachelor’s degree, and 46.5 per cent were employed in the public sector, 36.6 per cent in municipalities and 16.9 per cent in the private sector. The results revealed that their burnout levels for emotional exhaustion and personal accomplishment were high, and moderate for depersonalisation. Perceived work-related stress was more associated with emotional exhaustion and depersonalisation than with work satisfaction. Home healthcare nurses were suffering from high levels of burnout. Interventions are needed to improve job satisfaction, to reduce the burden of burnout among nurses, and to prevent them from leaving their jobs and retiring earlier.

Background: Family medicine is a field in which complete and detailed set of healthcare services are provided to the patients and their families. In developed countries, freshly graduated students choose family medicine as a priority for their career. However, in Pakistan it is not the case. The present study is undertaken to determine the perceptions of final year medical students’ about Family Medicine as a viable career. Methods: A total number of 504 students participated in the study. This was a cross-sectional study. The study participants were in their final year of medical college. Data was collected using a preapproved questionnaire. Data was entered and analyzed via SPSS version 17 and Chi-Square test was used post-stratification. Results: Only 14.3% (n=72) medical students had heard about Family Medicine. Only 18% (n=92) would select family medicine as a profession. The most frequent rationale for choosing the field of Family Medicine was the variety of patients seen in general practice (55.4% n=51). Conclusion: There is a dire need to focus on increasing awareness about the field of family medicine among medical students. The students should be counseled on the advantages along with the disadvantages of choosing this field as a medical profession.

Abstract Background Patient awareness surveys on polypharmacy have been reported previously, but no previous study has examined the effects of sending feedback to health professionals on reducing medication use. Our study aimed to conduct a patient survey to examine factors contributing to polypharmacy, feedback the results to health professionals, and analyze the resulting changes in the number of polypharmacy patients and prescribed medications. Methods After conducting a questionnaire survey of patients in Study 1, we provided its results to the healthcare professionals, and then surveyed the number of polypharmacy patients and oral medications using a before-after comparative study design in Study 2. In Study 1, we examined polypharmacy and its contributing factors by performing logistic regression analysis. In Study 2, we performed a t-test and a chi-square test. Results In the questionnaire survey, significant differences were found in the following 3 items: age (odds ratio (OR) = 3.14; 95% confidence interval (CI) = 2.01–4.91), number of medical institutions (OR = 2.34; 95%CI = 1.50–3.64), and patients’ difficulty with asking their doctors to deprescribe their medications (OR = 2.21; 95%CI = 1.25–3.90). After the feedback, the number of polypharmacy patients decreased from 175 to 159 individuals and the mean number of prescribed medications per patient decreased from 8.2 to 7.7 (p < 0.001, respectively). Conclusions Providing feedback to health professionals on polypharmacy survey results may lead to a decrease in the number of polypharmacy patients. Factors contributing to polypharmacy included age (75 years or older), the number of medical institutions (2 or more institutions), and patients’ difficulty with asking their physicians to deprescribe their medications. Feedback to health professionals reduced the percentage of polypharmacy patients and the number of prescribed medications. Trial registration UMIN. Background: Pressure ulcer (PU) development remains a significant complication among at-risk patients. It is considered “never events” because they are preventable and should “never” happen. It has been associated with increased morbidity, mortality, hospital cost and length of stay in the hospital. Prevention of pressure ulcers is considered a nurse-sensitive indicator hence nurses should have adequate knowledge about current evidence-based prevention and management of pressure ulcer. However, non adherence to these guidelines is frequent and lack of knowledge may act as barriers to using guidelines in clinical practice.Objectives: The objectives of the study were to 1. assess the knowledge of pressure ulcer management among nurses 2. determine the level of knowledge of pressure ulcer management among nurses 3. assess the association of knowledge of nurses and baseline variables.Methods: A descriptive cross sectional study was conducted, using pretested self administered questionnaire to assess the knowledge on pressure ulcer management among purposively selected 100 nurses working in Gandaki Medical College Teaching Hospital in 2015. Descriptive statistics as frequency and percentage were used and inferential statistics (Chi-square test) was done to find out association between knowledge and selected baseline variables.Results: The study revealed that 59% of the respondents had adequate knowledge where as 41% of nurse’s knowledge was found to be inadequate. Significant association was not found between age, working ward, experience, education, training, duration and knowledge of pressure ulcer management among Nurses.Conclusion: This result indicates that more than half of the nurses had the adequate knowledge but there is still need of education and training related pressure ulcer management.Journal of Gandaki Medical CollegeVolume, 09, Number 2, July December  2016, Page: 47-51

Objective: The present study aimed at finding the resistance pattern of Pseudomonas aeruginosa and other Pseudomonas species isolated from various clinical specimens in the laboratory.Methods: A total of 150 isolates of different species of Pseudomonas obtained from various clinical specimens processed at the Microbiology laboratory of Kasturba Medical College, Manipal Academy of Higher Education, were taken for this study. Antibiotic susceptibility testing was performed by Kirby-Bauer disc diffusion method and interpreted according to the CLSI guidelines. Biofilm assay was performed by modified O’Toole and Kolter method. The results were analyzed using SPSS 17.0 and Student’s unpaired t-test, Kruskal–Wallis, Mann–Whitney, ANOVA, and Chi-square test. p<0.05 was considered statistically significant.Results: Increased resistance was observed by P. aeruginosa to cefotaxime, cotrimoxazole, levofloxacin, ofloxacin, and ticarcillin clavulanate. There was also a good correlation between antibiotic resistance to aztreonam, netilmicin, and ceftazidime and biofilm production. Results of the present study, therefore, demonstrated the occurrence of resistance to various antipseudomonal agents among the biofilm-producing P. aeruginosa isolates.Conclusion: The present study may help in assessing the seriousness of drug resistance caused by biofilm formation in P. aeruginosa and devise strategies through antibiotic policies to minimize such problems.

Objective: The objective of this study is to find out association between scar thickness, assessed sonographically, and intraoperative findings (IOF). Study Design: Descriptive study. Place and Duration of Study: Department of Obstetrics and Gynecology, Sri Krishna Medical College and Hospital, Muzaffarpur, Bihar from June 2019 to May 2020. Methodology: A total of 70 pregnant patients were included in this study. Transabdominal ultrasound was done for scarred uteri. Sonographic findings were co-related with introperative findings. All the given data were entered on SPSS version 23. Age was expressed as mean ± SD. Parity, gestational age, and interval between cesarean sections were expressed as frequencies with percentages. Statistical analysis was done by using Chi-square test for categorical data for association between sonographic scar thickness and intraoperative findings. The statistical significance was set at p-value <0.05. Results: The age range of the patients was 20- 36 years with a mean of 27.91 ±3.690 years. Gestational age at the time of cesarean section was between 27-40 weeks of gestation with a mean of 37 ±2.126 weeks. The interval from previous cesarean was 10 months at the minimum, and 6 years at the maximum with a mean of 2.29 ±1.0 months. Mean scar thickness was 2.5 mm. Association between scar thickness (<1-3 mm) and intaoperative findings of dehiscence and rupture showed a p-value of <0.001. Conclusion: Sonographic assessment of a uterine scar has a practical application to determine the thickness of previous scar, and assess its integrity.

Background: Liver is not only involved in maintaining homeostasis but also exhibits significant role in metabolism and detoxification of various drugs and toxins. Aim: To explore the hepato-protective role of N-acetylcysteine against methotrexate induced hepato-toxicity. Study design: Randomized controlled trial. Methodology: This study having mice (n=18) was carried out after ethical review committee’s (ERC) approval at Foundation university medical college in collaboration of National institute of health, Islamabad in 2017. Single intraperitoneal injection (20mg/kg) of methotrexate induced hepato-toxicity. Hepatoprotective effect was assessed by oral administration N-acetylcysteine (50mg/kg) alone with methotrexate. The extent of liver damage and effect of protective agents were determined by measuring serum ALT, AST, ALP after 24 hours of respective treatment. Liver samples were taken for histological analysis. One way ANOVA followed by Post Hoc Tukey test was applied for multiple comparisons of biochemical markers between the groups. Histopathological findings were analyzed by Chi Square test. p < 0.05 was considered significant. Results: Significant (p < 0.05) hepatotoxicity was seen with substantial elevation in serum ALT, AST and ALP with methotrexate. N-acetylcysteine attenuated the methotrexate induced hepatotoxicity significantly (p < 0.05). The histopathological examination showed mild steatosis along with focal pleomorphism in the liver of mice that received methotrexate in comparison to group treated with N-acetylcysteine and methotrexate though minimal inflammation was seen. Conclusion: We concluded that N-acetylcysteine ameliorates the methotrexate induced hepatotoxicity on when used concomitantly. Keywords: Hepatotoxicity, N-acetylcysteine and Methotrexate
