Abstract Objectives: Sonoelastography is a relatively new technique used to evaluate the musculoskeletal system, but shear wave elastography of the knee menisci has not previously been reported. The purpose of this study is to study sonoelastographic features of the knee menisci. Methods: 34 knee joints were studied, in 17 healthy subjects. Shear wave elastography was used to evaluate the anterior horn of the medial meniscus. Normal sonoelastographic values were obtained. Results: The mean shear elastic modulus of the anterior horn of the right medial meniscus was 24.86 kPa (range 15.3- 36.1 ±6.35 SD), and 23.86 kPa (range14.6-37.6 ± 4.49 SD) for the anterior horn of the left medial meniscus. Inverse correlation was noted between the right medial meniscus elasticity and height. Inverse correlation was noted between the left medial meniscus elasticity and age, while positive correlation was noted between left medial meniscus elasticity and BMI. Conclusion: The elastic modulus of the knee menisci has been determined in healthy subjects and can serve as a reference when studying pathological conditions of these structures .

Objective To determine the ability of shear wave elastography to measure the stiffness of the knee menisci in healthy adults. Methods This observational cross-sectional study evaluated knee joints in healthy adults. Shear wave elastography was used to evaluate the anterior horn of the medial menisci bilaterally. The correlations between the mean elasticity bilaterally and age, weight, height and body mass index (BMI) were calculated using Pearson’s correlation coefficient test. Results A total of 34 knee joints in 17 healthy subjects were evaluated. The mean ± SD shear elastic modulus of the anterior horn of the right medial meniscus was 24.86 ± 6.35 kPa and of the anterior horn of the left medial meniscus was 23.86 ± 4.49 kPa. A significant inverse correlation was observed between the right medial meniscus elasticity and height. Other demographic factors showed no significant relationship to the anterior horn of the right medial meniscus elasticity. A significant inverse correlation was observed between the anterior horn of the left medial meniscus elasticity and age, while a significant positive correlation was observed between left medial meniscus elasticity and BMI. Conclusion These preliminary results suggest that shear wave elastography could be a potential tool to aid in studying the stiffness of the knee menisci.

Objective This study was performed to examine the sonoelastographic features of the radial nerve in healthy subjects. Methods In this observational cross-sectional study, shear wave elastography was used to evaluate the radial nerve. The cross-sectional area and stiffness were measured. Results The study included 37 nerves in 20 healthy adult subjects. The mean cross-sectional area of the radial nerve at the arm was 6.1 mm2. The mean stiffness of the radial nerve in the short axis was 30.3 kPa, and that in the long axis was 34.9 kPa. The elasticity measurements were significantly different between the long axis and short axis. Conclusion The elastic modulus of the radial nerve was studied in healthy subjects and can serve as a reference for future assessment of different radial nerve pathologies.

Objectives Shear wave elastography is a rapidly evolving new technique. The purpose of the present study was to determine the sonoelastographic features of the submandibular glands in healthy individuals. Methods We conducted a cross-sectional study of 45 healthy individuals, whose submandibular glands were studied using shear wave elastography. The stiffness of both submandibular glands was measured and its relationships with the age, sex, body mass, body mass index (BMI), and height of the participants were assessed. Results The participants had a mean age of 31.82 ± 5.53 years, a mean height of 158.30 ± 7.16 cm, a mean body mass of 57.83 ± 8.84 kg, and a mean BMI of 22.99 ± 2.72 kg/m2. The mean shear elastic modulus of the right and left submandibular glands were 18.02 kPa and 19.09 kPa, respectively. Positive correlations were found between the elasticity of the right submandibular gland and the anthropometric parameters. Conclusion The elastic modulus of the submandibular glands has been determined in healthy individuals and the data generated can serve as a reference for future studies of pathological conditions of these structures.

Background: Shear wave electrography (SWE) is a novel non-invasive imaging technique which demonstrate tissue elasticity. Recent research evaluating the elasticity properties of normal and pathological tissues emphasize the diagnostic importance of this technique. Aims: Polycystic ovarian syndrome (PCOS), which is characterized by menstrual irregularity, hyperandrogenism, and polycystic overgrowth, may cause infertility. The aim of this study was to evaluate the elasticity of ovaries in patients with PCOS using SWE. Methods: 66 patients diagnosed with PCOS according to the Rotterdam criteria (PCOS = group I) and 72 patients with non-PCOS (Control = group II), were included in the study. Demographic and clinical characteristics of the participants were recorded. Ovarian elasticity was assessed in all patients with SWE, and speed values were obtained from the ovaries. The elasticity of the ovaries was compared between the two groups. Results: While there were statistically significant differences between the groups in body mass index (BMI), right and left ovarian volumes, luteinizing hormone and testosterone levels (p<0.05), no significant differences were found between groups I and II in the velocity (for the right ovary 3.89±1.81 vs. 2.93±0.72, p=0.301; for the left ovary 2.88±0.65 vs. 2.95±0.80, p=0.577) and elastography (for the right ovary 36.62±17.78 vs. 36.79±14.32, p=0.3952; for the left ovary 36.56±14.15 vs. 36.26±15.10, p=0.903) values, respectively. Conclusion: We could not obtain different velocity and elastography values from the ovaries of the patients with PCOS using SWE. Therefore, further large-scale studies are needed to elucidate this issue.

Shear wave elastography (SWE) is a type of ultrasound elastography with which the elastic properties of breast tissues can be quantitatively assessed. The purpose of this study was to determine the impact of different regions of interest (ROI) and lesion size on the performance of SWE in differentiating malignant breast lesions. The study included 150 female patients with histopathologically confirmed malignant breast lesions. Minimal (Emin), mean (Emean), maximal (Emax) elastic modulus and elasticity ratio (e-ratio) values were measured using a circular ROI size of 2, 4 and 6 mm diameters and the lesions were divided into large (diameter ≥ 15 mm) and small (diameter < 15 mm). Highest Emin, Emean and e-ratio values and lowest variability were observed when using the 2 mm ROI. Emax values did not differ between different ROI sizes. Larger lesions had significantly higher Emean and Emax values, but there was no difference in e-ratio values between lesions of different sizes. In conclusion, when measuring the Emin, Emean and e-ratio of malignant breast lesions using SWE the smallest possible ROI size should be used regardless of lesion size. ROI size has no impact on Emax values while lesion size has no impact on e-ratio values.

In children, the quality and muscle function are altered in many pathologic conditions, including temporomandibular disorders. Although several methods have been used to evaluate muscle tonus, none became a golden standard. Moreover, the masseter muscle characteristics in children have not been investigated to date. This study aimed to measure the stiffness of the masseter muscle using shear-wave elastography in healthy children. We enrolled 30 healthy children (mean age 10.87 ± 3.38 years). The stiffness of masseter muscles was measured with shear wave elastography. Stiffness for the total sample was 6.37 ± 0.77 kPa. A comparison of the measurements did not show significant differences between the right and the left masseter muscles (left—6.47 ± 0.78 kPa; right—6.24 ± 0.76 kPa; p = 0.3546). A significant difference was seen between boys and girls (boys—5.94 ± 0.50 kPa; girls—6.63 ± 0.80; p = 0.0006). Shear-wave elastography is a promising diagnostic tool. It may help to detect changes in the stiffness of the masseter muscle and draw attention to pathological processes within the jaw muscles. Directions for further research shall include determining stiffness values in pathological conditions and the impact of biological and functional factors on the stiffness of the masseter muscle.

This study aimed to determine the measurements of the cochlea among healthy subjects and hearing deafness subjects using a High Resolution Computed Tomography (HRCT). A total of 230 temporal bone HRCT cases were retrospectively investigated in the period spanning from 2011 to 2015. Three 64-slice units were used to examine patients with clinical complaints of hearing loss conditions at three Radiology departments in Khartoum, Sudan. For the control group (A) healthy subjects, the mean width of the right and left cochlear were 5.61±0.40 mm and 5.56±0.58 mm, the height were 3.56±0.36 mm and 3.54±0.36 mm, the basal turn width were 1.87±0.19 mm and 1.88 ±0.18 mm, the width of the cochlear nerve canal were 2.02±1.23 and 1.93±0.20, cochlear nerve density was 279.41±159.02 and 306.84±336.9 HU respectively. However, for the experimental group (B), the mean width of the right and left cochlear width were 5.38±0.46 mm and 5.34±0.30 mm, the height were 3.53±0.25 mm and 3.49±0.28mm, the basal turn width were 1.76±0.13 mm, and 1.79±0.13 mm, the width of the cochlear nerve canal were 1.75±0.18mm and 1.73±0.18mm, and cochlear nerve density were 232.84±316.82 and 196.58±230.05 HU, respectively. The study found there was a significant difference in cochlea’s measurement between the two groups with a p-value < 0.05. This study had established baseline measurements for the cochlear for the healthy Sudanese population. Furthermore, it found that HRCT of the temporal bone was the best for investigation of the cochlear and could provide a guide for the clinicians to manage congenital hearing loss.

Introduction: In this study, we attempt to determine the diagnostic performance of shear wave elastography of the optic nerve and adjacent fat tissue in patients with optic neuritis. Methods: The study included a patient group consisting of 72 eyes of 36 patients who were diagnosed with unilateral optic neuritis, and an age-matched control group of 36 eyes of 18 healthy subjects. The patient group consisted of 25 multiple sclerosis patients and 11 recurrent isolated optic neuritis patients. The mean shear wave elastography values of the optic nerves and intraorbital fat tissue adjacent optic nerves were recorded using m/s and kPa as units. ROC curve analysis was performed, and the diagnostic accuracy of shear wave elastography values was determined. Results: The mean shear wave elastography values of the optic nerves with neuritis (2.49 ± 0.41 m/s and 17.56 ± 4.42 kPa) were significantly higher than the values of the contralateral normal optic nerves (1.71 ± 0.32 m/s and 9.02 ± 2.34 kPa) (p = 0.006 and p = 0.004, respectively) in the optic neuritis group. The mean shear wave elastography values of intraorbital fat tissue adjacent optic nerves with neuritis (1.87 ± 0.32 m/s and 9.65 ± 1.12 kPa) were significantly higher than the values of the contralateral normal side (1.47 ± 0.27 m/s and 6.78 ± 1.14 kPa) (p = 0.025 and p = 0.022, respectively) in the optic neuritis group. ROC curve analysis showed a high diagnostic accuracy for determining optic neuritis with shear wave elastography values of the optic nerves (AUC 0.955 [95% CI, 0.933–0.978] in m/s and AUC 0.967 [95% CI, 0.940–0.985] in kPa). Conclusions: Shear wave elastography may be an important alternative diagnostic tool in the diagnosis of optic neuritis.

Background: Ulnar nerve compression at the elbow level is the second-most common entrapment neuropathy. The aim of this study was to use shear-wave elastography for the quantification of ulnar nerve elasticity in patients after ulnar nerve decompression with anterior transposition and in the contralateral non-operative side. Method: Eleven patients with confirmed diagnosis and ulnar nerve decompression with anterior transposition were included and examinations were performed on an AixplorerTM ultrasound system (Supersonic Imagine, Aix-en-Provence, France). Results: We observed significant differences at 0-degree (p < 0.001), 45-degree (p < 0.05), 90-degree (p < 0.01) and 120-degree (p < 0.001) elbow flexion in the shear elastic modulus of the ulnar nerve in the operative and non-operative sides. There were no statistically significant differences between the elasticity values of the ulnar nerve after transposition at 0-degree elbow flexion and in the non-operative side at 120-degree elbow flexion (p = 0.39), or in the ulnar nerve after transposition at 120-degree elbow flexion and in the non-operative side at 0-degree elbow flexion (p = 0.09). Conclusion: Shear-wave elastography has the potential to be used postoperatively as a method for assessing nerve tension noninvasively by the estimation of mechanical properties, such as the shear elastic modulus.

Abstract Aim The primary aim of this study was to determine the inter-system variability of liver stiffness measurements (LSMs) in patients with varying degrees of liver stiffness. The secondary aim was to determine the inter-observer variability of measurements. Materials and Methods 21 individuals affected by chronic hepatitis C and 5 healthy individuals were prospectively enrolled. The assessment of LSMs was performed using six ultrasound (US) systems, four of which with point shear wave elastography (p-SWE) and two with 2 D shear wave elastography (2D-SWE) systems. The Fibroscan (Echosens, France) was used as the reference standard. Four observers performed the measurements in pairs (A-B, C-D). The agreement between different observers or methods was calculated using Lin’s concordance correlation coefficient. The Bland-Altman limits of agreement (LOA) were calculated as well. Results There was agreement above 0.80 for all pairs of systems. The mean difference between the values of the systems with 2D-SWE technique was 1.54 kPa, whereas the maximum mean difference between the values of three out of four systems with the pSWE technique was 0.79 kPa. The intra-patient concordance for all systems was 0.89 (95 % CI: 0.83 – 0.94). Inter-observer agreement was 0.96 (95 % CI: 0.94 – 0.98) for the pair of observers A-B and 0.93 (95 % CI: 0.89 – 0.96) for the pair of observers C-D. Conclusion The results of this study show that the agreement between LSMs performed with different US systems is good to excellent and the overall inter-observer agreement in “ideal conditions” is above 0.90 in expert hands.
