Indonesia’s work accident rate is one of the highest in Southeast Asia, particularly in the construction sector. Work at height is one of the job that causes the most accidents in construction work. One of the work performed at height is a roof work. The purpose of this study was to conduct a risk assessment of roof work of building The Samator Surabaya by PT. PP. This research was conducted with observational approach. The object of this study is the roof work. Population and samples were taken from labor of roof work and SHE officer. The variable in this study is risk assessment and risk control. Data collected by observation and interviews with labor of roof work and SHE Officer. The results showed that there are 35 causes of hazard on the roof work, with 17 hazard classified as medium risk and 18 hazard classified as high risk. Most potential hazard is fall from height and fall of material. Assessment of risk control are in the range of 50% -90%. Residual risk assessment of hazard shows that there are 21 hazard classified as low risk and 14 hazard classified as medium risk. It is recommended that the company need to think about the compliance mechnanism of using PPE on workers that aligned with company policy and using reward and punishment system.

Health Risk Assessment (HRA) is one part of risk assessment. In the utility area at PT Pertamina Lubricant Production Unit of Gresik use a machines that have impact on health noise and temperature hazards. The aim of this study was to identify health risk assessment in the utility area at PT Pertamina Lubricant Production Unit Gresik. This was an observational descriptive study with cross sectional design. This study uses total population of 9 people consist of 1 supervisors Health Safety Enviroment (HSE), 3 HSE officer, and 5 workers. Primary data obtained from interviews, observations, noise and temperature measurements. Secondary data were got in the documents from the company. The collected data were assessed as likelihood and severity to count the risk. Afterward risk control assessed and residual risk is calculated. The results of this study showed that there were 3 steps of work with 6 potential hazard noise and temperature. The risk assessment has levels of risk medium risk and low risk. Medium risk was found in the boiler monitoring and compressor. Low risk was found in the stage of preparing CNG gas and turning on the machines. Control effectiveness is 75%. Residual risk has a low risk and no risk. It can be concluded that the potential hazard in the utility area was noise. Most of the risk found in this area was categorized as low risk. Risk control was properly implemented. If the recommendation of control properly implemented there will be no residual risk. The suggestions given are monitoring the implementation of controls, fit to work, use of PPE, socialization of the use of PPE, giving reward and punishment to motivate workers to safe behavior at workKeyword: health risk assessment, utility area, PT Pertamina Lubricant Production Unit Gresik

Installation of precast concrete in construction of multi-storey buildings at risk of workplace accidents due to be carried out on the altitude. Installation of precast concrete must also use heavy equipment, because of the weight of the material to be install. Accidents on the installation of precast concrete can be very fatal and cause of death. Risk assessment is an attempt to calculating risk arising from hazards, take into account the adequacy of any existing controls, and deciding whether or not the risk is acceptable. The purpose of this research is to studies and perform  risk assessment level onprecast concrete installation at apartment project in Surabaya with hazard, risk, risk control, and residual risk observation. Primary data were collected through interview and observation. Object of this research is installation proses of precast concrete wall at Puncak Bukit Golf Apartment project by PT. Wijaya Karya Bangunan Gedung  in Surabaya. The data was analyzed descriptively using table and naration. Results showed that hazard identify in precast concrete installation at apartment project in Surabaya include: fall from height, squeezed,  fallen object, struck by precast concrete, crush by precast concrete, and electro shock. Risk assessment has obtained 21 hazards. Risk control with was done: installing safety net for falling objects, safety induction, work instruction, wear PPE, and wear fall protection. The result of risk assessment showed19% risk are high risk, 38% with moderate risk category, and 43% with low risk category. Keywords: risk assessment, precast concrete, installing

Hazard identification, risk assesment and risk control and the application of risk mapping at Hospital of the Animal Prof. Soeparwi Universitas Gadjah Mada Purpose: This research was held to analyze the work accident using HIRARC method and the applied of Risk Mapping at the hospital of the animal of Prof. Soeparwi Gadjah Mada University, Yogyakarta.Method: This research used descriptive qualitative research method. The qualitative method used to describe the condition, assess, and to recognize the work accident analysis. The data used was grabbed from Hazard Identification Risk Assessment and Risk Control (HIRARC) method and questionnaire.Results: Hazard identification used HIRARC method at the hospital of the animal of Prof. Soeparwi Gadjah Mada University was about 198. The work hazard commonly includes physical hazard, chemistry, biologist, ergonomics, and psychosocial. The risk assessment which held denoted that there were 61 low risks, 108 medium risks, and 15 high risks. The recommendations of risk restraint that can be given were technician control, administrative, and the use of personal protective equipment (PPE).Conclusion: HIRARC method had been applied at the hospital of animal of Prof. Soeparwi with the result to that grooming unit was high risk work place, assessed with risk mapping. The risk was from animal contact phisically.

Background: Forklift is one of the most used craning plane in the manufacture and logistics industry. The operational of forklift can cause fatality work accident. PT SIGK uses forklift in the daily production activities, so it is necessary to identify hazard and risk assessment. Purpose: This study was to identify and assess the risk of physical and mechanical hazard at every stage of forklift operation. Methods: The type of this research was descriptive observational research, the object of the study were potential hazard and risk of all forklift operation stages in the AAC production area, consisted of 4 stages which were initial inspections stage, operational preparation stage, operational stage and post operations stage. Results: The result showed that there were 25 hazards, which were 4 hazards at the initial inspections stage, 2 hazards at the operational preparations stage, 14 hazards at the operational stage and 5 hazards at the post operations stage. Initial risk assessment obtained 2 hazards with high risk level, 22 hazards with medium risk level and 1 hazard with low risk level. The most prevalent hazard controls performed by PT SIGK were administrative (SOP, for klift training, safety sign) and PPE (helmet, gloves, mask), the controls got the value of effectiveness between 50% to 75%. Residual risk assessment, obtained 5 hazards with medium risk level and 20 hazards with low risk level. Conclusion: Additional control should be done by the company to reduce the residual risk level of medium category to be low, if the control recommendation is implemented well then the overall level of residual risk with low category will be obtained.

Work accidents that occured in Indonesia, 32% are accidents from the construction sector. The construction  sector is a sector with a work environment where there were so many hazard risks. Cable tray installation is one of the jobs in a construction project. The purpose of this research is to get an overview of the steps in the installation of cable tray and the its risk. The process of identifying work accident risk in this study uses Job Safety Analysis (JSA). Falling from height is one of the workplace accidents in the cable tray installation process. Risk control is needed to prevent work accidents that can occur due to cable tray installation.

The entire activity involving human factors, machinery, chemicals, and through some work processes had the potential hazard that increased the risk of accidents and health problems. Aircraft paint removal job was a high risk of an accident. The purpose of this research was to identify hazards, assess pure risks, identify risk control, and assess risks after risk control were conducted on aircraft paint removal job in  PT. X. Type of this research was descriptive research. Primary data retrieval techniques in the study was observational and interview. The subject of research were 1 PDCA HSE and 4 aircraft paint removal workers. Variable in this study is the hazards, likelihood, severity, risk levels, and risk control. The results of hazard identification obtained 14 risk in Aircraft Paint Removal such as 1 low risk, 2 middle risks and 13 high risks. Risk control by the company showed that there are 13 high risk decrease to 7, 2 middle risks increased to 6 risk, and 1 low risk increased to 3 risks. In conclusion, PTX. has 14 risks, 13 of them included in the high risk category. There was a decrease in risk due to the control carriedd out by the company into 13 risks and 7 of them included in high risk.Keywords: aircraft paint removal, hazard identification, risk assessment, risk control

The Covid-19 pandemic is endemic throughout the world, especially in Indonesia. One of the effects of the covid-19 pandemic was the flight system at the airport which was later shut down. Some of them cause some losses from this incident. To deal with the New Normal situation, therefore OHS risk management is an effort to manage OHS risk to reduce Covid-19 virus transmission. This study is to determine hazard identification, risk assessment, and risk control with the Danger Identification, Risk Assessment and Risk Control (HIRARC) methods at the Airport. The research method used is descriptive research that is the result of data research, in the form of research journals and online media. The object of research is the activity that has the potential danger of co-19 virus transmission at the airport. The results of the study can be seen that the activities at the airport there are 12 main activities, Among them are 3 extreme risk category activities, 5 high risk category, and 4 medium risk category. The proposed COVID-19 countermeasures are limiting flight quotas at the airport, providing hygiene kits according to recommended health protocols and Personal Protective Equipment. Pandemi Covid-19 sedang mewabah di seluruh dunia, terutama di Indonesia. Salah satu dampak dari pandemi covid-19 ini adalah system penerbangan di bandara yang belakangan sempat ditutup. Beberapa diantaranya menyebabkan beberapa kerugian dari kejadian ini. Untuk menghadapi keadaan New Normal, maka dari itu manajemen risiko K3 merupakan salah satu upaya untuk mengelola risiko K3 untuk mengurangi penularan virus Covid-19. Pada penelitian ini adalah untuk mengetahui identifikasi bahaya, penilaian risiko, dan pengendalian risiko dengan metode Hazard Identification, Risk Assessment And Risk Control(HIRARC) di Bandara. Metode penelitian yang digunakan adalah penelitian deskriptif yaitu Hasil riset data, berupa Jurnal-jurnal penelitian dan Media online. Objek penelitian adalah kegiatan yang berpotensi bahaya penularan virus covid-19 di Bandara. Hasil penelitian dapat diketahui bahwa pada kegiatan-kegiatan di bandara terdapat 12 kegiatan utama, Di antaranya 3 kegiatan kategori risiko ekstrem,  5 kegiatan kategori risiko tinggi, dan 4 kegiatan kategori resiko sedang. Upaya penanggulangan COVID-19 yang diusulkan adalah melakukan pembatasan kuota penerbangan di bandara, penyediaan peralatan kebersihan sesuai protokol kesehatan yang dianjurkan dan perlengkapan Alat Pelindung Diri.

Abstrak Setiap tempat kerja selalu mempunyai risiko kemungkinan terjadinya kecelakaan dan penyakit akibat kerja. Pada tahun 2013 telah terjadi 406 kasus kecelakaan kerja di Kabupaten Boyolali, 372 pekerja (91,6%) berhasil sembuh, 25 pekerja (6,1%) sementara tidak mampu bekerja, 4 pekerja (0,9%) mengalami cacat, dan 5 pekerja (1,2%) meninggal dunia. Insiden kecelakaan dan cedera di tempat kerja dapat dikurangi dengan penggunaan Job Hazard Analysis. Penelitian ini dilaksanakan pada tahun 2018. Tujuan dari penelitian ini adalah untuk mengetahui potensi bahaya serta pengendalian yang tepat pada industri logam UD. A&D dengan metode Job Hazard Analysis. Jenis penelitian ini adalah deskriptif kualitatif dengan pendekatan observasional. Hasil penelitian menunjukkan terdapat 46 bahaya dan 82 risiko yang teridentifikasi. Hasil penilaian risiko terdapat 24 (29,6%) risiko rendah, 27 (32,9%) risiko sedang dan risiko tinggi berjumlah 31 (37,8%). Hasil penilaian dan pengendalian risiko dirancang dalam bentuk form Job Hazard Analysis. Kesimpulan penelitian ini adalah potensi bahaya yang ada belum dilakukan pengendalian secara optimal.   Abstract Each workplace always has a risk of possible accidents and occupational diseases. In 2013 there were 406 work accident cases in Boyolali District, 372 workers (91.6%) recovered, 25 workers (6.1%) were unable to work, 4 workers (0.9%) were disabled, and 5 workers (1.2%) died. Incidents of accidents and workplace injuries can be reduced by the use of Job Hazard Analysis. This research was conducted in 2018. The purpose of this research is to know the potential danger and proper control on UD metal industry. A & D with Job Hazard Analysis method. The type of this research is descriptive qualitative with observational approach.The results showed 46 hazards and 82 identified risks. Risk assessment results were 24 (29.6%) low risk, 27 (32.9%) medium and high risk 31 (37.8%). The results of risk assessment and control are designed in the form of Job Hazard Analysis form. The conclusion of this research is the potential hazard that has not been done optimally.

Many banks are looking for a better core banking system to support their business growth with a more efficient and flexible core banking system to improve their sales and services in the competitive market and to fulfill regulatory requirements. The decision of replacing the legacy core banking system is difficult due to the high IT investment cost required for banks because they are also trying to cut costs. But maintaining the legacy system is costly in terms of upgrade. Changing the core banking system is also a difficult process and increases risks. To have a successful Core Banking System implementation, risk assessment is required to be performed prior to starting any activities. The assessment can help project teams to identify the risks and then to mitigate the risks as part of the plan. In this research the Core Banking System replacement risks were assessed based on ISACA Framework for IT Risk. Fourteen risk scenarios related to Core Banking System Replacement were identified. The high and medium rated inherent risks can become medium and low residual risk after assessment by putting the relevant control in place. The result proves that by adding mitigation plan it will help to mitigate the Residual Risk to become low risk. There are still three residual risk which categorized as medium risk and should be further mitigated they are Software Implementation, Project Delivery and Selection/Performance of Third Party Suppliers. It is also found that COBIT 5 has considered some specific process capabilities that can be used to improve the processes to mitigate the medium risks.
