Abstrak Setiap tempat kerja selalu mempunyai risiko kemungkinan terjadinya kecelakaan dan penyakit akibat kerja. Pada tahun 2013 telah terjadi 406 kasus kecelakaan kerja di Kabupaten Boyolali, 372 pekerja (91,6%) berhasil sembuh, 25 pekerja (6,1%) sementara tidak mampu bekerja, 4 pekerja (0,9%) mengalami cacat, dan 5 pekerja (1,2%) meninggal dunia. Insiden kecelakaan dan cedera di tempat kerja dapat dikurangi dengan penggunaan Job Hazard Analysis. Penelitian ini dilaksanakan pada tahun 2018. Tujuan dari penelitian ini adalah untuk mengetahui potensi bahaya serta pengendalian yang tepat pada industri logam UD. A&D dengan metode Job Hazard Analysis. Jenis penelitian ini adalah deskriptif kualitatif dengan pendekatan observasional. Hasil penelitian menunjukkan terdapat 46 bahaya dan 82 risiko yang teridentifikasi. Hasil penilaian risiko terdapat 24 (29,6%) risiko rendah, 27 (32,9%) risiko sedang dan risiko tinggi berjumlah 31 (37,8%). Hasil penilaian dan pengendalian risiko dirancang dalam bentuk form Job Hazard Analysis. Kesimpulan penelitian ini adalah potensi bahaya yang ada belum dilakukan pengendalian secara optimal.   Abstract Each workplace always has a risk of possible accidents and occupational diseases. In 2013 there were 406 work accident cases in Boyolali District, 372 workers (91.6%) recovered, 25 workers (6.1%) were unable to work, 4 workers (0.9%) were disabled, and 5 workers (1.2%) died. Incidents of accidents and workplace injuries can be reduced by the use of Job Hazard Analysis. This research was conducted in 2018. The purpose of this research is to know the potential danger and proper control on UD metal industry. A & D with Job Hazard Analysis method. The type of this research is descriptive qualitative with observational approach.The results showed 46 hazards and 82 identified risks. Risk assessment results were 24 (29.6%) low risk, 27 (32.9%) medium and high risk 31 (37.8%). The results of risk assessment and control are designed in the form of Job Hazard Analysis form. The conclusion of this research is the potential hazard that has not been done optimally.

Severe natural disasters and related secondary disasters are a huge menace to society. Currently, it is difficult to identify risk formation mechanisms and quantitatively evaluate the risks associated with disaster chains; thus, there is a need to further develop relevant risk assessment methods. In this research, we propose an earthquake disaster chain risk evaluation method that couples Bayesian network and Newmark models that are based on natural hazard risk formation theory with the aim of identifying the influence of earthquake disaster chains. This new method effectively considers two risk elements: hazard and vulnerability, and hazard analysis, which includes chain probability analysis and hazard intensity analysis. The chain probability of adjacent disasters was obtained from the Bayesian network model, and the permanent displacement that was applied to represent the potential hazard intensity was calculated by the Newmark model. To validate the method, the Changbai Mountain volcano earthquake–collapse–landslide disaster chain was selected as a case study. The risk assessment results showed that the high-and medium-risk zones were predominantly located within a 10 km radius of Tianchi, and that other regions within the study area were mainly associated with very low-to low-risk values. The verified results of the reported method showed that the area of the receiver operating characteristic (ROC) curve was 0.817, which indicates that the method is very effective for earthquake disaster chain risk recognition and assessment.

Public-private partnership (PPP) projects require comprehensive risk assessment and management, including Urban Rail Transit (URT). A more effective risk management can benefit from an accurate understanding of the two-way influence of PPP project risk factors. This paper uses the content analysis method to filter out, compare, and analyze PPP-related literature; 12 categories of 22 PPP risk factors are extracted and identified, and the possible correlations between these risk factors are judged preliminarily. With the knowledge and advice provided by PPP experts, the initial risk relationships are adjusted and supplemented, which then help to determine a reasonable logical relationship among risk factors. The logical relationship helps analyze the risk factors based on the ISM model analysis method and builds a hierarchical structure relationship of risk factors including 6 levels. Finally, the direct, intermediate, and autonomous factors that lead to problems or failures in PPP projects are analyzed which explains in detail the paths of risk transmission and risk prevention measures of PPP companies operating URT. It lays a foundation for PPP project companies operating URT to recognize, manage, and control risks in a targeted and systematic manner.

<p><em>H</em><em>ealth and safety has been known as one of the requirements in carrying out tasks and a form of human rights factor for workers in carrying out work activities. Lack of awareness and concern for K3 resulted in the existence of work risks, namely the presence of accidents that occur in welding activities carried out on the maintenance of tugboats carried by PT. Margasurya Shipindo like being hit by welding sparks, slipping, touched by hot iron which causes injury to workers. Therefore the purpose of this study is to identify and classify potential hazards, and control risks in work activities at PT. Marga Suryashipindo. Hazard analysis of risk in this study used the Hazard Identification, Risk Assessment, and Risk Control (HIRARC) method. Based on the results of observations and interviews with 3 maitenance workers and 1 maintenance coordinator, there were 22 potential hazards. Based on the risk assessment, the risk level values were not with the HIGH category, 10 risks were MEDIUM category and 12 risk were LOW category. Control of the above risks is to use APD with the standards of welding workers, carry out administrative controls in the form of the application of SOP welding and K3 supervision to evaluate work activities. In addition, Engineering control is also carried out in the form of holding lift aids and doing maintenance on a regular basis, procuring toolsbox, and arranging tools and materials in the welding location.</em><strong><em></em></strong></p>

Introduction: Tasikmalaya Regency is one of the areas in West Java that has a large industrial potential, especially small and handicraft industries. Every workplace, both formal and informal, has various potential hazards that can affect the health of workers or can cause occupational diseases. As bamboo is sharp, the craftsmen must be very careful in treating bamboo as a woven materialbecause the risk of accidents such as cutting off hands, being punctured by bamboo slats, and the risk of other potential mechanical hazards can occur. The purpose of this study is to analyze the occupational health and safety risks of woven craftsmen in the Rajapolah District, Tasikmalaya Regency. Methods: This study was a descriptive study. 5 Bamboo weaving craftsmen were the subjects of this study. Meanwhile, the objects of this study were the work process and workplace environment. Data collection was carried out by observation and interviews. Data analysis was performed using a semi-quantitative risk analysis method according to AS/NZS 4360: 2004 standard. Results: 23 potential hazards are identified from all stages of the bamboo process, and 23 health and safety risks are identified from all potential hazards, with 1 risk at a lowrisk level, 15 risks at a medium risk level, and7 risks at a highrisk level. Conclusion: There are 7 risks at the high-risk level that must be prioritized for control, originating from the potential hazards of ergonomics and chemicals.

Indonesia’s work accident rate is one of the highest in Southeast Asia, particularly in the construction sector. Work at height is one of the job that causes the most accidents in construction work. One of the work performed at height is a roof work. The purpose of this study was to conduct a risk assessment of roof work of building The Samator Surabaya by PT. PP. This research was conducted with observational approach. The object of this study is the roof work. Population and samples were taken from labor of roof work and SHE officer. The variable in this study is risk assessment and risk control. Data collected by observation and interviews with labor of roof work and SHE Officer. The results showed that there are 35 causes of hazard on the roof work, with 17 hazard classified as medium risk and 18 hazard classified as high risk. Most potential hazard is fall from height and fall of material. Assessment of risk control are in the range of 50% -90%. Residual risk assessment of hazard shows that there are 21 hazard classified as low risk and 14 hazard classified as medium risk. It is recommended that the company need to think about the compliance mechnanism of using PPE on workers that aligned with company policy and using reward and punishment system.

As one of the particulate chemicals, dust could occur in most of the production process and can create interference for workers health and safety. As one of the air pollution sources, dust could became a potential hazard which exist in room maintenances. Protection to workers is a must in order to reduce the risk of respiratory tract syndrome that often could be found in this cases. The aim of this study is to conduct a risk assessment to dust exposure in room maintenance, which held by contractors in PT. X (Persero) building in Surabaya. This is an cross sectional study with obsevation approach. The object of this research is the repairing  works. The results indicate that the activities which could produce dust, such as: walls sanding using sandpaper, the tiles dismantle, sawmilling, the wood fiber refining, grinding, mixing and stirring cast  materials, and room cleaning. Dust produced from a variety of works including sanddust, cement, lime, wood and dust mixed with paint. The results show that three types of works considere as high-risk activity (value > 12-25), 3 types of work consider as midle risk activities (value > 5-12), and one activity considered as a low-risk work (grades 1-5). The dusk factors controlling should be held regularly, in order to minimize the risk leveln againts the workers.

The entire activity involving human factors, machinery, chemicals, and through some work processes had the potential hazard that increased the risk of accidents and health problems. Aircraft paint removal job was a high risk of an accident. The purpose of this research was to identify hazards, assess pure risks, identify risk control, and assess risks after risk control were conducted on aircraft paint removal job in  PT. X. Type of this research was descriptive research. Primary data retrieval techniques in the study was observational and interview. The subject of research were 1 PDCA HSE and 4 aircraft paint removal workers. Variable in this study is the hazards, likelihood, severity, risk levels, and risk control. The results of hazard identification obtained 14 risk in Aircraft Paint Removal such as 1 low risk, 2 middle risks and 13 high risks. Risk control by the company showed that there are 13 high risk decrease to 7, 2 middle risks increased to 6 risk, and 1 low risk increased to 3 risks. In conclusion, PTX. has 14 risks, 13 of them included in the high risk category. There was a decrease in risk due to the control carriedd out by the company into 13 risks and 7 of them included in high risk.Keywords: aircraft paint removal, hazard identification, risk assessment, risk control

The right and appropriate system of receiving and transferring goods is needed by the company. In the process of receiving and transferring goods from the central warehouse to the branch warehouse at PDAM Tirta Kerta Raharja, Tangerang Regency, which is currently done manually is still ineffective and inaccurate because the Head of Subdivision uses receipt documents, namely PPBP and mutation of goods, namely MPPW in the form of paper as a submission media. The Head of Subdivision enters the data of receipt and mutation of goods manually and requires a relatively long time because at the time of demand for the transfer of goods the Head of Subdivision must check the inventory of goods in the central warehouse first. Therefore, it is necessary to hold a design of information systems for the receipt and transfer of goods from the central warehouse to a web-based branch warehouse that is already database so that it is more effective, efficient and accurate. With the web-based system of receiving and transferring goods that are already datatabed, it can facilitate the Head of Subdivision in inputing data on the receipt and transfer of goods and control of stock inventory so that the Sub Head of Subdivision can do it periodically to make it more effective, efficient and accurate. The method of data collection is done by observing, interviewing and studying literature from various previous studies, while the system analysis method uses the Waterfall method which aims to solve a problem and uses design methods with visual modeling that is object oriented with UML while programming using PHP and MySQL as a database.
