Difficulty in breathing or shortness of breath may be simply termed as Shwasa (Asthma), As per Ayurveda, Shwasa is mainly caused by the Vata and Kapha Doshas. Shwasa is broadly classified into five types in Maha Shwasa (Dyspnoea major), Urdhawa Shwasa (Expiratory Dyspnoea), Chinna Shwasa (Chyne-stroke respiration), Kshudra Shwasa (Dyspnoea minor), Tamaka Shwasa (Bronchial Asthma). In modern science Tamaka Shwasa can be correlated with Asthma, Asthma which is a chronic inflammatory disease of airway. In modern medicine there is no cure for Asthma, symptoms can typically be improved. In Ayurveda, Asthma can be effectively and safely manage the condition without inducing any drug dependency where Pachakarma procedures and use of internal medication detoxifies the body, provides nutrition and increases the elasticity of lung tissue it also develops natural immunity of the body thus decreasing episodic recurrence of the disease.

Nicotinamide riboside (NR), a new form of vitamin B3, is an effective precursor of nicotinamide adenine dinucleotide (NAD+) in human and animal cells. The introduction of NR into the body effectively increases the level of intracellular NAD+ and thereby restores physiological functions that are weakened or lost in experimental models of aging and various pathologies. Despite the active use of NR in applied biomedicine, the mechanism of its transport into mammalian cells is currently not understood. In this study, we used overexpression of proteins in HEK293 cells, and metabolite detection by NMR, to show that extracellular NR can be imported into cells by members of the equilibrative nucleoside transporter (ENT) family ENT1, ENT2, and ENT4. After being imported into cells, NR is readily metabolized resulting in Nam generation. Moreover, the same ENT-dependent mechanism can be used to import the deamidated form of NR, nicotinic acid riboside (NAR). However, NAR uptake into HEK293 cells required the stimulation of its active utilization in the cytosol such as phosphorylation by NR kinase. On the other hand, we did not detect any NR uptake mediated by the concentrative nucleoside transporters (CNT) CNT1, CNT2, or CNT3, while overexpression of CNT3, but not CNT1 or CNT2, moderately stimulated NAR utilization by HEK293 cells.

Unilateral laterothoracic exanthem (ULE) is relatively rare self-limited pediatric skin inflammatory disease; its etiology still remains unknown. The diagnosis can be established due to specific clinical findings: presence of spotted and papular rash on one side of the body, more often in the axillary or inguinal area with possible generalization. This article presents the clinical case of ULE in the girl (4 years 8 months), it shows the dynamics of exanthematous syndrome at this pathology on different stages of the disease course.

Efficient synthesis of NAD+ is critical to maintaining cell viability in all organs of the body. However, little is known of the pathway(s) by which cells of the central nervous system produce NAD+. The aim of this study was to investigate the relationship, between tryptophan degradation via the kynurenine pathway (KP) and de novo NAD+ synthesis in human astrocytes, a major cell type within the brain. In this study we observed that inhibition of single enzymes of the KP resulted in significant decreases in NAD+ levels in astroglial cells after a 24 hr period. We also observed that astrocytes cultured in media deficient in tryptophan, nicotinic acid and nicotinamide resulted in a 50% decrease in NAD+ levels after 24 hrs. This decrease in NAD+ was partially restored by supplementation of the culture media with either tryptophan or kynurenine, or nicotinic acid or with supply of the salvage pathway precursor nicotinamide.

 Rheumatoid arthritis is a chronic systemic inflammatory disease whichinvolve multiple joints  of the body.The clinical symptoms of the Rheumatoid Arthritisresemble with Amavata described in Ayurvedic literature. In thisdisease, the vitiated Vatadosha and Ama spread all along the body and  localisein kaphasthana likejoints and producing symptoms like pain, stiffness, swelling, tendernessetc.Treatment principle recommended are to correct the status of the digestive fireof the bodyand balance of Vata in the body. In this case, a male patient of 28 years old presented with the symptoms of inabilityto walk and stand without support associated with pain, swelling and  stiffnessover multiple jointsalong with deformity of hands. Pain was assessed by Visual analogue score(VAS).The patient was admitted in IPD of the hospital andtreatedwith Ayurvedic  medicines and Panchakarma therapy for 28 days. Remarkable improvement was seen in pain, swelling and stiffness after the treatment.

Sarcoidosis is an inflammatory disease with multisystem involvement characterized by the presence of noncaseating granulomas. It can affect virtually every organ of the body, with lung involvement being most common occurring in >90% of patients. Other organs affected are skin, eye and liver. Skin involvement is common, affecting 25-35%. Here we present a rare case of a 15 year-old male with isolated oculocutaneous sarcoidosis without systemic involvement.

Abstract Background: Lipomatosis is a condition in which multiple lipomas are present on the body. Different entities which are accompanied by multiple lipomas include Proteus syndrome, Cowden syndrome and related disorders due to PTEN gene mutations, MEN1, benign symmetric lipomatosis (Madelung or Launois-Bensaude disease), Dercum’s Disease, familial lipodystrophy, hibernomas, epidural lipomatosis, familial angiolipomatosis, and meso-somatous lipomatosis (LMS) still called Roch-Leri lipomatosis. LMS is characterized by the presence of many discrete, encapsulated lipomas of 2 to 5 cm, painless, at the level of the trunk and forearms. The aim of the present study was to determine the clinico-biological phenotype of LMS, as compared to controls. Patients and methods: In this single-center study (NCT01784289), 18 healthy controls (C) and 11 LMS were included after examining the cohort of 76 patients referred for suspected lipodystrophic syndrome between 2009 and 2019 in the Endocrinology Department of a University Hospital. Clinical (sex, age, weight, BMI, blood pressure (BP), alcohol consumption), metabolic (fasting blood glucose (FBG) and insulin levels, lipid balance, ASAT, ALAT, GammaGT, leptin), immuno-inflammatory (CBC, lymphocyte immunophenotyping), and anthropometric (% of body fat in DEXA, steatosis and intra-abdominal / total abdominal fat ratio (IAF / TAF) in MRI) were evaluated. Results: The following parameters, expressed as % or median, differed significantly between LMS vs. C groups, respectively: weight 100 vs. 69kg (p<0.01), BMI 30.8 vs. 22.7 (p<0.01), systolic BP 140 vs. 115 (p<0.01) and diastolic BP 80 vs. 70 mmHg (p<0.05), gammaGT 74 vs. 18 IU / L (p<0.01), fasting insulin levels 7.3 vs. 4.7 microIU / mL (p<0.05), leptin 28 vs. 5 ng / mL (p<0.01), CD3 (867 vs. 1444 / mm3 (p<0.01), CD4 499 vs 866 / mm3 (p<0.05), CD8 227 vs. 546 (p<0.05), fat mass 41 vs. 22 (p<0.05). Three LMS patients had hepatic steatosis and 3 a first-degree family member with LMS. Percentage of men (63% vs. 52%, FBG, lipid levels, ASAT, ALT, CBC, IAF / TAF (0.25 vs 0.19) did not differ significantly. The number of lipomas was > 5 in 82% of LMS patients and lipomas were localized first to the forearms (82%), then the thighs (73%) and the abdomen (55%). At diagnosis, the age of LMS patients was 20 years old; 55% of the LMS patients had a BMI above 30 and 45% above 25. No patient had excessive alcohol consumption. Five had a history of auto-immuneor inflammatory disease: 1 hyperthyroidism, 1 hypothyroidism, 1 multiple sclerosis, 1 vitiligo, 1 Raynaud syndrome. Conclusion: LMS mainly affects overweight men and is associated with hypertension, hyperinsulinism, increased gammaGT and a decrease in CD3, CD4 and CD8 lymphocytes, suggesting an immune dysregulation, all the more so that 45% had an associated auto-immune/inflammatory disease.

Niacin (nicotinic acid) is a potent lipid-lowering agent that has been used for prevention of coronary heart disease. Niacin activates the HCAR2 receptor found on adipocytes, macrophages and various immune cells throughout the body. Activation of the HCAR2 receptor by niacin results in beneficial anti-inflammatory effects that are independent of lipid lowering. This review summarizes the use of niacin in treatment of dyslipidemia, the pharmacogenetics of niacin response and the potential role of HCAR2 signaling in the treatment of a variety of inflammatory and metabolic diseases.
