Background and Aim: Evaluation of word recognition score requires multiple lists that must be similar in terms of difficulty level. There is currently no such word lists for the Persian language. The aim of this study was to construct several lists of Persian monosyllabic words with psychometric homogeneity. Methods: The most common monosyllabic words were collected from a book of Persian word frequency. The selected monosyllabic Consonant-Vowel-Consonant (CVC) words were presented randomly to 30 normal hearing participants with the age range of 18 to 25 years. The presentation level was from 0 to 40 dB in 8 dB increments. The characteristics of psychometric function were determined for all words using the logistic regression. Results: The Persian CVC monosyllabic words have different difficulty levels with threshold varying from 2.8 to 37.2 dB HL and the slope from 2.3 to 16.4 %/dB. Conclusion: The final result of the present study is three full lists of monosyllabic words with CVC syllabic structure that have the same mean threshold and slope of psychometric function. Background: So that portions of the classic Miller, Heise, and Lichten (1951) study could be replicated, new recorded versions of the words and digits were made because none of the three common monosyllabic word lists (PAL PB-50, CID W-22, and NU–6) contained the 9 monosyllabic digits (1–10, excluding 7) that were used by Miller et al. It is well established that different psychometric characteristics have been observed for different lists and even for the same materials spoken by different speakers. The decision was made to record four lists of each of the three monosyllabic word sets, the monosyllabic digits not included in the three sets of word lists, and the CID W-1 spondaic words. A professional female speaker with a General American dialect recorded the materials during four recording sessions within a 2-week interval. The recording order of the 582 words was random. Purpose: To determine—on listeners with normal hearing—the psychometric properties of the five speech materials presented in speech-spectrum noise. Research Design: A quasi-experimental, repeated-measures design was used. Study Sample: Twenty-four young adult listeners (M = 23 years) with normal pure-tone thresholds (≤20-dB HL at 250 to 8000 Hz) participated. The participants were university students who were unfamiliar with the test materials. Data Collection and Analysis: The 582 words were presented at four signal-to-noise ratios (SNRs; −7-, −2-, 3-, and 8-dB) in speech-spectrum noise fixed at 72-dB SPL. Although the main metric of interest was the 50% point on the function for each word established with the Spearman-Kärber equation (Finney, 1952), the percentage correct on each word at each SNR was evaluated. The psychometric characteristics of the PB-50, CID W-22, and NU–6 monosyllabic word lists were compared with one another, with the CID W-1 spondaic words, and with the 9 monosyllabic digits. Results: Recognition performance on the four lists within each of the three monosyllabic word materials were equivalent, ±0.4 dB. Likewise, word-recognition performance on the PB-50, W-22, and NU–6 word lists were equivalent, ±0.2 dB. The mean recognition performance at the 50% point with the 36 W-1 spondaic words was ˜6.2 dB lower than the 50% point with the monosyllabic words. Recognition performance on the monosyllabic digits was 1–2 dB better than mean performance on the monosyllabic words. Conclusions: Word-recognition performances on the three sets of materials (PB-50, CID W-22, and NU–6) were equivalent, as were the performances on the four lists that make up each of the three materials. Phonetic/phonemic balance does not appear to be an important consideration in the compilation of word-recognition lists used to evaluate the ability of listeners to understand speech.A companion paper examines the acoustic, phonetic/phonological, and lexical variables that may predict the relative ease or difficulty for which these monosyllable words were recognized in noise (McArdle and Wilson, this issue).

A chart review study was conducted to determine the prevalence of syphilis and explore the associated risk factors among men who have sex with men (MSM) who attended a large HIV clinic in Trinidad during the period January–December 2019. Patients were routinely screened for syphilis annually, and demographic, clinical, and laboratory data were extracted from the medical records. Descriptive and bivariate analyses were performed, and factors significantly associated with a syphilis diagnosis were assessed using multivariate logistic regression. During the period, 218 MSM were seen, age range 19–67 years, and median age 34.0 years. The prevalence of syphilis was 41.3% (90/218), and 71.1% (64//90) of these infections were asymptomatic. Multivariate analysis using logistic regression showed that MSM living with HIV in the 30–34 years old-age group (OR, 4.32; 95% CI, 1.04–18.02), and those with a previous history of treated syphilis (OR, 10.18; 95% CI, 4.60–22.53) were more likely to be diagnosed with syphilis. The prevalence of syphilis is high among MSM attending the HIV clinic in Trinidad, and most of these infections were asymptomatic; hence, targeted and sustained interventions to reduce syphilis transmission are urgently required. Repeat episodes of syphilis may play a role in the transmission dynamics of syphilis in MSM.

Sektor perladangan kelapa sawit merupakan sektor yang sangat penting dalam industri pertanian dan merupakan penyumbang terpenting kepada ekonomi Malaysia. Namun begitu, isu tenaga buruh di sektor ini sering diperkatakan kerana masih berintensif buruh. Penyertaan golongan belia dalam sektor perladangan kelapa sawit adalah kurang memberangsangkan berbanding sektor lain walaupun sektor ini menawarkan peluang pekerjaan yang lebih luas. Oleh sebab itu, kajian ini bertujuan untuk mengenal pasti golongan belia yang lebih bersedia untuk melibatkan diri dalam sektor perladangan kelapa sawit berdasarkan faktor demografi mereka. Borang soal selidik digunakan bagi mengumpul data daripada responden yang terdiri daripada golongan belia berusia 18 hingga 35 tahun. Seramai 724 orang responden yang telah menjawab soal selidik ini mewakili setiap negeri di Malaysia. Data yang dikumpul dianalisis menggunakan kaedah analisis faktor dan model regresi logistik. Hasil kajian menunjukkan bahawa faktor jantina, bangsa, tempat tinggal, pengalaman, dan penglibatan ahli keluarga dalam sektor perladangan kelapa sawit merupakan faktor yang menyumbang kepada kesediaan belia untuk melibatkan diri dalam sektor tersebut. Faktor gaji, faedah, persekitaran kerja dan keselamatan perlu ditambah baik bagi menarik lagi minat golongan belia untuk melibatkan diri dalam industri pertanian negara khususnya di sektor perladangan kelapa sawit.   Abstract The oil palm plantation sector is a very important sector in the agricultural industry and is a major contributor to the Malaysian economy. However, the issue of labour in this sector is often addressed as it is labour-intensive. The participation of youths in the oil palm plantation sector is less favourable than the other sector although it offers greater employment opportunities. Therefore, this study aimed to identify youths who are better prepared to participate in the oil palm plantation sector based on their demographic factors. The questionnaire was used to collect data from respondents in the age range of 18 to 35 years. A total of 724 respondents answered this questionnaire representing each state in Malaysia. The data collected were analysed using the factor analysis method and logistic regression model. The results show that the gender, race, residence, experience and involvement of family members in the oil palm plantation sector are factors contributing to youths’ readiness to participate in the sector. Factors such as salary, benefits, work environment and safety factors need to be improved to attract more youths to participate in the agricultural industry, especially in the oil palm plantation sector. Abstract Background Speech audiometry materials are widely available in many different languages. However, there are no known standardized materials for the assessment of speech recognition in Arabic-speaking children. Purpose The aim of the study was to develop and validate phonetically balanced and psychometrically equivalent monosyllabic word recognition lists for children through a picture identification task. Research Design A prospective repeated-measure design was used. Monosyllabic words were chosen from children's storybooks and were evaluated for familiarity. The selected words were then divided into four phonetically balanced word lists. The final lists were evaluated for homogeneity and equivalency. Study Sample Ten adults and 32 children with normal hearing sensitivity were recruited. Data Collection and Analyses Lists were presented to adult subjects in 5 dB increment from 0 to 60 dB hearing level. Individual data were then fitted using a sigmoid function from which the 50% threshold, slopes at the 50% points, and slopes at the 20 to 80% points were derived to determine list psychometric properties. Lists were next presented to children in two separate sessions to assess their equivalency, validity, and reliability. Data were subjected to a mixed design analysis of variance. Results No statistically significant difference was found among the word lists. Conclusion This study provided an evidence that the monosyllabic word lists had comparable psychometric characteristics and reliability. This supports that the constructed speech corpus is a valid tool that can be used in assessing speech recognition in Arabic-speaking children.

Acute coronary syndrome (ACS), a life-threatening manifestation of coronary artery disease, ranges from unstable angina (UA) to acute myocardial infarction (AMI). To reduce the morbidity and mortality resulting from acute coronary syndrome, we should have to find out some predictor or prognostic indicator. A prognostic indicator should be available at the time of initial patient’s evaluation, in order to maximize the potential benefits of early risk assessment. This study designed to evaluate plasma BNP as a prognostic tool in patients with acute coronary syndrome. A prognostic cohort study was carried out with 90 (ninety) acute coronary syndrome patients on the basis of their clinical and laboratory criteria with age range of 30 to 90 years in the department of biochemistry, BSMMU, in collaboration with department of cardiology, NICVD, BSMMU and BIRDEM. Plasma BNP concentrations were measured on enrollment and then grouping of the study subjects were done on the basis of their empirical cut off value of plasma BNP concentration. All the patients were free from heart failure, renal disease, thyroid disease and hepatic disorder. Main outcomes were mortality, morbidity and survival after hospital discharge with or without any disability. All the subjects were categorized into two and to see the significance between two groups in relation to age and sex Unpaired –t test and Chi square test were done. Finally, Binary logistic regression was done. Among 90 acute coronary syndromes patients, there were 74(82.2%) male and 16(17.8%) female with mean age of the study population 51.8 years and the age range of 30 t0 90 years. Among enrolled patients, 24 (26.7%) were NSTEMI and 66 (73.3%) were STEMI that includes 29 (32.2%) anterior MI, 21 (23.3%) inferior MI, 16 (17.8%) other varieties of MI. All the study subjects were grouped into two on the basis of empirical cut off value of plasma BNP 640pg/ml on enrollment. Group I with plasma BNP level less than 640pg/ml includes 57 (63.3%) subjects and group II with plasma BNP more than 640pg/ml includes 33 (36.7%) subjects. Among group I (n=57) good recovery, morbidity and mortality found to be in 41(71.9%), 15(26.3%) and 1(1.8%) patients and those in group II (n=33) found in 6(18.2%), 19(57.6%) and 8(24.2%) patients respectively. Keeping the group I in reference category binary logistic regression analysis done, showing odds ratio 11.5 with p- value 0.000. The odds ratio 11.5 indicates that there is 11.5 times higher chance of getting bad outcome in ACS patients having higher plasma BNP concentrations. Bangladesh Med J. 2017 May; 46 (2): 32-37

Abstract Background English materials for speech audiometry are well established. In Spanish, speech-recognition materials are not standardized with monosyllables, bisyllables, and trisyllables used in word-recognition protocols. Purpose This study aimed to establish the psychometric characteristics of common Spanish monosyllabic, bisyllabic, and trisyllabic words for potential use in word-recognition procedures. Research Design Prospective descriptive study. Study Sample Eighteen adult Puerto Ricans (M = 25.6 years) with normal hearing [M = 7.8-dB hearing level (HL) pure-tone average] were recruited for two experiments. Data Collection and Analyses A digital recording of 575 Spanish words was created (139 monosyllables, 359 bisyllables, and 77 trisyllables), incorporating materials from a variety of Spanish word-recognition lists. Experiment 1 (n = 6) used 25 randomly selected words from each of the three syllabic categories to estimate the presentation level ranges needed to obtain recognition performances over the 10 to 90% range. In Experiment 2 (n = 12) the 575 words were presented over five 1-hour sessions using presentation levels from 0- to 30-dB HL in 5-dB steps (monosyllables), 0- to 25-dB HL in 5-dB steps (bisyllables), and −3- to 17-dB HL in 4-dB steps (trisyllables). The presentation order of both the words and the presentation levels were randomized for each listener. The functions for each listener and each word were fit with polynomial equations from which the 50% points and slopes at the 50% point were calculated. Results The mean 50% points and slopes at 50% were 8.9-dB HL, 4.0%/dB (monosyllables), 6.9-dB HL, 5.1%/dB (bisyllables), and 1.4-dB HL, 6.3%/dB (trisyllables). The Kruskal–Wallis test with Mann–Whitney U post-hoc analysis indicated that the mean 50% points and slopes at the 50% points of the individual word functions were significantly different among the syllabic categories. Although significant differences were observed among the syllabic categories, substantial overlap was noted in the individual word functions, indicating that the psychometric characteristics of the words were not dictated exclusively by the syllabic number. Influences associated with word difficulty, word familiarity, singular and plural form words, phonetic stress patterns, and gender word patterns also were evaluated. Conclusion The main finding was the direct relation between the number of syllables in a word and word-recognition performance. In general, words with more syllables were more easily recognized; there were, however, exceptions. The current data from young adults with normal hearing established the psychometric characteristics of the 575 Spanish words on which the formulation of word lists for both threshold and suprathreshold measures of word-recognition abilities in quiet and in noise and other word-recognition protocols can be based.

The Picture Identification Task was developed to estimate the word-recognition performance of nonverbal adults. Four lists of 50 monosyllabic words each were assembled and recorded. Each test word and three rhyming alternatives were illustrated and photographed in a quadrant arrangement. The task of the patient was to point to the picture representing the recorded word that was presented through the earphone. In the first experiment with young adults, no significant differences were found between the Picture Identification Task and the Northwestern University Auditory Test No. 6 materials in an open-set response paradigm. In the second experiment, the Picture Identification Task with the picture-pointing response was compared with the Northwestern University Auditory Test No. 6 in both an open-set and a closed-set response paradigm. The results from this experiment demonstrated significant differences among the three response tasks. The easiest task was a closed-set response to words, the next was a closed-set response to pictures, and the most difficult task was an open-set response. At high stimulus-presentation levels, however, the three tasks produced similar results. Finally, the clinical use of the Picture Identification Task is described along with preliminary results obtained from 30 patients with various communicative impairments.

The purpose of this study was to identify how to build an educational game model with numerical recognition, numerical ordering and simple counting for kindergarten students with an age range of 3-6 years, where the method used is leveling up difficulty automatically based on user experience, where the level of difficulty will increase based on the success of the user completing the game, the more often the user plays the game, the difficulty level will continue to increase, so hopefully with this model education games mobile-based can develop education games model that can help students recognize numbers and improve their ability to sort and count numbers with more easy and fun way.

AbstractTwo previous experiments from our laboratory with 70 interrupted monosyllabic words demonstrated that recognition performance was influenced by the temporal location of the interruption pattern. The interruption pattern (10 interruptions/sec, 50% duty cycle) was always the same and referenced word onset; the only difference between the patterns was the temporal location of the on- and off-segments of the interruption cycle. In the first study, both young and older listeners obtained better recognition performances when the initial on-segment coincided with word onset than when the initial on-segment was delayed by 50 msec. The second experiment with 24 young listeners detailed recognition performance as the interruption pattern was incremented in 10-msec steps through the 0- to 90-msec onset range. Across the onset conditions, 95% of the functions were either flat or U-shaped.To define the effects that interruption pattern locations had on word recognition by older listeners with sensorineural hearing loss as the interruption pattern incremented, re: word onset, from 0 to 90 msec in 10-msec steps.A repeated-measures design with ten interruption patterns (onset conditions) and one uninterruption condition.Twenty-four older males (mean = 69.6 yr) with sensorineural hearing loss participated in two 1-hour sessions. The three-frequency pure-tone average was 24.0 dB HL and word recognition was ≥80% correct.Seventy consonant-vowel nucleus-consonant words formed the corpus of materials with 25 additional words used for practice. For each participant, the 700 interrupted stimuli (70 words by 10 onset conditions), the 70 words uninterrupted, and two practice lists each were randomized and recorded on compact disc in 33 tracks of 25 words each.The data were analyzed at the participant and word levels and compared to the results obtained earlier on 24 young listeners with normal hearing. The mean recognition performance on the 70 words uninterrupted was 91.0% with an overall mean performance on the ten interruption conditions of 63.2% (range: 57.9–69.3%), compared to 80.4% (range: 73.0–87.7%) obtained earlier on the young adults. The best performances were at the extremes of the onset conditions. Standard deviations ranged from 22.1% to 28.1% (24 participants) and from 9.2% to 12.8% (70 words). An arithmetic algorithm categorized the shapes of the psychometric functions across the ten onset conditions. With the older participants in the current study, 40% of the functions were flat, 41.4% were U-shaped, and 18.6% were inverted U-shaped, which compared favorably to the function shapes by the young listeners in the earlier study of 50.0%, 41.4%, and 8.6%, respectively. There were two words on which the older listeners had 40% better performances.Collectively, the data are orderly, but at the individual word or participant level, the data are somewhat volatile, which may reflect auditory processing differences between the participant groups. The diversity of recognition performances by the older listeners on the ten interruption conditions with each of the 70 words supports the notion that the term hearing loss is inclusive of processes well beyond the filtering produced by end-organ sensitivity deficits.
