Objectives. Past several studies have proven that caffeine facilitates attentional enhancement by acting as an adenosine antagonist once it is absorbed by the body, resulting in improved psycho-behavioral function. Modern clinical olfactory function tests are usually assessed by psychophysical tests but due to a paucity of data, the influence of enhanced attention by caffeine on olfactory function still remains unclear. The objective of this study was to compare results of cognitive function (attention) and olfactory function before and after caffeine administration in order to analyze effects of caffeine on olfactory function in normosmic subjects.Methods. This study enrolled 49 participants of Konkuk University Hospital with a mean age of 27.7 years who had patent olfactory clefts and no olfactory dysfunction from May 2015 to February 2016. Subjects were restrained from caffeine 10 hours before the test. On day 1, participant’s subjective olfactory function was evaluated before and after uptake of either caffeinated or decaffeinated coffee using visual analog scale (VAS) score, minimum cross-sectional area (MCA) measured by acoustic rhinometry, and the Korean version of Sniffin’ Stick II (KVSS II). Evaluation of participant’s attentional degree was measured by d2 test. On day 2, the same procedure was carried out with counterpart substance. The type of coffee initially administrated was randomly selected.Results. After administration, caffeinated coffee resulted in significant attentional enhancement than decaffeinated coffee. Results of d2 test showed statistically significant differences in the parameters of total number of errors and omission errors. In both the caffeinated and decaffeinated groups, the patients showed slight increase in VAS score and nasal cavity area; however, the difference was not statistically significant. Also, caffeinated coffee intake compared to decaffeinated coffee intake showed no significant relevance to olfactory function.Conclusion. Caffeine may significantly improve attentional congnitive function, while not have acute effects on olfactory function.

This study aims to determine the effect of body mass index (BMI), sleep patterns, working years, physical workload, mental workload, and work time on work fatigue on nurses at Hasanuddin University Hospital Makassar. This research is a quantitative study with a cross-sectional design with a sample size of 56 people. Sampling using a simple random sampling method. Data collection was carried out by means of a questionnaire. Bodyweight is measured by weight scales, body temperature is measured by microtoice, and physical workload is measured by a pulse oximeter, which is by looking at the pulse of the nurse before and after work. Data analysis in this study used the SPSS program. The results of this study indicate that there is an influence between Body Mass Index (BMI) on work fatigue (p = 0.001), there is an effect of sleep patterns on work fatigue (p = 0.019), there is an effect of tenure on work fatigue (p = 0.017), the relationship between physical workload and work fatigue (p = 0.000), there is an effect of mental workload on work fatigue (p = 0.000) and there is an effect of length of work on work fatigue (p = 0.017). The physical workload is the variable with the greatest influence on work fatigue on nurses at Hasanuddin University Hospital Makassar with Exp. (B) = 14,526. The results of this study concluded that there was a significant influence between the Body Mass Index (BMI), sleep patterns, years of work, physical workload, mental workload, and length of work for nurses at Hasanuddin University Hospital Makassar.

BackgroundResults from earlier publications on the association of coffee and caffeine and risk of ovarian cancer are inconsistent.ObjectiveTo evaluate the link between coffee, caffeine, caffeinated coffee, and decaffeinated coffee consumption and risk of ovarian cancer.MethodsWe searched PubMed/Medline, ISI Web of Science, Scopus, and Google Scholar to identify relevant publications up to April 2018. All case–control studies that considered coffee, caffeine, caffeinated coffee, or decaffeinated coffee as the exposure variables and ovarian cancer as the main outcome variable or as one of the outcomes were included in the systematic review. Publications in which odds ratios (ORs) or rate or risk ratios (RRs) and 95% confidence intervals (CIs) were reported, were included in the meta-analysis.ResultsA total of 22 case–control studies were included in the systematic review, and 20 studies in the meta-analysis. Overall, 40 140 participants, including 8568 patients with ovarian cancer, aged ≥ 17 years were included. Combining 21 effect sizes from 18 studies, no significant association was observed between total coffee intake and risk of ovarian cancer (OR=1.09; 95% CI 0.94 to 1.26). There was no significant association between total caffeine intake and ovarian cancer risk (OR=0.89; 95% CI 0.55 to 1.45). In addition, caffeinated coffee intake was not significantly associated with ovarian cancer (OR=1.05; 95% CI 0.87 to 1.28). However, combining effect sizes from five studies, we found an inverse significant association between decaffeinated coffee intake and risk of ovarian cancer (OR=0.72; 95% CI 0.58 to 0.90).ConclusionsOur findings indicated an inverse association between decaffeinated coffee consumption and risk of ovarian cancer. No significant association was found between coffee, caffeine or caffeinated coffee intake and risk of ovarian cancer.

Abstract Background Maternal caffeine intake has an effect on fetal growth through cross the placenta freely. The objective of this study is to examine the association of pre-pregnancy coffee consumption pattern with the risk of threatened abortion. Methods A cross-sectional analysis was conducted for a total of 4,111 pregnant women who completed baseline examination for the Korean Pregnancy Outcome Study between 2013 and 2017. Coffee consumption pattern before pregnancy was measured by a questionnaire. The frequency of coffee consumption was divided into seldom (<1 cup/week), light (<1 cup/day), moderate (1 cup/day), and heavy coffee drinkers (≥2 cups/day). Threatened abortion was defined as the occurrence of vaginal bleeding in the first 20 weeks of pregnancy. Multiple logistic regression models were applied to examine the association between pre-pregnancy coffee consumption and the risk of threatened abortion, after adjusting for age, body mass index, systolic blood pressure, smoking, drinking, physical activity before and after pregnancy, stress level, history of depression, and reproductive information. Results Women with heavy coffee drinking were at increased odds for having threatened abortion in unadjusted model (odds ratio [OR], 1.271; 95% confidence interval [CI], 1.008 to 1.602). Compared to women with seldom coffee drinkers, the fully adjusted ORs for the risk of threatened abortion were significant in moderate (OR, 1.420; 95% CI, 1.001 to 2.014) and heavy coffee drinkers (OR, 1.750; 95% CI, 1.156 to 2.651). Conclusions Heavy coffee drinking was independently associated with a higher risk of threatened abortion among Korean pregnant women.

Purpose Observational studies have demonstrated increased colon cancer recurrence in states of relative hyperinsulinemia, including sedentary lifestyle, obesity, and increased dietary glycemic load. Greater coffee consumption has been associated with decreased risk of type 2 diabetes and increased insulin sensitivity. The effect of coffee on colon cancer recurrence and survival is unknown. Patients and Methods During and 6 months after adjuvant chemotherapy, 953 patients with stage III colon cancer prospectively reported dietary intake of caffeinated coffee, decaffeinated coffee, and nonherbal tea, as well as 128 other items. We examined the influence of coffee, nonherbal tea, and caffeine on cancer recurrence and mortality using Cox proportional hazards regression. Results Patients consuming 4 cups/d or more of total coffee experienced an adjusted hazard ratio (HR) for colon cancer recurrence or mortality of 0.58 (95% CI, 0.34 to 0.99), compared with never drinkers (Ptrend = .002). Patients consuming 4 cups/d or more of caffeinated coffee experienced significantly reduced cancer recurrence or mortality risk compared with abstainers (HR, 0.48; 95% CI, 0.25 to 0.91; Ptrend = .002), and increasing caffeine intake also conferred a significant reduction in cancer recurrence or mortality (HR, 0.66 across extreme quintiles; 95% CI, 0.47 to 0.93; Ptrend = .006). Nonherbal tea and decaffeinated coffee were not associated with patient outcome. The association of total coffee intake with improved outcomes seemed consistent across other predictors of cancer recurrence and mortality. Conclusion Higher coffee intake may be associated with significantly reduced cancer recurrence and death in patients with stage III colon cancer.

Abstract Background and Aims The Chronic kidney disease (CKD), like many chronic illnesses, is invariably associated with various psychiatric conditions and poorer quality of life. This study aims to assess the prevalence of depression and anxiety among CKD patient and their determinant factors. Method this is a cross sectional single center study in a Moroccan university hospital. Patients aged ≥ 18 years old and followed for more than one year were included. The data was collected using a questionnaire for sociodemographic and clinical information and the Hospital anxiety and depression scale (HADS) to assess depression and anxiety prevalence. After the description of the population’s characteristics, the statistical analysis aimed to assess the association between depression and anxiety disorders and the estimated glomerular filtration rate before and after adjustment on several confounding factors. Results 88 patients were included (63.6% of them were women, the mean age was 61.8±14.0 years), 21 were on stage 3, 46 were on stage 4, and 21 were on stage 5 of the CKD. The median of depression sub-score was 5.00[2.00; 10.0], the median of anxiety sub-score was 6.00[4.00; 9.00], and the median of the global score was 11.0[7.00; 20.0], 22.0% of included patients had depression and 22.0% had anxiety. Both depression and anxiety scores were associated to eGFR before and after adjustment (p= 0.001, p&lt;0.001and p=0.04, p=0.03 respectively). Conclusion This study showed that depression and anxiety are strongly related to the CKD progression, which should motivate both doctors and nurses to improve their psychological care toward CKD patients.

The situation of lack of oxygen supply to cells and tissues is often not realized by many people (hypoxia). Hypoxia can occur in various situations in life. The main effect of hypoxia is the effect on the brain, so the body will do everything it can to restore the state of homeostasis. HIF-1α protein is a marker of hypoxic conditions. HIF-1α regulates the synthesis of many genes to maintain and restore body homeostasis from hypoxia to normoxia. This study was a descriptive study with cross-sectional design. The sample of this study were six swimming athletes with a 12-19 year age range who met the inclusion and exclusion criteria. The HIF-1α protein is measured by the ELISA method. Data were analyzed statistically. The results showed higher levels of HIF-1α after anaerobic exercise than the levels of HIF-1α before and after aerobic exercise. This is greatly influenced by the intensity of the exercise carried out. This proves that cellular adaptation to hypoxia is more stable in aerobic exercise, where the body's metabolism during aerobic exercise is more stable

Introduction: Fatigue is the exhaustion of the body or spirit that can be caused by stress, excessive work, drug use, physical or mental illness. Fatigue is a factor in reducing productivity and incidents. Therefore, measuring fatigue is important. The aim of this study was to determine the fatigue measurement based on subjective and objective indices. Method: A cross-sectional study was performed on 120 male and female students. By simulating the real environment of each person in the 50 and 70dB pressure levels for 40 minutes, before and after exposure to noise, the amount of mental fatigue was measured using the subjective index Visual Analog Scale (VAS) and the objective index continuous performance test (CPT).  The results were analyzed using paired t-test and Spearman correlation coefficient in SPSS 20 software. Results: Measurement results with both self-declaration scale and continuous performance test showed that mental exhaustion  was increased significantly after exposure to the mentioned pressure level, but the results of two methods were not consistent with each other. Conclusion: Considering that the VAS self-declaration scale is a subjective method, it seems that the mismatch of the two methods is more related to the ineffectiveness of this scale. Therefore, it is suggested that re-examination has to be done with more precise methods such as electroencephalogram.

The 6 minute walk test (6MWT) and the incremental shuttle walking test (ISWT) have been used as an alternative to the Cardiopulmonary Exercise Test (CPX) for functional evaluation as well after rehabilitation programs. The objective was to analysis the cardiorespiratory and metabolic demands among the ISWT, 6MWT and CPX in the obese and the contribution of adiposity markers on this response. An observational and cross-sectional study was conducted with fifteen obese (10 women; 39.4+10.1years), performing CPX, 6MWT and ISWT. The subjects were as initial part of a rehabilitation program and bariatric surgery at the University Hospital. Metabolic and ventilatory variables were recorded by a telemetry system during all tests. Peak oxygen uptake-VO2peak was similar between CPX (18.6±4.0ml/kg/min) and ISWT (15.4±2.9ml/kg/min) and different from 6MWT (13.2±2.5ml/kg/min). There was agreement (3.2ml/kg/min; 95%; IC-3.0-9.4) between VO2peak of CPX and ISWT. CPX duration (R2=0.61;p=0.001) was best-fit by waist circumference (WC) and the body adiposity index(BAI) that reduced 4.7% and 3.2% CPX duration respectively. Forced vital capacity-FVC and WC predicts increasing of carbon dioxide production (VCO2) on CPX (R2=0.95;p=0.001) and ISWT (R2=0.67;p=0.001). In conclusion the obese individuals perform the ISWT and CPX test with similar physiological responses. It is suggested that, ISWT could be an alternative to CPX and that metabolic monitoring of ISWT by telemetry can be useful for the clinical assessment of the functional capacity of the obese.

Introduction: Determination of dry weight is one of the daily goals to achieve in hemodialysis. The aim of this study was to validate the use of bioelectrical impedance analysis (BIA) in estimation of dry weight in a population of Senegalese chronic hemodialysis patients. Patients and methods: A 9-week cross-sectional study was carried out at the hemodialysis unit of Aristide Le Dantec University Hospital. Adult patients with no previous hospital history were included. The total body water (TBW) was measured with a single frequency bioelectric impedance foot-to-foot analyzer, before and after six successive hemodialysis sessions. These results were compared with those from clinical measurements with the Watson equation using a Student’s t-test and Bland-Altman analysis. Results: 264 measurements were made in 22 patients (46.6 years, 54.5% men, 92.3 months on dialysis, 62.7 kg mean dry weight). A significant reduction in weight (ΔWeight = 2.0 ± 1.1 kg; p < 0.0001) and in TBW measured by the BIA (ΔTBWBIA = 3.3 ± 1.0 liters; p < 0.0001)) or calculated by Watson’s equation (ΔTBWWatson = 0.5 ± 0.2 liter; p = 0.0001) was observed. There was a strong linear correlation and agreement between the 2 TBW measurements in pre-dialysis. In post-dialysis the concordance diagram indicated a bias = –2.2 and wide agreement limits. Conclusion: The BIA allows reproducible and reliable measurements and a fair estimate of the TBW in pre-dialysis.

Background. Psoriasis is a chronic, inflammatory, and multifactorial dermatosis that impairs quality of life (QoL). Health-related QoL has become an important element in medical decision-making along with the effectiveness and the harmlessness of the treatments. Objective. To assess the impact of psoriasis in the QoL of patients with psoriasis by using the DLQI scales. Methods. A cross-sectional study from January to June 2018 was conducted in the Department of Dermatology of the University Hospital Joseph Raseta Befelatanana, Antananarivo, Madagascar, including patients more than 18 years old with mild to severe psoriasis. The severity of psoriasis was assessed using the “Psoriasis Area and Severity Index (PASI)”. QoL of patients with psoriasis was evaluated by using the DLQI scales. Results. 80 patients were included, their mean age was 36.5 years, and the male to female was 1.5 : 1. The mean DLQI score was 13.8. Symptoms, feelings, and psychic were the most altered dimensions. QoL was impaired in young patients, single, having medium level education. Even though patients with disease duration more than 5 years had higher DLQI score than other patients, the difference was not statistically significant (p=0.36). Furthermore, the clinical presentation of psoriasis did not influence the patient’s QoL (p=0.73). Patients with nail involvement had QoL impaired but the difference with another localization was not statistically significant (p=0.2). The quality of life was influenced by body area involved. The higher the body surface area involved, the more QoL is impaired (p=0.002). Furthermore, the higher the PASI, the more QoL is altered (p=0.002). Conclusion. Psoriasis has a negative impact in the quality of life in Malagasy patients with psoriasis, especially in younger and single patients. Worse quality of life is correlated to severity of psoriasis.
