Aim. To determine the saliva level of immunoregulatory proteins in patients with rampant caries and 25-hydroxyvitamin D (25(OH)D) deficiency and evaluate the association of their concentration with 25(OH)D plasma level.Materials and methods. The study was performed in two groups. The experimental group included 15 patients aged 20–22 years with rampant caries and the 25(OH)D plasma level of < 20 ng / ml. The control group encompassed 15 healthy age-matched volunteers with the 25(OH)D plasma level of 20–100 ng / ml. The concentrations of B7.2 (CD86), free active TGF-β1, CTLA-4, PD-1, Tim-3, LAG-3, IGFBP-4, and ICAM-1 were assessed using flow cytometry. The levels of LL-37 and secretory immunoglobulin A (sIgA) were measured using ELISA. The Spearman’s rank correlation coefficient was used to reveal a correlation between the indicated proteins and the 25(OH)D plasma level.Results. A decrease in B7.2 (CD86), PD-1, Tim-3, sIgA, and LL-37 and elevation of IGFBP-4 and ICAM-1 saliva levels were detected in patients with rampant caries and 25-hydroxyvitamin D deficiency. A positive Spearman’s rank correlation coefficient was revealed between plasma 25(OH)D and saliva levels of free active TGF-β1, CTLA4, B7.2 (CD86), LL-37, and sIgA. A negative correlation was revealed between 25(OH)Dand ICAM-1.Conclusion. 25(OH)D deficiency in patients with rampant caries is associated with decreased levels of B7.2 (CD86), PD-1, Tim-3, sIgA, and LL-37 and elevated levels of IGFBP-4 and ICAM-1 in the saliva.

Early detection of dorsopathies is an urgent task for primary care physicians, since such conditions can combine with other chronic noncommunicable diseases (NСD) and adversely affect the course all comorbidities, consequently disimproving the quality of life of patients and increasing the frequency of their requests for medical assistance. This study aimed to determine the value of routine checkups in the context of detection of dorsopathies, NCD, and identification of risk factors (RF). We have retrospectively analyzed the patient records database of a rural outpatient clinic in the Tver region (years 2015 to 2017). The prevalence of dorsopathies and NCD RF were the subjects investigated. Fisher's exact test and Spearman's rank correlation coefficient (SRCC) were applied for the purposes of statistical processing of the results. We discovered that dorsopathy most often was a comorbidity to arterial hypertension and gastrointestinal tract diseases; it was strongly related to the NCD (SRCC = 0.506), age (SRCC = 0.383), slightly less so — to hypodynamia (SRCC = 0.146), type of the patient's occupation (intellectual or physical labor) (SRCC = 0.07). Routine checkups improve the rate of detection of dorsopathy: the more patients undergo such examinations, the more cases of dorsopathy are diagnosed. Thus, it is necessary to increase the number of working people attending the checkups in order to detect dorsopathies early and prevent them effectively.

Abstract. The research problem lies in the need to study the contradiction between the high values of life-meaning orientations and the reflexivity of employees of the internal affairs bodies. Purpose of the article: to investigate the indicated contradiction and reveal the peculiarities of the subjectivity of the personality of employees. The analysis and interpretation of the characteristics of the personal development of employees in lifelong education was carried out by the method of clustering the sample using the values of the tests "Life-meaning orientations" D. Krambo, L. Makholik, adaptation - D.А. Leontiev, reflexivity (A.V. Karpov), as well as the subjectivity of a professional's personality and focus on the personal development of an employee, including in lifelong education based on the author's methodology. The work used the correlation analysis of the studied features by means of nonparametric statistical criteria (Spearman's rank correlation coefficient), cluster analysis. As a result of the study, it was revealed that reflexivity, orientation towards development in professional activity and focus on continuing education in clusters are expressed unevenly. The hypothesis of a strict inverse relationship between manifestations of reflexivity and life-meaning orientations was not confirmed. At the same time, a feature of the subject-personal orientation of employees is an insufficient connection between reflexivity and high values of meaningful orientations, subordination of the personal principle to the "professional role and position" (according to the terms of V.A. Lefebvre) due to the subordination and discipline in the profession, as well as the substitution of the moral imperative with legal norms. Strategies with extremely high values of life-meaning orientations of adult employees of internal affairs bodies, which have a negative correlation with reflexivity, regardless of subjectivity or the severity of orientation towards development in the profession, are assessed by us as unfavorable for personal development.

Taking as a basis for discussion the Schumpeter’s innovation theory, this paper analyses the relationship between enterprises activity in the field of research and development, and their efficiency at the core business level. This analysis was performed in two ways – with the assumption shift in time between research and development activities and companies business efficiency, and without it – using the Spearman’s rank correlation coefficient. The sample was accounted for 252 companies from the technology sector, whose shares are traded on NYSE or NASDAQ, and the analysis time period consisted of three years (2011–2013). The results obtained in the course of analysis generally indicate lack of strong relationship between distinguished categories. A noticeable, but only at moderate level, positive correlation was found in both considered approaches only in respect of the relationship between the intensity of expenditures on research and development or y/y change of these expenditures and gross margin on sales. Therefore, it seems to be relevant to extend this research at least in such directions as: identification and characterization of factors determining efficiency of companies research and development activities, as well as examination considered relationship taking into account business diversity within the sector and wider time shift between realized research and development activities and various measures of core business efficiency.

Background:Matrics metalloproteinases (MMPs) are the key enzymes responsible for the joint destruction in rheumatoid arthritis (RA).Objectives:The aim of this study was to examine the association of baseline levels of metalloproteinases-9 (MMP-9) in serum and synovial fluid (SF) with structural damage of hand and feet joints in patients with early RA and also with imunoserological markers of the disease. Methods:The study enrolled 134 subjects with knee synovitis: 72 patients with early DMARD-naïve RA (symptom duration ≤12 months) and 62 patients with osteoarthritis (OA), as control group. Synovial fluid was obtained by an arthrocentesis of the knee joint. Joint damage was estimated by hands, knee and feet radiography. With regard to the presence of destructive joint changes on initial x-ray, RA patients were classified as erosive and nonerosive form of disease. ELISA assay was used for the detection of MMP–9 activity in serum and SF as well for the imunoserology tests: rheumatoid factor RF (IgG) and anti-CCP antibody (ACPA).Results:MMP–9 activity in serum and SF of RA patients was significantly higher compared to its activity in serum and SF of control group (p<0.01 and p <0.001 respectly) (table 1).Table 1.MMP–9 activity (ng/ml) in serum and SF of RA and control group patientsGroupMMP-9 activity in BP(ng/ml); mean±SDMMP-9 activity in SF(ng/ml)mean±SDRA(n=72)18.28±7.54**15.07±13.24***Controls -OA(n=62)13.58±3.070.65±0.41RA – rheumatoid arthritis; SD – standard deviation; n – number of subjects; ** – p<0.01, *** p<0,001We did not establish a significant correlation between the activity of MMP 9 in serum and SF in the RA and control groups (Spearman’s rank correlation coefficient in RA was 0,02 and 0,06 in control group). Most of the subjects from RA group (52–72.22%) had verified radiographic erosive changes in joints. Nonerosive arthritis was present in remaning 20 (27.78%) of RA patients. No differences were obtained according to the sex, age or disease duration between erosive and nonerosive RA patients.Table 2 represents MMP–9 activity in serum and SF of patients with erosive and nonerosive RA. The values of MMP–9 activity measured in serum were higher in nonerosive compared to erosive RA (20.35±10.30 vs. 17.46±6.25), but the difference was not statistically significant (p>0.05). However, MMP–9 activity in SF was significantly higher in erosive compared to nonerosive RA (17.53±12.87 vs. 8.76±7.72; p<0.05).Table 2.MMP–9 activity (ng/ml) in serum and SF of patients with erosive and nonerosive RAMMP-9 activityErosive RA(n=52)[ng/ml]Nonerosive RA(n=20)[ng/ml]Serum17.46±6.2520.35±10.30Synovial fluid17.53±12.87*8.76±7.72RA – rheumatoid arthritis; SD – standard deviation; n – number of subjects; * p<0.05;We also examined the correlation of MMP 9 activity in serum and SF with standard imuno serological markers of disease (RF and ACPA). Our results indicate that there is a significant correlation of MMP 9 activity in SF with ACPA level in RA group (Spearman’s rank correlation coefficient is 0,48), but not with RF (Spearman’s rank correlation coefficient is 0.06).Conclusion:MMP–9 activity in serum and synovial fluid of patients with early RA is significantly higher compared to patients with osteoarthritis. High activity of MMP–9 in synovial fluid of patients with early RA correlated with ACPA level and may be a predictor of rapid radiographic progression of disease.References:[1]Prodanovic S, Radunovic G, Babic D. et al. The main idea of the presented work is the assumption that innovative behavior is largely predetermined by the synthesis of values and motives, the relationship of which has not been sufficiently studied. The article presents a theoretical and empirical analysis of the value-motivational characteristics of managers with different levels of expression of innovativeness. On the basis of the theoretical study, the authors of the article formulated a hypothesis about the presence of significant differences in value-motivational characteristics among managers with different levels of innovativeness. The hypothesis determined the goal of the empirical study, which was to establish links between value-motivational characteristics and the specificity of the severity of innovativeness among middle managers working in manufacturing companies. The study involved 83 respondents from 25 to 50 years old, working in manufacturing companies in the Moscow region. The survey was carried out using the following methods: (1) methodology "Diagnostics of the real structure of personal value orientations" by S.S. Bubnova; (2) questionnaire "Diagnostics of the motivational structure of personality" by V.E. Milman; (3) "Scale of self-assessment of innovative personality traits" (N.M. Lebedeva, A.N. Tatarko). For statistical verification of the results, the Kruskal-Wallis H-test and Spearman's rank correlation coefficient were used. The empirical results obtained made it possible to identify managers with different levels of self-assessment of innovativeness among the respondents and to obtain in each of the groups specific data on the manifestation of value-motivational characteristics of behavior and activity. At the end of the article, the authors formulated recommendations to enhance the professional activity of managers with different levels of self-assessment of innovativeness. In particular, it is important to develop training programs that correct the value-motivational sphere of the individual, taking into account the synthesis of those values and motives of activity that are characteristic of groups with high, medium and low self-esteem of innovativeness.

Nurses are always part of the team that performs resuscitation procedures. In this paper the authors explore the influence that resuscitation procedures on nurses who perform them. For data collection, the authors used a survey with a convenience sample of nurses who work in emergency settings. For statistical processing of data, the authors used the calculation of frequency, standard deviation, means, and median. Authors used Spearman's rank correlation coefficient and calculated the p-value. The respondents in the survey completed the Post-Code Stress Scale. The results show that the majority of the respondents experienced resuscitation cases as burdensome situations; however, the level of stress was moderate. Nurses still experience some physical and psychological symptoms during resuscitations.
