Hazard identification, risk assesment and risk control and the application of risk mapping at Hospital of the Animal Prof. Soeparwi Universitas Gadjah Mada Purpose: This research was held to analyze the work accident using HIRARC method and the applied of Risk Mapping at the hospital of the animal of Prof. Soeparwi Gadjah Mada University, Yogyakarta.Method: This research used descriptive qualitative research method. The qualitative method used to describe the condition, assess, and to recognize the work accident analysis. The data used was grabbed from Hazard Identification Risk Assessment and Risk Control (HIRARC) method and questionnaire.Results: Hazard identification used HIRARC method at the hospital of the animal of Prof. Soeparwi Gadjah Mada University was about 198. The work hazard commonly includes physical hazard, chemistry, biologist, ergonomics, and psychosocial. The risk assessment which held denoted that there were 61 low risks, 108 medium risks, and 15 high risks. The recommendations of risk restraint that can be given were technician control, administrative, and the use of personal protective equipment (PPE).Conclusion: HIRARC method had been applied at the hospital of animal of Prof. Soeparwi with the result to that grooming unit was high risk work place, assessed with risk mapping. The risk was from animal contact phisically.

HIRARC atau Hazard Identification Risk Assessment and Risk Control adalah upaya metode melakukan identifikasi terhadap bahaya yang digunakan suatu proses menentukan nilai risiko dan memasukkan kedalam kategori tingkat bahaya yang ada di tempat kerja serta menentukan pengendalian risiko yang digunakan pada setiap kegiatan prosedur yang ada di lingkungan kerja. Setiap tempat kerja selalu mempunyai risiko yang besar tergantung dari jenis pekerjaanya begitu pula upaya pengendalian risiko yang dilakukan di tempat kerja oleh karena itu dibutuhkan suatu pengukuran risiko kecelakaan kerja. Penelitian ini bertujuan untuk dapat mengetahui analisis risiko keselamatan dan kesehatan kerja yang ada di bagian produksi kerja PT. Varia Usaha Beton Cabang Makassar Tahun 2020. Jenis penelitian ini adalah studi kuantitatif dengan menggunakan  metode pendekatan deskriptif. Populasi dalam penelitian ini adalah seluruh pekerja yang ada di lingkungan kerja di PT. Varia Usaha Beton Cabang Makassar dengan teknik total sampling yaitu sebanyak 32 responden yang ada dibagian produksi beton.Data dianalisis dengan menggunakan univariat untuk mendeskripsikan karakkteristik responden serta menganalisa setiap variabel. Hasil penelitian menunjukkan identifikasi bahaya memiliki risiko sebanyak 75% sedangkan penilaian risiko di setiap kegiatan produksi beton terdapat tingkat risiko paling tinngi di kegiatan pengoperasian mesin sebanyak 20 WRAC dan tingkat risiko rendah di kegiatan perbaikan mobil truck sebanyka 02 WRAC. Pengendalian risiko yang ada di PT. Varia Usaha Beton telah menimalisir dari segala bentuk upaya terhadap bahaya dan risiko yang ada di lingkungan kerja bagian produksi beton. Kesimpulannya dari identifikasi risiko, penilaian risiko serta pengendalian risiko yang didapatkan di PT. Varia Usaha Beton Cabang Makassar memiliki tingkat risiko yang berbeda-beda sesuai dengan potensi bahaya pekerjaan atau kegiatan yang sedang dikerjakan. ABSTRAK Rumah Potong Hewan merupakan suatu bangunan yang memiliki desain dan konstruksi khusus digunakan sebagai tempat pemotongan hewan. Aktivitas kerja di RPH memiliki potensi bahaya yang dapat mengancam keselamatan dan kesehatan kerja. Berdasarkan observasi awal, RPH Kota Pekanbaru ini tidak memiliki laporan bulanan maupun tahunan mengenai data kecelakaan, dan juga belum pernah melakukan identifikasi, dan penilaian terhadap bahaya yang ada. Adapun tujuan penelitian ini yaitu untuk menganalisis keselamatan kesehatan kerja (K3) terhadap risiko bahaya lingkungan fisik di tempat kerja dengan metode “HIRARC” (Hazard Identification, Risk Assessment, and Risk Control). Jenis penelitian ini adalah kualitatif analitik. Lokasi penenlitian dilakukan di rumah potong hewan Kota Pekanbaru, penelitian dilakukan pada bulan Juli-Agustus 2020. Informan dalam penelitian ini sebanyak 6 orang, sebagai informan utama yaitu Kepala UPTD, 2 orang informan pendukung yaitu koordinator lapangan dan dokter hewan, dan informan kunci yaitu pekerja, metode yang digunakan peneliti yaitu wawancara mendalam dan observasi langsung. Berdasarkan hasil penelitian yang dilakukan potensi bahaya tingkatan risiko “Ekstrim” seperti diseruduk sapi pada proses penggiringan sapi menuju killing box, tertimpa katrol dan sapi pada proses penggantungan dan pemindahan sapi menggunakan katrol. Rumah Potong Hewan belum pernah melakukan identifikasi bahaya, penilaian risiko akan tetapi untuk pengendalian risiko sudah dilakukan beberapa upaya di rumah potong hewan, seperti SOP, Shift kerja, dan juga APD. Rekomendasi yang diberikan yaitu diharapkan sebaiknya RPH memiliki fasilitas yang berhubungan dengan kesehatan pekerja. Perlu adanya upaya pengendalian risiko secara engineering control, contohnya, membuat kerangkeng sapi. administrative control, seperti, memberikan sosialisasi, maintenance, housekeeping dan inspeksi, terhadap alat, mesin dan pekerja. ABSTRACTSlaughterhouse is a building having the design and construction specifically used as animal slaughterhouse. Slaughterhouse’s work activities have potential of hazards which can cause harm to workers’ safety and health. Based on pre observation, the slaughterhouse in Pekanbaru does not have any monthly and annual report about the data of accident. This livestock production also has no doing identification and assessment of hazard. The purposes of this study are to analyze occupational health and safety toward risk of danger physical environment at work, this study uses Hazard Identification, Risk Assessment, and Risk Control “HIRARC” method which using likelihood and severity level of each hazard as references to define a risk value. There are 6 informants the maininfromants is a UPTD head, 2 supporting informants are the field coordinator and venetarian, and the key informant is a worker, The methode that use in this study is interview And observation. The study shows that the most extreme are getting hit by a cow in cattle herding to killing box, falling of a cow hoist material from conveyor railing in shackling and hoisting process. Slaughterhouse has never done hazard identification and risk assessment. However, for risk control have done several attempts in slaughterhouse, such as Standard Operating Procedures, work shifts, and Personal Protective Equipment. Recommendation is given that the slaughterhouse should has facilities related to health of workers. It needs an effort to risk control through engineering control such as making cow cages, administrative control, such as giving socialization, maintenance, housekeeping and inspection of tools, machines and workersKeywords : Slaughterhouse, Hazard, Risk, Physical Environtmen, HIRARC.

ABSTRACTRevolution in the industry sector has been rapidly grown to fill up all the needs of the consumer products. One involves  supporting advanced machinery such as “Cutting, Skiving, Stitching, Emboss Logo, Roving, Punch Hole, Juki, BrushingEdge, Hammer Over Lapping and Two Molding”. In the factory production process, there are various types of high-risk activities, especially on line upper. The main of this research is to study the risk assessment on export companies line the upper part of the shoes export company using Job Safety Analysis. This research was conducted observational crosssectional design. Observations made to the hazards and control measures. Interviews were conducted to 12 employees. Variables in this research is production activity, hazard identification, risk assessment, risk control and residual risk. The results of hazard identification has been done, there are 91 known potential hazards, for risk assessment found 7 high risk and low risk 5. Machine classified as high risk on the risk assessment is roving machine, whereas low-risk is two molding machine. Control efforts on the upper line in accordance with the hierarchy of controlling a number of 91 controls, whereas for the residual risk still remains as much as 30 residual risk. Control has been applied quite well by pressing the consequences of hazards and risk management.Keywords: risk assessment, controlling, residual risk

Zero accident was considered as the primary occupational health and safety value in manufacturing and service enterprises.  PT. ABC provided  electrical and instrument services, including a low-voltage installation, high-voltage installation and an instrument installation. This company is known to excellent electricity and instrument service providers across the country . Moreover, men’s power is used as a primary source to maintain  these kind  of services. Installation projects depend not only on the high working speed, but also on  high working  accuracy within  certain timeframes . As a result, work  accidents  have occurred with a classification varying from minor to fatal accidents.  The purpose of this research is to identify potential hazards to assess risk levels and  obtain recommendations for  preventing accident. The HIRARC method is employed as a research approach. This research is initiated through Hazard Identification, Risk Assessment and Risk Control.  This research identified that two hazards were classified as  extreme risk, three hazards as high- risk, eight hazards as moderate risk , and two hazards have been identified as low risk . Furthermore, a number of action lists have been suggested in order to minimize accident rates and finally the working conditions are designed to maintain zero accidents.

Risiko kecelakaan kerja merupahal hal yang tak dapat dihindari dari kegiatan proyek pembangunan. PT Gran Anugerah Wijaya merupakan pengusaha pengembang perumahan yang sedang mengerjakan proyek pembangunanan 58 unit rumah tipe 36 yang berlokasi di daerah Palembang. Dari hasil pengamatan lapangan, alur proses pembangunan rumah yang dikerjakan meliputi pemasangan pondasi, pemasangan dinding, pemasangan kusen kayu, pemasangan rangka atap dan finishing. Hazard Identification, Risk Assessment dan Risk Control telah dilakukan dlaam penelitian ini. Hasil yang didapat bahwa terdapat 27 potensi risiko dengan risk level diantara rendah hinggi tinggi. Pengendalian risiko yang bisa dilakaukan untuk mengurangi bahaya kerja terhadap karyawan bangunan diantaranya substitusi, administrasi dan Alat Pelindung Diri (APD).

Indonesian Navy ships (KAL) is one element of the Integrated Fleet Weapon System (SSAT), which plays an important role in carrying out tasks in the defense field. So, we need an optimal KAL readiness to support the passage of the operation to be carried out successfully and smoothly. One of the ways used to prepare KAL is to carry out routine maintenance through the docking process. However, in practice there are accidents  suffered by workers.This research was conducted using the HIRARC method. HIRARC is a method of hazard identification, risk assessment and risk control measures that are used for this method is considered moreappropriate and more accurately where the dangers that arise in the process described docking of any work activity. In this method also provides appropriate control measures for each hazard. Based on the results of the HIRARC analysis, each suction work activity of docking based Risk Ranking will be sorted according to the level of Risk Matrix and will be discussed further against high risks and implemented controls. Then the resulting draft recommendation to minimize workplace accidents in the docking process is by way of elimination, Administration Control and Use of PPE (Personal Protective Equipment).Keywords: HIRARC method, Docking process, KAL.

Perkembangan di dunia industri menyebabkan banyaknya permasalahan yang penting untuk dikaji mengingat hazard dapat mengakibatkan risiko gangguan kesehatan bahkan kecelakaan kerja. Suatu industri membutuhkan sistem Kesehatan Keselamatan Kerja (K3) dengan metoda Hazard Identification, Risk Assessment, and Risk Control (HIRARC), yang jika diterapkan, akti-vitas berbahaya dapat mudah ditemukan dan dapat segera dilakukan pengendalian sesuai besarnya tingkat risiko (urgent, high, medium, dan low). Penelitian ini dilakukan di Industri Seri-kat Pekerja Aluminium Sorosutan dan bertujuan ingin mengetahui identifikasi bahaya, penilaian risiko dan pengendalian risiko, khusunya di Bagian Finishing 2. Hasil penelitian menunjukkan bahwa persentase potensi hazard yang masuk kategori urgent adalah 5 % di ruang pembubut-an dan 6 % di ruang las. Kemudian, persentase potensi hazard yang masuk kategori high ada-lah 22 % di ruang polish, 38 % di ruang brom, 36 % di ruang gerinda, 33 % di ruang quality control, dan 20 % di ruang pembersihan produk. Berdasarkan hasil tersebut dapat disimpulkan bahwa hazard tertinggi yang masuk kategori urgent ada di dua ruang produksi yaitu ruang pembubutan dan las, sedangkan ruang produksi yang masuk kategori high terdapat di lima ru-ang produksi yaitu ruang polish, brom, gerinda, quality control, dan pembersihan produk (cuci).

Kegiatan di Rumah Sakit mempunyai risiko berasal dari faktor fisik, kimia,  biologi, ergonomi  dan  psikososial, variasi, ukuran, tipe dan kelengkapan Rumah Sakit  menentukan tingkat risiko K3. RSD Gunung Jati sebagai sarana pelayanan kesehatan tempat berkumpulnya orang sakit maupun orang sehat yang memungkinkan terjadinya risiko timbulnya kecelakaan akibat kerja dan penyakit akibat kerja. Sumber bahaya yang ada di Rumah Sakit harus diidentifikasi dan dinilai untuk menentukan tingkat risiko, yang merupakan tolak ukur kemungkinan terjadinya kecelakaan akibat kerja dan penyakit akibat kerja. Dari hasil rekapitulasi insiden di ruang rawat inap Pangeran Suryanegara (Psikiatri) pada Desember 2015 sampai Juni 2017 terdapat 20 insiden. Tujuan penelitian ini untuk mengetahui manajemen risiko K3 menggunakan Hazard Identification Risk Assessment And Risk Control (HIRARC). Penelitian ini merupakan penelitian kualitatif. Teknik yang digunakan dalam pengumpulan data yaitu observasi lapangan, telaah dokumen, dan wawancara mendalam. Analisis data diawali dengan mengidentifikasi bahaya potensial dengan metode HIRARC untuk menganalisa potensi bahaya dari aktivitas kerja serta memberikan penilaian risiko, dan melakukan upaya pengendalian risiko.Berdasarkan hasil penelitian, diketahui bahwa terdapat 10 aktivitas kerja di ruang rawat inap psikiatri yang memiliki potensi bahaya, identifikasi bahaya yaitu pencahayaan, disinfektan, tertular penyakit HIV/AIDS, Hepatitis A, Hepatitis B, Tuberkulosis, postur tubuh yang salah, melakukan pekerjaan berulang, mendapat  serangan pasien, sering kontak dengan pasien, panik, dan kerja berlebih. Penilaian risiko diketahui 1 aktivitas kerja dengan risiko extreme, 7 aktivitas kerja dengan risiko high, 1 aktivitas kerja moderate dan 1 aktivitas kerja low.Kata Kunci : Manajemen Risiko K3, HIRARC   ABSTRACTActivities in hospital have a risk which comes from physical factor, chemistry, biology, ergonomics, and psychosocial, variety, size, type, and completeness of hospital determine the OHS risk degree. Regional Gunung Jati hospital as a health service facility is a gathering place for sick people or healthy people where it is possible a risk occurs due to working accidence and disease due to working. The dangerous source which exists in hospital must be identified and measured to determine the risk level which is measuring basis for the possibility of accidence occurs due to working and disease due to working. From the recapitulation result of incidence in overnight-patient room Pangeran Suryanegara (psychiatry) from Des 2015 to June 2017 has 20 incidences. The goal of this research is to know the OHS risk management uses HIRARC. This research is a qualitative research. The technique used to collect data is observing field, analyzing data begins by identifying the potential danger using HIRARC method, analyzing potential danger from working activities and risk measurement and do the effort for controlling the risk.According to the research's result, it is known that there are 10 working activities in overnight-psychiatry patient room which have potential danger, identifying danger such as lightning, desinfectan, infected HIV/Aids, hepatitis A, hepatitis B, tuberculosis, wrong body from, doing repetitive work, getting patients attack, contacting patient frequently, panic, and workaholic. The risk measurement is known 1 working activity with the extreme risk, 7 working activities with high risk, 1 moderate working activity and 1 low working activity.Keywords : OHS Risk Management, HIRARC
