AbstractBecause of the difficulties involved in the invasive monitoring of conscious patients, core temperature monitoring is frequently neglected during neuraxial anaesthesia. Zero heat flux (ZHF) and double sensor (DS) are non-invasive methods that measure core temperature from the forehead skin. Here, we compare these methods in patients under spinal anaesthesia. Sixty patients scheduled for elective unilateral knee arthroplasty were recruited and divided into two groups. Of these, thirty patients were fitted with bilateral ZHF sensors (ZHF group), and thirty patients were fitted with both a ZHF sensor and a DS sensor (DS group). Temperatures were saved at 5-min intervals from the beginning of prewarming up to one hour postoperatively. Bland–Altman analysis for repeated measurements was performed and a proportion of differences within 0.5 °C was calculated as well as Lin`s concordance correlation coefficient (LCCC). A total of 1261 and 1129 measurement pairs were obtained. The mean difference between ZHF sensors was 0.05 °C with 95% limits of agreement − 0.36 to 0.47 °C, 99% of the readings were within 0.5 °C and LCCC was 0.88. The mean difference between ZHF and DS sensors was 0.33 °C with 95% limits of agreement − 0.55 to 1.21 °C, 66% of readings were within 0.5 °C and LCCC was 0.59. Bilaterally measured ZHF temperatures were almost identical. DS temperatures were mostly lower than ZHF temperatures. The mean difference between ZHF and DS temperatures increased when the core temperature decreased.Trial registration: The study was registered in ClinicalTrials.gov on 13th May 2019, Code NCT03408197.

The possibility of using a smartwatch as a rehabilitation tool to monitor patients’ heart rates during exercise has gained the attention of many researchers. This study aimed to evaluate the accuracy and precision of the HR measurement performed by two wrist monitors: the Fitbit Charge 4 and the Xiaomi Mi Band 5. Thirty-one healthy volunteers were asked to perform a stress test on a treadmill. Their heart rates were recorded simultaneously by the wristbands and an electrocardiogram (ECG) at 1 min intervals. The mean absolute error percentage (MAPE), Lin’s concordance correlation coefficient (LCCC), and Bland–Altman analysis were calculated to compare the precision and accuracy of heart rate measurements. The estimated validation criteria were MAPE < 10% and LCCC < 0.8. The overall MAPE and LCCC of the Fitbit were 10.19% (±11.79%) and 0.753 (95% CI: 0.717–0.785), respectively. The MAPE and LCCC of the Xiaomi were 6.89% (±9.75) and 0.903 (0.886–0.917), respectively. The precision and accuracy of both devices decreased with the increased exercise intensity. The accuracy of wearable wrist-worn heart rate monitors varies and depends on the intensity of training. Therefore, the decision to use such a device as a heart rate monitor during in-home rehabilitation should be made with caution.

For longitudinal overdispersed Poisson data sets, estimators of the intra-, inter-, and total concordance correlation coefficient through variance components have been proposed. However, biased estimators of quadratic forms are used in concordance correlation coefficient estimation. In addition, the generalized estimating equations approach has been used in estimating agreement for longitudinal normal data and not for longitudinal overdispersed Poisson data. Therefore, this paper proposes a modified variance component approach to develop the unbiased estimators of the concordance correlation coefficient for longitudinal overdispersed Poisson data. Further, the indices of intra-, inter-, and total agreement through generalized estimating equations are also developed considering the correlation structure of longitudinal count repeated measurements. Simulation studies are conducted to compare the performance of the modified variance component and generalized estimating equation approaches for longitudinal Poisson and overdispersed Poisson data sets. An application of corticospinal diffusion tensor tractography study is used for illustration. In conclusion, the modified variance component approach performs outstandingly well with small mean square errors and nominal 95% coverage rates. The generalized estimating equation approach provides in model assumption flexibility of correlation structures for repeated measurements to produce satisfactory concordance correlation coefficient estimation results.

Objective: Handheld dynamometers are common tools for assessing/monitoring muscular strength and endurance. Health/fitness Bluetooth load sensors may provide a cost-effective alternative; however, research is needed to evaluate the validity and reliability of such devices. This study assessed the validity and reliability of two commercially available Bluetooth load sensors (Activ5 by Activbody and Progressor by Tindeq). Methods: Four tests were conducted on each device: stepped loading, stress relaxation, simulated exercise, and hysteresis. Each test type was repeated three times using the Instron ElectroPuls mechanical testing device (a gold-standard system). Test–retest reliability was assessed through intraclass correlations. Agreement with the gold standard was assessed with Pearson’s correlation, interclass correlation, and Lin’s concordance correlation. Results: The Activ5 and Progressor had excellent test–retest reliability across all four tests (ICC(3,1) ≥ 0.999, all p ≤ 0.001). Agreement with the gold standard was excellent for both the Activ5 (ρ ≥ 0.998, ICC(3,1) ≥ 0.971, ρc ≥ 0.971, all p’s ≤ 0.001) and Progressor (ρ ≥ 0.999, ICC(3,1) ≥ 0.999, ρc ≥ 0.999, all p’s ≤ 0.001). Measurement error increased for both devices as applied load increased. Conclusion: Excellent test–retest reliability was found, suggesting that both devices can be used in a clinical setting to measure patient progress over time; however, the Activ5 consistently had poorer agreement with the gold standard (particularly at higher loads).

We aimed to evaluate the stability of radiomic features in the liver of healthy individuals across different three-dimensional regions of interest (3D ROI) sizes in T1-weighted (T1w) and T2-weighted (T2w) images from different MR scanners. We retrospectively included 66 examinations of patients without known diseases or pathological imaging findings acquired on three MRI scanners (3 Tesla I: 25 patients, 3 Tesla II: 19 patients, 1.5 Tesla: 22 patients). 3D ROIs of different diameters (10, 20, 30 mm) were drawn on T1w GRE and T2w TSE images into the liver parenchyma (segment V–VIII). We extracted 93 radiomic features from the different ROIs and tested features for significant differences with the Mann–Whitney-U (MWU)-test. The MWU-test revealed significant differences for most second- and higher-order features, indicating a systematic difference dependent on the ROI size. The features mean, median, root mean squared (RMS), 10th percentile, and 90th percentile were not significantly different. We also assessed feature robustness to ROI size variation with overall concordance correlation coefficients (OCCCs). OCCCs across the different ROI-sizes for mean, median, and RMS were excellent (>0.90) in both sequences on all three scanners. These features, therefore, seem robust to ROI-size variation and suitable for radiomic studies of liver MRI.

AbstractThis study provides a quantitative assessment of the accuracy of a commercially available deformable image registration (DIR) algorithm to automatically generate prostate contours and additionally investigates the robustness of radiomic features to differing contours. Twenty-eight prostate cancer patients enrolled on an institutional review board (IRB) approved protocol were selected. Planning CTs (pCTs) were deformably registered to daily cone-beam CTs (CBCTs) to generate prostate contours (auto contours). The prostate contours were also manually drawn by a physician. Quantitative assessment of deformed versus manually drawn prostate contours on daily CBCT images was performed using Dice similarity coefficient (DSC), mean distance-to-agreement (MDA), difference in center-of-mass position (ΔCM) and difference in volume (ΔVol). Radiomic features from 6 classes were extracted from each contour. Lin’s concordance correlation coefficient (CCC) and mean absolute percent difference in radiomic feature-derived data (mean |%Δ|RF) between auto and manual contours were calculated. The mean (± SD) DSC, MDA, ΔCM and ΔVol between the auto and manual prostate contours were 0.90 ± 0.04, 1.81 ± 0.47 mm, 2.17 ± 1.26 mm and 5.1 ± 4.1% respectively. Of the 1,010 fractions under consideration, 94.8% of DIRs were within TG-132 recommended tolerance. 30 radiomic features had a CCC > 0.90 and 21 had a mean |%∆|RF < 5%. Auto-propagation of prostate contours resulted in nearly 95% of DIRs within tolerance recommendations of TG-132, leading to the majority of features being regarded as acceptably robust. The use of auto contours for radiomic feature analysis is promising but must be done with caution.

Ripe rot caused by Colletotrichum spp. is a serious threat in many vineyards, and its control relies mainly on the repeated use of fungicides. A mechanistic, dynamic model for the prediction of grape ripe rot epidemics was developed by using information and data from a systematic literature review. The model accounts for i) the production and maturation of the primary inoculum; ii) the infection caused by the primary inoculum; iii) the production of a secondary inoculum; and iv) the infection caused by the secondary inoculum. The model was validated in 19 epidemics (vineyard × year combinations) between 1980 and 2014 in China, Japan, and the USA. The observed disease incidence was correlated with the number of infection events predicted by the model and their severity (ρ = 0.878 and 0.533, respectively, n=37, P≤ 0.001). The model also accurately predicted the disease severity progress during the season, with a concordance correlation coefficient of 0.975 between the observed and predicted data. Overall, the model provided an accurate description of the grape ripe rot system, as well as reliable predictions of infection events and of disease progress during the season. The model increases our understanding of ripe rot epidemics in vineyards and will help guide disease control. By using the model, growers can schedule fungicides based on the risk of infection rather than on a seasonal spray calendar.

The use of sensor technologies to monitor cows’ behavior is becoming commonplace in the context of dairy production. This study aimed at validating a commercial collar-based sensor system, the AFICollar® (Afimilk, Kibbutz Afikim, Israel), designed to monitor dairy cattle feeding and ruminating behavior. Additionally, the performances of two versions of the software for behavior classification, the current software AFIfarm® 5.4 and the updated version AFIfarm® 5.5, were compared. The study involved twenty Holstein-Friesian cows fitted with the collars. To evaluate the sensor performance under different feeding scenarios, the animals were divided into four groups and fed three different types of feed (total mixed ration, long hay, animals allowed to graze). Recordings of hourly rumination and feeding time produced by the sensor were compared with visual observation by scan sampling at 1 minute intervals using Spearman correlation, concordance correlation coefficient (CCC), Bland–Altman plots and linear mixed models for assessing the precision and accuracy of the system. The analyses confirmed that the updated software version V5.5 produced better detection performance than the current V5.4. The updated software version produced high correlations between visual observations and data recorded by the sensor for both feeding (r = 0.85, CCC = 0.86) and rumination (r = 0.83, CCC = 0.86). However, the limits of agreement for both behaviors remained quite wide (feeding: −19.60 min/h, 17.46 min/h; rumination: −15.80 min/h, 15.00 min/h). Type of feed did not produce significant effects on the agreement between visual observations and sensor recordings. Overall, the results indicate that the system can provide farmers with adequately accurate data on feeding and rumination time, and can be used to support herd management decisions. Despite all this, the precision of the system remained relatively limited, and should be improved with further developments in the classification algorithm.

This research project had the aim to validate the possible alternative and less-painful sampling method of cutting feathers close to the skin instead of plucking them for subsequent feather corticosterone analysis, confirming recently-published results for other species in captivity. Analyzing CORTf is often used in animal welfare studies in combination with behavioral monitoring. The background of this idea was to act in the sense of animal welfare and reduce the burden of animal studies according to the 3-R-Principle (Replacement, Reduction, and Refinement) by refining procedures. To confirm the hypothesis that the sampling method itself has no influence on CORTf levels measured, plucked and cut samples of the respective bird were collected. Birds of two wild species were used: the Mallard (Anas platyrhynchos) and the Greater Flamingo (Phoenicopterus roseus). The CORTf was measured by using an enzyme-linked immunosorbent assay (ELISA). The determined values were inspected for their mean values, standard deviation (SD), and average differences. Afterwards, the CORTf levels of both species were compared, according to the sampling method, with the concordance correlation coefficient (CCC). In the Bland-Altman (BA) plot the differences of the methods were displayed against the mean values. Additionally, sex, as a possible factor influencing CORTf, was analyzed using the Mann-Whitney U test. The values of CCC showed poor agreement in the comparability of the two methods, whereas the concordance of the BA plot was decent. The average differences between the methods were marginal for both species (Mallards: −0.16 pg/mm, Flamingos −0.13 pg/mm). In summary, all anomalies or differences between the methods were negligible. Therefore, the alternative sampling method seems to be as suitable as the common standard method. No significant difference was found between females and males. Nevertheless, our results suggest that CORTf should not be interpreted in just considering the values themselves, but the results they should be analyzed in the context of a wider set of parameters. Hence, further studies are encouraged to create a larger data pool.
