Aim of the study. To evaluate the effect of professional teeth whitening on the level of oral hygiene. Material and methods. A randomized controlled, prospective clinical trial was conducted in 2 groups of 30 people, in each. In group 1, professional oral hygiene and professional teeth whitening were performed using 25% hydrogen peroxide. In group 2, only professional oral hygiene was performed. Evaluated the dynamics of the hygienic index of Quigley-Hein in the modification of S. Turesky, 2 weeks, 1 month and 6 months after the procedure. Results. 6 months after the start of the study in group 1, the average value of the Quigley-Heine index in the modification of S. Turesky was significantly lower than before the at start of the study (1.24 ± 0.11 and 2.34 ± 0.14 respectively). In group 2, the index value after 6 months exceeded 2.0 and almost returned to the initial level (2.05 ± 0.06 and 2.44 ± 0.08, respectively). During the entire observation period in patients underwent professional teeth whitening, the level of oral hygiene was significantly higher than in the control group. Conclusion. Professional teeth whitening has a positive effect on the level of oral hygiene and is a significant motivating factor in maintaining oral health.

 Background: The aim of this study was to compare the efficacy of infiltration of local anaesthetic bupivacaine at incision site (port site) and intraperitoneally on postoper­ative pain in patients undergoing laparoscopic cholecystectomy.Methods: This was a randomized controlled study carried out on patients undergoing laparoscopic cholecystectomy. The patients were divided into three groups of 60 patients each. Group 1 was the control group which did not receive either intraincisional or intraperitoneal bupivacaine. Group 2 received intraincisional bupivacaine 0.25% 20 ml, while group 3 received 20 ml solution of bupivacaine 0.25% 20 ml intraperitoneally. Postoperative pain was recorded for 24 hours post-operatively.Results: The incidence of abdominal pain was significantly lower in the group which received intraincisional bupivacaine upto 12 hours postoperatively. Right shoulder pain was low in group receiving intraperitoneal bupivacaine as compared to group 2, but it was not statistically significant.Conclusions: Intraincisional (port site) infiltration of bupivacaine is more effective than intraperitoneal infiltration for postoperative pain relief after laparoscopic cholecystectomy. It is easier to apply and there is less requirement of postoperative analgesics.

Purpose: To study the effect of anterior capsule polishing during phacoemulsification on the incidence of post operative YAG laser capsulotomy. Method: A retrospective controlled study of 159 patients who underwent uncomplicated phacoemulsification with anterior capsular polishing between October 1998 and March 2000. 169 age matched patients who underwent phacoemulsification but without anterior capsule polishing served as controls. Main outcome measure: Incidence of visually significant YAG capsulotomy, which improved the Snellen acuity for more than 1 line or at least 1 line with subjective improvements in symptoms. Results: 2.51 % of patients with anterior capsular polishing (Group 1) had YAG capsulotomy compared to 7.1% of patients in control group at one year. However, after two years, 11.3% of patients in Group I had YAG capsulotomy compared to 12.4% in Group 2. Conclusion: Anterior capsular polishing during cataract surgery may delay the opacification of posterior capsule during the early postoperative period. But does not decrease the incidence of YAG capsulotomy after two years.

Aim. To establish the association between the presence of chronic infection in oral cavity and the severity of SARSCoV-2 infection.Materials and methods. The study was conducted among 30 people aged between18 and 22 who had had coronavirus infection from mild to severe cases. The assessment of oral health was carried out with main and additional examination methods, CFE index, PMA index, Greene, Wermillion oral hygiene index.Results. In group 1, the average value of CFE index was 4.2, in the second group – CFE index was twice higher at 7.8. PMA index in patients of group 2 was significantly higher (p> 0.01) and was at the level of 41.5%. In group 1, the PMA index was 13.3%. It was found that 17% of the respondents in the control group and 70% patients in the experimental group had an episodic exacerbation of dental diseases during COVID-19.Conclusions. The data obtained indicates a correlation between oral diseases and the severity of COVID-19. It is necessary to consider that chronic infection in the oral cavity as well as poor oral hygiene can act as a risk of complications of viral infections, in particular, of COVID-19.

Background: Direct laryngoscopy and endotracheal intubation frequently induce cardiovascular stress response characterised by hypertension and tachycardia due to reflex sympathetic stimulation. Considering the clinical significance of these changes stress attenuation is needed to blunt these responses. β- blockers are used to reduce the unwanted hemodynamic responses. Esmolol is ultra-short acting cardio selective β blocker. Labetalol is useful in not only attenuating the response to laryngoscopy and intubation but also in preventing perioperative cardiovascular events. Present study compared the efficacy of esmolol and labetalol for attenuation of sympathomimetic response to laryngoscopy and intubation.Methods: It was a prospective, randomized, controlled study carried out in 75 adult patients with ASA 1 and ASA 2 posted for elective surgeries. Patients were allocated randomly into Group-1 (esmolol) and Group-2 (labetalol) and Group-3 (placebo) of 25 patients each. Inj. esmolol (1mg/kg) or Inj. labetalol (0.4 mg/kg) or placebo (0.9NS) dissolved up to 5 ml in distilled water was injected intravenously 5 minutes prior to intubation. All patients premeditated with Inj. glycopyrrolate (4µg/kg). Patient were then induced with Inj. propofol (2mg/kg), Inj. scoline (2mg/kg) given followed by laryngoscopy and intubation. Haemodynamic readings were noted at T0 (baseline before injecting the drug), T1 (1 minute after injecting the drug), T2 (after intubation), T3 (2 minutes after intubation), T4 (4 minutes after intubation), T5 (6 minutes after intubation), T6 (8 minutes after injection), T7 (10 minutes after injection).Results: Gr-1, Gr-2 and Gr-3, when compared with each other for systolic BP, diastolic BP, mean BP, and HR, showed a statistically significant difference at different intervals.Conclusions: Both the drugs are found to be effective in attenuation of hemodynamic reflex without any side effects. Further studies are recommended to substantiate the findings in present study.

Objective: To evaluate the effect of chronic pancreatitis (CP) and exocrine insufficiency of the pancreas on the carotid intima media thickness (cIMT) in patients with type 2 diabetes mellitus (DM-2). Materials and methods: 91 patients were examined, they were divided into groups: 1st group-patients with DM-2 (n = 31) group 2nd – DM-2 combined with CP (n = 60). Fasting plasma glucose (FPG), HbA1c, immunoreactive insulin (IRI), HOMA-IR index, C-reactive protein (CRP), serum α-amylase and fecal-1 elastase (FE-1) were assessed. In order to assess the thickness of the cIMT, ultrasound of the common carotid artery was performed. The control group of healthy volunteers was representative by age and sex. Results: In the 1st and 2nd groups, the average value of cIMT was (1.02 ± 0.1 vs 1.21 ± 0.15) mm. Parameters of DM-2 management in the 1st and 2nd groups were (FPG 8.18 ± 0.92vs8.57 ± 1.2) mmol / l; HbA1c (7.23 ± 0.21vs7.49 ± 0.36)%; IRI (20.31 ± 0.9vs22.53 ± 1.2) μIU / ml; HOMA-IR (6.55 ± 1.5vs8.38 ± 2.2) μIU / ml * mmol / l; PSA (1.3 ± 0.12vs6.77 ± 0.31). HOMA-IR correlated with: CRP (r = 0.43, p <0.05 in group 1; r = 0.61, p <0.05 in group 2); FE-1 (r = -0.55, p <0.05 of the 2nd group); and cIMT (r = 0.42, p <0.05 in the 1st group; r = 0.53, p <0.05 in the 2nd group). IRI have relationship with: FE-1 (r = 0.41, p <0.05 in group 1; r = -0.3, p <0.05 in group 2); CRP (r = 0.42, p <0.05 in group 1; r = -0.28, p <0.05 in group 2); HbA1c had close relationships with cIMT (r = 0.38, p <0.05 in group 1; r = 0.51, p <0.05 in group 2). The relationship between cIMT and CRP was related in the 2nd group (r = 0.39, p <0.05); with α-amylase (r = 0.2, p <0.01 2nd group). There was no significant relationship between cIMT and FE-1 in the study. Conclusions: Obtained data allows us to conclude that hyperinsulinemia and insulin resistance have a direct atherogenic effect on the walls of blood vessels. It is established that the accession of the inflammatory process leads to increasing development of atherosclerotic lesions of the vessel, at the same time, the presence of exocrine dysfunction of the pancreas does not have a significant direct effect on the cIMT.

Aims: This study aims to compare the efficacy in alleviating pain between intracanal medicaments, namely Nitrofurantoin and Calcium Hydroxide Paste. Study Design:  Randomized Controlled Trial. Place and Duration of Study: Sample: Department of Operative Dentistry, Sir Syed College of Medical Sciences, Pakistan, between January 2021 and May 2021. Methodology: 60 patients were randomly divided into 3 groups, each group having 20 patients as following: Group 1: Nitrofurantoin, Group 2: Calcium Hydroxide and Group 3: Control. Preoperative pain was recorded using a numerical pain scale. After access preparation, chemomechanical preparation was performed with subsequent placement of intracanal medicaments. Pre-operative pain score was initially recorded, followed by post-operative pain at 12, 24, 48, and 72 hours, respectively. Root canal treatment was performed in single rooted teeth with patients suffering from symptomatic irreversible pulpitis Results: Majority of the patients in all 3 groups were initially presented with moderate to severe pre-operative pain. After 72 hours post-operatively, 50% patients in Group 1 reported no pain, while 5% patients in both Group 2 and 3 had no pain. Pain significantly subsided in Group 1 as compared to Group 2 and 3. Regarding age and gender, both had no significant relationship with the pain scores in all of the 3 groups Conclusion: Nitrofurantoin has been proven to be an effective intracanal medicament in alleviating immediate post-operative pain in patients with symptomatic irreversible pulpitis as compared to calcium hydroxide. While the control group with no intracanal medicament showed little reduction in pain scores. So, nitrofurantoin can be used as a substitute to currently available standard intracanal medicaments.

This study was conducted to evaluate and compare the clinical and functional outcome of OA knee patients treated with proximal fibular osteotomy with or without trabeculotomy.This was a randomized controlled study amongst patients of osteoarthritis knee for a period of 1 year on patients with early osteoarthritis of knee. All patients were randomly allocated into two groups; group 1 patients were managed using PFO with trabeculotomy whereas group 2 patients were managed with proximal fibular osteotomy alone. All the patients were followed up post operatively at 15 days and at 2, 6 and 12 months post operatively. At each follow up patients were assessed for presence of Pain using Visual analogue scale. Also functional outcome was assessed using WOMAC.In present study, patients of two groups were comparable in their base line characters (p&#62;0.05). Post operatively, mean VAS score and WOMAC score improved significantly in both the groups, the improvement was significantly higher in group 1 as compared to group 2 at all the follow up (p&#60;0.05).Proximal fibular osteotomy with or without trabeculotomy can be used effectively as a promising tool for management of cases with early osteoarthritis of knee. It is safe, effective, and cost effective surgical technique for pain relief and improving functional outcome. PFO with trabeculotomy yields better results in terms of clinical as well as functional outcome as compared to PFO without trabeculotomy.

Until now, there is no radical method of treating children with cerebral palsy, which allows us to consider scientific research in this direction reasonable and promising.The aim of the research: to study the effect of exercises on the mini-simulator “Kinesioplatform-swing” with biofeedback on the indicators of motor skills in children with spastic cerebral palsy.Materials and methods. We conducted an open, non-randomized, prospective, comparative, controlled study in pairs (each participant in the main group corresponds to a participant in the control group). The study involved 53 patients aged 4–12 years with cerebral palsy: the intervention group (group 1: n = 27 (13 boys, 14 girls)) and the control group (group 2: n = 26 (11 boys, 15 girls)), comparable in terms of gender, age and severity of movement disorders. Characteristics of the medical intervention: group 1 – botulinum toxin type A (BTA, for exercising against the background of relative muscular normotonus) + exercise therapy + exercises on the mini-simulator “Kinesioplatform-swing” with biofeedback (BFB); group 2 – BTA + exercise therapy. Duration of the study: 2019–2020. The difference between groups in terms of motor skills on the day of hospitalization and after completion of the training program (10 sessions each) was determined. Motor skills were assessed according to the GMFM-66/88 (Gross Motor Function Measure) table using the “Scales for measuring global motor functions”.Results. The inclusion of additional exercises on the mini-simulator “Kinesioplatformswing” with biofeedback in the rehabilitation of patients with movement disorders with spastic cerebral palsy (BTA + exercise therapy) in comparison with the control group. However, the question of the long-term effects of such training remains open and requires further study.

Abstract Background Polycystic ovary syndrome (PCOS) is defined as a complex endocrine syndrome, and the mechanisms underlying its various clinical signs and symptoms are still poorly understood. It is critical to precisely diagnose the phenotypes of PCOS in order to provide patients with individualized therapy[1, 2] . However, the criteria by which to diagnose different phenotypes, which are mostly based on symptoms, physical examination, and laboratory evaluation, remain unclear. The aim of this study is to compare the accuracy and precision of metabolomic markers with common clinical characteristics to determine a more effective way to diagnose and treat two subgroups, one based on clinical indexes and another based on metabolomic indexes[3], of PCOS patients. The effects of different the interventions based on the two subgroups will also be observed. Methods This is a prospective, multicenter, analyst-blinded, randomized controlled trial. There will be one healthy control group and two parallel experimental arms in this study: (1) people without PCOS (health control group); (2) PCOS patients diagnosed based on clinical indexes (group 1); (3) PCOS patients diagnosed based on metabolomic indexes (group 2). A total of 276 eligible people will be recruited, including 60 healthy people and 216 PCOS patients, who will be randomly assigned to different diagnosis groups in a 1:1 ratio. Patients in the two different diagnosis groups will be divided into two different subgroups based on their clinical characteristics (group 1 based on clinical indexes, group 2 based on metabolomic indexes); thereafter, they will receive a 6-month different treatment. The primary outcome for experimental groups will be the treatment effect of PCOS. Discussion The purpose of this trial is to determine whether integrated metabolomic indexes are more accurate and effective than clinical characteristics in the diagnosis of the phenotypes of reproductive females with PCOS. This trial will therefore contribute to the provision of a solid foundation for the precise clinical diagnosis of two PCOS subgroups, as well as for future research on individualized PCOS therapy.

Abstract The article describes the results of comparative studies on the influence of new immunostimulants on the course of postnatal recovery of the cows under conditions of intensive milk production technology. Injections of PS-2 (group 1) and Prevention-N-E (group 2) at a dose of 10 ml forty, twenty and ten days before calving, and a injection of PDE at a dose of 20 ml with E-Selenium of 10 ml (group 3) twenty days before calving prevent the occurrence of obstetric and gynecological pathology, improve reproductive qualities, as compared to the control group, where no biologicals were injected. This fact impacts the acceleration of the first heat onset by 13.7-21.1 days, the conception rate by 0.5-0.7 times, the service period by 16-29.9 days. A positive effect of the tested agents on metabolic processes, morphological and biochemical blood parameters, nonspecific immunological reactivity of the organism was also established. All these factors led to an increase in milk productivity for 305 days of lactation by 186-478 kg, at the same time, milk quality indicators improved correspondingly. Thus, biological preparations PS-2 and Prevention-N-E are highly effective means of preventing obstetric and gynecological pathology.
